Diagnostic Usefulness of Fine Needle Aspiration Cytology on Lymphadenopathy (림프절종대의 세침흡인 세포검사의 진단적 유용성 - 림프절의 세침흡인 세포검사 1,216예의 분석 -)

  • Clinical lymphadenopathies are subjected to fine needle aspiration cytology(FNAC) for diagnosing not only benign lesions but also malignant ones, as the first diagnostic procedure. While the diagnostic reliability in metastatic carcinoma is high, it is difficult to differentiate malignant lymphoma from reactive conditions. We evaluated the diagnostic reliability of FNAC in lymphadenopathy, and discuss the diagnostic limitation and its place in clinical practice in this study, Over 8 years from January 1988, FNAC of 1,216 lymphadenopathies were analyzed and among them 170 cases were compared with histopathology. The results are as follows. 1. Of ail the cases, 890 cases(73.2%) were diagnosed cytologically as benign, 312 cases(25.7%) as malignant, and 14 cases(1.1%) as unsatisfactory material. Reactive hyperplasia was diagnosed in 585 cases(65.7%) of the benign lesions, and among the malignant diseases, metastatic carcinoma was diagnosed in 248 cases(79.5%), and malignant lymphoma in 62 cases(19.9%). 2. The overall diagnostic accuracy was 89.2%, and no false positive case and 9 false negative results were observed among 170 cases which were proven by histopathology. Six cases of sampling error of false negative diagnoses included 3 of metastatic carcinomas and 3 of malignant lymphomas. The causes were difference between aspiration and biopsy site, poor fixation, or scanty cellularity with bloody smear. All 3 cases of misinterpretation error were malignant lympliomas, one of mixed type on biopsy which was diagnosed as reactive hyperplasia cytologically. In summary, FNAC technique is thought to be useful in the initial diagnosis of lymphadenopathies as well as in the follow-up of patients with known malignancy. Although the results of malignant lymphoma was less accurate than other malignant lesions, the application of strict cytologic criteria or lymphoid marker studies of aspiration material will reduce the false negative rate.

Diagnostic Value of Ceruloplasmin in the Diagnosis of Pediatric Wilson's Disease

  • Purpose: Measurement of serum ceruloplasmin level is the first step in screening for Wilson's disease (WD). Despite the rarity of WD in the general population, ceruloplasmin levels are routinely measured through hepatitis screening in both adults and children. Herein, we evaluated the diagnostic value of ceruloplasmin for the diagnosis of WD among children with hepatitis. Methods: We retrospectively reviewed data on serum ceruloplasmin levels measured as a serologic marker for patients with hepatitis at Asan Medical Center (Seoul, Korea) between from January 2004 to November 2013. The diagnosis of WD was confirmed by the identification of pathogenic variants in the ATP7B gene. To determine the diagnostic accuracy of ceruloplasmin, receiver operation characteristic (ROC) curves were constructed and the area under curve (AUC) were calculated. Results: Measurements of serum ceruloplasmin were performed in 2,834 children who had hepatitis. Among these, 181 (6.4%) children were diagnosed with WD. The sensitivity, specificity, and accuracy of a ceruloplasmin level of <20 mg/dL in the discrimination of WD were 93.4%, 84.2%, and 84.8%, respectively. In this study, 418 (14.7%) false-positive cases and 12 (0.4%) false-negative cases were noted. Using a ROC curve, a ceruloplasmin level of ${\leq}16.6mg/dL$ showed the highest AUC value (0.956) with a sensitivity of 91.2%, a specificity of 94.9%, and an accuracy of 94.7%. Conclusion: The measurement of serum ceruloplasmin was frequently used for the screening of WD in children, despite a low positive rate. The diagnostic value of ceruloplasmin may be strengthened by adopting a new lower cut-off level.

Fine Needle Aspiration Cytology of Palpable Lymph Nodes -A Single Institutional Experience of 1,346 Cases- (촉지 림프절의 세침흡인 세포검사 - 단일 기관의 1,346예 경험 -)

  • The aim of this study was to evaluate the diagnostic value of fine needle aspiration cytology (FNAC) for the assessment of palpable enlarged lymph nodes. The authors reviewed the results of 1,346 FNACs of palpable enlarged lymph nodes performed at Pusan National University Hospital from 1998 to 2004. Of the 1,346 cases, 1,265 (94.0%) were satisfactory and 81 (6.0%) unsatisfactory. Cytologic diagnoses were judged in 488 cases, based on subsequent histologic diagnoses, clinical follow up, or both. Global results for all malignancies (lymphoid and non-lymphoid neoplasms) based on cases with final diagnoses, showed a sensitivity of 87.4% and a specificity of 98.7%. The overall diagnostic accuracy was 93,2%, and the false negative rate reduced from 12,6% to 7,3% when lymphomatous cases were excluded. The annual data for this period showed that the number of diagnostic lymph node biopsies and the rate of inadequately sampled material markedly decreased. Gene rearrangement studies for IgH and TCR ${\gamma}$ were helful in 30 cases. FNAC is a useful initial diagnostic procedure for the evaluation of palpable enlarged lymph nodes. However, the technique should be assisted by the appropriate ancillary studies and by proper interpretation by a cytopathologist.

