요로 종양의 세포병리 (Cytopathology of Urinary Tract Neoplasms)

  • Urine cytology is the most useful technique for detecting either primary or recurrent neoplasms in the urinary tract. Although urine cytology is the traditional method of detecting these neoplasms, its diagnostic accuracy has been underevaluated because of low sensitivity. The cytologic interpretation of urinary samples is not an easy task, even with some expertise in this area, for many reasons. In low-grade urothelial carcinoma, no reliable or reproducible diagnostic cytologic criteria can be provided because of the lack of obvious cytologic features of malignancy, which is one of the main factors lowering its diagnostic accuracy. Many diagnostic markers have been developed recently to enhance its diagnostic yield, but the results have not been satisfactory. However, urine cytology plays a role in detecting high-grade urothelial carcinoma or its precursor lesions. It still shows higher specificity than any of the newly developed urine markers. Understanding the nature of urine samples and the nature of neoplasms of the urinary tract, recognizing their cytologic features fully, and using cytologic findings under appropriate conditions in conjunction with a detailed clinical history would make urine cytology a very valuable diagnostic tool.

경계물과 경계만들기로서 구제역 간이진단키트: 국가기술중심주의와 분권주의의 충돌 (The FMD Diagnostic Kit as a Boundary Object and Boundary Making: Conflicts and Negotiations Between the State-centered and Decentralized Sociotechnical Orders)

  • 이 글은 지난 2010년-2011년 사이에 한국사회를 충격으로 몰아넣었던 구제역의 확산과정에서 질병진단방식을 둘러싼 논쟁에 대해서 논의하게 될 것이다. 당시 구제역 방역과 질병진단 방식을 둘러싸고 다양한 사회집단들 사이에서 치열한 논쟁이 있었으며 그 중심에는 휴대용 항체/항원 간이진단키트가 논쟁의 대상이 되었다. 이 항체/항원 간이진단키트의 효용성으로 부터 사용권한까지 논쟁은 구제역 확산기간 동안 지속되었으며 특히 중앙정부를 중심으로 하는 방역당국과 지방정부와 시민단체들은 이 간이진단키트의 사용권과 병성감정권한을 둘러싸고 논쟁을 벌이게 된다. 이 논문은 항체/항원 간이진단키트를 둘러싼 의미부여방식과 기술적인 판단에 대한 각기 다른 해석방식을 설명하기 위해 "경계물(boundary objects)" 개념을 사용하여 설명할 것이다. 특히 경계물의 구성방식과 각기 다른 사회집단이 이 경계물을 포함하거나 배제하는 과정을 경계만들기(boundary making)로 해석하면서 경계물과 경계만들기 과정의 상호작용과 협상과정을 추적하게 될 것이다. 이를 통해 의학기술적 대상물인 간이진단키트가 경계물로 구성될 뿐 아니라 이를 둘러싼 사회집단의 경계만들기는 상호작용을 통해 구성되는 공구성의 과정이라는 것을 보이게 될 것이다.

Performance Evaluation of Biozentech Malaria Scanner in Plasmodium knowlesi and P. falciparum as a New Diagnostic Tool

  • The computer vision diagnostic approach currently generates several malaria diagnostic tools. It enhances the accessible and straightforward diagnostics that necessary for clinics and health centers in malaria-endemic areas. A new computer malaria diagnostics tool called the malaria scanner was used to investigate living malaria parasites with easy sample preparation, fast and user-friendly. The cultured Plasmodium parasites were used to confirm the sensitivity of this technique then compared to fluorescence-activated cell sorting (FACS) analysis and light microscopic examination. The measured percentage of parasitemia by the malaria scanner revealed higher precision than microscopy and was similar to FACS. The coefficients of variation of this technique were 1.2-6.7% for Plasmodium knowlesi and 0.3-4.8% for P. falciparum. It allowed determining parasitemia levels of 0.1% or higher, with coefficient of variation smaller than 10%. In terms of the precision range of parasitemia, both high and low ranges showed similar precision results. Pearson's correlation test was used to evaluate the correlation data coming from all methods. A strong correlation of measured parasitemia (r2=0.99, P<0.05) was observed between each method. The parasitemia analysis using this new diagnostic tool needs technical improvement, particularly in the differentiation of malaria species.

