Effect of chlorhexidine application on the bond strength of resin core to axial dentin in endodontic cavity

  • Objectives: This study evaluated the influence of chlorhexidine (CHX) on the microtensile bonds strength (${\mu}TBS$) of resin core with two adhesive systems to dentin in endodontic cavities. Materials and Methods: Flat dentinal surfaces in 40 molar endodontic cavities were treated with self-etch adhesive system, Contax (DMG) and total-etch adhesive system, Adper Single Bond 2 (3M ESPE) after the following surface treatments: (1) Priming only (Contax), (2) CHX for 15 sec + rinsing + priming (Contax), (3) Etching with priming (Adper Single Bond 2), (4) Etching + CHX for 15 sec + rinsing + priming (Adper Single Bond 2). Resin composite build-ups were made with LuxaCore (DMG) using a bulk method and polymerized for 40 sec. For each condition, half of specimens were submitted to ${\mu}TBS$ after 24 hr storage and half of them were submitted to thermocycling of 10,000 cycles between $5^{\circ}C$ and $55^{\circ}C$ before testing. The data were analyzed using ANOVA and independent t-test at a significance level of 95%. Results: CHX pre-treatment did not affect the bond strength of specimens tested at the immediate testing period, regardless of dentin surface treatments. However, after 10,000 thermocycling, all groups showed reduced bond strength. The amount of reduction was greater in groups without CHX treatments than groups with CHX treatment. These characteristics were the same in both self-etch adhesive system and total-etch adhesive system. Conclusions: 2% CHX application for 15 sec proved to alleviate the decrease of bond strength of dentin bonding systems. No significant difference was shown in ${\mu}TBS$ between total-etching system and self-etching system.

The effect of a desensitizer and $CO_2$ laser irradiation on bond performance between eroded dentin and resin composite

  • PURPOSE. This study was aimed to evaluate effect of the desensitizing pretreatments on the micro-tensile bond strengths (${\mu}TBS$) to eroded dentin and sound dentin. MATERIALS AND METHODS. Forty-two extracted molars were prepared to form a flat dentin surface, and then they were divided into two groups. Group I was stored in distilled water while group II was subjected to a pH cycling. Each group was then subdivided into three subgroups according to desensitizing pretreatment used: a) pretreatment with desensitizer (Gluma); b) pretreatment with $CO_2$ Laser (Ultra Dream Pluse); c) without any pretreatment. All prepared surfaces were bonded with Single Bond 2 and built up with resin composite (Filtek Z250). The micro-tensile bond test was performed. Fracture modes were evaluated by stereomicroscopy. Pretreated surfaces and bonded interfaces were characterized by scanning electron microscope (SEM). The data obtained was analyzed by two-way ANOVA (${\alpha}$=0.05). RESULTS. For both sound and eroded dentin, samples treated with desensitizer showed the greatest ${\mu}TBS$, followed by samples without any treatment. And samples treated with $CO_2$ laser showed the lowest ${\mu}TBS$. SEM study indicated that teeth with eroded dentin appeared prone to debonding, as demonstrated by existence of large gaps between adhesive layers and dentin. CONCLUSION. Pretreatment with Gluma increased the ${\mu}TBS$ of Single Bond 2 for eroded and sound teeth. $CO_2$ laser irradiation weakened bond performance for sound teeth but had no effect on eroded teeth.

치근단절제시 노출된 상아세관의 밀폐효과에 대한 Nd : YAG 레이저의 효과 (THE EFFECT OF ND : YAG LASER ON DENTINAL TUBULE SEAL AFTER ROOT END RESECTION)

  • If root and resection is done during surgical endodontic treatment, newly exposed dentinal tubules form pathways between the canal and the peripheral tissue. Nd : YAG laser was used to block this phenomenon, and its effect was studied with dye penetration and SEM techniques. 40 intact single rooted teeth were divided into 4 groups(10 each) : control group and test groups, in which retrograde cavity surface, cutting surface, retrograde cavity surface & cutting surface were treated with laser(1 watt 15pps) and finally retrograde filling with IRM was conducted. After that, they were stained with 2 % methylene blue, sectioned and evaluated by the maximum infiltration depth. And to observe surface change, they were prepared for SEM. The results were as follows ; 1. All experimental groups showed microleakage with variation in amount. 2. The 2nd group which treated both the retrograde cavity and cutting surface showed significantly less microleakage than the other groups(p<0.05). There was no significant difference between groups treated on one side only. 3. As a result of SEM observation of dentin surface, obstruction of dentinal tubules with marble shaped granules, which were different from normal dentin could be seen. Cracks could be seen also. 4. In summary of this experiment, it is thought that effort to obstruct the exposed dentinal tubules as well as retrograde cavity after root end resection is needed.

