Factors Affecting Malnutrition in Hemodialysis Patients (혈액투석 환자의 영양장애 영향요인)

  • Purpose: The aim of this study was to evaluate the nutritional status and to identify factors affecting malnutrition in hemodialysis patients. Methods: Data were collected from a convenience sample of 125 hemodialysis patients who agreed to participate in the study, between August 1 and October 4, 2014. Five structured questionnaires were used: Patient-generated Subjective Global Assessment, Simplified Nutritional Appetite Questionnaire, Self-rating Depression Scale, Fatigue Scale, and Family Support Scale. Data were analyzed by descriptive statistics, t-test, Mann-Whitney U test, ${\chi}^2$-test, and stepwise multiple regression analysis by using SPSS/WIN 22.0 program. Results: Of 125 patients, 30.4% were found to be malnourished. Malnutrition was related to age, family household income, depression, fatigue, social support, appetite, and levels of C-reactive protein and serum albumin. Stepwise multiple regression analysis showed that appetite, C-reactive protein level, fatigue, and albumin level were significant factors affecting malnutrition in hemodialysis patients. Conclusion: Regular and consistent nutritional assessment is essential in hemodialysis patients. Nurses who care for hemodialysis patients need to consider the factors identified from these findings when assessing their patients' nutritional status and needs.

Educational Needs Assessment in Pediatric Nursing Handoff for Nursing Students (간호대학생을 위한 아동간호 인수인계 교육요구도 조사)

  • Purpose: This descriptive research was conducted to identify educational needs in pediatric nursing handoff training to improve students' handoff skills. Methods: Data were collected using a survey with 188 senior nursing students and 48 pediatric nursing professors and clinical nurses. The survey included items on general information as well as experiences in handoff training, necessity, training content, and items for a handoff training program in pediatric nursing. Results: Of the nursing students, 30.5% reported receiving handoff training during their clinical hours. After their handoff training, the students' confidence index was only 3.78 out of 10. Significantly, 98.3% of the respondents said that pediatric handoff training is necessary. In addition, participants reported that simulation practice (26.5%) is an appropriate educational method, and the time required for handoff training should be 8.16 hrs. Admission process was placed first as the most critical circumstance for handoff (56.8%). High demands were observed for the necessity of training content for patients with respiratory problems. Conclusion: The results of this study show the various educational needs for developing a patient safety pediatric handoff training program to promote nursing students' skills in handoff.

Blood glucose levels, insulin concentrations, and insulin resistance in healthy women and women with premenstrual syndrome: a comparative study

  • Objective: To compare the blood glucose levels, insulin concentrations, and insulin resistance during the two phases of the menstrual cycle between healthy women and patients with premenstrual syndrome (PMS). Methods: From January of 2011 to the August of 2012, a descriptive cross-sectional study was performed among students in the School of Medicine of Jahrom University of Medical Sciences. We included 30 students with the most severe symptoms of PMS and 30 age frequency-matched healthy controls. We analyzed the serum concentrations of glucose, insulin, and insulin resistance by using the glucose oxidase method, radioimmunometric assay, and homeostasis model assessment of insulin resistance equation, respectively. Results: No significant differences between the demographic data of the control and PMS groups were observed. The mean concentrations of glucose of the two study groups were significantly different during the follicular and luteal phases (p=0.011 vs. p<0.0001, respectively). The amounts of homeostasis model assessment of insulin resistance of the two study groups were significantly different in the luteal phase (p=0.0005). Conclusion: The level of blood glucose and insulin resistance was lower during the two phases of the menstrual cycle of the PMS group than that of the controls.

