지방기질세포 치료가 당뇨섬유아세포의 증식과 교원질합성에 미치는 영향 (Effect of lipoaspirate cell autograft on proliferation and collagen synthesis of diabetic fibroblasts in vitro)

  • Purpose: Human lipoaspirate cells are relatively easy to obtain in large quantities without cell culture. The aim of this in vitro pilot study was to determine the effect of cell therapy using uncultured lipoaspirate cells on cell proliferation and collagen synthesis of diabetic fibroblasts, which are the major contributing factors in wound healing. Methods: In order to get diabetic fibroblasts, dermis tissues were obtained from foot skin of diabetic patients who underwent debridements or toe amputations(n = 4). In order to isolate lipoaspirate cells, the same diabetic patients' abdominal adipose tissues were obtained by liposuction. The diabetic fibroblasts were co - cultured with or without autogenous lipoaspirate cells using porous culture plate insert. Initial numbers of the lipoaspirate cells and diabetic fibroblasts seeded were 15,000 cells/well, respectively. For cell proliferation assay, two treatment groups were included. In group I, diabetic fibroblasts were cultured with the insert having no cells, which serves as a control. In group II, the lipoaspirate cells were added in the culture plate insert. For collagen synthesis assay, one additional group(group III), in which diabetic fibroblasts were not seeded in the well and only lipoaspirate cells inside the insert were incubated without diabetic fibroblasts, was included for a reference. Results: One hundred to one hundred sixty thousand lipoaspirate cells were isolated per ml of aspirated adipose tissue. After 3 - day incubation, the mean cell numbers in group I and II were 17,294/well and 22,163/well. The mean collagen level in group I, II, and III were 29, 41, and 2 ng/ml, respectively. These results imply that both cell proliferation and collagen synthesis in the lipoaspirate cell treatment group were 28 and 44 percents higher than in the control group, respectively(p < 0.05). Conclusion: Uncultured lipoaspirate cell autografts may stimulate the wound healing activity of diabetic fibroblasts.

  • Purpose: Oncostatin M(OSM) has been known as a role in fibrosis and anti-inflammatory effects of various organs and tissues. Although there have been a number of studies which are focused on the roles and mechanisms of OSM, there are few reports on its effects in chronic wound healing. The purpose of this study is to evaluate the effects of OSM in wound healing activities of dermal fibroblasts of chronic wound in vitro. In particular, this study is focused on cell proliferation and synthesis of collagen and glycosaminoglycan(GAG), which are the major components of the extracellular matrices, of diabetic fibroblasts. Methods: Fibroblasts were isolated from excess skin that was obtained from diabetic foot ulcer patients who underwent debridement. The isolated fibroblasts were cultivated in presence of OSM(100 ng/mL). Cell proliferation, collagen synthesis and GAG levels were compared. Results: All the components tested in this study increased in OSM treatment group. In particular, collagen and GAG synthesis demonstrated statistically significant increases(p<0.05 in the Mann-Whitney U-test). Conclusion: These results indicate that OSM increases wound healing activities of dermal fibroblasts of chronic wound in vitro.

  • Objective : Skin aging is commonly observed in patients with diabetes mellitus, which can be accessed by the amount of skin collagen and matrix metalloproteinase-1 (MMP-1). In the present study, anti-skin-aging effects of Root Cortex of Paeonia Suffruticosa Andrews (PSA), which has been widely used to treat diabetes mellitus, are investigated. Methods : Streptozotocin (STZ) was intraperitoneally injected to rats to induce diabetes. Body weights, feed intake, organ weights, blood glucose, and other biochemical index are determined in both normal and diabetic rats. In order to study the effect of PSA on skin aging, the amount of skin collagen was measured in diabetic rats after PSA treatments. Also, MMP-1 synthesis in UVB-irradiated human skin fibroblasts was investigated. Results : 1. When PSA was administered to STZ-induced diabetic rats, feed intake was significantly increased and blood glucose and total cholesterol were decreased in a dose-dependent manner. However, there are no differences in individual organ weights, GOT, and GPT. 2. A decrease of skin collagen in diabetic rats was significantly suppressed when PSA was treated. 3. PSA also inhibited MMP-1 synthesis in UVB-irradiated normal human skin fibroblasts, similar to retinoid, a well-known effective anti-skin-aging substance. Conclusion: PSA suppressed a collagen decrease in diabetic rats and inhibited MMP-1 synthesis in UVB-irradiated human skin fibroblasts. Therefore, the treatment of PSA is very effective to slow down the skin aging process.

