Purpose: This study was conducted to investigate performance of self-monitoring of blood glucose and the utilization of results for people with type II diabetes. Method: Data was collected from 177 patients with type II diabetes using a questionnaire from August to September, 2009. Result: Most participants performed self-monitoring of blood glucose in the correct way while some misused the lancet or test strips. In the utilization of results for self-monitoring blood glucose data, 62% of participants always recorded the data, 46% always understood the cause for hyperglycemia or hypoglycemia, 25% changed their diet, 23% regulated their exercise-amount, and 11% of participants adjusted the drug dosage. There were significant differences in the performance of self-monitoring of blood glucose and the utilization of results according to the characteristics in the self-monitoring subjects such as awareness of HbA1c, target fasting glucose level, target fasting glucose level 2 hours after meal etc. Conclusion: Systemic self-monitoring of blood glucose education which includes the utilization of self-monitoring of blood glucose needs to be developed for type II diabetic patients.
Unsatisfied with the modern medicine's treatment of diabetes, patients diagnosed with the disease turn to alternative medicine for treatment. A look at the percentage of people using alternative medicine marked 72.8% in the United States and more than 60% in Korea in 2006. The most preferred form of alternative medicine turned out to be usage of dietary supplements and herbal medicine. Most of the dietary supplements and herbs that diabetic patients use largely originated from their usage in East Asian Traditional Medicine. As Western Medicine made its way into East Asia in late 18th century, excessive efforts were made to translate Western medical terminology into traditional medical terminology equivalents. In the process of doing so, wasting-thirst became a concept equivalent to diabetes. Theories regarding the pathogenic outbreak and transmission of wasting-thirst has been supplemented and progressed according to needs and even showed new tendencies. Profound understanding of wasting-thirst achieved through historical research is expected to lead to proper application of wasting-thirst treatment methods in treating modern diabetes.
Purpose: This study was performed to evaluate the effectiveness of self-home dressing with nanocrystalline silver dressing method on the treatment of chronic ulcer wounds of the foot. Material and Methods: One hundred-nine patients with chronic foot ulcer due to various causes were treated with nanocrystalline silver dressing material. Dressing was done by themselves in their home. Dressing changes were performed every 2 to 3 days until complete reepithelization. Results: One hundred two cases of all cases had a complete reepithelization. It took 49 days to have a complete reepithelization on average. Seven cases failed to complete reepithelization because of infection. There was no silver intoxication in any cases. Conclusion: Using nanocrystalline silver is a useful dressing method for various superficial chronic ulcer and it can be done by themselves at their home. Thus it is considered to be more comfortable to both patients and doctors.
본 연구는 2014년 5월부터 2015년 5월까지 세종특별자치시 고혈압 당뇨병 등록교육센터에서 교육 받은 당뇨병 환자 39명을 대상으로 교육 프로그램의 효과를 평가하고자 실시되었다. 프로그램은 제2형 당뇨병에 대한 질환, 영양, 운동 등에 관한 내용으로 2일 동안 2회의 세션(세션 당 45 분)으로 구성되었다. 교육 전에 당뇨병에 관한 지식, 자기효능감, 자기관리행위 등을 평가하였고, 교육 직후에 지식과 자기효능감 평가, 교육 후 12주째에 자기관리행위를 평가하여 다음과 같은 결과를 얻었다. 당뇨병 지식 점수(10점 만점)는 프로그램 전후 평균 5.72점에서 8.21점으로 유의하게 증가하였고(p<0.001), 자기효능감 점수(10점 만점)는 프로그램 전후 6.88점에서 8.16점으로 유의하게 증가하였다(p<0.001). 자기관리행위 점수는 프로그램 전 5점 만점 중 3.59점이었던 것이 프로그램 종료 12주째에 4.35점으로 유의하게 증가하였다(p<0.001). 본 프로그램은 당뇨병 지식, 자기효능감, 자기관리행위 등의 향상을 통해 등록환자의 효과적인 당뇨병 관리에 기여할 수 있을 것으로 사료된다.
