A Study on the Correlation about 7-Zone-Diagnostic System and Clinical Parameters (7구역 진단기와 임상지표와의 상관성 연구)

  • Objectives : The purpose of this study is to find correlation about 7-Zone-Diagnostic System and Clinical Parameters using Oriental Health Examination. Methods : Subjects of our study were 325 cases who took Oriental Health Examination. We collected the data of body composition analysis, 7-Zone-Diagnostic System, blood analysis and devided by 7-Zone-Diagnostic system to acceleration, normalcy and depression. Results and Conclusions : The values of muscle mass, body fat mass, abdomen obesity rate, right arm water index, left arm water index, trunk water index, right leg water index, left leg water index, BMI, BMR, GOT, GPT, ${\gamma}$-GTP, BUN, TC, triglyceride and hemoglobin were continuous with 7-Zone-Diagnostic system.

  • Objective : The purpose of this study is to analyze the naming of 'A diagnostic system based on Shanghanlun six meridian patterns and provisions' and to suggest an alternative naming. Methods : 1. The meaning of 'Six meridian(六經)' was reviewed on existing theories and Shanghanlun provisions. 2. Comparing the name of diangostic system with the term in 'Korean Standard Classification of Diseases-6(KCD-6)' and term in 'WHO international standard terminologies on traditional medicine in the western pacific region' was done. Results : 'Six meridian' is customary used in the Shagnhanlun study but its meaning is not match with original Shanghanlun system and could possibly make misunderstanding. So 'Disease pattern identification' is suitable than 'Six meridian' for this diagnostic system. Conclusions : This study suggests that using 'A disease pattern identification diagnostic system based on Shanghanlun provisions.'is more appropriate instead of using the name of the six meridian diagnostic system.

  • In the field of clinical medicine, diagnostic accuracy studies refer to the degree of agreement between the index test and the reference standard for the discriminatory ability to identify a target disorder of interest in a patient. The receiver operating characteristic (ROC) curve offers a graphical display the trade-off between sensitivity and specificity at each cutoff for a diagnostic test and is useful in assigning the best cutoff for clinical use. In this end, the ROC curve analysis is a useful tool for estimating and comparing the accuracy of competing diagnostic tests. This paper reviews briefly the measures of diagnostic accuracy such as sensitivity, specificity, and area under the ROC curve (AUC) that is a summary measure for diagnostic accuracy across the spectrum of test results. In addition, the methods of creating an ROC curve in single diagnostic test with five-category discrete scale for disease classification from healthy individuals, meaningful interpretation of the AUC, and the applications of ROC methodology in clinical medicine to determine the optimal cutoff values have been discussed using a hypothetical example as an illustration.

  • Objectives: The purpose of this study was to identify the distribution of diagnostic disease among applicants for shoulder musculoskeletal disease. Methods: In 2020, 47 diagnostic disease applicants were investigated for sociodemographic, health, work, job, and diagnostic disease characteristics. The data were corrected through on-site visits and analyzed using descriptive statistics with SPSS WIN23.0. Results: Most of the applicants were male and elderly. They had high blood pressure(38.3%) and diabetes (21.3%), and the drinking rate and smoking rate were also high. The most common type of employment was daily workers, and it was confirmed that the working condition was poor due to excessive working hours and short rest times. Most of the applicants for shoulder diagnostic diseases were in the construction industry, and the most common diagnostic disease was a rotator cuff tear. Conclusions: It is necessary to develop a musculoskeletal disease prevention program suitable for construction workers to reduce their work-related disease. When establishing a program, business type, task, and diagnostic disease must be considered.

  • In order to diagnose a vehicle, it is achieved by collecting diagnostic data within the ECU or between ECUs and managing the diagnostic data by utilizing various communication methods through an electronic device composed of an ECU(Electronic Control Unit), which is an automotive electronic device. As communication methods, LIN, CAN, FlexRay are mainly used. Recently, wired/wireless communication is being used based on Ethernet. In order to perform vehicle diagnosis, it is necessary to know the diagnosis code generated by the ECU and to collect diagnosis data using diagnosis communication. In addition, diagnostic data can be managed from the ECU only when the application software required for vehicle diagnosis is configured. If many automobile manufacturers are manufacturing ECUs based on the AUTOSAR standard, which is an automotive electronic standard, the software structure is also configured to be applied according to the standard. In this paper, we understand the vehicle diagnosis communication method of the AUTUSAR standard, study the configuration and processing method of diagnosis data, and study the contents of software components, diagnosis communication, and diagnosis event processing.

