  • Purpose: The aim of this study is to analyze the series of panoramic radiograph of implant patients using the system to measure peri-implant crestal bone loss according to the elapsed time from fixture installation time to more than three years. Methods: Choose 10 patients having 45 implant fixtures installed, which have series of panoramic radiograph in the period to be analyzed by the system. Then, calculated the crestal bone depth and statistics and selected the implant in concerned by clicking the implant of image shown on the monitor by the implemented pattern recognition system. Then, the system recognized the x, y coordination of the implant and peri-implant alveolar crest, and calculated the distance between the approximated line of implant fixture and alveolar crest. By applying pattern recognition to periodic panoramic radiographs, we attained the results and made a comparison with the results of preceded articles concerning peri-implant marginal bone loss. Analyzing peri-implant crestal bone loss in a regression analysis periodic filmed panoramic radiograph, logarithmic approximation had highest $R^2$ value, and the equation is as shown below. $y=0.245Logx{\pm}0.42$, $R^2=0.53$, unit: month (x), mm (y) Results: Panoramic radiograph is a more wide-scoped view compared with the periapical radiograph in the same resolution. Therefore, there was not enough information in the radiograph in local area. Anterior portion of many radiographs was out of the focal trough and blurred precluding the accurate recognition by the system, and many implants were overlapped with the adjacent structures, in which the alveolar crest was impossible to find. Conclusion: Considering the earlier objective and error, we expect better results from an analysis of periapical radiograph than panoramic radiograph. Implementing additional function, we expect high extensibility of pattern recognition system as a diagnostic tool to evaluate implant-bone integration, calculate length from fixture to inferior alveolar nerve, and from fixture to base of the maxillary sinus.

  • Objectives: This study aimed to analyze the correlation between Ryodoraku diagnosis and three pattern identification questionnaires, namely, Spleen Qi Deficiency, Stomach Qi Deficiency, and Food Retention in functional dyspepsia (FD) patients. Methods: Forty FD patients who met the Rome IV diagnostic criteria for FD participated in this study. The Ryodoraku test was conducted, and three pattern identification questionnaires were filled up by all patients. The average Ryodoraku score was called total average (TA), and the scores on the Ryodoraku points were measured. The degree of Spleen Qi Deficiency, Stomach Qi Deficiency, and Food Retention was assessed by Spleen Qi Deficiency questionnaire (SQDQ), Scale for Stomach Qi Deficiency pattern (SSQD), and Food Retention Questionnaire (FRQ). Results: The TA scores in the SQDQ patient group were lower than those in the SQDQ control group (p=0.091). The TA scores in the SSQD patient group were higher than those in the SSQD control group (p=0.651). The TA scores in the FRQ patient group were lower than those in the FRQ control group (p=0.851). The scores on the LH5, RH5, and RH6 points were significantly lower in the SQDQ patient group than in the SQDQ control group. However, no significant difference was found in the Ryodoraku scores among the other groups. Conclusions: The results suggest that the low TA and low Ryodoraku scores on the LH5, RH5, and RH6 points could be a quantitative indicator to diagnose Spleen Qi Deficiency in FD patients in a simpler manner. Larger studies on the Ryodoraku test in FD patients, the health control group, and other pattern identification groups are required.

  • Objectives: This study aimed to comprehend the characteristics of Korean medicine patterns in relation to varying degrees of cognitive impairment in an elderly population. Methods: The dataset included 127 elderly individuals with cognitive impairment obtained from three Korean medicine hospitals between 2018 and 2021. The participants were categorized into two groups based on Clinical Dementia Rating-Sum of Boxes (CDR-SB) scores: those with questionable impairment (QI) and those with very mild dementia (VMD). A diagnostic framework for Korean medicine patterns encompassing Qi deficiency, Yin deficiency, Phlegm dampness, and Heat-fire was employed. Liver blood markers, including aspartate aminotransferase (AST), alanine aminotransferase (ALT), and the AST/ALT ratio, were also analyzed. Results: The scores of the cognitive assessment tools (MoCA-K, MMSE-DS, and K-IADL) of the QI group significantly differed from those of the VMD group. CDR-SB exhibited a positive correlation with the scores of each pattern of Qi deficiency, Yin deficiency, Phlegm dampness, and Heat-fire, whereas ALT and AST values displayed negative correlations. Binomial logistic analysis, controlling for potential confounders, such as age, education years, body mass index, the presence of chronic disease, and the presence of medication, verified that the VMD group showed higher pattern scores and lower ALT and AST values than the QI group. Conclusions: Increases in pattern scores along with decreased AST and ALT levels might be indicative of more severe cognitive impairment.

