The Clinical Study of Tongue Change in Diabetes Mellitus Patients as Oriental Diagnostic Method (糖尿환자의 27例의 舌診에 關한 臨床 硏究)

  • Tongue examination is the most unique and important diagnostic method of oriental medicine. It reveals patients condition and give some information about direction of therapy and background of disease as well as produces clue of duration of disease changing that grasps it from outside. The aims of this study show that results of tongue examination is related to somewhat special pattern. we study 27 patients(age of means: 63.01 years old, male: 14, female: 13) who come to Wonkwang University Oriental-Medicine Hospital at Chonju with diabetes mellitus or diabetes mellitus complications. Tongue colors of this study result in pale-red 8 cases, pale 6 cases, red 6 cases, crimson 5 cases, blue-green-purple 2 cases and tongue fur of this study result in white-fur 16 cases, yellow-fur 7 cases, black-fur 1 case, none-fur 3 cases. And the condition of tongue fur with grimy and thin result in thin-white-fur 12 cases, white-grimy-fur 4 cases, thin-yellow-fur 5 cases, grimy-yellow-fur 2 cases, black-fur 1 case. Means level of fasting glucose during 7 days was $223.24{\pm}32.l7mg/dl$ and postprandial 2hours was $286.37{\pm}24.54mg/dl$ There were no changes in tongue body and tongue color but changes occur in tongue fur with 9cases in this period. 5 cases of patient make a difference between FBS(fasting blood glucose) and postprandial blood glucose level more than l00mg/dl (2 cases of patient with thin-white-fur gradually turned to slight-yellow-fur. There were no changes in 2 cases of patient with crimson-none-fur and pale-white-fur. 1 case of patient with slimy-yellow-fur turned to black fur.) 11 cases of patient had lesser than $10\%$ hemoglobin $A_lC$ and tongue color of these patient were pale-red 5cases, pale 2 cases, red 2 cases, crimson 2 cases. 16 cases of patient had more than $10\%$ Hemoglobin $A_1C$ and tongue color of these patients were pale-red 3 cases, pale 4 cases, red 4 cases, crimson 3 cases, blue 2 cases. This result shows that quantity of Hemoglobin $A_1C$ make a somewhat role in tongue color. The above results show that tongue color, fur color, condition and change of fur in diabetes mellitus patients is various in pale-red, pale, crimson, none-fur. So it is difficult to give an exact diagnosis on pathology of diabetes mellitus only with tongue examination because there are a little matches between blood glucose level, prevalence-period, short-term blood glucose regulation and tongue and fur colors.

Psychogenic Dizziness for Psychiatrists in Korea (정신건강의학과 의사를 위한 심인성 어지럼)

  • Many patients with dizziness present with a symptom pattern that does not reveal the cause by neurotologic diagnostic approaches. In such cases, the physician frequently diagnoses psychogenic dizziness. Psychogenic dizziness is not characterized by true vertigo, and occurs in combination with other psychiatric symptom cluster. One out of two to four patients with dizziness are psychogenic dizziness. But there are few concern about this including clinical practice and study in Korea. I wrote this paper to increase concerning and attending to this for psychiatrists in Korea. I reviewed etiology including biological and psychological relations between dizziness and psychiatric disorder(especially anxiety), diagnostic approaches of, characteristics of dizziness of various psychiatric disorders related to, and the treatment of psychogenic dizziness. I also briefly reviewed the central and peripheral dizziness for psychiatrists. I suggest psychiatrists and clinicians in the psychosomatic field in Korea to acknowledge, concern, and attend to psychogenic dizziness. In turn, it will be helpful to well treat the patients with psychogenic dizziness.

MRI Predictors of Malignant Transformation in Patients with Inverted Papilloma: A Decision Tree Analysis Using Conventional Imaging Features and Histogram Analysis of Apparent Diffusion Coefficients