CaridexTM Caries Removal System이 우식상아질 제거에 미치는 영향 (AN EXPERIMENTAL STUDY OF THE EFFECT OF CARIDEXTM CARIES REMOVAL SYSTEM ON THE REMOVAL OF CARIOUS DENTIN)

  • In order to determine if GK-101E(N-monochloro-DL-2-aminobutyrate) solution was effective in the removal of carious dentin, this study was conducted on 75 extracted human carious teeth. GK-101E solution was compared in effectiveness to saline solution.75 extracted human carious teeth were randomly assigned to two groups accordingly : (1) 50 teeth were treated with GK-101E solution : and (2) 25 teeth were treated with saline solution. Teeth in each group were divided into the medium or medium-hard consistency. Treatment consisted of directing the pulsating stream of each coded test solution utilizing the Caridex$^{TM}$ Caries Removal System against the carious surface for 4 mimutes. One operator treated all teeth with corded solution, and four investigators scored on the treated teeth based on visual examination and tactile evaluation with a dental explorer in conjunction with staining method with Caries Detector$^{TM}$ The results were as follows : 1. All groups showed various effect of carious dentin removal. 2. GK-101E solution was statistically superior to physiologic saline solution in the removal of carious dentin, regardless of carious consistency. 3. GK-101E solution in the medium-hard consistency was statistically superior to that in the medium consistency in the caries removal(p=0.001). 4. Saline solution in the medium consistency was equivalent to that in the medium-hard consistency in caries removal(p>0.1).

유치 상아질에 대한 수종의 상아질 결합제의 전단결합강도에 대한 연구 (THE STUDY ON SHEAR BOND STRENGTH OF VARIOUS DENTIN BONDING SYSTEMS IN PRIMARY DENTIN)

  • 소아환자의 수복치료에서 시술시간과 시술과정을 줄이는 것은 중요한 점이다. 복합레진은 물성의 증가로 영구치, 유치 전치부 심미수복재로써 뿐만 아니라 구치부 수복에도 많이 쓰이고 있다. 최근에 개발된 7세대 상아질 결합제는 기존의 복합레진을 치면에 부착하기 위해 필요했던 산부식, 프라이밍, 접착제 도포의 복잡한 과정을 한번의 처리로 단순화하였다. 본 연구에서는 유치 상아질에서 7세대 상아질 결합제의 결합력을 기존에 사용되고 있는 여러 결합제의 결합력과 비교하였다. 1, 2, 3, 4군은 각각 4, 5, 6, 7세대 상아질 결합제를 사용하였다. 유치 상아질을 노출시키고 군에 따라 상아질 면을 처리한 후에 직경 2.5mm, 높이 2.0mm의 주형을 이용하여 복합레진을 접착시키고 열순환을 시행한 다음 전단결합강도를 측정하여 다음과 같은 결론을 얻었다. 1. 5세대 상아질 결합제인 Clearfil SE Bond가 가장 높은 전단결합강도인 $27.87{\pm}5.33\;MPa$를 보였다. 이것은 4, 6, 7세대와 비교했을 때 통계학적으로 유의하였다. 2. 4세대 상아질 결합제의 전단결합강토는 $20.11{\pm}5.73\;MPa$로 6, 7세대보다는 약간 높게 나타났지만 통계적으로 유의한 차이는 보이지 않았다. 3. 6, 7세대 상아질 결합제의 전단결합강도는 유사하게 나타났다.

