Rehabilitation using mandibular implant overdenture with CAD/CAM milled bar: A case report (CAD/CAM 시스템으로 제작한 Milled Bar를 이용한 하악 임플란트 피개의치 수복증례)

  • Long-term alveolar bone resorption in edentulous patient causes difficulty in denture use. Applying an implant overdenture with 2 to 4 implants to edentulous patient is easily approachable. Moreover, it improves denture stability, support, and retention. Milled bar, the attachment used in implant overdenture, can be used to induce better stability and retention to the supporting structure than conventional bar. It has become convenient to use due to the development of CAD/CAM system which had allowed the simplification of dental techniques. In this case, application of conventional maxillary full denture and mandibular overdenture made of CAD/CAM milled bar with 4 implants showed satisfactory results in the patient who had used upper and lower full dentures for a long time.

  • The purpose of this study was to evaluate a cases which was treated with magnetic attachments on the patients in edentulous mandible and maxilla. Author made magnetic overdenture for two patients and then clinical examination were used for evaluation of the function and change in retention during 30months. Patients showed difficulty in oral hygiene care at the beginning but they go used to it soon. It was effective in promotion of denture retention and stability and also gave patient emotionally uplifting psychologic state. This study suggest that magnetic attachment was effective for the treatment of patients presenting a few remaining teeth or teeth under severe caries or mobility.

  • Purpose: Prosthodontics for edentulous patients is a treatment technique using implant, which has impactful results in retention and support effects. Methods: As a retention technique, SCRP (screw and cement retained implant prosthesis) has reported in many studies as a beneficial method for both patients and curers, which can reduce errors in process of making abutment and top implant. Results: Prosthesis manufacturing, as polymerization method of hardened resin teeth with thermoplastic resin, is helpful for patients with aesthetic and financial situations regarding residual ridge and interocclusal relationship, also indicates reliable results in both retention and care. Conclusion: Using SCRP technique, we notably obtained a clinical and aesthetic outcome from five implants in anterior tooth, which are half fixable and detachable implants on screw of implant abutment by the technicians at anytime.

A STUDY ON THE CHANGES IN RETENTION OF CLIPS USED TO RETAIN IMPLANT-SUPPORTED OVERDENTURE (임플랜트지지 overdenture용 clip의 유지력 변화에 관한 연구)

  • Implant-supported overdenture is known as a useful appliance, instead of using the conventional complete denture, for better retention and stability. In this study 4 types of materials such as, gold bar/plastic clip(group AuP), gold bar/metal clip(group AuM), palladium bar/plastic clip(group PdP), and palladium bar/metal clip (group PdM) were used to evaluate the retention forces according the type of clips and alloys used for bar fabrication, in the Hader bar system. Repeated insertions and removals of overdenture were conducted in each group. and the retention forces were measured and compared the data of each group according to the number of insertion and removal. The obtained results were as follows, 1. In the comparison of retention forces according to type of bar-clip, retention was increased in the order of group AuM, PdM, PdP AuP. and the retention force of group AuM was significantly increased compared with those of others (p<0.05). 2. In the comparison of retention forces according to the number of insertion, only group PdP showed significant decrease in retention(p<0.05). 3. In the comparison of retention forces according to the type of bar and clip. there was no significant difference in the type of bar, but the retention of plastic clip was significantly higher than that of metal clip when Au bar was used(p<0.05). 4. In the observation of the bar surface, group AuM using Au bar and metal clip showed the most scratches among bar groups.

Implant-assisted removable partial denture using Milled-Bar and Attachment in partially edentulous maxilla: A case report (상악 부분 무치악 환자에서 Milled-bar와 어태치먼트를 이용한 임플란트 부분 피개의치 수복 증례 보고)

  • For treatment of partially edentulous patients, a treatment using implant is widely used. Treatment method using implant are implant fixed prostheses and removable partial dentures, and for patients with severe bone resorption, removable implant overdenture with the effects of aesthetic and reducing cost can be used as treatment options. Specially, prosthesis with milled-bar and attachment has the effect of being splinted between implant fixtures, higher retention and stability than conventional removable partial denture. And it has the effect of improvement of aesthetic through lip support by denture base. In this case, the patient with severe alveolar bone resorption and partial edentulous maxilla and mandible was treated by implant-assisted removable partial denture using Milled-bar and ADD-TOC attachment. The esthetic was improved by removing the clasp because of effects of additional retention by using the attachment, and reducing palatal coverage of implant-assisted removable partial denture. The clinical results were satisfactory on the aspect of aesthetic and masticatory function.