  • Objectives: Hyungsang medicine (HM) is an inspection which emphasizes the relationships between pathological patterns and one's facial or bodily shape, color and movement. The purpose of this study was to review previously published study results of the HM. Methods: The authors reviewed a total of 51 HM-related studies published from 2000 to 2012 found on PubMed and various domestic Oriental medicine journals. Each study was classified into theoretical, diagnostic, or treatment-related category. Results: Theoretical studies were mostly based on the Nei-Ching and Dong-Eui-Bo-Gam texts, and dealt with differences in form between types such as masculine and feminine, the five organ image types, the four-type categorization of body essence, vital energy (Qi), mentality, and Blood, Gallbladder and Bladder, the four-type categorization of fish, bird, horse, and turtle types, and the six meridian types. Research on diagnosis has been performed on diagnostic characteristics of HM, correlation between HM and general coordinative manipulation (GCM), and Hyungsang medicine and ante-disease pattern (未病類型). Studies on treatments could largely be classified as treatments for specific diseases using certain acupuncture or herbal prescriptions based on HM. Treatments were mostly evaluated solely through subjective symptom improvement of patients. Conclusions: Our review results suggest that HM-related studies were focused on the constitutional characteristics and clinical utility of HM. To strengthen the theoretical basis of HM and its clinical utility, clinical trials including randomized, treatment-placebo and blind methods are needed.

  • We have experienced a case of sarcoidosis appearing asymmetrical diffuse interstitial lung lesions with multiple lymphadenopathy. This patient was a 57 year-old female who had been in good health until 2 months ago. At that time she noted the onset of an exertional dyspnea and weakness. Pulmonary function tests showed moderate obstructive pattern with mild decreased DLco. In the chest CT, multiple lymphadenopathy with small nodular lesions are scattered, and the impression was a metastatic lymph nodes with lymphangitic carcinomatosis. In bronchofiberscopy, we noted luminal narrowing by extrinsic compression in the right middle and lower lobe bronchi. And microscopic examination of by bronchofiberscopic biopsy showed chronic inflammation. Thus we performed subcarinal and tight supraclaviclar lymph nodes aspiration biopsy cytology, and that revealed class 0 and class 1, respectively. Finally, we performed an excisional biopsy for the right scalene lymph node, which revealed the specimen as a noncaseating granuloma. The angiotensin convecting enzyme level was overt two folds compared to normal value. And the patient had negative PPD skin test and hyperglobulinemia. After 18 weeks treatment with prednisone, the signs and symptoms which the patient clad suffered from, disappeared.

  • To determine the pattern of disease in patients presenting with breast lumps and to determine the sensitivity and specificity of fine needle aspiration cytology of benign and malignant diseases as a diagnostic tool by correlating with histopathology findings. This retrospective study was carried out in the Department of Pathology, Maharaja Agrasen Medical College, Agroha, from Jan 2008 to April 2012. Fine needle aspiration cytology was performed on 370 cases and out of these 52 cases were received in the Department for histopathological examination. Fibroadenoma was the most common disease encountered, in 88 (24%), with a peak incidence in second and third decade of life. Fibrocystic disease was second, being common in the third and fourth decades of life. Peak incidences of duct ectasia, granulomatous and tubercular mastitis were seen in the third decade. Gynaecomastia showed two peak incidences in second and sixth decades of life. Out of total 370 cases undergoing fine needle aspiration, benign cases were 316 (85.4%), malignant and suspicious were 54 (14.6%) and 10 (2.70%) respectively. Out of total 22 histological confirmed malignant lesions 19 were interpreted as malignant cytologically while two as suspicious and one as benign. All thirty histologically confirmed benign cases were diagnosed as benign cytologically. The sensitivity, specificity, positive and negative predictive values were 98%, 100%, 100% and 96.4% respectively. FNA cytology is highly accurate for diagnosis of breast masses. However, the clinician should correlate FNA cytological results with physical examination and imaging findings to prevent false negative and false positive events and to obtain optimal management of their patients.