신경모세포종에서 $^{18}F-FDG$ PET의 임상 이용 (Clinical Application of $^{18}F-FDG$ PET in Neuroblastoma)

  • Neuroblastoma is the most common extracranial solid tumor in children. In diagnostic assessment of neuroblastoma, $^{18}F-FDG$ PET has been reported to have high diagnostic performance, especially, very high sensitivity in staging, restaging, and assessment of therapeutic efficacy. In comparison with conventional diagnostic imaging modalities including a, bone scan, and MIBG scan, $^{18}F-FDG$ PET showed better diagnostic performance. According to clinical research data hitherto, $^{18}F-FDG$ PET is expected to be an effective diagnostic tool in the management of neuroblastoma.

  • Fine needle aspiration cytology (FNAC) has become a highly preferred, minimally invasive diagnostic tool of choice in the diagnosis of a palpable breast mass owing to its sensitivity, specificity, cost-effectiveness, and expediency. Although breast needle biopsies have been widely employed recently due to the increased detection rate of non-palpable early lesions, the importance of the use of FNAC cannot be underestimated. It comprises part of the diagnostic triad for the breast along with a physical examination and mammography, which together contribute to an increasing diagnostic accuracy. The differential diagnosis of a benign and malignant lesion is of the utmost importance in the diagnosis of breast lesions, and therefore the understanding of the possible diagnostic pitfalls is of great importance.

  • In order to assure safety of both patient and operator, and to provide uniform quality radiographs, it is necessary to perform periodic calibration of diagnostic X-ray equipment. A basic parameter of diagnostic equipment's and its image sharpness is the size(and shape the energy distribution) of the focal spot as viewed along the central X-ray beam. This size determines the resolution possible with the equipment and also determines the heat characteristics of an anode. A fine focus tube gives high resolution but causes high local heating of target. In past, the pin-hole and star pattern image measurement for evaluation of resolution have been widely used, but it produced blurring and inaccuracy of image. So newly inverted Ug-meter has advantage in more convenient measurement method and less out-put bias than other image measurement. The authors intended to compare measured focal size between Ug-meter and focal spot test tool, changed state from setting to now of units.

진단검사의 특성 평가를 위한 Receiver Operating Characteristic (ROC) 곡선의 활용 (Application of Receiver Operating Characteristic (ROC) Curve for Evaluation of Diagnostic Test Performance)

  • In the field of clinical medicine, diagnostic accuracy studies refer to the degree of agreement between the index test and the reference standard for the discriminatory ability to identify a target disorder of interest in a patient. The receiver operating characteristic (ROC) curve offers a graphical display the trade-off between sensitivity and specificity at each cutoff for a diagnostic test and is useful in assigning the best cutoff for clinical use. In this end, the ROC curve analysis is a useful tool for estimating and comparing the accuracy of competing diagnostic tests. This paper reviews briefly the measures of diagnostic accuracy such as sensitivity, specificity, and area under the ROC curve (AUC) that is a summary measure for diagnostic accuracy across the spectrum of test results. In addition, the methods of creating an ROC curve in single diagnostic test with five-category discrete scale for disease classification from healthy individuals, meaningful interpretation of the AUC, and the applications of ROC methodology in clinical medicine to determine the optimal cutoff values have been discussed using a hypothetical example as an illustration.

방사선치료계획 정도관리 방법에 따른 신뢰도 평가 (Reliability estimation about quality assurance method of radiotherapy planning)

  • According as radiation therapy technique develops, standardization of radiation therapy has been complicated by the plan QA(Quality Assurance). However, plan QA tools are two type, OADT (opposite accumulation dose tool) and 3DADT (3 dimensional accumulation dose tool). OADT is not applied to evaluation of beam path. Therefore tolerance error of beam path will establish measurement value at OADT. Plan is six beam path, five irradiation field at each beam path. And beam path error is 0 degree, 0.2 degree, 0.4 degree, 0.6 degree, 0.6 degree, 0.8 degree. Plan QA accomplishes at OADT, 3DADT. The more path error increases, the more plan QA error increases. Tolerance error of OADT path is 0.357 using tolerance error of conventional plan QA. Henceforth plan QA using OADT will include beam path error. In addition, It will increase reliability through precise and various plan technique.
