  • Radiographs can help in the diagnosis and treatment planning, but the exposure to ionizing radiation may elevate the risk of developing cancer in a person's lifetime. The objective of this review is to briefly summarize 1) radiation risk, especially cancer risks associated with diagnostic imaging, 2) linear, non-threshold (LNT) hypothesis, 3) the risks of radiation exposure to a fetus, and 4) the campaign of Image Gently. The individual risk of radiation-related cancer from any single medical imaging procedure is extremely small and it is not likely to be cancer risk at doses lower than 100 mGy, but patients may be harmed by avoiding diagnostic imaging due to fear of radiation hazard. Dentists need to understand the radiation doses delivered by various radiographic techniques and the acceptable exposure thresholds to effectively advise the patient and to reduce the unnecessary radiation

  • Background and Objectives: The introduction of highly sensitive imaging techniques has made it possible to detect many non-palpable nodules, or“incidentaloma”in the thyroid. Discovery of these lesions raises concerns about their malignancy, but the optimal strategy for managing these lesions has not been clearly established. This study was designed to evaluate the usefulness of ultrasonographic exam with new diagnostic criteria and presume the value of mass screening for thyroid cancer. Materials and Methods : Mass screening for thyroid cancer were carried out in conjunction with mass screening for breast cancer. The subjects were 630 women aged 30 years or over. Thyroid glands were examined with 10 MHz transducer ultrasonography by one radiologist. Needle aspiration biopsy were performed when suspicious of malignancy under the new diagnostic criteria. Results and Conclusion : The new ultrasonographic criteria to diagnose thyroid cancer provided useful information and ensured more accurate evaluation. 7 cases of thyroid cancer were discovered and successfully managed. It should be further demonstrated that there is sufficient value of mass screening for thyroid cancer to perform it independently despite early cancer detection.

  • Extrapulmonary tuberculosis (EPTB) constitutes about 20% of all cases of tuberculosis (TB) in Korea. Diagnosing EPTB remains challenging because clinical samples obtained from relatively inaccessible sites may be paucibacillary, thus decreasing the sensitivity of diagnostic tests. Whenever practical, every effort should be made to obtain appropriate specimens for both mycobacteriologic and histopathologic examinations. The measurement of biochemical markers in TB-affected serosal fluids (adenosine deaminase or gamma interferon) and molecular biology techniques such as polymerase chain reaction may be useful adjuncts in the diagnosis of EPTB. Although the disease usually responds to standard anti-TB drug therapy, the ideal regimen and duration of treatment have not yet been established. A paradoxical response frequently occurs during anti-TB therapy. It should be distinguished from other causes of clinical deterioration. Surgery is required mainly to obtain valid diagnostic specimens and to manage complications. Because smear microscopy or culture is not available to monitor patients with EPTB, clinical monitoring is the usual way to assess the response to treatment.

  • Osteochondral lesions of the talus are isolated cartilage and/or bone lesions that are known cause of chronic ankle pain. They can occur as the result of a single acute ankle injury or from repetitive loading of the talus. Technical development in radiologic imaging and ankle arthroscopy have improved diagnostic capabilities for detecting osteochondral lesions. Characteristics which are important in assessing an osteochondral lesions include: the size, the type (chondral, subchondral, cystic), the stability, the displacement, the location, and the containment of lesion. Nonoperative treatment involving period of casting and non-weight-bearing is recommended for acute, non-displaced osteochondral lesions in select pediatric and adolescent patients. Operative treatment is recommended for unstable lesions or failed conservative management. Marrow stimulation techniques (abrasion chondroplasty, multiple drilling, microfracture), osteochondral autograft or allograft, autologous chondrocyte implantation, are frequently employed. The purpose of this article is to review the historical background, etiology, classification systems, diagnostic strategies, and to describe a systematic approach to management of osteochondral lesions of the talus.

