지구성 운동이 STZ-당뇨 유발 쥐의 허혈/재 관류 손상 후 심근의 당대사 관련 표현형 단백질과 HSP-60 단백질 발현에 미치는 영향 (The Effect of Exercise Training (EXE) on Myocardium Glucose Metabolic Phenotypic Proteins and HSP-60 Protein Expression after Ischemia/Reperfusion Injury in STZ-induced Rats)

  • The objective of this study was to identify EXE (1 hr a day at 21 m/min for 5 day/wk, at 0 % grade for 6 wk) on myocardium glucose metabolic phenotypic proteins (AMPK-PGC-1${\alpha}$-GLUT-4) and HSP-60 protein expression after ischemia/reperfusion injury (IRI) in STZ-induced rats. EXE was performed using STZ-induced diabetic rats on a rodent treadmill (28 m/min, 1 hr/day, 5 day/wk for 6 wk). The results of this study suggest that i) serum insulin level was not changed among groups (p>l0.05). ii) the LVDP level increased significantly in the STZ-EXE-IRI group compared to the STZ-IRI group at 60 min (p<0.01), 70 min (p<0.05) and 80 min (p<0.05) after reperfusion, respectively, and iii) AMPK phosphorylation (p<0.01), PGC-1${\alpha}$ protein (p<0.001), GLUT-4 protein (p<0.001) and HSP-60 protein expressions (p<0.05) increased significantly in the STZ-EXE-IRI group compared to the STZ-IRI group. In conclusion, the findings of the present study reveal that EXE may provide therapeutic value to insulin dependent diabetic patients with peripheral insulin resistance and myocardium injury by improving glucose metabolic proteins (AMPK-PGC-1${\alpha}$-GLUT-4) and heat shock protein-60 (HSP-60), along with increasing LVDP levels and decreasing glucose levels. Therefore, EXE protects the STZ-induced diabetic myocardium injury against ischemia/ reperfusion injury.

당뇨 쥐에서 상엽추출물을 첨가한 보리면의 혈당조절효과 (Effects of Barley Noodles Contained Mulberry Leave Extracts on Blood Glucose Regulation in Diabetic Mice)

  • 본 연구는 상엽추출물이 포함된 보리면의 혈당조절능력을 입증하기 위하여 4주간 고지방/고탄수화물 식이 후 streptozotocin과 nicotinamide로 당뇨를 유발하고 음성대조군, 양성대조군, 보리면대조군, 상엽대조군, 보리면+상엽추출물 2.5%군, 보리면+상엽추출물 5%군의 6군으로 나누었다. 그 후 2주간 식이를 진행하며 체중, 혈당, 간 기능 및 혈액 내 지질의 변화를 측정하였다. 그 결과 보리면과 상엽 단독 섭취군보다 혼합섭취군에서 체중 감소가 적었고 혈당조절효과도 뛰어났으며 이는 경구 당 부하검사에서도 동일한 결과를 보였다. 또한 간기능, 혈중 지질 개선 효과도 혼합 섭취군에서 강하게 나타났다. 따라서, 보리면과 상엽추출물을 함유한 식이는 복합식이에서 단독식이보다 강한 혈당조절효과와 간 기능 개선, 지질 강하효과도 나타났다. 결과적으로 상엽을 포함하는 보리면은 혈당을 조절하고, 간 기능, 지질 강하 기능을 보여 만성질환 관리에도 도움이 될 것으로 사료된다.

수풍순기환 투여가 고지방, 고탄수화물 식이로 유발된 비만형 제2형 당뇨병 동물모델에 미치는 영향 (The Effects of Supungsunki-hwan on High Fat, High Carbohydrate Diet-induced Obese Type 2 Diabetic Mouse Model)