  • Objectives: The objective of this study is to review the diagnostic methods of neurologic symptoms by non-organic causes. Methods: We experienced a case of a motor and sensory disturbance patient without abnormal findings in the diagnostic tests. Using this process, we became aware that it is important to remember some of the methods when treating patients who are suspicious of neurologic symptoms by non-organic causes. Results: First, we should perform a variety of diagnostic tests to exclude the organic causes. Second, we should persistently observe a consistency of the patient's symptoms and attitude. Finally, we should perform a neurological examination in order to acquire the objective physical findings. Conclusions: Because there are limitations in the methods above, we have to be aware of making a mistake regarding the disability by organic causes as that by non-organic causes.

Epidemiological investigation of equine hindlimb ataxia with Setaria digitata in South Korea

  • Background: Since 2013, the number of requests for diagnosis for horses based on neurological symptoms has increased rapidly in South Korea. The affected horses have commonly exhibited symptoms of acute seasonal hindlimb ataxia. A previous study from 2015-2016 identified Setaria digitata as the causative agent. Objectives: This study is an epidemiological investigation to find out risk factors related to the rapid increase in hindlimb ataxia of horses due to aberrant parasites in South Korea. Methods: An epidemiological investigation was conducted on 155 cohabiting horses in 41 horse ranches where the disease occurred. The surrounding environment was investigated at the disease-causing horse ranches (n = 41) and 20, randomly selected, non-infected ranches. Results: Hindlimb ataxia was confirmed in nine cohabiting horses; this was presumed to be caused by ectopic parasitism. Environments that mosquitoes inhabit, such as paddy fields within 2 km and less than 0.5 km from a river, had the greatest association with disease occurrence. Conclusions: Most horse ranches in South Korea are situated in favorable environments for mosquitoes. Moreover, the number of mosquitoes in the country has increased since 2013 due to climate change. Additional research is required; however, these data show that it is necessary to establish guidelines for the use of anthelmintic agents based on local factors in South Korea and disinfection of the environment to prevent disease outbreaks.

Use of "Diagnostic Yield" in Imaging Research Reports: Results from Articles Published in Two General Radiology Journals

  • Objective: "Diagnostic yield," also referred to as the detection rate, is a parameter positioned between diagnostic accuracy and diagnosis-related patient outcomes in research studies that assess diagnostic tests. Unfamiliarity with the term may lead to incorrect usage and delivery of information. Herein, we evaluate the level of proper use of the term "diagnostic yield" and its related parameters in articles published in Radiology and Korean Journal of Radiology (KJR). Materials and Methods: Potentially relevant articles published since 2012 in these journals were identified using MEDLINE and PubMed Central databases. The initial search yielded 239 articles. We evaluated whether the correct definition and study setting of "diagnostic yield" or "detection rate" were used and whether the articles also reported companion parameters for false-positive results. We calculated the proportion of articles that correctly used these parameters and evaluated whether the proportion increased with time (2012-2016 vs. 2017-2022). Results: Among 39 eligible articles (19 from Radiology and 20 from KJR), 17 (43.6%; 11 from Radiology and 6 from KJR) correctly defined "diagnostic yield" or "detection rate." The remaining 22 articles used "diagnostic yield" or "detection rate" with incorrect meanings such as "diagnostic performance" or "sensitivity." The proportion of correctly used diagnostic terms was higher in the studies published in Radiology than in those published in KJR (57.9% vs. 30.0%). The proportion improved with time in Radiology (33.3% vs. 80.0%), whereas no improvement was observed in KJR over time (33.3% vs. 27.3%). The proportion of studies reporting companion parameters was similar between journals (72.7% vs. 66.7%), and no considerable improvement was observed over time. Conclusion: Overall, a minority of articles accurately used "diagnostic yield" or "detection rate." Incorrect usage of the terms was more frequent without improvement over time in KJR than in Radiology. Therefore, improvements are required in the use and reporting of these parameters.