  • The neutral zone technique is an alternative approach for the construction of complete dentures on highly atrophic ridges with history of denture instability. This technique achieves two objectives. First, the teeth will not interfere with the normal muscle function, and second, the force exerted by the musculature against the denture is more favorable for stability and retention. In this case of a 78-years-old female patient with severely atrophic ridges who had been using unstable complete dentures, the neutral zone impression technique was used to increase the stability and the retention of dentures. The neutral zone for both arch was located with modeling compound. After the arrangement of artificial teeth within the neutral zone, the external impression was taken to determine the tissue surface. The final dentures showed enhanced stability and retention and the patient was satisfied with the new dentures with respect to functional and esthetic aspects.

Complete denture rehabilitation of fully edentulous patient with severe bone resorption and class II jaw relation using piezography (심한 골 흡수와 2급 악간관계를 보이는 완전 무치악 환자의 Piezography를 이용한 총의치 수복)

  • Piezography, prosthetic space recorded by pronunciation, can be used as a reference for arrangement of artificial teeth and polishing surface of a denture. In this case, a 67 year old female patient was presented for new dentures. Old dentures had class II relationship and poor retention. For fabrication of stable dentures, using piezography and lingualized occlusion was planned. After taking impressions with conventional method, conventional denture bases with wax rim were fabricated. Then, additional mandibular denture base was fabricated for piezography. With fast setting silicon impression material, piezography was recorded by using six pronunciations, 'si', 'so', 'me', 'te', 'de', and 'mu'. According to the piezographic space, mandibular artificial teeth were arranged and modified for lingualized occlusion. As a result, the patient was satisfied with new dentures functionally and esthetically.

  • In recent years, digital technology has been developed in dentistry, which denture frameworks can be manufactured using DMLS (Direct Metal Laser Sintering) technique. A traditional impression method can be replaced by oral scanning and wax pattern production process can be achieved by the use of CAD/CAM techniques. The designed STL files can be sent to DMLS devices to fabricate final components of removable partial dentures (RPD). The advantages of digital dentistry are concision and precision. In this case study, a fracture of occlusal rests providing support and indirect retention was repaired by DMLS and laser welding techniques. It shows satisfactory results in adaptation accuracy and functional properties of the repaired denture.

Functional impression technique using temporary denture for rehabilitation of severely atrophic maxillary and mandibular ridges (심한 치조제 흡수를 보이는 무치악 환자에서 임시 의치를 사용한 기능 인상에 의한 총의치 수복 증례)

  • Soft liner is used to functional impression technique when dental stone is immediately poured after taking impression because of viscoelasticity. In this case, a 78-year-old male visited for new dentures. Due to severe resorption of mandibular edentulous ridge, functional impression taking by closed mouth technique was planned. First of all, making maxillary and mandibular provisional dentures was done, and lined by soft liner to rehabilitate pressured maxillary and mandibular edentulous ridge. After this, Functional impression was taken by closed mouth technique using provisional dentures which are transformed to healed maxillary and mandibular edentulous ridge, and final denture were fabricated using maxillary provisional denture as a reference of artificial teeth arrangement. Consequently, restoring a complete edentulous patient with taking functional impression using provisional dentures resulted in recovering satisfying retention and function.

Changes in periodontal tissue and denture around the implants in the mandibular milled-bar implant overdenture: A 12-year follow-up (하악 Milled Bar 임플란트 피개의치에서 12년 간의 임플란트 주변 치주조직 및 의치의 변화)

  • The mandible has a smaller support area than the maxilla, and in particular, in patients with generalized alveolar bone resorption, the stability of the denture and the masticatory efficiency are significantly low in the case of conventional complete denture, due to the movement of the tongue and mandible. In these patients, implant overdenture is evaluated as a highly predictable treatment method with high retention and stability, and excellent pronunciation and masticatory force. In this clinical case, patient had an edentulous maxilla and generalized alveolar bone resorption in mandible. Therefore, complete denture was placed in the maxilla and implant overdenture using milled bar attachment on 4 implants was placed in the mandible. During the 12-year follow-up period, changes in periodontal tissue and denture around the implants were periodically managed and observed.