Clinical evaluation of thoracoscopy (흉강경검사의 임상적 고찰)

  • During a 12-month period, 31 patients underwent diagnostic and therapeutic thoracoscopy for previously undiagnosed thoracic diseases. In all patient, the diagnosis had been unobtainable by the usual diagnostic modalities of thoracentesis, closed pleural biopsy, bronchoscopy, or mediastinoscopy. The patients ranged from 4 years to 84 years old. One procedure was performed for mediastinal mass, 8 for parenchymal lesions, 21 for pleural diseases, and 1 for diaphragmatic disease. A correct diagnosis was obtained by thoracoscopy in 31 procedures for a 90% overall accuracy rate. There was no clinically significant morbidity in this series and no procedure-related mortality. Thoracoscopy, performed under intercostal nerve block and regional anesthesia, has proved to be a very attractive method of the diagnosis of thoracic disease.

The Usefulness of FNAB for the Diagnosis of Thoracic Surgical Disease (흉부외과 질환의 진단시 FNAB 의 유용성과 객담세포검사 및 기관지 내시경 검사와의 비교 고찰)

  • From March 1986 to June 1990, the percutaneous Fine Needle Aspiration Biopsy[FNAB] of 102 thoracic lesions were performed with Westcott needle [slotted 20G or 22G thin needle], for the purpose of identifying and diagnosing thoracic lesions. There were 94 lung lesions [67 malignant tumors, 27 benign lesions] and 8 mediastinal and chest wall lesions. The results of FNAB were compared with sputum cytology and bronchoscopic examinations. The sputum cytologic examinations were performed in 54 cases of malignant lung tumor and the malignant cell was found at the 18 cases [33%]. We bronchoscopic examinations were performed in 24 cases and the malignant cell was found at the 12 cases [55%]. The positive diagnostic rate of malignancy was 100% by FNAB. Among them, 55 cases [82%] were diagnosed by cytologic examinations and 43 cases[64%] by both. The specific diagnoses for benign lung lesions in 15 cases[55%] and for mediastinal and chest well lesions in 5 cases[62%]. The 7 patients[6.9%] developed the pneumothorax and 5 of them required the treatment. Therefore, the FNAB of thoracic lesions may be a preferred diagnostic method because of its safety, simplicity and accuracy.

The Role of Capsule Endoscopy in the Diagnosis of Crohn's Disease

  • The examination of small bowel in Crohn's disease (CD) is very important. Capsule endoscopy (CE) has been recognized as a good tool for evaluation of small bowel. The capsule placement is achieved endoscopically for Children not to swallow capsule. CE is superior to any other modalities for examination of small-bowel. The large portion of pediatric patients with known CD were found with CE to have more extensive and newly diagnostic small-bowel disease. All of them had therapeutic changes. The most side effect of CE is capsule retention. The capsule retention rate in pediatric CD is about 7.3%. The patency capsule helps to predict the possibility of capsule retention. For the improving of the diagnostic accuracy, the experience of more than 20 readings of CE is needed.

Chest Wall Implantation of Lung Cancer after Percutaneous Fine Needle Aspiration - Report of one case - (경피적 폐생검술후 흉벽에 전이된 폐암;1례 보고)

  • Percutaneous needle aspiration has been widely used in the diagnosis of pulmonary lesions, because it is a fairly simple procedure with good diagnostic accuracy and low complication rate. Among its complications, the spread of malignant cells along the needle tract is rare but serious one. We report a case of chest wall implantation of lung cancer after the percutaneous fine needle aspiration biopsy. A 57-year-old man had undergone a right upper lobectomy for squamous cell carcinoma [T2N0M0] of the lung, 3 months after the operation, a growing mass, located far from the previous thoracotomy incision, developed on the right anterior chest wall where the diagnostic thin needle biopsy had been performed before the lobectomy. A wide excision of the chest wall mass was performed, and permanent histology showed squamous cell carcinoma as noted before.