본 연구는 제2형 당뇨병으로 진단받은 환자 중 의료급여수급권자를 대상으로 12주간 영양교육을 실시하여 저소득층 당뇨병환자들의 영양교육을 통한 식습관 및 혈당개선의 효과를 알아보고자 하였다. 총 45명(남자 25명, 여자 20명)으로 구성되었으며 교육 후 여자에서 공복혈당 및 당화혈색소가 유의하게(P<0.05) 감소하였다. 혈중지질은 남자는 교육후 총콜레스테롤이 유의하게(P<0.05) 감소하였으나 LDL-콜레스테롤과 HDL-콜레스테롤, 중성지방에서는 유의한 변화가 없었다. 여자는 총콜레스테롤과 중성지방이 유의하게 (P<0.05) 감소하였으며 LDL-콜레스테롤과 HDL-콜레스테롤은 유의한 차이가 없었다. 식습관의 총 점수는 남녀 모두에서 교육 후 상승하는 경향을 보였으며, 항목별로는 남녀 모두에서 중정도의 식사 섭취와 식사 외의 간식을 먹지 않는다는 항목에서 유의적인 향상이 있었다. 영양지식의 경우도 남녀 모두에서 총 지식점수의 상승경향을 보였고, 아침결식 후 점심과 식사섭취, 떡과 감자류 등의 탄수화물 조성, 술의 영양가에 대한 항목에서 유의적인 지식 향상이 있었다. 영양소섭취 상태에 있어서 교육 후 탄수화물이 여자에서 유의하게 감소하였으며, 단백질은 두 군 모두 유의하게 상승하였고 지방은 여자에서 유의하게 상승하였다. 비타민은 여자에서 비타민 A, B1이 유의하게 상승하였으며, 무기질은 남자가 교육 후 칼슘, 인, 철분, 나트륨, 칼륨, 아연에서, 여자는 칼슘, 인, 나트륨에서 유의한 변화가 있었다. 에너지 구성 영양소의 비율은 남자는 교육 전 탄수화물 : 단백질 : 지방 : 알코올이 62.4%:12.7%:14.6%:10.1%에서 교육 후 65.1%:14.5%:15.0%:5.6%로 변화되었고, 여자는 탄수화물 : 단백질 : 지방의 비율이 교육 전 70.5%:14.8%:14.6%에서 교육 후 66.4%:16.3%:17.6%로 변화되었다. 본 연구는 저소득층 제2형 당뇨병환자를 대상으로 저소득층 당뇨병환자의 특성을 파악하고, 이들의 식습관 문제점을 도출하여 그것을 토대로 적절한 영양교육을 실시한 결과 혈당, 식습관, 영양지식 및 영양소섭취상태의 개선에 효과가 있었다. 저소득층 당뇨병 환자에게는 경제적으로나 현실적으로 어려움이 많기 때문에 구체적인 대안이 필요하며, 실천 가능한 영양교육이 지속적으로 필요할 것으로 사료된다.
Mulberry, Mori folium leaves (ML) and fruits (MF), Lycii fructus (LF), and Panax ginseng have been known to maintain normal blood sugar levels (BSL) in folk medicine. Here we investigated the effects of coated rice with these herb extract to diabetic patients. Alloxan-induced diabetes mellitus (DB) mice (80 mg/kg BW., i.v.) separated into 3 groups. One group received the extract in high dose (6.5 mg/kg B.W.), and another one group received in low dose (1.3 mg/kg B.W.) perorally for 30 days. (+) control received no extracts. (-) control received neither of them. BSL, volume of water consumed (VWC), body weights (BW) of each group were measured and compared. BSL in both of the extracts-received groups got close to normal levels on 4th week after starting the experiments. VWC also showed the same pattern. BSL in (+) control remained high through the experiment periods. BW in (+) control remained high, but those in the extracts-received groups recovered to normal levels. The clinical trials for both of healthy volunteers and DB patients, conducted in a University Hospital, also showed that the rice coated with the extracts could control the BSL. From these results, we suggest that the functional rice with herb extracts could be used as a new alimentotherapy for DB patients.
Purpose: This study was a systematic review and meta-analysis of the effects of exercise programs in patients with type II diabetes mellitus. Method: Two investigators systematically searched and reviewed English articles from PUBMED from 1988 to 2004, selecting randomized controlled trials on structured exercise programs for DM patients. Out of 87 studies identified, a meta analysis was done for eleven studies which satisfied inclusion criteria and focused on glycemic indices, lipid indices, and cardiac function indices. Results: The means and standard deviations were compared for experimental groups that received exercise-only or exercise and diet programs and control groups that received no intervention or only diet education. The groups were considered homogeneous as the p value of the Q score in each variable group was over 0.05. The experimental groups demonstrated a moderate positive effect on HbA,c and $VO_{2max}$ (d=0.55 & 0.5), and a small positive effect on fasting blood glucose and cholesterol (d=0.38 & 0.27) compared to the control groups. HDL and LDL cholesterol levels, however, showed a very low positive effect (d=0.11 & 0.12) in the experimental groups. Aerobic exercise was more beneficial than resistance exercise on $HbA_1c$ (d=0.59 vs 0.28) in the groups. Conclusions: Regular exercise has a positive effect on $HbA_1c$, fasting blood glucose, total cholesterol, HDL cholesterol, LDL cholesterol, and $VO_{2max}$ in Type 2 diabetic patients.
