대영전가미방(大營煎加味方)으로 호전된 상열감(上熱感)과 우울(憂鬱)을 동반한 갱년기 장애 치험 3례 (The Clinical Study of 3 Menopausal Disorder Patients with Hot Flush and Depression Treated by Daeyeongjeongami-bang)

  • Objectives : The purpose of this study is to evaluate the effects of Daeyeongjeongami-bang on menopausal disorder patients. Methods : We recruited 3 menopausal disorder patients who have been suffering from hot flush, fatigue and depression. In addition, the patients had been administrated Daeyeongjeongami-bang. The patients of menopausal disorder had been estimated with Kupperman's index, Menopause rating scale, Beck's depression Inventory, Self rating depression scale, Visual analogue scale and Heart rate variability. We measured Kupperman's index, Menopause rating scale, Beck's depression inventory and Self rating depression scale for every seven days. Heart rate variability was estimated twice, before and after the treatment. Results : After the treatments, symptoms of menopausal disorder were decreased. Conclusions : This study suggests that Daeyeongjeongami-bang is significantly effective on menopausal disorder patients with hot flush, fatigue and depression.

성인여성의 수면장애와 우울에 관한 연구 (A Study on Adult Women's Sleep Disorder and Depression)

  • Purpose: This study is to study sleep disorder, sleep pattern, and depression in adult women, and to offer basic data for a program of the improvement of women's health and nursing intervention. Method: A structured questionnaire was carried out from September 1, 2001 to October 30, 2001 on the subject of 441 females, who are above 18 years old and lived in Seoul. This research tool measured insomnia with APA's insomnia scale(1994), sleep pattern and sleep disorder factors scale(Shin et al(1999)'s), and depression with Radloff's CES-D scale (1977). The data were analyzed with a SPSS program for descriptive statistics, ${\chi}^2-test$, and t-test. Result: The result were as follows; 1) 35.1% of all participants have insomnia. 2) Sleep disorder group appeared shorter($50.98{\pm}29.41min$.) than normal group($73.03{\pm}38.7min$.) in daytime sleepiness(p=0.001). 3) Factors of sleep disorder were stress(76.6%), worry(55.6%), without reason(39.6%), and noise outside(37.4%). 4) In relation between sleep disorder and depression, group with sleep disorder($20.20{\pm}11.06$) marked higher depression score than normal group($14.25{\pm}8.81$)(p<0.0001). Conclusion: Based upon these finding, sleep disorder in adult women was influenced by psychological factors, group with sleep disorder showed a higher depression score than normal group, and which indicated correlation with sleep disorder and depression. This study shows that nursing intervention on women's sleep disorder and depression is needed and further research be done to verify the results.

기분부전장애와 비전형우울증의 약물치료 (Pharmacological Treatments for Dysthymic Disorder and Atypical Depression)

  • Objectives : The diagnosis of depression is based on a highly variable set of symptoms. Therefore, depression should not be viewed as a single disease, but a heterogenous syndrome comprised of different pathophysiologies. There are several subtypes of depression which were already incorporated in DSM-IV. This article provides a systematic review of pharmacological treatments of two recognized subtypes of depression-dysthymic disorder and atypical depression. Methods : Systematic search of relevant literatures on dysthymic disorder and atypical depression was performed by proposed search strategy of the Clinical Research Center for Depression of Korean Health 21 R&D Project. All identified literatures were carefully reviewed and classified according to SIGN grading system and summarized in a narrative manner. Results : For the treatment of dysthymic disorder and atypical depression, selective serotonin reuptake inhibitors( SSRIs) and moclobemide have more evidence than the other antidepressants. SSRIs and moclobemide showed superior tolerability than tricyclic antidepressants. Conclusions : The authors proposed treatment recommendations for dysthymic disorder and atypical depression by the methods of evidence-based medicine(EBM). However, guideline developing methods of EBM also have several inevitable limitations. Therefore, in the absence of clear and significant differences in efficacy, the choice of medication must be individualized for a particular patient based on psychiatrist's own clinical decision.

