DIAGNOSIS OF MESIODENS BY $SCANORA^{(R)}$ ($SCANORA^{(R)}$를 이용한 Mesiodens의 진단)

  • Mesiodens are supernumerary teeth in the region of the maxillary central incisors, a condition which can lead to disorders of the dentition. Their presence may lead to disorders, such as delay in eruption of permanent teeth, development of dentigerous cysts, resorption of adjacent roots and eruption of a supernumerary tooth into nasal cavity. The optimal time for surgical intervention is controversial. Early diagnosis is important so as to enable good prognosis. Diagnosis primarily depends on x-ray films. Panorama film, occlusal film, periapical film have been used for detection of mesiodens. But, all of them have disadvantages. $SCANORA^{(R)}$ is a multimodal radiology system which utilizes the principles of narrow beam radiology and spiral tomography. Pre-programmed imaging procedure are provided for many dental situations An optional personal computer can be connected into the unit to help design the examination. We report two cases diagnosed by $SCANORA^{(R)}$. When compared with tube shift technique, it is simple and exact method of detecting mesiodens.

A Case of Tuberculous Otitis Media in Children (소아에서 결핵성 중이염 1례)

  • Tuberculous otitis media is a very rare disease in extrapulmonary tuberculosis and in chronic otitis media nowadays. The characteristic signs seem to have altered from the past ones. And the suspicion index is very low that there is often a considerable delay prior to diagnosis. This might result in irreversible complications such as hearing loss, facial nerve paralysis and labyrinthitis. It is difficult to be defined by culture, so many patients with tuberculous otitis media are subjected to have surgery without a correct etiologic diagnosis. Therefore bacteriologic study of secretion and pathologic examinations of biopsied tissue are mandatory. We report one year and 11 months old boy with tuberculous otitis media having painful otorrhea and perforated tympanic membrane diagnosed by histologic finding and polymerase chain reaction(PCR). And we would like to emphasize the fact that tuberculous otitis media must be considered in the differential diagnosis of persistent suppurative otitis media despite appropriate antibiotic therapy considering the increasing tendency in incidence of Tuberculosis in Korea these days.

Identifying Predictive Factors for the Recurrence of Pediatric Intussusception

  • Purpose: The aim of the study was to identify factors related to the recurrence of intussusception in pediatric patients. Methods: The medical charts of patients diagnosed with intussusception and treated at Dongsan Medical Center, between March 2015 to June 2017, were retrospectively reviewed. Univariate and multivariate analyses were performed. Results: Among 137 patients, 23 patients (16.8%) had a recurrent intussusception and 8 of these patients (6%) had more than 2 episodes of recurrence. The age at diagnosis was significantly different between the non-recurrence and recurrence group (p=0.026), with age >1 year at the time of diagnosis associated with a greater rate of recurrence (p=0.002). The time interval from symptom onset to the initial reduction (<48 vs. ${\geq}48$ hours) was significantly longer in the recurrence group (p=0.034) and patients in the recurrence group had higher levels of C-reactive protein (CRP) (p=0.024). Bloody stools and a history of infection were significantly more frequent in the non-recurrence group (p=0.001 and p<0.001, respectively). On stepwise regression analysis, age >1 year at the time of presentation (odds ratio [OR], 4.79; 95% confidence interval [CI], 1.56-14.06; p=0.016) and no history of infection (OR, 0.18; 95% CI, 0.06-0.58; p=0.004) were retained as predictors of recurrence. Conclusion: Patients with intussusception who are older than 1 year at diagnosis, have an elevated CRP level, a delay of ${\geq}48$ hours between symptom onset and the initial reduction, an absence of bloody stools, and no history of infection should be closely monitoring for symptoms and signs of a possible recurrence.

Diffuse Sclerosing Variant of Papillary Thyroid Carcinoma in a Child: A Case Report (소아의 미만성 경화 변종 갑상선 유두암: 증례 보고)

  • The diffuse sclerosing variant of papillary thyroid carcinoma (DSPTC) is uncommon. Herein, we report a rare case of DSPTC in a 9-year-old girl who initially presented with a painless diffuse goiter. Thyroid peroxidase antibody testing yielded positive results, and the initial clinical diagnosis was Hashimoto's thyroiditis. However, thyroid ultrasonography revealed characteristic findings of DSPTC, which was confirmed through the postoperative histopathological diagnosis. Although thyroid cancers are rare in the pediatric population, DSPTC should be included in the differential diagnosis of goiter in these patients. Moreover, ultrasonography may prevent a diagnostic delay and facilitate the detection of a concomitant malignancy.

