  • Evaluation of clinical usefulness of current perception threshold test and vibration sense perception threshold test in diagnosing the diabetic poly-neuropathy patients is one of the diagnosis methods for diabetic poly-neuropathy. Up to the present, some diagnostic methods were used for diabetic poly neuropathy patients. For example, there are neuropathy impairment score test of lower limbs, nerve conduction test, cooling detection threshold test, heat-pain threshold test and so on. However, most of the above tests require very expensive cost and take a lot of time in test. In this paper, a new apparatus estimating vibration sense ability is introduced. For this purpose, the VCM(voice coil motor) stimulating patient's peripheral nerve and current amplifier were manufactured. Also, softwares sensing and driving the vibration detection threshold test in order to measure the quantitative vibration sensory levels in diabetic poly-neuropathy patients were developed.

Fresh Fibroblast Allograft as a Treatment for Diabetic Foot Ulcers (당뇨족 궤양의 치료를 위한 신선 섬유아세포 동종이식)

  • Purpose: In order to overcome the limitations of the conventional cryopreserved fibroblast or keratinocyte allograft method used in the treatment of diabetic foot ulcers, we reported a pilot study in 2004 demonstrating promising results of a fresh fibroblast allograft method in eight patients. However, the number of cases was insufficient for full evaluation and the follow-up duration was not long enough to determine the efficacy and safety of the method. This encouraged us to conduct this follow-up study to fully evaluate the use of noncryopreserved fresh human fibroblast allografts in treating diabetic foot ulcers. Methods: Thirty-seven patients with diabetic foot ulcers were treated using fresh fibroblast allografts. Human dermal fibroblasts from healthy teenagers were cultured in DMEM/F-12 medium supplemented with 10% serum. The cultured cells were applied on the wounds immediately following debridement, with fibrin being used as a cell carrier. In eight weeks, percentages of complete healing, mean healing time, and patient satisfactions were assessed, with follow-up time ranging from 6 to 40 months. Results: Our study showed that 83.8% of the treated patients were complete healed. The time required for complete healing was $30.9{\pm}10.1$ days. Patient satisfaction scores for the experimental treatment were higher than those for the conventional method(mean scores of $8.1{\pm}1.1$ and $4.8{\pm}1.4$, respectively). No adverse events related to the study treatment occurred. Conclusion: The use of fresh human fibroblast allografts was found to be a safe and effective treatment for diabetic foot ulcers.

A Study on the Development of High-fiber supplements for the Diabetic Patients -Effect of Seaweed Supplementation on the Lipid and Glucose Metabolism in Streptozotocin-induced Diabetic Rats- (당뇨벙 환자를 위한고식이섬유 보충물의 개발을 위한 연구(II) -해조류 투여가 당뇨쥐의 당질과 지질대사에 미치는 영향-)

  • The ultimate aim of this study is to find high-fiber sources among Korean common foods and to develop a high-fiber supplement which can be useful in the therapeutic diet for the diabetic patients. For this purpose the effect of four kinds of seaweeds(mixture of purple laver & sea lettuce : MPS, sea tanle : ST, sea mustard : SM, agar agar : AA) on the glucose and lipid mtabolism were examined. Seven groups of normal and streptozotocin-induced diabetic rats were fed dietary fiber-free control diet or one of experimental diets containing 7% of one of four seaweeds for six weeks. The effects of seaweeds were campared with the effects of fiber-free diet or pectin diet. ST, SM, and AA showed a tendency of improving glucose tolerance improvement by those seaweeds, however, was less than that by pectin. MPS was found to possess a serum cholesterol-lowering effect which is comparable to that of pectin. All the supplementations of seaweeds induced significant increase in fecal steroids excretion. The amounts of fecal cholesterol excretion follwing in feeding of MPS and SM were as high as the level cause by pectin. The excretion of bile acids in the MPS group was much higher than that in the pectin group. Based on its effects of alleviating the diabetic symptoms in the previous study and of improving the glucose tolerance, sea mustard seems to have a benefical effect on glucose metabolism. The serum cholesterol-lowering effect of MPS possibly due to the significant increase in fecal steroids excretion suggests that MPS may be effective in improving abnormalities of lipid metabolism. Therefore, sea tangle and mixture of purple laver & sea lettuce seem to be promising as an effective source of high-fiber supplement for the diabetic patients.

