Common Hip Diseases and Their Imaging Studies (흔히 접할 수 있는 고관절 질환들과 영상 검사)

  • Hip joint is one of the most deeply located joint in human body. Moreover, most clinical symptoms of various hip diseases are vague and confusing. In these reasons, the imaging modalities of hip have been extensively studied and developed. From a plain radiograph to more complex MR arthrography, various techniques are now available. In this brief review, the long and short of imaging modalities and clinical examples of common hip diseases will be introduced. The most important significance of diagnostic imaging is its clinical relevance. It is because when we know the characteristics of the imaging modalities and the disease itself, the optimal approach to a patient can be achieved, but when we do not know, the expensive images can only be a meaningless picture.

  • A ganglion cyst contains clear high viscous fluid within dense fibrous connective tissue wall communicating with joint or tendon sheath, but is not frequent around the knee joint. A hard mass at the medial side of the knee joint of the 73 years old woman, mimic to hard tumor mass was diagnosed as the ganglion using ultrasonography. Ultrasonography was a useful diagnostic tool of the cystic lesion although the consistency of the mass was hard enough to be suspected as a tumor mass.

  • Background: Liver cytology is indicated and requested for evaluating hepatic masses in symptomatic or serendipitous cryptic discovered lesions. Objective: To determine the cytomorphological patterns of hepatic lesions identified among a group of Sudanese patients. Materials and Methods: This is an analytical descriptive hospital-based study included 165 patients undergoing ultrasound-guided fine needle is an aspiration cytology (FNAC)for hepatic lesions, at Al-Amal Military Hospital & Khartoum Teaching Hospital in Khartoum, Sudan. Clinical data were reviewed. Air dried Diff Quick stained smears were grouped into unsatisfactory samples, benign lesions, and malignant neoplasms. Results: Our population were consisted of 35 (21.2%) females and 130 (78.8%) males, with a male to female ratio 3.7:1 and an age ranged between 47 to 80, and a mean age $57{\pm}7$. Of 165 cases, 57 (34.5%) were benign, no atypia were noticed, 101 (61.2%) were malignant. Most investigated patients were found to have metastatic lesions. Conclusion: FNAC is a useful tool for investigating hepatic lesions.

Compressive Neuropathy in Upper Extremity (상지의 압박 신경병증)

  • Compressive neuropathy in the upper extremity can be clinically diagnosed by careful history taking, physical examination of the involved nerve. Electrodiagnosis for the suspected nerve informs severity of compression of the involved nerve and indicates specific site of the lesion. In the early stage of the disease, non-operative treatment generally cures the symptom, however, if the conservative treatment fails, confirmation of the exact site of the lesion should be preceded before the operation. Recently, ultrasonography, as a supportive tool for the diagnosis of compressive neuropathy has increasing popularity for its ability to find space occupying lesion, anatomical change of the nerve, and the pathologic change in the nerve itself. For the successful treatment of the compressive neuropathy, these various diagnostic tools have to be introduced in the orthopaedic clinic.

  • Portal blood flow was measured with pulsed doppler ultrasound in thirty normal dogs. In normal dogs, the average portal blood flow velocity was $17.03{\pm}1.75cm/sec$ and the average portal blood flow was $41.59{\pm}10.10ml/min/kg$. The incident angle between the doppler beam and the portal vein averaged $65^{\circ}$. The average portal vein sectional area was $0.41{\pm}0.14cm^2$. The Congestion index was $0.0245{\pm}0.0081cm{\cdot}sec$. Conclusively, the spectral doppler ultrasonography was quick, non-invasive and simple diagnostic method in circulatory disorders of liver.

  • Simple radiography, CT and MRI were used for the diagnosis of the hip disease. However ultrasonography could be a good diagnostic tool for the lesion of tendons and bursae. Another advantage of ultrasonography is its usefulness for the follow-up of these lesions. Nowadays hip ultrasonography expanded the horizon to the follow-up study of total hip arthroplasty. However, the limitation of ultrasonography is that there is disparity in results according to the physician's experiences. In this article ultrasonographic findings of normal structures and pathologic lesions of the hip joint are introduced and explained briefly.

  • Traditionally the diagnosis and treatment of the diseases of the knee is based on the findings of the x-rays and the MRI. The x-rays provide good information of the changes of the internal structure of the bone. However, there is a limitation in providing information of the soft tissue and the cartilage. The MRI is one of the most expensive diagnostic modalities and it can not give us a dynamic and real time information. The sonography has a role in diagnosis and treatment of the soft tissue disease and surface of the bone. It gives us a real time dynamic information and it is really cheap. In this article the sonographic findings of the normal and pathologic conditions of the knee joint are introduced in relation to the findings of the x-rays and the MRI.

  • We experienced three cases with chronic shoulder pain due to a knot impingement after arthroscopic rotator cuff tear repair and treated with arthroscopic revision surgery. Ultrasonography is commonly used for an imaging scan and an useful diagnostic tool to follow up after rotator cuff repair recently. We also could diagnose three cases with the knot impingement using ultrasonography obviously. And we report these cases with a review of current literature.

  • In this study, in order to overcome the weakness of acoustic field analysis which is generally used for ultrasonic probe performance evaluation, automatic acoustic field measurement system and evaluation parameters were proposed. The comparisons between acoustic field simulation and measured acoustic distribution data of normal and abnormal channels were conducted to evaluate the availability of proposed system and evaluation parameters. First, the impulse response characteristic of sample probe was investigated to classify the normal elements and abnormal elements. And then, normal channels and abnormal channels with abnormal element were chosen. The suggested 12 evaluation parameters were calculated using the acoustic fields of these channels. The availability of proposed automatic acoustic field measurement system and evaluation parameters was confirmed. And the performance evaluation of ultrasonic probe using acoustic field analysis could be easier and faster.

  • This paper describes a resonant driving piezoelectric micro cantilever practicable to ultrasound thrombolysis device for the treatment of ischemic stroke. The proposed piezoelectric cantilever was designed to be a unimorph structure of Si/$SiO_2$/Ti/Pt/PZT/Pt, and fabricated by top-down sequence etching process. The red blood cell (RBC) lysis experiment was carried out to confirm the usability of the proposed cantilever. Total 87.76 % of RBCs were ruptured using the ultrasound generated by up-down actuations of the fabricated cantilever with AC voltage having the frequency of 19.36 Hz and the magnitude of $30V_{p-p}$.