加味托里消毒飮이 瘡傷 治癒에 미치는 影響 (The Effects of GamiTakliSodocyum on Wound Healing)

  • In order to investigate the effects of GamiTakliSodocyum(GTS) on wound healing, migration of epidermis, formation of granulation tissue and number of capillary within the granulation tissue were measured in diabetic mice by local application and NZW rabbits by local application and prescription of medicine in vivo, and proliferation of human epidermal keratinocytes and human dermal fibroblasts and composition of extracellular matrix were measured in vitro. The results were summerized as follows. 1. $2\%,\;10\%$ GTS remarkably increased migration of epidermis in diabetic mice by local application. 2. $2\%,\;10\%$ GTS remarkably increased formation of granulation tissue, number of neovascularization within the granulation tissue in diabetic mice by local application. 3. 5\%,\;10\%$ GTS remarkably increased migration of epidermis in NZW rabbits by local application. 4. $5\%,\;10\%$ GTS remarkably increased fonnation of granulation tissue, number of neovascularization within the granulation tissue in NZW rabbits by local application. 5. $5\%,\;10\%$ GTS increased migration of epidennis in NZW rabbits by prescription of medicine. 6. $5\%,\;10\%$ GTS increased formation of granulation tissue, number of neovascularization within the granulation tissue in NZW rabbits by prescription of medicine. 7. GTS didn't show effect on the proliferation of human epidermal keratinocytes. 8. GTS increased the proliferation of cultured human dermal fibroblasts. 9. GTS increased the expression of procoliagen ${\alpha}1(I) mRNA in cultured human dermal fibroblasts. 10. GTS increased the expression of fibronectin mRNA in cultured human dennal fibroblasts according to dosage of GTS using northern blot hybridization but didn't increase, using RT-PCR. From the above results, it is conclude that GTS might use on wound healing.

소갈 치료 처방 가감이 3T3-L1 Adipocytes에서 인슐린 유사성과 인슐린 민감성에 미치는 영향 (Insulin-like and Insulin Sensitizing Effects of Modified Anti-diabetic Remedies in 3T3-L1 Fibroblasts)

  • Based on the data from our previous studies, four new diabetic remedies were composed with the addition of Coicis Semen into Okchun-san (OCH), Commelinae Herba into Gangsim-tang (GST) , Scrophulariae Radix into Sunki-san (SKS), and Erythrinae Cortex into Yukmijihuang-hwan (YMG). The water extracts of these new remedies were treated in 3T3-L1 fibroblasts and adipocytes in order to investigate insulin-like substances and insulin sensitizers, respectively. With and without differentiation inducers, unmodified SKS (SKS-O) treatment induced 3T3-L1 fibroblasts into adipocytes more than the control. However, without inducers, YMG treatment, but not SKS, induced the differentiation more than the control among modified remedies. Without inducers, SKS, OCH as well as YMG increased the induction of differentiation from 3T3-L1 fibroblasts into adipocytes, compared to the control. The treatment of OCH and YMG with 1 ng/mL insulin increased glucose uptake much more than only insulin 1 ng/mL treatment. Thus, OCH and YMG contained increased insulin actions. In conclusions, the modified remedies, OCH and YMG, contained insulin-like substances and insulin sensitizers, and they can be improved the hypoglycemic effects.