Effects of applying antioxidants on bond strength of bleached bovine dentin

  • Objectives: Some antioxidants are believed to restore dentin bond strength after dental bleaching. This study was done to evaluate the influence of antioxidants on the bond strength of bleached bovine dentin. Materials and Methods: Thirty incisors were randomly assigned to 10 groups (two unbleached control and eight bleached groups:immediate bonding IB, 4 wk delayed bonding DB, 10% sodium ascorbate treated SA, 10% ${\alpha}$-tocopherol treated TP groups). Teeth in half of groups were subjected to thermal stress, whereas the remaining groups were not. Resin-dentin rods with a cross-sectional area of $2.25mm^2$ were obtained and microtensile bond strength was determined at a crosshead speed of 1 mm/min. Fifteen specimens were prepared for SEM to compare the surface characteristics of each group. The change in dentin bond strength from thermal stress and antioxidant treatment was evaluated using two-way analysis of variance (ANOVA) and Sheffe's post hoc test at a significance level of 95%. Results: The control group exhibited the highest bond strength values, whereas IB group showed the lowest value before and after thermocycling. The DB group recovered its bond strength similar to that of the control group. The SA and TP groups exhibited similar bond strength values with those of the control and DB groups before thermocycling. However, The TP group did not maintain bond strength with thermal stress, whereas the SA group did. Conclusions: Applying a 10% sodium ascorbate solution rather than 10% ${\alpha}$-tocopherol solution for 60 sec is recommended to maintain dentin bond strength when restoring non-vitally bleached teeth.

CO2레이저 조사후 상아질 조직변화에 대한 주사전자혐기경적 연구 (A SCANNING ELECTRON MICROSCOPIC STUDY OF HUMAN DENTIN SURFACE WITH THE CO2 LASER IRRADIATION)

  • The main purpose of this study was to investigate the possibility of obstruction of dentinal tubules through irradiation of the $CO_2$ laser on the dentin surface. For this study, 104 extracted caries - free premolars and molars were grinded their mesial or distal surface to expose dentin completly. And these specimens were divided into three Groups. Group I : those with their smear layer not removed and Group IT : those with their smear layer removed, were irradiated by the $CO_2$ laser(SPACE LASER - $CO_2$ VEGA 25) at five different energy levels ranging 4 - 20J/$cm^2$, and all specimens were observed with scanning eletron microscope (Hitachi S - 450). Group m was divided into 2 sub - groups: one with smear layer, another one without and those were irradiated with $CO_2$ laser of the same condition as Group I & II. All specimens were immersed in, $37^{\circ}C$, 0.5% methylene blue solution for 48 hours after 100 times of thermocycling at $4^{\circ}C$ and $60^{\circ}C$, and were observed with stereo -light microscope (Olympus SZH - ILLD) to evaluate dye penetration. The following results were obtained; 1. The dentinal tubules were obstructed through laser irradiation. 2. The dentinal tubules were still obstructed after treatment of 10% HCl/2sec., then it was proved that acid resistance of detin surface was also increased. 3. The Group of laser irradiation showed less dye penetration than control Group. 4. The smear layer hardly affected obstruction of the dentinal tubules through laser irradiation.

상아질에 대한 10% Carbamide peroxide가 미치는 영향 (EFFECT OF 10% CARBAMIDE PEROXIDE ON DENTIN)

  • 최근 들어 심미적 목적으로 치아 미백제를 이용한 치아 미백술이 널리 사용되고 있다. 현재까지 치아 미백제의 정확한 기전은 밝혀져 있지 않지만 미백제로부터 여러 가지 방법으로 분해된 free radical의 복잡한 산화작용에 의한 것으로, 치아변색의 원인물질의 화학적 구조가 바뀌어짐으로써 희게 되는 것으로 이해하고 있다. 본 연구의 목적은 치아 미백제에 의한 상아질의 특성변화를 관찰하고 미백제의 안전성을 평가하고자 하였다. 이를 위해 10% carbamide peroxide를 1일에 6시간 2주동안 사람의 소구치 법랑질에 적용한 후 상아질에서 나타나는 형상변화, 경도변화, 조성변화 및 무기질 함량변화를 주사전자현미경(SEM), 미세경도기, FT-Raman 분광기 그리고 전자탐침미세분석기(EPMA)를 이용하여 분석하고 다음과 같은 결과를 얻었다. 법랑질 표면에 도포한 10% carbamide peroxide가 상아질로 침투하여 미친 영향은 증류수에만 담가둔 시료와 비교하였을 때 형상변화의 측면에서는 차이를 발견할 수 없었다. Peritubular dentin과 intertubular dentin의 표면에서는 nanometer range에서 아무런 변화를 발견할 수 없었다. 미백처리한 치아와 증류수에만 담가둔 치아의 상아질에서 통계적으로 유의한 경도의 차이를 발견할 수 없었다. FT-Raman 분석결과 상아질 구성성분 중 어떤 변화도 분광학적으로 관찰되지 않았다. 유기질에 의한 peak와 무기질에 의한 peak 모두 무시 할 수 있을 정도의 Raman intensity 변화만 관찰되었다. 무기질 함량변화를 측정한 결과 아무런 처리도 하지 않은 치아의 상아질, 증류수에만 담가둔 치아의 상아질 그리고 미백후 증류수에 담가둔 치아의 상아질에서 무기질 총량이 각각 $98.73{\pm}1.89,\;98.56{\pm}2.11,\;97.47{\pm}2.51$로 나타났다. 이들 값은 통계학적으로 유의한 차이를 보이지 않았다. 이상의 결과들로 볼 때 10% carbamide peroxide 미백이 상아질에 미치는 영향은 극히 미약하다고 판단되어 상아질에는 안전한 것으로 생각된다.