노인성우울증에 대한 한방 집중치료 프로그램의 효과 (Effects of Korean Medical Intensive Treatment Program on Geriatric Depression Disorder)

  • Objectives: As the elderly population grows and the prevalence of depression increases, the incidence of geriatric depression disorder will continue to increase. The effect of an intensive Korean medicine treatment program on geriatric depression disorder was studied. Methods: Geriatric depression disorder patients who received treatment over a period of three years were reviewed, with the geriatric depression score (GDS) used as the primary outcome. The intensive Korean medical treatment program applied to the geriatric depression disorder patients consisted of herbal medicine, acupuncture, pharmacopuncture, moxibustion, meditation, qigong exercises, and reminiscence therapy. Results: The GDS scores of patients who received the intensive Korean medical treatment program decreased from an average of 19.57 (${\pm}5.52$) to 15.76 (${\pm}6.99$) and there was a significant difference. Conclusions: When the intensive Korean medical treatment program was applied to geriatric depression disorder patients, the symptoms of depression improved. This treatment program will be helpful for improving symptoms in patients and can be applied by trained clinicians.

혈액투석환자의 수면장애에 영향을 미치는 요인 (Study on the Factors Affecting the Sleep Disorder of Hemodialysis Patients)

  • Purpose: The purpose of this study is to investigate the degree of sleep disorder and its related factors in hemodialysis patients. Method: The subjects of this study consisted of 210 hemodialysis patients. Data were analyzed by descriptive statistics, t-test, ANOVA, pearson's correlation coefficient, or stepwise multiple regression analysis by using SPSS/PC+ program. Result: Sleep disorder degree averaged 2.24. Stress of hemodialysis patients averaged 2.52. Depression degree of hemodialysis patients averaged 1.14. There was significant difference in sleep disorder according to age, marriage status, education, family income and help-givers. Sleep disorder was significantly associated with stress and depression. Stress, depression and marriage status explained 31.3% of the variance in the sleep disorder. Conclusion: In conclusion, the sleep disorder of hemodialysis patients is the nursing problem. The sleep disorder degree relates with stress, depression. Especially, it is strongly affected by stress among them.

The Relationship Between Eating Disorders, Body Image, Depression and Self-Esteem among College Women

  • This study was conducted to investigate the relationship between an eating disorder, body image, depression and self-esteem amongst a subject group of 282 college women between Jan.,1st and 30th,2002, to provide base data for eating disorder levels of college women and to provide base data for health control. The evaluative instrument was a constructive questionnaire that consisted of 8 items of general characteristics and weight control questions, 24 questions relating to eating disorders, 17 questions pertaining to body image, 10 questions pertaining to depression and 10 questions pertaining to self-esteem. To identify the levels of an eating disorder, body image, depression and self-esteem perceived by subjects, the researcher used means and SDs. To test the significance of a subject's eating disorder, body image, depression and self-esteem according to the BMI, the researcher used the t-test. To test the correlation between the BMI, an eating disorder, body image, depression and self-esteem, the researcher used Pearson correlation coefficient. The results of this study are as follows. The mean score for anorexia nervosa was 31.52 and the mean score for bulimia nervosa was 24.12. Differences between the underweight group and the normal weight group in the level of eating disorders were significant (t=-6.94, p=.000). There were high positive correlations between the BMI and an eating disorder (r=.383, p<.01), between an eating disorder and depression (r=.161, p<.01), between body image and self-esteem (r=.653, p<.01). In addition, there were high negative correlations between an eating disorder and body image (r=-.214, p<01), between an eating disorder and self-esteem (r=-.196, p<.01), between depression and body image (r=-.541, p<.01), and between depression and self-esteem (r=-.537, p<.01). These results indicate that Korean college women need more education and counseling on dietary habits. Also, the systemic efforts reestablish the social standard of the beauty should be taken. Further empirical and experimental studies would be required for investigating for the factors influencing the eating attitude of the college women and discriminating the variables affecting the various specific dimensions of the eating attitude.