Amelioration of Bleomycin-induced Pulmonary Fibrosis of Rats by an Aldose Reductase Inhibitor, Epalrestat

  • Aldose reductase (AR) is known to play a crucial role in the mediation of diabetic and cardiovascular complications. Recently, several studies have demonstrated that allergen-induced airway remodeling and ovalbumin-induced asthma is mediated by AR. Epalrestat is an aldose reductase inhibitor that is currently available for the treatment of diabetic neuropathy. Whether AR is involved in pathogenesis of pulmonary fibrosis and whether epalrestat attenuates pulmonary fibrosis remains unknown. Pulmonary fibrosis was induced by intratracheal instillation of bleomycin (5 mg/kg) in rats. Primary pulmonary fibroblasts were cultured to investigate the proliferation by BrdU incorporation method and flow cytometry. The expression of AR, TGF-${\beta}_1$, ${\alpha}$-SMA and collagen I was analyzed by immunohistochemisty, real-time PCR or western blot. In vivo, epalrestat treatment significantly ameliorated the bleomycin-mediated histological fibrosis alterations and blocked collagen deposition concomitantly with reversing bleomycin-induced expression up-regulation of TGF-${\beta}_1$, AR, ${\alpha}$-SMA and collagen I (both mRNA and protein). In vitro, epalrestat remarkably attenuated proliferation of pulmonary fibroblasts and expression of ${\alpha}$-SMA and collagen I induced by TGF-${\beta}_1$, and this inhibitory effect of epalrestat was accompanied by inhibiting AR expression. Knockdown of AR gene expression reversed TGF-${\beta}_1$-induced proliferation of fibroblasts, up-regulation of ${\alpha}$-SMA and collagen I expression. These findings suggest that AR plays an important role in bleomycin-induced pulmonary fibrosis, and epalrestat inhibited the progression of bleomycin-induced pulmonary fibrosis is mediated via inhibiting of AR expression.

자가 섬유아세포-히알루론산 복합체를 이용한 당뇨발의 치료 (Treatment of Diabetic Ulcer Using Autologous Fibroblast-Hyaluronic Acid Complex)

  • Purpose: To evaluate clinical efficacy and safety of hyaluronic acid based autologous dermal fibroblasts (Hyalograft 3D) in the treatment of diabetic foot ulcers. Methods: A total of 28 patients with diabetic ulcers were randomized to either the control group with nonadherent foam dressings(n=14) or the treatment group with autologous tissue - engineered grafts(n=14). Weekly assessment contained vital sign checks, ulcer size measurements, and wound photos. At 12th week, percentages of complete wound healing and mean healing times were compared. Safety was also monitored by adverse events. Results: Complete wound healing was achieved in 84.6% of the treatment group and 23.1% of the control group (p<0.005). The mean times of closures for the treatment versus control groups were 6.1 weeks and 10.9 weeks, respectively. No adverse events related to the study treatment occurred. Conclusion: The use of hyaluronic acid based autologous fibroblast grafts was found to be a safe and effective treatment for diabetic foot ulcers.

당뇨족 궤양의 치료를 위한 신선 섬유아세포 동종이식 (Fresh Fibroblast Allograft as a Treatment for Diabetic Foot Ulcers)

