Even though an immediate denture (ID) is a practical prosthesis, fabricating an ID may be challenging, as unexpected removals of periodontally compromised teeth may occur during an impression procedure. This clinical report introduces a digital approach to a maxillary ID. An intraoral scanner was applied to prevent accidental extraction. A physical cast and a resin pattern of a framework were fabricated with rapid prototyping technology. A proper border and retention was also achieved by an altered cast impression.

Construction of an obturator for rehabilitation of a patient who underwent a maxillectomy is vital. Routinely a constructed obturator includes denture portion. A patient who may present anatomical limitations due to surgical or radiotherapy complications often challenges the clinician. Purpose: This clinical report describes a patient with severe trismus after surgical resection and radiotherapy treatment of a tumor in the upper left maxilla. Conclusion: This report describes the concepts of using a rotational path insertion for an obturator and a separately constructed maxillary denture. The stability and retention of the obturator were obtained from anatomical features. Where as these were achieved through magnetic attachments and the remaining edentulous ridge to ensure esthetic and function of the prosthesis.

Proponents of bilaterally balanced occlusion believe that this occlusal concept is important for denture retention and stability. However, the critical appraisal of the literature reveals no scientific evidence to support the bilaterally balanced occlusion as the ideal occlusal concept for conventional complete dentures. Current evidence suggests that the occlusal concept has little influence on clinical outcomes or patient satisfaction. Additional randomized controlled clinical trials should be developed taking into account the influence of mucosal resiliency, alveolar ridge anatomy, and parafunctional activities on occlusal concept choice. In this paper, various types of complete denture occlusion (bilaterally balanced occlusion, lingualized occlusion, canine-guided occlusion) will be reviewed and compared.

본 증례는 구순구개열 환자에서 상순의 반흔, 상악 잔존치조제의 과도한 흡수, 협소한 구개 및 상악골의 열성장을 동반한 3급 악간관계로 인해 통상의 가철성 국소의치 수복에 한계를 보이기 때문에 상악 무치악 부위에 4개의 임플란트를 식립 후, CAD/CAM 시스템으로 밀링한 titanium bar를 이용한 상악 가철성 임플란트 피개 국소의치를 계획하였다. 통상의 주조방법은 주조 후 수축을 하게 되지만 CAD/CAM 시스템으로 밀링한 titanium bar는 높은 정밀성과 경제성 그리고 가벼운 장점이 있다. 하지만 실제 임상에서 milled bar는 아주 균등하고 정확한 마찰결합을 얻기 힘들며 의치의 반복적인 착탈에 의해 마찰 유지력이 감소 할 수 있다. 이에 milled bar와 함께 사용할 수 있는 다양한 보조 유지장치(e.g. friction pin, $ERA^{(R)}$, $CEKA^{(R)}$, magnetics, $Locator^{(R)}$ 등)를 같이 사용함으로써 부가적인 유지력, 경제성 및 교체의 용이성 등을 추가적으로 얻을 수 있다. 본 증례에서 milled titanium bar의 상부면에 drill and tapping 방법으로 $Locator^{(R)}$ 유지장치를 연결하였다. milled bar에 screw hole을 형성하고 3개의 $Locator^{(R)}$ 유지장치를 20 Ncm으로 체결하여 추가적인 유지력을 확보하였다. 보철수복 후, 환자가 원하는 심미적인 안모, 저작 기능 및 의치 유지에 만족할 만한 결과를 얻었기에 이를 보고하고자 한다.

이중관 의치는 지대치 장축 방향으로의 교합력 전달, 구강위생관리 용이, 유지 및 안정의 증가, 지대치 간의 2차적인 연결 고정으로 인한 부목효과 등의 장점을 가지며 일반적인 국소의치에 비해 높은 성공률이 보고되고 있으나 기공과정이 복잡하고 치료기간 및 비용이 증가하며 합병증의 발생이 많은 단점이 있다. 본 증례보고는 기능적으로 만족스러운 결과를 얻은 상악의 4점 지지의 텔레스코프관을 이용한 이중관 의치를 제작한 증례와 상악의 2점 지지의 원추관을 이용한 이중관 의치를 제작한 증례를 보고하고자 한다. 여러 문헌을 통하여 보고되는 이중관 의치의 합병증을 고찰하고, 이와 관련하여 임상 및 기공과정에서 주의할 점을 고려하여 치료를 진행하였다.