  • Objective: Preoperative differentiation between inverted papilloma (IP) and its malignant transformation to squamous cell carcinoma (IP-SCC) is critical for patient management. We aimed to determine the diagnostic accuracy of conventional imaging features and histogram parameters obtained from whole tumor apparent diffusion coefficient (ADC) values to predict IP-SCC in patients with IP, using decision tree analysis. Materials and Methods: In this retrospective study, we analyzed data generated from the records of 180 consecutive patients with histopathologically diagnosed IP or IP-SCC who underwent head and neck magnetic resonance imaging, including diffusion-weighted imaging and 62 patients were included in the study. To obtain whole tumor ADC values, the region of interest was placed to cover the entire volume of the tumor. Classification and regression tree analyses were performed to determine the most significant predictors of IP-SCC among multiple covariates. The final tree was selected by cross-validation pruning based on minimal error. Results: Of 62 patients with IP, 21 (34%) had IP-SCC. The decision tree analysis revealed that the loss of convoluted cerebriform pattern and the 20th percentile cutoff of ADC were the most significant predictors of IP-SCC. With these decision trees, the sensitivity, specificity, accuracy, and C-statistics were 86% (18 out of 21; 95% confidence interval [CI], 65-95%), 100% (41 out of 41; 95% CI, 91-100%), 95% (59 out of 61; 95% CI, 87-98%), and 0.966 (95% CI, 0.912-1.000), respectively. Conclusion: Decision tree analysis using conventional imaging features and histogram analysis of whole volume ADC could predict IP-SCC in patients with IP with high diagnostic accuracy.

A Study of In-sole Plantar Pressure Distribution in Functional Tennis Shoes (기능성 전문테니스화의 족저압력분포 분석)

  • The aim of this study is to evaluate tennis shoes's plantar pressure distribution in tennis prayers and to determine the influence of the shoe on various tennis movements. When investigating the biomechanics of movement in tennis, one of the first things to do is to understand the movement patterns of the sport, specifically how these patterns relate to different tennis shoes. Once these patterns are understood, footwear company can design tennis shoes that match the individual needs of tennis players. Plantar pressure measurement is widely employed to study foot function, the mechanical pathogenesis for foot disease and as a diagnostic and outcome measurement tool for many performance. Measurements were taken of plantar pressure distribution across the foot and using F-Scan(Tekscan Inc.) systems respectively. The F-Scan system for dynamic in-shoe foot pressure measurements has enabled us to assess quantitatively the efficacy of different types of footwear in reducing foot pressures. The Tekscan F-Scan system consists of a flexible, 0.18mm thick sole-shape having 1260 pressure sensors, the sensor insole was trimmed to fit the subjects' right, left shoes. For this study 4 university male, high level tennis players were instructed to hit alternated forehand stroke, backhand stroke, forehand volley, backhand volley, smash, service movement in 4 different tennis shoes. 1. When impact in tennis movement, peak pressure distribution of landing foot displayed D>C>B>A, A displayed the best low pressure distribution. A style's tennis shoes will suggest prayer with high impact. If prayer with high impact feeling during pray in tennis wear A style, it will decrease injury, will have performance improvement. 2. When impact in tennis movement, plantar pattern of pressure distribution in landing foot displayed B>A>C>D in stability performance. During tennis, prayer want to stability movement suggest B style tennis shoes when tennis movement impact keep stability of human body. B style tennis shoes give performance improvement 3. When impact in tennis movement, plantar pattern of center of force(C.O.F.)trajectory in landing foot analyzed this : 1) When stroke movement and volley movement in tennis, prayer better to rearfoot movement. 2) when service movement, prayer midfoot strike movement. 3) when smash movement, prayer have forefoot strike movement.

Epidemiological Pattern of Breast Cancer in Iranian Women: Is there an Ethnic Disparity?

  • Introduction: Northeastern Iran is known as a high risk area of upper gastrointestinal cancers. Recent reports have suggested a declining trend for these cancers as well as an increase in the incidence of other malignancies including breast cancer. Our present aim was to describe the epidemiological pattern of breast cancer in this region during 2004-2009. Methods: All new cancer cases from public and private diagnostic and therapeutic centers of Golestan province were registered. A structured questionnaire was prepared and used based on the standerds of the International Association of Cancer Registries. The international classification of diseases for oncology was considered for coding. Age standardized incidence rates (ASR) of breast cancer were calculated. Results: A total of 11,038 new cancer cases were registered during 2004-2009, of which, 1,101 (10%) were females with breast cancer. The median age of the breast cancer patients was 46 years. The ASR for breast cancer was 28 per 100,000 person-years. We found an unusual rapid increase in breast cancer rate at the age of 25 years. The ASR of breast cancer was significantly lower in females from Turkmen ethnicity and those from rural areas(P value <0.01). Conclusion: Our study showed high rate of breast cancer in Golestan province of Iran. We found an unusual peak of breast cancer in young women. So, the age of starting screening programs may need to be revised in this area. The rate of breast cancer was significantly lower in women from Turkmen ethnicity. Further studies are warranted to clarify the role of important determinants, especially regarding the ethnic disparity, on breast cancer in this region.