The Mediating Effects of Nutritional Status on the Relationship between Symptom Experience and Functional Status in Patients with Chronic Obstructive Pulmonary Disease (만성 폐쇄성 폐질환 환자의 증상경험과 기능상태의 관계에 미치는 영양상태의 매개효과)

  • Purpose: This study was conducted to explore the mediating effect of nutritional status on the relationship between symptom experience and functional status of patients with Chronic Obstructive Pulmonary Disease (COPD). Methods: A total of 141 COPD patients visiting D hospital and I hospital in B city were enrolled in this study. Data were collected from January 2017 to July 2017. Outcome variables were measured by Mini Nutritional Assessment Short-Form (MNA-SF) for nutritional status, The Memorial Symptom Assessment Scale (MSAS) for symptom experience, and The Functional Performance Inventory Short Form (FPI-SF) for functional status. The data were analyzed with descriptive statistics, Pearson's correlation, and path analysis using SPSS/WIN 21.0 and AMOS 25. Results: There was a significant negative correlation between symptom experience and nutritional status (r=-.61, p<.001), a significant negative correlation between symptom experience and functional status (r=-.40, p<.001), and significant positive correlation between nutritional status and functional status (r=.47, p<.001). Symptom experience had indirect effects on functional status through nutritional status. Conclusion: For enhancing functional status in COPD patients, it is necessary to develop nursing intervention programs to enhance symptom management as well as nutritional status.

Nurses' patient safety activities observed by nursing students (간호학과 실습생이 관찰한 간호사의 환자안전 간호활동 수행 정도)

  • Purpose: Objective of this study was to investigate how often nurses to perform patient safety activities. Methods: 521 observations were collected in 9 hospitals by 107 nursing students. Nurses' patient safety care activities were measured 0 (not at all) to 10 (all the time) scores. Descriptive statistics, independent t-test, one way ANOVA, correlation, multiple regression used to analyse data. Results: Items like 'Initial nursing assessment', 'drug management bring on', 'preparation for radiology test', 'falls assessment', 'nursing record' got high scores. But, scores of 'patient identification', 'verbal order management', 'hand hygiene' were lower than others. Each scores were different significantly according to institutions and departments. Within a same institution, the variance of scores, especially in 'patient identification', 'hand hygiene' were great. Scores of activities were different according to characteristics of institutions like type, location, number of beds, teaching hospital, number of accreditation, JCI accreditation. Predictors influencing nurses' patient safety activities were type, location and accreditation. These predictors account for 19.4% of variance. Conclusion: Performance of nurses' patient safety activities were different significantly according to characteristics of institutions. The important items like 'patient identification', 'hand hygiene' had achieved lowest performance. Further researches are needed to improve the basic safety activities.

The Administration Patterns of Sedatives for Sedation of Mechanically Ventilated Patients and Patient Assessment by Nurses (기계환기기 치료 환자들의 진정상태유지를 위한 투약양상 및 사정내용에 대한 일 조사연구)

  • Purpose: The purpose of this descriptive study was to investigate the administration patterns of sedatives and paralytic agents for mechanically ventilated patients. Method: The participants were 18 adult patients who received mechanical ventilation therapy for more than 1 day in the MICU. Collected data were type of injected medications, administration interval, injected amount, administration purpose and patient assessment by nurses. Result: Mean age of the patients was 61 years old. Seventy-two percent were male. Only 2 drugs (midazolam and norcuron) were administered to most of subjects. The type of medication administered was less heterogeneous than in previous studies. Most of the patients received sedatives and paralytics intermittently. Six different methods were used by nurses to administer sedatives and paralytics. The mean injection frequency was 9.3 times/day for sedatives and 10.8 times/day for paralytic agents. The most common purpose for administration of the drugs was full sedation of the patient (57%). Conclusion: Since a few patients received sedatives and paralytic agents higher than usual dose and a few Patients received those drugs lower than usual dose, nurses need to use proper protocols and guidelines for sedation to avoid oversedation and undersedation.

Evaluation of Mental Status in High-Risk Neonates using Infants Coma Scale (고위험신생아의 의식수준 사정을 위한 Infants Coma Scale의 개발과 적용)

  • Purpose: This was a cross sectional descriptive study to introduce the Infants Coma Scale (ICS), describe mental status of high risk infants using ICS and explore the relationships between ICS and clinical variables in infants hospitalized in a neonatal intensive care unit of a university hospital in Korea. Methods: After ICS was developed and tested by the authors, a research nurse evaluated the mental status of the infants using the English version of ICS and obtained clinical information on the infants from their medical records. Results: Data from 88 infants were analyzed. About 60% were male, 90% were preterm births, and 40% had pathologic abnormalities. Their mean gestational age was 32.4 (${\pm}3.50$) weeks and the mean birth weight was 1,842 (${\pm}728.6$) grams. The Cronbach's alpha for the ICS was .78. There was a statistically significant positive correlation between ICS total score and five clinical variables including gestational age, birth weight, 1 and 5 min Apgar scores and respiration status. Conclusion: Mental status is an important parameter in nursing assessment. ICS is a valid and reliable instrument, which clinicians can easily use to evaluate the mental status of high risk infants.