  • Objective : Obesity is an important cause of diabetes, and lipotoxicity causes insulin resistance. Recently a lot of research is being done on PPAR-${\alpha}$. TNF-${\alpha}$. adiponectin, and leptin, which are important obesity related factors. In this study, we investigated the effects of Supungsunki-hwan on high fat. high carbohydrate diet-induced obese type 2 diabetic mouse models. Methods: Diabetes was induced in ICR male mouse (30${\pm}$5g) with Surwit's high fat, high sucrose diet. Mice were divided into 4 groups(n=10) of Normal. Control. Supungsunkj-hwan group. and acarbose group. The Supungsunki-hwsn group was given 10% Supungsunkj-hwan in their diet. and the acarbose group was given 0.5% acarbose in their diet. After 6 weeks. body weight. food intake, FBS and OGTT. lipid profile and liver enzymes, epididymal fat weight, and gene expression of leptin, adiponectin, TNF-${\alpha}$ and PPAR-${\alpha}$ were measured. Leptin. adiponectin. tumor necrosis factor(TNF)-${\alpha}$ and peroxisome proliferator-activated receptor (PPAR)-${\alpha}$ were evaluated by reverse transcription-polymerase chain reaction. Results : Supungsunkj-hwan increased the gene expression of PPAR-${\alpha}$, which reduces lipotoxicity and insulin resistance. Supungsunkj-hwan also significantly reduced triglyceride. AST. ALT serum levels. and 1 hour oral glucose tolerance levels. Conclusion : These results show that Supungsunkj-hwan improves insulin resistance in the liver and muscles, by reducing triglyceride levels and lipotoxicity through increased PPAR-${\alpha}$ gene expression. This is supported by the fact that Supungsunkj-hwan significantly reduces 1 hour oral glucose tolerance levels. Therefore we suggest that Supungsunkj-hwan would be an effective treatment for obese type 2 diabetic patients.

  • Drug interactions with food, on occasion, lead to serious nutritional and functional changes in the body as well as alterations of pharmacological effect. It, therefore, should be necessary to take drug interactions with food into consideration for effective and safe therapeutics. Diabetes mellitus is a heterogeneous group of disorders characterzed by abnormal glucose homeostasis, resulting in hyperglycemia, and is associated with increased risk of microvascular, macrovascular, and neuropathic complications. However, the precise mechanism of diabetes mellitus remains unclear. Three basic objectives in the care of diabetic patients are maintaining optimal nutrition, avoiding hypo- or hyperglycemia and preventing complications. Laminaria japonica is a brown macroalgae which can be used as a functional diet due to high content of diatery fiber. The purpose of this study was to investigate the effect of Laminaria japonica diet on the pharmacokinetics of metformin which are frequently used in the treatment of diabetes. Diabetic rats induced by streptozotocin were employed in this study. Blood concentrations of oral hypoglycemic agent, metformin, were measured by HPLC and resultant pharmacokinetic parameters were calculated by RSTRIP. The mechanisms of drug interaction with food were evaluated on the basis of pharmacokinetic parameters such as $k_{a},\;t_{1/2},\;C_{max},\;t_{max}$, and AUC. Administration of metformin in normal and diabetic rats treated with Laminaria japonica diet showed significant decrease in AUC, $C_{max},\;and\;k_a$, and increase in $t_{max}$, compared to those with normal diet. This might result from adsortion of metformin on components of Laminaria japonica, causing delayed absorption. The oral glucose test showed that Laminaria japonica diet could lower blood glucose level probably through either inhibiting the activity of disaccharidases, intestinal digestive enzymes, or delaying the absorption of glucose. More studies should be followed to fully understand pharmacokinetic changes of metformin caused by long-term Laminaria japonica diet.

오매 추출물의 혈당 강하 효과 (Hypoglycemic Effects of Crude Extracts of Prunus mume)