Evaluation of nasolabial angle in adult patients with skeletal Class III malocclusion (성인 골격성 III급 부정교합 환자의 올바른 비순각 평가에 관한 연구)

  • The purposes of this study were to evaluate the nasolabial angle changes between closed lip position at centric occlusion and relaxed lip position at which the bite is open so that the lips do not touch and to elucidate the significance of the relaxed lip position for dentofacial diagnosis. Methods: The subjects consisted of 60 (35 Males, 25 Females) skeletal Class III malocclusion adult patients (mean age 23.3 years) with anterior crossbite. Results: In Class III malocclusion adult patients, there were significant differences in the nasolabial angle changes between closed lip position and relaxed lip position. Using the cluster analysis, the subjects were divided into three groups according to the pattern of nasolabial angle change: Group 1 (N = 27, 45%, $-8\;{\sim}\;1$), Group 2 (N = 30, 50%, $2^{\circ}\;{\sim}\;17$), and Group 3 (N = 3, 5%, over 18). Conclusion: The results showed that the pattern of the nasolabial angle change between closed lip position and relaxed lip position varies in skeletal Class III malocclusion patients. Thus, relaxed lip position should be taken into account when diagnostic records are obtained and analyzed to accurately to evaluate the facial soft tissues and predict facial esthetics after surgical-orthodontic treatment.

Prevalence and Expression Pattern of Cytokines in Porcine Respiratory Disease Complex (PRDC) (돼지호흡기복합증후군(Porcine respiratory disease complex, PRDC)에 대한 발생상황의 분석 및 cytokine의 변화)

  • Porcine respiratory disease complex (PRDC) is a common respiratory disease in nursery and grow-finishing pigs. A complex of viral and bacterial agents is known to be involved in the etiology of PRDC. The purpose of this study was to investigate common etiologic agents associated with PRDC in the field and compare detection methods for identifying these agents. To understand the mechanism of polymicrobial infection in PRDC, changes in the expression of cytokines were investigated. In 461 pig samples examined, most of the affected pigs ranged from 3 to 10 weeks old (73.4%), and 348 (75.4%) samples were confirmed as polymicrobial infection. Of the polymicrobial-infected cases, two (50.3%), three (32.2%), four (13.8%), five (3.2%), and six (0.5%) agents were detected. Two- or three-agent infections were the most common, with PRRSV/PCV-2 (44.6%) the most common two-agent infection. PRRSV/PCV-2/H. parasuis (11.0%) was the most common three-agent infection. Comparison of two detection methods (PCR and IHC) in the polymicrobial cases showed that 78.4% were PCV-2 positive with the PCR method, and 26.2% were PCV-2 positive with IHC. SIV was 7.8% by the PCR method and 3.7% positive by the IHC. This result indicates that the PCR method is more useful than IHC for detecting causative agents in PRDC. In the analysis of cytokines in the two- and three-agent infected samples, interleukin (IL)-$1{\alpha}$, IL-2, IL-4, IL-6, IL-10, and INF-${\alpha}$ showed the same expression pattern. All cytokines were suppressed, except IL-6. These findings indicate that changes in cytokine expression could be used to understand the mechanism of polymicrobial infection in PRDC.

Chracteristics of Primary Health Practice and Diagnosis-Cluster Pattern in Health Insurance (의원의 특성에 따른 상병진단군의 분포에 대한 연구)

  • This study is designed to find out some intra-clinic factors affecting the content of practice provided by primary care physicians in Korea, and proposed factors in this study are characteristcs of each private clinc --- physician-related variables(age, sex, specialty), bfed-related variables for inpatient care, laboratory-related variables for precise diagnosis. We have tried to estimate the difference of disease entities cared by each primary care physician according to above factors by analyzin gdisease data claimed during one month(April, 1992) to National Federation of Medical Insurance. The diagnosis codes by ICD-9 in the research disease data were reclassified to 'diagnosis clusters' by virtue of clinical similarities for effective analyses. We have converted frequent-tsing ICD-9 codes to 86 diagnosis clusters, which incorporated 97.4 percents of all ambulatory visits to private clinics. This result means proposed diagnosis-cluster method is effective tool for analysis of the content of ambulatory medical care carried out by primary care physicians. Comparisons and analyses of multiple diagnosis-clusters made on the basis of presented factors were done and the results were as follows; - Major factors affecting the difference between diagnosis-cluster pattern by each variables were phyusician's age, sex, specialty and bed counts of each private clinic for inpatient care and the size of laboratories of each clinic. - Middle aged(30th to 40th) group physicians are providing more comprehensive care than 20th or above 50th aged groups. Male physicians are more adequate for comprehensive care than female physicians, because woman-doctors are providing narrow-spectrum care. The content of practice of obstetricians and gynecologists shows much difference from primary medical practice, and they cannot be included in primary care physician, this study suggested. Pediatricians are also providing short-spectum acre, and nearly all visits to pediatricians were incorporated only 2-3 diagnosis-clusters. General surgeons' practices are very similar to general practioners' or family physicians' practices, the means they are providing primary care rather than special surgical care. And small number of beds(under 5 beds) and only basic(2-3 sorts of)diagnostic apparatuses are sufficient for primary physicians' clinic to carry out primary care. In conclusion, to reinforce primary care department in Korea, there must be support with health policy to expand office-based primary care practice-- with small number of beds for inpatient care and only basic laboratories-- provided by general practitioner of family physician.