The Accuracy of Hysterosalpingography for Evaluating Female Infertility (불임 검사시 자궁난관 조영술의 진단 정확도)

  • Objective: This study was performed to evaluate the accuracy of hysterosalpingography (HSG) for evaluating female infertility patients by comparison with hysteroscopic and laparoscopic examination. Methods and Material: Total 219 infertile patients were retrospectively analyzed between January 1, 2002 and December 31, 2003. Ninety seven patients (44.3%) were primary infertility, 122 patients (55.7%) were secondary infertility. We performed hysteroscopic and laparoscopic examination on next cycle when HSG revealed any abnormal finding, and 3~6 cycles later if HSG was normal. Results: The accuracy of HSG was 65.2% compared with hysteroscopic examination (sensitivity 88.4%, specificity 46.4%, false positive rate 53.6%, false negative rate 11.6%). The most common abnormal finding of hysteroscopy was uterine synechia (67.4%) followed by endometrial polyp, uterine anomaly (e.g. uterine septum), endometrial hyperplasia. Compared with laparoscopic examination, the accuracy of HSG was 76.9% (sensitivity 98.9%, specificity 70.6%, +LR 3.36, -LR 0.02). The positive predictive value of normal patent tube was excellent (99.6%) but that of proximal tubal blockage was only 46.7%. The unilateral tubal obstruction of HSG was poor accuracy (+LR 3.85 -LR 0.68) and 70% of those was patent by laparoscopic examination. Laparoscopic examination also revealed that 53% of patients had peritubal adhesion and 37% of patients has additional pelvic findings, especially endometriosis. Among the patients had normal HSG, 53.5% patients with normal ultrasonography was diagnosed endometriosis (25.6% of them had endometriosis stage I-II). Conclusion: Normal HSG shows a high negative predictive value. Nevertheless, the incidence of associated pelvic disease in the normal HSG group is high enough to warrant diagnostic laparoscopy if nonsurgical treatment is unsuccessful. Because HSG has poor accuracy in predicting distal tubal blockage and peritubal adhesion, and poor positive predictive value of proximal tubal blockage, laparoscopic examination could be considered in abnormal HSG group.

The Reduction of Question Items in Korea Sasang Constitutional Diagnostic Questionnaire (KS-15) (단축형 사상체질 진단설문지(KS-15)의 문항 축소 연구)

  • Objectives The purpose of this study is to reduce the question items in Korea Sasang Constitutional Diagnostic Questionnaire (KS-15) and to assess the validity. Methods The subjects of this study were 1,553 people who had information on body shape, personality, and symptoms among the data established at the Korean Medicine Data Center. Among them, 155 people were separated into a test group to assess the validity, and the remaining 1,398 were used as training groups for model development. The reduction question items were finally confirmed through discussion by two experts. The model development went through Multinomial Logistic Regression. Results & Conclusions The Percent Correctly Predicted (PCP) in the test group was 60.00% (58.49% in men and 60.78% in women). The PCP of each constitution was 72.6% in Taeeumin, 53.3% in Soeumin, and 50% in Soyangin. The accuracy rate of KS-10 seems to be reasonable considering the ease of use of reduced time required.

Correlation of Ultrasound and Scintigraphic Findings in Scrotal Disease (음낭 질환의 초음파 및 씬티그라피에 관한 방사선학적 연구)

  • Recently ultrasound and scintigraphic study are most useful modality for diagnosis of scrotal diseases. They are able to determine the nature, anatomic extent of the lesion, and possible to differential diagnosis of the scrotal lesion. The ultrasound and scintigraphic findings were analyses in 59 patients of confirmed scrotal disease at Seoul Red Cross Hospital from June 1986 to Aug. 1989. The results were as follows. 1) Most patients were below the 30 years old age (69.4%), and most prevalent age was $11\sim30$ Yrs. in trauma, $31\sim40$ Yrs. in inflammation, and $51\sim60$ Yrs. in mass lesion of scrotum. 2) The diagnostic accuracy of scrotal disease was 94.9% in ultrasound and 72% in scintigraphic study. The ultrasound study showed high accuracy in all kind of scrotat disease, but scintigraphy showed only high detection rate in inflammatory disease. 3) Scrotal ultrasound is first choice of study for the diagnosis of scrotal lesion, and useful determine the cystic or solid lesion and anatomic extent of disease. However it can't be differentiate the acute epididymitis with acute testicular torsion. 4) Scrotal scintigraphy is useful study for the diagnosis of acute painful scrotal disease, and showed specific findings in acute epididymitis and acute torsion for differential diagnosis. 5) In consideration of age, history and clinical findings of patient, appropriate study will be chosen firstly in ultrasound and scintigraphy, and combined all of findings of these studies will be helpful for the accurate diagnosis and adequate treatment of scrotal disease.
