Clinical, Biochemical, and Genetic Characterization of Glycogen Storage Type IX in a Child with Asymptomatic Hepatomegaly

  • Glycogen storage disease type IX (GSD IX) is caused by a defect in phosphorylase b kinase (PhK) that results from mutations in the PHKA2, PHKB, and PHKG2 genes. Patients usually manifest recurrent ketotic hypoglycemia with growth delay, but some may present simple hepatomegaly. Although GSD IX is one of the most common causes of GSDs, its biochemical and genetic diagnosis has been problematic due to its rarity, phenotypic overlap with other types of GSDs, and genetic heterogeneities. In our report, a 22-month-old boy with GSD IX is described. No other manifestations were evident except for hepatomegaly. His growth and development also have been proceeding normally. Diagnosed was made by histologic examination, an enzyme assay, and genetic testing with known c.3210_3212del (p.Arg1070del) mutation in PHKA2 gene.

Prospective Observational Study of Acupuncture Adverse Events: 50 Patients Hospitalized for Peripheral Facial Palsy Treatments (침치료 이상반응에 대한 전향적 관찰: 말초성 안면신경마비 입원 환자 50명을 대상으로)

  • Objectives : The aim of this study is to survey adverse events of acupuncture in the course of peripheral facial palsy treatments. Methods : The prospective observational study included patients hospitalized to receive acupuncture treatments for peripheral facial palsy. All patients were surveyed daily for adverse events associated with the acupuncture treatments applied the day before. Results : A total of 50 patients received $37.68{\pm}20.37$ acupuncture treatments in average. 36 patients reported experiencing at least one adverse event. Total 163 adverse events were reported. Common adverse events were bleeding(62), bruise(33), pain(30), tiredness(11), hematoma(7). The others were paresthesia(4), erythema(3), delay of removing needles(2), edema(2), aggravation of symptoms(2), declining of appetite(2), pruritus(1), drowsiness(1), anxiety(1), sweating(1), headache(1). Conclusions : Acupuncture has adverse events, like any therapeutic approaches. However, it is a relatively safe treatment. In future study, organized large scale prospective survey should be carried out to fully prove that acupuncture is safe.

  • Niemann-Pick disease is a group of autosomal recessive disorders associated with hepatosplenomegaly, variable neurologic deficits, and the storage of sphingomyelin and other lipids. Seven cases have been reported in Korea. We report an additional case presenting with hypotonia, early neurodevelopmental delay, hepatosplenomegaly and death by persistent pneumonia and asphyxia at the age of 23 months. MRI of brain and fundoscopic findings of our case at 4 months of age were normal. However, abnormal intensity of the thalamus and atrophy of the right temporal lobe on the MRI and macular cherry red spots were noticed at the age of 17 months. A bone marrow biopsy showed large foamy cells, while hexosaminidase A and B levels were normal. Although biochemical or molecular workup was not done, these findings led to the diagnosis of infantile onset Niemann-Pick disease, probably type A. A brief review of the related literatures was made.

Changes of Clinical Practice in Gastrointestinal Perforation with the Increasing Use of Computed Tomography

  • Purpose: The use of computed tomography (CT) to evaluate acute abdominal complaints has increased over the past two decades. We investigated how the clinical practice of patients with intestinal perforation has changed with the increasing use of abdominal CT in the emergency department (ED). Methods: We compared ED arrival to CT time, ED arrival to surgical consultation time, and ED arrival to operation time according to the method of diagnosis from 2003-2004 and 2013-2014. Results: In patients with gastrointestinal perforation, time from ED arrival to CT was shorter ($111.4{\pm}66.2min$ vs. $199.0{\pm}97.5min$, p=0.001) but time to surgical consultation was longer ($135.1{\pm}78.8$ vs. $77.9{\pm}123.7$, p=0.006) in 2013-2014 than in 2003-2004. There was no statistically significant difference in time to operation for perforation confirmed either by plain film or CT between the two time periods. There was no statistically significant difference in length of hospital or ICU stay or mortality between the two groups. Conclusion: With the increasing use of abdominal CT in ED, ED arrival to CT time has decreased and ED arrival to surgical consultation time has increased in gastrointestinal perforation. These changes of clinical performance do not delay ED arrival to operation time or adversely influence patient outcome.

  • It is well known that multipoint and computerized intraoperative mapping systems improve the results of surgery for Wolff-Parkinson-White syndrome and show tremendous potential for opening an entirely new era of surgical intervention for the more common and lethal types of supraventricular tachyarrhythmias such as atrial flutter and atrial fibrillation. In addition, the ability to map and ablate the sometimes fleeting automatic atrial tachycardia is greatly enhanced by computerized mapping systems. In this study, we have developed 64 channel computerized data analysis system using microcomputer (Macintosh IIx) for basic research of electrophysiology and electrical propagation. The bipolar electrogram information is acquired from 64 cardiac sites simultaneously at a sampling rate of 1ksamples/sec with continuous and total data storage of up to 30 seconds. When the reference electrogram is selected and reference point is picked up, delay time from the reference point in displayed on two dimensional diagram of the heart. System design permits easy expansion to almost 256 simultaneous sites, This system is expected to enable us to study pathophysiology of cardiac arrhythmia and to improve the results of diagnosis and surgical treatment for cardiac arrhythmia.