A Research on The Pulse & Disease-patterns and Diagnostic Theory of Exogenous Febrile Disease in the "Sanghanjeonsaengjip(傷寒全生集)" ("상한전생집(傷寒全生集).변상한발열례(辨傷寒發熱例)" 등에 대한 연구(硏究))

  • "Sanghanjeonsaengjip(傷寒全生集)" is a classic medical work, written by the outstanding physician Dohwa(陶華) in the Ming Dynasty. The characteristic of "SangHanJeonSaengJip" is that this book succeeded to the spirit of pattern identification and treatment of Treatise on Cold Damage Diseases, newly changed a table of contents by symptoms, and together with this indicated the prescriptions in accordance with diswase-pattern at "YujeunghwalInseo(類證活人書)", "Hwajegookbang(和劑局方)" etc. Also because this kept the existing ephedra decoction, cinnamom twig decoction, minor decoction of bupleurum, decoction for reinforcing middle-energizer and replenishing qi etc.'s name on and unlikely indicated the medicine composition, it caused confusion, but at the later ages "Euhakipmun(醫學入門)" the so-called 'Doci(陶氏)' was added to the prescription name, so we are able to distinguish. Together with this, this book dose not indicate the dosage of medicine and indicates the first, the second, and the third classes[上中下] below medicine. As this dose not mean the three grades of quality"good, fair, and poor[上中下] of "Shennong's Classic of Materia Medica" but expresses the sovereign medicinal as the first class[上], minister medicinal as the second class[中] and assistant and courier medicinal as the third class[下], doctors can voluntarily decide the dosage of medicine in accordance with the degree of disease. At this thesis, I single out ten chapters in contents of 2nd volume named Hyeong(亨) corresponding to the details, among "Sanghanjeonsaengjip(傷寒全生集)". I discussed superficial fever types of exogenous febrile disease in chapter 1, aversion to cold types of exogenous febrile disease in chapter 2, syndrome caused un-sufficient sweating in chapter 3, organic fever types of exogenous febrile disease in chapter 4, aversion to wind types of exogenous febrile disease in chapter 5, Tidal fever types of exogenous febrile disease in chapter 6, Alternative attacts of chills and fever in chapter 7, Dysphoria with smothery sensation in chapter 8, Fidgetiness of exogenous febrile disease in chapter 9, and Headache of exogenous febrile disease in chapter 10, and together with this I discussed, in detail, which influence the prescriptions which are listed on each chapter have caused on future generations In accordance with this, I think that the above-mentioned symptoms and prescriptions are important when I research cold damage and warm disease study. So I orderly research revision, annotation, rendering and an investigation.

Improved Transmission Path Visualization of Vibration Power Flow for Stiffened Plate Using Streamlines Representation (유선 표현법을 이용한 보강판의 진동파워흐름에 대한 개선된 전달경로 가시화)

  • Vibration intensity has been used to localize vibration source of a vibrating system. Not only that, vibration intensity has also been used for structural diagnostic in identifying crack and mounted stiffeners. To clearly identify the location of vibration source and understand the changes of energy transmission path, clear flow visualization is required. Most of previous works used vectors to indicate the magnitude and direction of emerging vibration energy and transmission paths. However, due to the large surface area of a plate like-structure, clear transmission paths cannot be achieved using vector visualization. This becomes an issue when detail vector flow at all locations of the whole plate surface is required. In this study, streamlines visualization is used to clearly indicate the power flow transmission path at all plate surface. By using streamlines representation, not only clear transmission paths are obtained, but also improves the vector visualization which helps us to understand the changes of the energy flow especially for stiffened plates. In this study, vibration intensity computation is firstly compared to previous work to validate the vibration intensity computation. To clearly show the power flow transmission paths, streamlines representation is shown. This representation overcomes the unclear vector direction especially for stiffened plates. Different pattern of energy transmission path can be observed using streamlines representation for stiffened and unstiffened plate. The complex streamlines pattern can also be observed at high resonance frequencies which is unclear by using vector representation.