심미와 기능이 조화를 이루는 총의치 제작을 위해서는 상하악 관계를 정확히 기록하는 것이 필수적이다. 고딕아치 묘기법은 하악의 운동을 시각적으로 보여주며, 정확하고 반복재현성 있는 중심위를 확립하는데 유용하다. 또한, 고도의 치조제 흡수를 갖는 환자에서는 총의치의 적절한 유지 및 안정을 얻기가 어려운데, 이런 경우 환자의 생리적인 동작으로 변연을 형성하여 의치상연 주위가 가동점막으로 봉쇄되도록 하는 폐구 인상법이 유용할 수 있다. 본 증례에서는 우측 하악과두골절 병력과 고도의 하악 치조제 흡수를 보이는 환자에서 폐구인상법과 고딕아치 묘기법을 이용한 총의치로 수복하였고 만족할 만한 결과를 얻어 보고하는 바이다.

본 증례는 상악골 결손이 있는 무치악 환자에서 3D printing을 이용해 closed hollow bulb obturator로 수복한 증례이다. Magic $denture^{TM}$ 시스템(Cozahn, Seoul, Korea)에서 제공하는 트레이와 인상법을 이용하여 구내 인상을 채득하였고, 시적의치상에서 수직고경과 안모, 유지력 등을 확인하였다. 환자가 요구하는 바와 오차를 시적의치에서 수정한 후 이를 반영하였다. 의치의 무게를 줄이고 균일한 두께를 부여하기 위해 상악골 결손부위를 closed hollow bulb로 디자인하여 최종의치상을 프린팅하였다. 치료 후 심미적 및 기능적으로 만족스러운 결과를 얻었기에 이를 보고하는 바이다.

다수의 치아 상실은 교합의 부조화를 일으키며, 수직고경 상실과 하악 운동의 변위를 유발할 수 있기 때문에 중심위에서 적절한 수직 고경과 교합을 재형성하는 것이 중요한 치료 목표가 될 수 있다. 한편, 임플란트 보조 국소의치는 국소의치의 유지, 지지 및 안정을 증가시켜 환자의 만족도를 높이기 때문에 최근에 자주 사용되는 임상 술식이다. 본 증례는 편측 구치부 및 전치부 지지의 소실로 편측 교합면간 거리가 소실된 환자에서 치료 의치를 제작하고 기존에 식립된 양측의 임플란트를 지대치로 이용한 임플란트 보조 국소의치를 제작하였고 양호한 임상 결과를 확인하였기에 이를 보고하고자 한다.

가철성 보철물에서는 교합을 어떻게 형성하느냐에 따라 의치에 가해지는 힘의 크기가 결정되고 그에 따라 의치의 안정에도 영향을 미친다. 즉, 의치에 가해지는 외력이 의치의 유지력보다 크면 의치가 불안정해지지만 가해지는 이탈력을 줄여서 의치의 유지력보다 작아지도록 하면 의치의 안정을 얻을 수 있다. 그러므로 주의 깊은 검사와 진단과정을 통해 환자에게 적절한 교합을 설계하는 것은 의치 성공의 중요한 기준인 안정에 필수적이다.

Sometimes in clinic, we have found that cast crown construction is necessary to fit the existing partial denture clasp, when the abutment is involved with caries, a failing restoration, periodontal disease, or fracture. This is important not only to protect the abutment tooth from missing, but to restore retention, bracing and support for partial denture. For constructing the cast crown, several methods have been reported by many of previous authors. The number of techniques reported during the past are ideal but have uncertain results However, with more improved and practical technic developed by me, the problem becomes more successful. One method in which does not need the laboratory technecian is the wax pattern has been made in chairside waxing by doctor himself. The other in which requires the laboratory technician is to use special impression method and articulator for laboratory waxing. In one case of the reported here, we have successful results in restoring the abutment with cast crown for original denture clasp.