  • This paper presents the novel fault diagnostic expert system based on dissolved gas analysis(DGA) techniques in power transformer. The uncertainty of key gas analysis, norm threshold, and gas ratio boundaries are managed by using a fuzzy set concept. The uncertainty of rules are handled by fuzzy measures. Trend analysis through the monthly increment of key gas and DGA analysis are combined by the Dempster-Shafer theory, and the state of transformer and confidence factor are yielded by using this combined analysis. To verify the effectiveness of the proposed diagnosis technique, the expert system has been tested by using KEPCO's transformer gas records.

  • Pleural adhesions are a major challenge in standard and nonintubated video-assisted thoracic surgery. The currently available imaging techniques help to assess the presence and extent of pleural adhesions, but do not provide information on tissue deformability, which is crucial for intraoperative management. In this report, we describe the utilization of real-time elastography mapping of pleural adhesions. This technique enabled us to detect areas with softer adhesions, and helped establish the surgical plan in a difficult case of a patient scheduled for nonintubated video-assisted thoracic surgery.

  • Although small bowel the mainly occupies the most part of the gastrointestinal tract, small intestine tumors are rare, insidious in clinical presentation, and frequently represent a diagnostic and management challenge. Small bowel tumors are generally classified as epithelial, mesenchymal, lymphoproliferative, or metastatic. Familial adenomatous polyposis and Peutz-Jeghers syndrome are the most common inherited intestinal polyposis syndromes. Until the advent of capsule endoscopy (CE) and device-assisted enteroscopy (DAE) coupled with the advances in radiology, physicians had limited diagnostic examination for small bowel examination. CE and new radiologic imaging techniques have made it easier to detect small bowel tumors. DAE allows more diagnosis and deeper reach in small intestine. CT enteroclysis/CT enterography (CTE) provides information about adjacent organs as well as pictures of the intestinal lumen side. Compared to CTE, Magnetic resonance enteroclysis/enterography provides the advantage of soft tissue contrast and multiplane imaging without radiation exposure. Treatment and prognosis are tailored to each histological subtype of tumors.

  • A USW based diagnostic procedure is presented for estimating the depth of surface-breaking cracks. The diagnosis is demonstrated on seven lab-scale SFRC beam specimens, which are subjected to the CMOD controlled three-point bending test to create real bending cracks. Then, the recorded multiple ultrasonic signals are examined with the signal processing techniques, including wavelet transform and two-dimensional Fourier transform, to investigate the relationships between the crack depth and two diagnostic indices, namely the attenuation coefficient and dispersion index (DI). Finally, the reliabilities of these indices for depth estimation are verified with the visually measured crack depths as well as the crack features obtained with a digital image processing algorithm. It is found that the DI outperforms the attenuation coefficient in depth estimation, where this index displays good agreement with the visual inspection for 86% of the inspected specimens.

  • One of the most prevalent disease among women that leads to death is breast cancer. It can be diagnosed by classifying tumors. There are two different types of tumors i.e: malignant and benign tumors. Physicians need a reliable diagnosis procedure to distinguish between these tumors. However, generally it is very difficult to distinguish tumors even by the experts. Thus, automation of diagnostic system is needed for diagnosing tumors. This paper attempts to improve the accuracy of breast cancer detection by utilizing deep learning convolutional neural network (CNN). Experiments are conducted using Wisconsin Diagnostic Breast Cancer (WDBC) dataset. Compared to existing techniques, the used of CNN shows a better result and achieves 99.66%% in term of accuracy.

  • Glossopharyngeal neuralgia (GPN) is a rare disease that must be differentiated from trigeminal neuralgia. The purpose of this article is to provide a comprehensive review of anatomy, pathophysiology, diagnostic criteria, and several options of treatment for GPN. Lessons learned through our experience of treating GPN are presented in detail, as well as cases of misdiagnosis and diagnostic pitfalls. Microvascular decompression (MVD) should be primarily considered for medically intractable GPN. Techniques employed in MVD for GPN are categorized and described. Especially, we underscore the advantages of the 'transposition' technique where insulating material is positioned 'off' the root entry zone (REZ), instead of 'on' it. We believe this 'off-the-REZ' technique can fundamentally prevent recurrence, if applicable. In addition, Gamma Knife radiosurgery can be an alternative option when a patient is ineligible for MVD, though it is categorized as a destructive procedure.