Menstrual Cycle Characteristics and Premenstrual Syndrome Prevalence Based on the Daily Record of Severity of Problems in Korean Young Adult Women

  • Purpose: This descriptive study aimed to identify the menstrual cycle characteristics and premenstrual syndrome (PMS) prevalence in Korean young adult women using the retrospective and prospective Daily Record of Severity of Problems (DRSP). Methods: In the first stage, participants included 151 nursing students studying in a university located in Seoul. Data were collected from April 20 to June 2, 2017, using the questionnaire on menstrual characteristics, pictorial blood assessment chart, and retrospective DRSP. In the second stage, participants included 17 students with PMS, based on the screening conducted in the first stage. Data were collected using the prospective DRSP from May 29 to 2 September 2, 2017. Results: Of the study sample, 104 participants (68.9%) had regular periods. Those with regular periods had 11.97 periods annually with a menstrual cycle of 29.38 days and a period duration of 5.72 days. Fifty-five participants (37.4%) showed menorrhagia. Sixty-four participants (42.4%) were found to have PMS based on their retrospective DRSP. When the ratio of women (52.9%) with PMS shown in the prospective DRSP was used as a positive predictive value, the estimated PMS prevalence was 22.4%. Conclusion: This study provides clinically significant PMS prevalence among Korean young adult women, positive predictive value of the retrospective DRSP, and valid data to basically understand the menstrual cycle characteristics experienced by these women.

Assessment of Gestational Age using New Ballard Examination in Premature Infants (미숙아에 있어 New Ballard Examination(NBE)을 이용한 재태기간 사정)

  • Purpose: Knowing the accurate gestational age(GA) is critical in nursing care of premature infants. A descriptive study was performed to examine the reliability and clinical applicability of the New Ballard examination(NBE) in premature infants. Method: A NBE was performed to measure GA by assessing the neuromuscular and physical maturity in the course of physical examination of a convenient sample of 74 premature infants. Result: 1. The highest item of NBE score was posture (mean=2.82) and the lowest item was breast (mean=1.84). 2. There was a highly correlation between both the GA by LMP(GA-LMP) and GA by NBE (GA-NBE)(r=.844, p=.000). 3. There was a greater positive relationship in neuromuscular maturity than physical maturity in the GA-NBE of the premature infants(r=.786 vs r=.933). 4. There was a positive correlation between neuromuscular, physical, total maturity, GA-LMP and GA-NBE in the birth weight, length, 1, 5 minute apgar score. 5. There was no significant difference neuromuscular, physical, total maturity in NBE by delivery history. Conclusion: The study supports the reliability a clinical relevance of NBE in assessment of the accurate GA in premature infants.

Preparatory Applications for Performance-based Regulatory System in Japanese Landscape Architecture Related Fields (일본 조경관련 분야의 성능규정화 대비방안)

  • WTO/TBT requires that, for technical regulations affecting trade, technical regulatory requirements must be specified where possible in terms of performance rather than design or descriptive characteristics. The movement which made "performance" a keyword in landscape architecture was activated in Japan, one of the leading counties in performance-based regulatory system(PBRS). The Japanese recent movement of systematization activity on performance-based standards and specifications was reviewed and operational applications for performance-based regulatory system in South Korea are summarized as follows: 1. The establishment of performance standards that can be properly evaluated by assessment indicators is necessary in cases when quantitative evaluation is difficult. 2. As a preparation for PBRS, a brief procurement system by technical proposal for the landscape design and construction is necessary. 3. As a preparation for PBRS, activation of an environmental performance evaluation on experimental construction is necessary. 4. As a preparation for PBRS, a certification system of environmental performance on various landscape construction methods is necessary. 5. The Private Finance Initiative Project is the most similar to PBRS therefore, activation of the Private Finance Initiative Project is necessary in landscape architecture projects for park rehabilitation.