  • The re-examination of morphological and anatomical characters of Laminaria appressirhiza and L. inclinatorhiza collected from different localities in the Sea of Okhotsk was performed. Despite their commercial and ecological importance to the region they have not been comprehensively reviewed since their first description in 1970. Our results show that some original diagnostic key characters (e.g., shape of holdfast, shape of sporangial sori, and dissection of blade) are not stable and have deviations from the type morphology when plants grow in different environments. In L. inclinatorhiza, the sporangial sori development occurred differently to the pattern indicated in original species description as they did not develop simultaneously on both sides of the blade. Instead, the sporangial sori outlines on both sides of the blade did not coincide at first and only became coincident later. Also, a deep-water population of L. inclinatorhiza with an unusual and interesting morphology, growing at depths of 15-25 m on opened rocky coasts in Taujskaya Bay (northern part of the Sea of Okhotsk) was found. The stable diagnostic key characters to distinguish these two species are the cone-like, multilayered, very thick and massive holdfast (in L. inclinatorhiza) and rolled margins of blades, lamellar rosette-like part of thallus, and sporangial sori developing only on one side of the blade (in L. appressirhiza). The ecological characteristics, distribution, and abundance of both species in the Sea of Okhotsk are discussed. Both species are perennial and widely distributed in the region. L. appressirhiza is more often found as a subdominant species among other kelps, forming maximum biomass and density of 7-9 kg and 8-25 plants per $1m^2$, respectively. L. inclinatorhiza sometimes forms local mono-species communities with maximum biomass and density of 10-12 kg and 10-15 plants per $1m^2$, respectively.

  • The purpose of this study was to develop generalized validity evaluation indexes for the Sasang type diagnostic test. As a generalized evaluation index for Sasang type diagnostic test, Generalized Squared Correlation (GC2), Mutual Information (I) and Mutual Information Coefficient (IC) as overall validity index, and Correlation Coefficient (C) and Mutual Information Contribution (ID) as type-specific validity index were suggested and the practicability of these indexes was assessed along with Percentage Correctly Predicted (PCP), adjusted PCP, type-specific sensitivity and type-specific specificity which was proposed previously. The usefulness of the nine validity indexes was examined using previously reported data and three mock Sasang type diagnosis results. We demonstrated here that the PCP is not adequate for the analysis of validity. We also showed that our validity index is recommendable for the comparison and improvement of Sasang type diagnosis. The generalized validity analysis of Sasang type diagnosis can provide a guideline for the development of new Sasang type diagnosis or pattern identification. The working sheet for calculating validity indexes can be acquired at http://www.sasangtypology.org/validity.

  • The genus Actinidia has been recorded as having total six taxa, composed of four species and two varieties, in Korea. However, its taxonomy remains complex because of disagreement on taxonomic characters used for species boundaries. Based upon morphological, anatomical, and palynological characters, we re-evaluated species delimitations in the genus Actinidia. Critical observation of wild populations and extensive herbarium materials including types were also carried out. It is generally considered that leaf thickness and shape, hair type and color were important diagnostic characters to identify species, but these characters were insufficient to be diagnostic characters because of their wide variation ranges. On the other hands, pith pattern, winter bud protrusion rate, scale leaf number, anther color, fruit shape, and maturity season were taxonomic significance. A. arguta var. platyphylla and A. arguta var. rufinervis have been identified by hair type and color, but these characters showed continuous variation in A. arguta. Therefore, two varieties are treated as synonyms of A. arguta. Consequently four species are recognized in Korea: A. polygama, A. kolomikta, A. rufa, and A. arguta. A key is provided, as well as complete descriptions of the species examined, including information on nomenclatural types, and synonymies.

  • Miliary tuberculosis is quite a rare but serious cause of acute respiratory distress syndrome (ARDS). Therefore, the early detection of military tuberculosis as the underlying cause of ARDS is very important for the prognosis and survival of the patient. We report a case of military tuberculosis mimicking ARDS. A female patient was admitted due to repeated fever and dyspnea. The initial chest CT scan showed diffuse ground glass opacity, without a miliary pattern. The case was considered to be ARDS caused by pneumonia. She showed improvement after being treated with levofloxacin. However, she was re-admitted with fever seven days after discharge. The follow up chest CT scan showed micronodules in both lungs. An open lung biopsy confirmed the diagnosis of military tuberculosis.