Objectives: The purpose of this study was to investigate the effects of oral health behaviors of patients with hypertension and diabetic patients on the number of remaining teeth in Korean adults over 45 years of age, furthermore, we also aimed to develop oral health care programs and health promotion for patients with hypertension and diabetes. Methods: Altogether, 1,526 subjects with hypertension and diabetes from the 6th Korean National Health and Nutrition Survey (2013-2015) were analyzed. Results: A multiple logistic regression analysis was performed to determine the effects of oral health behavior and periodontal disease on residual teeth in patients with hypertension and diabetes after adjusting for age. We found 1.53 times the risk of having less than 20 remaining teeth in the group of brushing more than 3 times a day (95% CI: 1.00,2.35, p<0.001), 2.27 times (95% CI: 1.23-4.17, p<0.05) in the group not using the dental floss group 0.05), compared to the group undergoing regular dental check-up (95% CI: 0.93-1.85, p>0.05), compared to the non-smoking group 0.77 times (95% CI: 0.51-1.14, p>0.05), 1.33 times (95% CI: 0.98-1.80, p>0.05) in the drinking group compared to the non-drinking group, in the periodontal disease group, the risk of having less than 20 remaining teeth was 2.19 times higher (95% CI: 1.48-3.24, p>0.001) than in the normal group. Conclusions: As a super-aging society, develops in Korea, the prevalence of hypertension and diabetes will increase gradually. Therefore, an oral health policy should be implemented at the national level for the prevention and treatment of periodontal disease as well as the management of oral health the elderly.
BACKGROUND/OBJECTIVES: The current study examined trends in adherence to dietary recommendations and compared the levels of adherence between diagnosed and undiagnosed subjects with type 2 diabetes mellitus (T2DM) in Korea over the past 14 years. SUBJECTS/METHODS: Data were collected from the 1998-2012 Korea National Health and Nutrition Examination Surveys (KNHANES). Diagnosed diabetes was defined as giving a positive response to questions about awareness of the disease, a physician's diagnosis of diabetes, or medical treatment for diabetes, whereas undiagnosed diabetes was defined as having a fasting glucose level ${\geq}126mg/dl$. Assessment of adherence level was based on 6 components of dietary guidelines, considering meal patterns and intake levels of calories, carbohydrates, vegetable/seaweed, sodium, and alcohol. The participants received 1 point if they met the criteria for each of the 6 components, and the total possible score ranged from 0 to 6 points. Multivariate generalized linear regression was performed, taking into account the complex survey design. RESULTS: Among all diabetic patients aged 30 years or older, the proportion of diagnosed diabetes increased dramatically, from 40.9% in 1998 to 75.9% in 2012 (P for trend < 0.001). The overall adherence levels to dietary recommendations were low and did not significantly differ between diagnosed and undiagnosed subjects with T2DM for all survey years. Several improvements were observed, including increased adherence to maintaining sufficient vegetable/seaweed consumption (increased from 0.12 to 0.16 points) and limiting sodium intake (increased from 0.12-0.13 points to 0.19-0.24 points; P for trend < 0.001), while adherence to maintaining moderate alcohol consumption decreased. CONCLUSIONS: Analysis of data collected by the KNHANES indicates that Korean T2DM patients have poor adherence to dietary recommendations and maintenance of a healthy lifestyle, regardless of disease awareness. This finding suggests that development of practical, evidence-based guidelines is necessary and that provision and expansion of educational programs for T2DM patients is critical after diagnosis.
With the shift of cause of death from infection to chronic, the health expenditure has risen dramatically. To curb the increasing health expenditure, programs and campaigns to promote health were proposed and implemented. Most of them, however, were not successful in achieving satisfactory results. Customer Relationship Management has been gradually accepted as an innovative approach to health promotion. The objective of this research was to develop a Customer Relationship Management system for providing comprehensive health care services to the residents in the community. Detailed objectives were as follows: The first objective was the development of the CRM system for health promotion. The second objective was the satisfaction assessment for the health promotion program using the CRM system. The third was the proposal for the effective utilization of the CRM system. The development methodology of the CRM system was Rapid Warehouse Developing Method. As a CRM system equipment, a workstation with GIS of Windows 2000 was selected. SQL Server 2000 was used as a development tool and database. The subjects of study were diabetic mellitus patients, hypertension patients, and vaccin patients. The campaign channel of patients was an autocalling system. For the satisfaction assessment, a survey was performed. The main content of the survey was satisfaction level. The satisfaction level of the health promotion program using CRM system was 79.3%. In consideration of the above findings, we suggested ways of improving the Health Promotion Program by using CRM. The first was the efficient selection of the subjects of the Health Promotion Program. The second was the development for health promotion program using CRM system(life time health of individual etc).