일 대학 신입생들의 기분장애 유병률과 위험요인 (Prevalence and Risk Factors of Mood Disorders among One University Freshmen)

  • Purpose: Mood disorders such as depression and bipolar disorder are a major mental health problem in college students. We investigate the prevalence of depression and bipolar disorder and the relevance of risk factors for these mood disorders among one college freshmen. Methods: The subjects were 2,865 college students who entered one university located in Seoul and Ansung in 2009. We used BDI (Beck Depression Inventory) for depression assessment and K-MDQ (Mood Disorder Questionnaire) for bipolar disorder assessment. Demographic and socioeconomic factors were measured by questionnaire. Height, weight, blood pressure, total cholesterol, complete blood cell count, and liver function test data were obtained by physical examination for freshmen. Chi-square test and multiple logistic regression were performed to analyze the possible risk factors for depression and bipolar disorder. Results: With different BDI cutoff value, 16 and 21, the prevalence of depression was 8.7% (male: 7.6%, female: 10.1%) and 2.4% (male: 2.5%, female: 2.3%), separately. 'Low economic status', 'urban birth place', and 'low grade at entrance' were significantly associated with depression. Using the original cutoff criterion, defined as clustering of 7 or more symptoms that caused moderate or severe problems, the prevalence of bipolar disorder was 1.3% (male: 1.4%, female: 1.1%). The risk factor of bipolar disorder was academic fields (art fields). Conclusion: Depression and bipolar disorder are common disease in college freshmen. Therefore, Campus-based mental health service program is needed to help with prevention of and early intervention of these mood disorders.

  • Objectives Internet gaming disorder (IGD) is associated with attention deficit/hyperactivity disorder (ADHD) or depression etc. We tried to examine the mediating effects of depression in the relationship between ADHD and IGD. Methods This study was conducted on 2000 people who participated in on-line survey among 14-39 year-old men and women in 2017. And we clarified the relationship among IGD, ADHD, and depression and tested the mediation model. The mean and standard deviation of the main variables were calculated and correlation analysis was performed to confirm the relationship among the main variables. In order to test the mediating effect of depression on the relationship between ADHD and IGD, the structural equation model was implemented using AMOS 21 (IBM). Results There were significant correlations among the variables; IGD, ADHD and depression. Depression had a mediating effect 0.23 (95% confidence interval : 0.17-0.28) in the relationship between IGD and ADHD. Conclusions This study showed that depression can mediate ADHD and IGD. Therefore, the evaluation and management of depression and ADHD should be included in the diagnosis and treatment of IGD.

한방치료와 호흡명상으로 호전된 불면 및 우울증을 동반한 불안 장애 환자 치험 1례 (Clinical Report of One Case with Insomnia, Depression and Anxiety Disorder Improved by Traditional Korean Medical Treatment and Breathing Meditation)

  • Objectives : Insomnia, depression and anxiety disorder are widespread in elderly patients. Mood disorder and insomnia share complex theoretical and clinical relationships. We investigated if traditional Korean medical treatment and breathing meditation are effective in patient with insomnia, depression and anxiety disorder. Methods : We treated one patient with traditional Korean medical treatment such as accupuncture, herb-medication, moxa and breathing meditation Results : After Treatment, the chief complaint and accompanying symptoms were improved, and the score of BAI, SADS, BDI, PSQI, IQ were decresed. Conclusions : According to this study traditional Korean medical treatment is effective for the cure of insomnia, depression and anxiety disorder and breathing meditation forifies the traditional Korean medical treatment.

The Association between Attention Deficit/Hyperactivity Disorder Symptoms and the Number of Suicide Attempts among Male Young Adults with Unipolar and Bipolar Depression

  • Objectives: Mood disorder is highly comorbid with attention deficit/hyperactivity disorder (ADHD), and comorbid ADHD symptoms are associated with suicide risk. The aim of this study was to examine the association between comorbid ADHD symptoms and the number of suicide attempts among patients with unipolar and bipolar depression. Methods: In this cross-sectional study, 100 patients with either unipolar or bipolar depression constituted the final sample. Pearson's correlation and multiple regression analyses were conducted to examine the association between comorbid ADHD symptoms and suicide risk. Results: Among patients with bipolar depression, the number of suicide attempts was significantly correlated with ADHD symptoms (r=0.324, p<0.01). ADHD symptoms significantly predicted the number of suicide attempts (β=0.249, p<0.05). Conclusion: Our findings underscore the importance of screening and assessing ADHD symptoms in male young adults with depressive disorders.