  • Purpose: In order to overcome the limitations of the conventional cryopreserved fibroblast or keratinocyte allograft method used in the treatment of diabetic foot ulcers, we reported a pilot study in 2004 demonstrating promising results of a fresh fibroblast allograft method in eight patients. However, the number of cases was insufficient for full evaluation and the follow-up duration was not long enough to determine the efficacy and safety of the method. This encouraged us to conduct this follow-up study to fully evaluate the use of noncryopreserved fresh human fibroblast allografts in treating diabetic foot ulcers. Methods: Thirty-seven patients with diabetic foot ulcers were treated using fresh fibroblast allografts. Human dermal fibroblasts from healthy teenagers were cultured in DMEM/F-12 medium supplemented with 10% serum. The cultured cells were applied on the wounds immediately following debridement, with fibrin being used as a cell carrier. In eight weeks, percentages of complete healing, mean healing time, and patient satisfactions were assessed, with follow-up time ranging from 6 to 40 months. Results: Our study showed that 83.8% of the treated patients were complete healed. The time required for complete healing was $30.9{\pm}10.1$ days. Patient satisfaction scores for the experimental treatment were higher than those for the conventional method(mean scores of $8.1{\pm}1.1$ and $4.8{\pm}1.4$, respectively). No adverse events related to the study treatment occurred. Conclusion: The use of fresh human fibroblast allografts was found to be a safe and effective treatment for diabetic foot ulcers.

Paeonol accelerates skin wound healing by regulating macrophage polarization and inflammation in diabetic rats

  • Diabetic ulcer is usually seen in people with uncontrolled blood sugar. Reportedly, many factors such as impaired glucose metabolism, and macrovascular and microvascular diseases caused angiogenesis disorders and delayed the healing of diabetic ulcers, thus affecting the body's metabolism, nutrition, and immune function. This study aimed to explore the effect of paeonol on skin wound healing in diabetic rats and the related mechanism. A rat model of diabetic ulcer was established. High glucose-treated mouse skin fibroblasts were co-cultured with M1 or M2-polarized macrophages treated with or without paeonol. H&E and Masson staining were used to reveal inflammatory cell infiltration and collagen deposition, respectively. Immunohistochemistry visualized the expression of Ki67, CD31, and vascular endothelial growth factor (VEGF). Western blot was used to detect interleukin (IL)-1β, tumor necrosis factor (TNF)-α, IL-4, IL-10, CD31, VEGFA, and collagen I/III. The expression of iNOS and arginase 1 was revealed by immunofluorescence staining. Paeonol treatment augmented collagen deposition and the expression of Ki67, CD31, VEGF, and macrophage M2 polarization markers (IL-4 and IL-10) and reduced wound area, inflammatory cell infiltration, and macrophage M1 polarization markers (IL-1β and TNF-α) in the ulcerated area. In vitro, paeonol treatment promoted M2-polarization and repressed M1-polarization in macrophages, thereby improving the repair of cell damage induced by high glucose. Paeonol accelerates the healing of diabetic ulcers by promoting M2 macrophage polarization and inhibiting M1 macrophage polarization.

Royal jelly enhances migration of human dermal fibroblasts and alters the levels of cholesterol and sphinganine in an in vitro wound healing model

  • Oral administration of royal jelly (RJ) promotes wound healing in diabetic mice. Concerns have arisen regarding the efficacy of RJ on the wound healing process of normal skin cells. In this study, a wound was created by scratching normal human dermal fibroblasts, one of the major cells involved in the wound healing process. The area was promptly treated with RJ at varying concentrations of 0.1, 1.0, or 5 mg/ml for up to 48 hrs and migration was analyzed by evaluating closure of the wound margins. Furthermore, altered levels of lipids, which were recently reported to participate in the wound healing process, were analyzed by HPTLC and HPLC. Migration of fibroblasts peaked at 24 hrs after wounding. RJ treatment significantly accelerated the migration of fibroblasts in a dose-dependent manner at 8 hrs. Although RJ also accelerated the migration of fibroblasts at both 20 hrs and 24 hrs after wounding, the efficacy was less potent than at 8 hrs. Among various lipid classes within fibroblasts, the level of cholesterol was significantly decreased at 8 hrs following administration of both 0.1 ug/ml and 5 mg/ml RJ. Despite a dose-dependent increase in sphinganines, the levels of sphingosines, ceramides, and glucosylceramides were not altered with any concentration of RJ. We demonstrated that RJ enhances the migration of fibroblasts and alters the levels of various lipids involved in the wound healing process.