  • In the latest date, medical body area networks (MBANs) are emerging as a new technology for diagnosis the human body. MBANs in the health care fields are based on short-range and low-power (e.g. ubiquitous computing) among small-sized devices, and have been used by means of medical services. In this paper, we proposed an emergency message transmission protocol using carrier sense multiple access/time division multiple access in MBANs. This scheme focuses on dependability and power-efficiency. In order to increase the reliability of the transmission, this scheme modified a MCTA slot of IEEE 802.15.3 standard to a SR-MCTA slot. SR-MCTA slot is assigned by MBAN coordinator according to requesting terminal nodes. The method, having the priority of transmission, occurs a collision packet randomly. Results from this proposed solution revel that reservation-based TDMA medical body area network(MBAN) protocol for transmitting emergency message was improved in terms of transmission delay.

  • Shaken baby syndrome is a serious form of child abuse, mostly involving children younger than 2 years. It results from extreme rotational cranial acceleration induced by violent shaking. The characteristic injuries include subdural hemorrhage, retinal hemorrhage, and fracture of ribs or long bones. If physicians have no recognition of, or suspicion about, shaken baby syndrome, this syndome is difficult to diagnosis because of a lack of obvious external signs and failure of the abuser to admit his or her actions. In addition to the high mortality, 60% of survivors have significant long term neurologic and developmental abnormality. The authors experienced five cases of shaken baby syndrome presented with seizures or vomiting, without external signs of trauma. All of these cases had subdural hemorrhages, and four cases had retinal hemorrahges. In our follow up, two children were found to have sequelae such as quadriplegia, monoplegia, and developmental delay. We emphasize that early recognition and prompt treament are key to overall success of case management. The incidence of shaken baby syndrome can be reduced through public awareness and education of parents not to shake a babies.

  • Return of facial nerve function is important in patients with facial nerve paralysis by trauma. Sometimes, delay in diagnosis of facial nerve paralysis make recovery of facial nerve function difficult. Traumatic facial palsy mostly occur after temporal bone fracture in unilateral. Temporal bone fracture after head trauma are divided into the three group; longitudinal fracture, transverse fracture and mixed fracture. The most common symptoms are hearing impairment, bloody otorrhea, loss of consciousness and facial nerve paralysis. The early care of temporal bone fracture involves facial nerve paralysis. And there has been many discussion and study in the treatment of the immediate or delayed facial palsy ; examply, surgical approach, time and methods. We have managed a patient with unilateral facial nerve paralysis after longitudinal temporal bone fracture in mastoid process and conservative facial nerve decompression was performed. We have obtained good result and report this case with review of literatures.

Effects of Breast Self-Examination Consultation Based on the Health Belief Model on Knowledge and Performance of Iranian Women Aged Over 40 Years

  • Background: Breast cancer is the most prevalent malignancy in women worldwide; lack of awareness of symptoms and delay on diagnosis of breast cancer are the main causes of mortality among women. This study was conducted with the purpose of assessing the effect of educational consulting for breast self-examination (BSE) based on the health belief model (HBM) on the knowledge and performance of women over 40 years attending health care centers in Hamadan, Iran. Materials and Methods: In this quasi-experimental study, eligible women admitted to health centers in Hamadan city in 2015 were randomly assigned to intervention and control groups (n=75 in each group). The intervention group received 4 weekly sessions of breast cancer screening consulting based on the HBM. Control group received only routine care. Knowledge, HBM constructs, and BSE practice were compared between the groups before, immediately after and three months after the consultation. Results: Before the intervention, no significant differences were observed in knowledge, health belief and practice between two groups. However, after the intervention a significant difference was observed between two groups in mean scores of perceived benefits, perceived barriers, self-efficacy and the health motivations (p <0.05). Significant differences were also observed in terms of knowledge and BSE practice (p <0.01). Conclusions: The results indicate the importance of consultation on knowledge and beliefs to improve BSE performance and prevention of breast cancer in Iranian women.

Study on Wireless Acquisition of Vibration Signals (진동신호 무선 수집에 대한 연구)

  • A Wi-Fi signal network (WSN) system is introduced in this paper. This system consists of several data-transmitting sensor modules and a data-receiving server. Each sensor module and the server contain a unique intranet IP address. A piezoelectric accelerometer with a bandwidth of 12 kHz, a 24-bit analog-digital converter with a sampling rate of 15.625 kS/s, a 32-bit microprocessor unit, and a 1-Mbps Wi-Fi module are used in the data-transmitting sensor module. A 300-Mbps router and a PC are used in the server. The system is verified using an accelerometer calibrator. The voltage output from the sensor is converted into 24-bit digital data and transmitted via the Wi-Fi module. These data are received by a Wi-Fi router connected to a PC. The input frequencies of the accelerometer calibrator (320 Hz, 640 Hz, and 1280 Hz) are used in the data transfer verification. The received data are compared to the data retrieved directly from the analog-to-digital converter used in the sensor module. The comparison shows that the developed system represents the original data considerably well. Theoretically, the system can acquire vibration signals from 600 sensor modules at an accelerometer bandwidth of 15.625 kHz. However, delay exists owing to software processes, multiplexing between sensor modules, and the use of non-real time operating system. Hence, it is recommended that this system may be used to acquire vibration signals with up to 10 kHz, which is approximately 70% of the theoretical maximum speed of the system. The system can be upgraded using parts with higher performance