  • Developmental dysplasia or dislocation of the hip is the most pronounced form of a condition in which the femoral heads tends progressively to leave the acetabulum. In the young child the variability of findings and course suggest a spectrum of conditions in which there are a number of common features: restriction of movement, particularly of abduction in flexion; shortening; and abnormal radiology, including a sloping or dysplastic acetabulum and delay in the appearance of the upper femoral epiphysis. It is vital to make the diagnosis of a congenital dislocation as soon after birth as possible. Conservative treatment with an abduction brace before the child run begins to walk is completely adequate, but after the age of 4 even surgical repositioning is difficult and after the age of 7 it is almost impossible. The aim of the study was to gain insight into the value of physical therapy of developmental dysplasia or dislocation after operation.

  • The immature neonatal brain is susceptible to the development of seizures. Seizures occur in 1% to 5% of infants during the neonatal period. Neonatal seizures are most commonly associated with serious acute illnesses, such as hypoxic-ischemic encephalopathy, birth trauma, metabolic disturbances, or infections. Thus, newborn infants with seizures are at risk for neonatal death and survivors are at risk for neurologic impairment, developmental delay, and subsequent epilepsy. Experimental data have also raised concerns about the potential adverse effects of the currently used anticonvulsants in neonates on brain development. Therefore, in the management of neonatal seizures, confirmatory diagnosis and optimal, but shorter, duration of anticonvulsant therapy is essential. Nevertheless, there has been substantial progress in understanding the developmental mechanisms that influence seizure generation and responsiveness to anticonvulsants. The currently used therapies have limited efficacy and the treatment of neonatal seizures has not significantly changed in the past several decades, This review includes an overview of current approaches to the treatment of neonatal seizures.

  • Colorectal carcinoma is a well-known malignancy in adults. However, it is rare in children. Besides, it also has different behaviour in paediatric age-group and usually presents with non-specific symptoms like abdominal pain, weight loss, and anaemia. This usually leads to delay in diagnosis. Adenocarcinoma in children has unfavourable tumour histology (mucinous subtype) and advanced disease stage at presentation which lead to poorer prognosis in children. Family history, genetic typing and sibling screening are essential components of management as this malignancy is frequently seen associated with hereditary syndromes. We describe a case of unusual presentation of rectal carcinoma in a 12-year-old girl.

  • Mental retardation(MR) is one of the most common developmental disabilities, which is characterized by deficits in intellectual and adaptive functions. Most children with MR have cognitive limitation in the mild range. With respect to the etiology, it is believed that genetic and environmental factors are interrelated and show variable pictures. Most children with MR present with speech and language delay during the early years. The diagnosis can be made by clinical features and neuropsychological tests of intelligence and adaptive functioning. The treatment is limited, but many associated problems are amenable to multidisciplinary interventions. The article will review the recent advances in the management of MR and other neurodevelopmental disorders in children.

  • 본 논문에서는 규칙 기반 추론 기법을 이용하여 LAN 상의 지연 장애 검출 알고리즘과 장애를 유발시킨 호스트에 대한 위치 확인 알고리즘 및 장애 원인 분석 알고리즘을 제시하고자 한다. 이를 위해 지연 장애 검출 모델과 RBR 기반 장애 검출 규칙 모델을 제시하고 있다. 또한 충돌율 검출 규칙과 이용률 검출 규칙을 적용하여 지연 장애 검출 알고리즘을 설계하였고, 최대 패킷 출력 호스트 파악 규칙을 적용하여 장애 위치 탐색 알고리즘을 설계하였다. 그리고 패킷 유형 분석 규칙과 장애 원인 파악 규칙을 적용하여 장애 원인 분석 알고리즘을 설계하였다. 이를 통하여 LAN 상의 지연 장애를 검출하고 진단하는 기법을 제시하고자 한다. 이와 같이 제시한 지연 장애 검출 및 진단 기법을 실제 네트워크 환경에 직접 적용시켜 봄으로써 본 논문에서 제시한 장애 검출 및 진단 기법의 정확성과 적용성을 확인하였다. 이러한 기법은 네트워크 관리자가 LAN 상의 장애를 진단하고 원인을 해결하는데 큰 도움을 줄 것으로 기대된다.

  • RQ(Respiratory Quotient) value is obtained from the ratio of the consumed oxygen and the produced carbon dioxide during the patient's respiration. To investigate the efficacy of insulin and diagnosis the metabolic disorder in short time, the RQ value can be used as important parameter. The measurement of oxygen and carbon dioxide amounts is needed large chamber and complex sensors. But If the atmospheric oxygen and carbon dioxide concentrations do not change, the expiratory oxygen and carbon dioxide can be used to obtain RQ value. A convenient RQ measuring device has been developed by using two sensors for O2 and CO2. The estimation of RQ devices confirms that the RQ device can obtain accurate data by eliminating uncertain factor such as delay time and remaining gases.