The Amputation Rate and Associated Risk Factors within 1 Year after the Diagnosis of Diabetic Foot Ulcer (당뇨병성 족부 궤양 환자의 진단 1년 내의 절단율 및 위험 인자의 분석)

  • Purpose: This study investigates the amputation rate within 1 year after the diagnosis of diabetic foot ulcer and its associated risk factors. Materials and Methods: This study enrolled 60 patients with diabetic foot ulcer. The mean and standard deviation age was $64.4{\pm}12.8years$ (range, 32~89 years); the mean and standard deviation prevalence period for diabetes mellitus was $21.0{\pm}7.5years$ (range, 0.5~36 years). The amputation rate was evaluated by dividing the subjects into two groups - the major and minor amputation groups - within 1 year following the initial diagnosis of diabetic foot ulcer. Multivariate Cox proportional hazards regression analysis was used to identify the risk factors for amputation. Results: The total amputation rate of 38.3% (n=23) was comprised of the amputation rate for the major amputation group (10.0%) and rate for the minor amputation group (23.8%). There was a high correlation between peripheral artery disease (toe brachial pressure index <0.7) and amputation (hazard ratio [HR] 5.81, confidence interval [CI] 2.09~16.1, p<0.01). Nephropathy was significantly correlated with the amputation rate (HR 3.53, CI 1.29~9.64, p=0.01). Conclusion: Clinicians who treat patients with diabetic foot complications must understand the fact that the amputation rate within 1 year is significant, and that the amputation rate of patients with peripheral artery disease or nephropathy is especially high.

Two Cases of Diabetic Peripheral Polyneuropathy Improved by Bogan-tang. (보간탕으로 호전된 당뇨병성 말초신경병증 2례)

  • Diabetic Peripheral Polyneuropathy is one of the most distressing complications of diabetes, Drugs such as narcotic analgesics, tricyclic antidepressants, anticonvulsants and phenothiazines have been used to treat diabetic peripheral polyneuropathy, but these drugs are not very effective for the many patients and the side effects may become intolerable. In oriental medicine, usually the main etiology of diabetic Peripheral Polyneuropathy is insufficiency of Blood due to the state of dry-heat in the human body. Therefore, we use Bogan-tang to treat numbness of Diabetic Peripheral Polyneuropathy. Bogan-tang was administrated twice a day for 15 days, and VAS Scale was performed every day to evaluate numbness and insufficiency of blood. In both cases, numbness and insufficiency of blood were improved without any side effects. This study suggests that Bogan-tang is an effective drug in the treatment of Diabetic Peripheral Polyneuropathy.

Effects of Legume Supplementation on the Gastrointestinal Function and Diabetic Symptoms in Streptozotocin-Induced Diabetic Rats (두류의 첨가가 당뇨병 쥐의 장기능과 당뇨증세에 미치는 영향)

  • The present study was conducted to evaluate the usefulness of four kinds of legumes(black soybean: BS, yellow soybean: YS, green peas: GP, soybean curd residue: SCR) as a high-fiber supplement in the therapeutic diet for diabetic patients. Seven groups of normal and streptozotocin-induced diabetic rats were fed isocaloric experimental diets containing 8% dietary fiber from one of legumes for 6 weeks. The effects of legumes on the gastrointestinal function and diabetic symptoms in diabetic rats were also examined and compared with the effects of cellulose or pectin diet. Legume supplementations had no significant effects on alleviating typical diabetic symptoms such as polydipsia, polyphasia, polyuria, and urinary glucose excretion when compared with cellulose or pectin supplementations. The supplementations of legumes all resulted in remarkable changes in gastrointestinal functions: shortening of GI transit time and increases in fecal volume, fecal water and crude fat contents. Legume supplementations appeared to supress the damages of the small intestine villi and the enteritis symptoms in diabetic rats. GP led to regular villus patterns closer to normal villus. BS, YS and GP led to the enlargement of cecum in diabetic rats. In conclusion, the present studies have demonstrated that legumes may possess a significant physiological actions in the control of various diabetic conditions.

TGF-$\alpha$, -$\beta$$_1$, and bFGF mRNA Expression of Lens Epithelial Cells in Senile and Diabetic Cataract