  • 한의학에서 당뇨병 (소갈) 처방으로 사용되는 생진양혈탕과 황금탕 처방 성분 중의 하나인 오매의 포도당 이용에 대한 효과를 조사하기 위해서 오매를 70% 에탄올로 추출한 후 메탄올과 물을 섞은 용액으로 단계별로 XAD-4 column으로 분획하였다. 본 연구에서는 3T3-L1 섬유아세포와 지방세포에서 오매의 추출 분획물이 인슐린처럼 작용하는 인슐린성 물질이거나, 인슐린 작용을 향상시키는 인슐린 민감성 물질이거나, 또는 $\alpha$-glucoamylase 활성을 억제하는 물질로 작용하는 지 여부를 조사하였다. 오매 분획물은 인슐린성 물질로 작용하지 않았다. 반면에 20%나 40% 메탄올 분획층은 분화 유도 물질의 작용을 억제하여 3T3-L1 섬유아세포에서 지방세포로의 분화를 억제하였다. 한편 오매 분획층 중 60%, 80% 그리고 100% 메탄올 분획층은 3T3-L1 지방세포에서 인슐린 작용을 향상시키는 인슐린 민감성제로 작용하였다. in vitro 실험에서 20 그리고 40% 메탄올 분획층은 $\alpha$-glucoamylase의 활성을 저하시켜 말토스와 dextrin의 분해를 방해하였다. 그러나 in vivo에서는 오매 에탄올 추출물을 말토스나 dextrin을 투여하기 10분전에 투여하였을 때 최고 혈당 값과 area under the curve 값은 대조군과 차이가 없었다. 결론적으로 오매에는 지방 세포의 분화를 억제하는 물질과 인슐린 민감성을 향상시키는 물질이 함유하고 있으므로 비만을 동반한 당뇨병 및 인슐린 저항성의 치료와 예방에 중요한 역할을 할 것으로 사료된다.

관상동맥 약물 용출 스텐트 삽입 후 항혈소판제제 3제요법과 2제요법의 임상적 효과 비교 (Effect of Triple Compared to Dual Antiplatelet Therapy After Drug-Eluting Stent Implantation in Percutaneous Coronary Intervention)

  • ACC/AHA/SCAI Guideline recommends for administration dual antiplatelet therapy after drug-eluting stent (DES) to prevent restenosis and stent thrombosis in patients with percutaneous coronary intervention (PCI). Recently triple antiplatelet therapy including cilostazol is known to reduce restenosis and stent thrombosis significantly after DES implantation. However, there is lack of data providing the efficacy of triple antiplatelet therapy. The purpose of this study is to evaluate the clinical effects of the triple therapy after DES implantation compared with the dual therapy. This retrospective study collected data from medical charts of 251 patients who received DES implantation between Jul 2006 and Jun 2008. They received either dual antiplatelet therapy (N = 154 clopidogrel and aspirin; Dual group) or triple antiplatelet therapy (N = 97 cliostazol, clopidogrel and aspirin; Triple group). Major adverse cardiac event rates (MACE, included total death, myocardial infarction, target lesion revascularization) at 12 months, 24 months, stent thrombosis, rates of bleeding complications and adverse drug reactions were compared between these two groups. Compared with the dual group, the triple group had a similar incidence of the MACE rates at 24months (12.3% vs. 12.4%, p = 0.99). There is no difference in overall stent thrombosis between two groups (Dual group 2.6% vs. Triple group 4.1%, p = 0.5). Subgroup analysis showed that diabetic patients got more benefit in reducing MACE rates but, there is no statistical difference. Bleeding complications and adverse drug effects were not different significantly. As compared with dual antiplatelet therapy, triple antiplatelet therapy did not reduce the 12-months, 24-months MACE rates and stent thrombosis. Bleeding complications and adverse drug effects were not different.

Ratio of fat to energy intake independently associated with the duration of diabetes and total cholesterol levels in type 2 diabetes