  • Anterior subcapsular cataract was developed by opacification with transdifferentiation and abnormal proliferation of lens epithelial cells (LECs) and pathological accumulation of extracellular matrix (ECM). After-cataract also be caused by a similar transdifferentiation of LECs remaining after surgery and the accompanying increase of ECM deposits. It is blown that prostaglandin E2 and cytokine, such as TGF-$\beta$, bFGF, and IL-1, were associated with abnormal proliferation and transdifferentiation of LECs. The aim of this study was to detect the expression of transforming growth factor-$\alpha$ (TGF-$\alpha$), transforming growth factor-$\beta_1$(TGF-$\beta_1$) and basic fibroblast growth factor (bFGF) in LECs of senile and diabetic cataract. The expressions of these growth factors in lens epithelial cells were determined. The sample for growth factor determination were collected in senile cataract patients without metabolic disorder, especially diabetes mellitus and diabetic cataract patients. The mRNA expression of growth factors was detected by semi-quantitative reverse transcription - polymerase chain reaction (RT-PCR) followed by Southern blot analysis. Statistics were analysed using Wilcoxon rank sum test. Semi-quantitative RT-PCR/southern analysis of RNA obtained from thirty surgical specimens demonstrated that the level of mRNA expression of TGF-$\alpha$, -$\beta_1$ and bFGF was increased in diabetic cataract lens tissues compared with senile cataract specimens but non-significant, bFGF and TGF-$\beta_1$ mRNA expression were detected in most patients, expression level of TGF-$\beta_1$ was most high on the basis of normal ocular concentration. Detection rate of TGF-$\alpha$ in diabetic cataract was 1.5 fold higher than in senile cataract (P=0.098). TGF-$\alpha$, TGF-$\beta_1$, and bFGF mRNA expression of LECs were detected in senile and diabetic cataract. In both patient groups, expression level of TGF-$\beta_1$, mRNA was high, so We suggest TGF-$\beta_1$ strong influence in development of senile cataract and of diabetic cataract also. TGF-$\alpha$ expression level was similar but more frequently detected in diabetic cataract than in senile cataract. In conclusion, TGF-$\alpha$ may be associated with early development of diabetic cataract.

Lower Extremity Amputations for the Diabetic Foot Complication (당뇨병성 족부 합병증에 따른 하지 절단술)

  • Purpose: To evaluate the clinical manifestations, the patterns and the functional outcome of the amputations due to the diabetic foot complications. Materials and Methods: Fifty patients (50 feet) of diabetic foot amputations were followed for more than 1 year. The mean age was 62.5 years, and the mean follow-up period was 46 months. Retrospective analysis was performed using chart review and interview with the patients. The outcome was assessed with modified AOFAS scale. Results: The diabetic foot lesions were infection in 45 feet, gangrene in 35 feet and ulcer in 15 feet. Toe amputation was most commonly performed procedure (23 cases) followed by below knee and ray amputation. Postoperative modified AOFAS score was average 51.5 points, and 94% were satisfied with outcome. Minor amputations showed better outcome than the major amputations. Conclusion: Overall postoperative functional outcome was encouraging with high patient satisfaction rate (94%). Better outcome was obtained with the minor amputations.

The Riboflavin Status in Insulin-Dependent Diabetes Mellitus During Growing Period (성장기 인슐린 의존성 당뇨병 환자의 리보플라빈 영양상태)

  • Riboflavin status of 17 insulin-dependent diabetic mellitus(IDDM) patients in growing period was evaluated as a function of energy intake and expenditure, biochemical nutritional status and diabetic control indicators. Compared with recommended dietary allowances for Koreans(RDA, 1989), only 35.3% of subjects was at good levels of all nutrients intakes and 52.9% of subjects was below normal level of height and weight. Nutrients consumed below RDA levels were energy(=88.5% of subjects), niacin(64.7%), iron(52.9%) and protein(23.5%) respectively. The riboflavin status was within normal range by urinary riboflavin excretion but 17.6% of subjects was evaluated as showed riboflavin deficiency by erythrocyte glutathione reductase activity coefficient(EGRAC). Correlation between riboflavin intake, urinary riboflavin excretion, EGRA level and diabetic duration were not statistically significant. Correlation analyses indicated that EGRA level was inversely correlated with thiamin, niacin and cabohydrate intake. No significant correlations were found between the EGRA and glycosylated hemoglobin A1(HbA1) (r=-0.464, p=0.129). From this study, it is suggested that IDDM subjects need to maintain balanced diet containing nutrients above RDA for individual activity during growing period. It needs more study whether the current recommended riboflavin allowance is adequate for diabetic patients.

  • Objective: To carry out the oriental medical treatment on a patient with vitreous hemorrhage in the left eye caused by diabetic retinopathy and record the results of the treatment. Methods: 1. Diagnosis: Fundus photography, Colored paper, Dr. Hahn's standard test chart for 5M, Blood sugar measurement. 2. Treatment: Acupuncture, Electro-Acupuncture, Indirect moxibustion, Western medicines, Oryoungsan(Crude drug preparations) Results: Oriental treatment using Ohaeng-acupuncture, Electro-Acupuncture, Indirect moxibustion resulted in the Unaided visual acuity of 0.1 while it used to be the left eye visual acuity with only light sense I month ago. Looking from Fundus photography result, progress was achieved and diabetic retinopathy was found to be in progress in fluorescein fundus angiography to right eye also by revisiting the patient after treatment. Conclusions: 1. Vitrectomy has many advantages but there are instances where patients do not recover their visual acuity due to complications. Therefore it is necessary to prove the effect oriental medical treatment through more cases in future. 2 For diabetic retinopathy patients, diabetes must be treated together with visual acuity.