  • The importance of dietary intake in the treatment of type 2 diabetes was emphasized. This study was performed to investigate the dietary intakes of Korean type 2 diabetes patients according to the treatment and duration of diabetes and to examine the relationships between their diet and serum lipid profiles. The subjects were 111 type 2 diabetic patients who were treated by medical nutrition therapy only, oral hypoglycemic agents (OHA), or insulin with medical nutrition therapy. Dietary intake was assessed by a registered dietitian using semi-quantitative food frequency questionnaires Comparisons according to treatment type were made using covariance analyses. General linear models identified the independent effects of the different treatments after covarying for age, duration of diabetes, and 2-way interactions. There were no significant differences in age and BMI but was in duration of diabetes according to treatment type in these subjects. Carbohydrate to energy ratio was higher in the OHA group (P < 0.05), whereas the fat to energy ratio was higher in the insulin group for males (P < 0.05). Carbohydrate ($R^2$ = 0.24, P = 0.005) and fat ($R^2$ = 0.26, P = 0.02) to energy ratios were independently associated with the duration of diabetes after covarying for age, sex, treatment, and 2-way interactions. The levels of triglyceride (TG; $R^2$ = 0.32, P = 0.02) and total cholesterol (TC) were associated independently with energy intake and the carbohydrate ($R^2$ = 0.15, P = 0.02) and fat ($R^2$ = 0.15, P = 0.01) to energy ratios, respectively. The concern that the independent association of dietary intake with either duration of diabetes or dietary factors affects blood lipid levels could suggest that specific dietary recommendations may work better for identifiable groups of diabetes patients.

당뇨병 환자의 하지 혈관 질환 검사에서 색도플러 초음파의 이용 (Color Doppler Ultrasonogram for the Peripheral Vascular disease in Diabetes Patients)

  • Purpose: This study is to assess the involvement of vessels in lower extremity in diabetics, using color doppler ultrasonogram. Materials and Methods: Arteries of both lower extremities were divided into 3 groups-large vessel (above-knee arteries), popliteal vessel, and small vessel(below-knee arteries) -, and evaluated using color doppler ultrasonogram in 60 diabetics. In color doppler ultrasonogram, the wave forms of all vessels were divided into 5 grades; grade I was triphasic wave form, grade II was spectral broadening form, grade III was monophasic wave form, grade IV was pulsus tardus et parvus form, grade V was absence of wave. Grade III, IV, V were grouped into vessel obstruction. We reviewed the correlationships among the degree of the peripheral vascular involvement, duration of dibetes, existence of bilaterality, types of dibetes. Results: Bilateral involvement was high in both lower extremity. Luminal stenosis, vascular calcification and vessel obstruction were high incidence in the patients over ten years of diabetic duration. Prevalence of vascular calcification and vessel obstruction were high in the small vessel of ankle level. But, insulin injection was not related to the incidence of vascular abnormality. Conclusion: Color doppler ultrasonogram seems to be useful for evaluation of peripheral vascular status, decision making for necessity of additional test, periodic follow -up tool in diabetes patients.

당뇨병 환자의 자기효능, 환자역할행위 이행 및 대사조절간의 관계 (A Study of the Relationship among Self-Efficacy, Sick-Role Behavior and Metabolic Control in Diabetic Patients)

  • The purpose of this study was to identify the degree of self-efficacy, sick-role behavior and metabolic control in patients with diabetes and to identify the relationships among those variables. The subject of the study were composed of 204 NIDDM patients from 2 hospitals, who were visiting the outpatient clinic in Taegu. Data were collected from Sep. 18th to Oct. 5th, 1996. In data analysis, an SPSS/PC+ program was utilized for descriptives correlation, T-test, and ANOVA. The results are as follows. 1) The mean scores for self-efficacy were 65.74 (range 30.83-90.68), on a 100 point scale. 2) The mean scores for sick-role behavior were 3.36(range 1.80-4.67), on a 5 point scale. 3) The mean score for metabolic control was 7.63 (range 5.0-13.2). 4) Self-efficacy was significantly correlated with sick-role behavior(r=.3614, p<0.01) but there were no significant correlations with self-efficacy and metabolic control (r=-.1045, p>0.01), sick-role behavior and metabolic control (r= - .1288, p>0.01). 5) There were significant differences in self-efficacy according to sex(t=4.47, p=0.000) and education(F=8.27, p=0.000). 6) There were no significant differences in sick -role behavior according to demographic characteristics. 7) There were significant differences in metabolic control according to the duration of diagnosed diabetess(F=4.13, p=0.007) and complication (t=2.47, p=0.014). These results suggest that for improvement in sick-role behavior nursing intervention needs to be directed at promoting self-efficacy.