레진 시멘트를 이용한 레진 파이버 강화 레진포스트의 치근 상아질에 대한 미세인장결합강도 (MICROTENSILE BONDING OF RESIN FIBER REINFORCED POST TO RADICULAR DENTIN USING RESIN CEMENT)

  • Object The purpose of this study were to evaluate the microtensile bond strength of resin fiber reinforced post to radicular dentin using resin cement according to various dentin surface treatment and to observe the inter face between post and root dentin under SEM Material and Method A total 16 extracted human single rooted teeth were used. A lingual access was made using a #245 carbide bur in a high-speed handpiece with copious air water spray. The post space was mechanically enlarged using H-file(up to #60) and Gates Glidden bures(#3). This was followed by refining of the canal space using the calbrating drill set provided in ER Dentinpost(GEBR, BRASSELER GmbH&Co. KG). The 16 teeth were randomly distributed into 4 group of 4 teeth. Group 1 teeth had their post space prepared using 10% phosphoric acid as root canal surface treatment agent during 20s. The canal was then rinsed with saline and dried with paper point. Group 2 teeth had their post space prepared using 3% NaOCl as root canal surface treatment agent during 30min. The canal was then rinsed with saline and dried with paper point. Group 3 teeth had their post space prepared using 17% EDTA as root canal surface treatment agent during 1min. The canal was then rinsed with saline and dried with paper point. Group 4 teeth had their post space prepared using 17% EDTA as root canal surface treatment agent during 1min. After rinsing with saline, the canal was rinced 10m1 of 3% NaOCl for 30min. After drying with paper point, the post(ER Dentinpost, GEBR, BRASSELER GmbH&Co. KG) was placed in the treated canals using resin cement. Once the canal was filled with resin cement(Super bond C&B sunmedical co. Ltd.), a lentulo was inserted to the depth of the canal to ensure proper coating of the root canal wall. After 24 hours, acrylic resin blocks($10{\cdot}10{\cdot}50mm$) were made. The resin block was serially sectioned vertically into stick of $1{\cdot}1mm$. Twenty sticks were prepared from each group. After that, tensile bond strengths for each stick was measured with Microtensile Tester. Failure pattern of the specimen at the interface between post and dentin were observed under SEM. Results 1. Tensile bond strengths(meen{\pm}SD$) ) were expressed with ascending order as follows group 4, $12.52{\pm}6.60$ ; group 1, $7.63{\pm}5.83$ ; group 2, $4.13{\pm}2.31$ ; group 3, $3.31{\pm}1.44$. 2. Tensile bond strengths of Group 4 treated with 17% EDTA +3%NaOCl were significant higher than those of group 1, 2 and 3 (p<0.05). 3. Tensile bond strengths of Group 1 treated with 10% phosphoric acid were significant higher than those of group 2 (p<0.05). Tensile bond strengths of Group 4 treated with 17% EDTA +3% NaOCl was significant higher than those of other groups.

Targis 표면처리가 상아질과의 전단결합강도에 미치는 영향 (EFFECTS OF SURFACE TREATMENTS AND STORAGE CONDITIONS ON TARGIS/DENTIN BOND STRENGTH)

  • The purpose of this study was to estimate shear bond strength according to difference in Targis surface treatment and storage condition. 140 non-carious extracted human molars and Targis D210(Ivoclar, Liechtenstein) were used in the present study and were divided into 7 experimental groups respectively according to surface treatment of Targis. Group 1 ; No treatment, Group 2 ; $50{\mu}m$ aluminium oxide blasting, Group 3 ; 4% HF etching for 3 minutes, Group 4 ; 4% HF etching after blasting, Group 5 ; silane treatment after blasting, Group 6 ; silane treatment after 4% HF etching, Group 7 ; silane treatment after blasting and 4% HF etching. In Each group, one half of 20 specimens was stored in distilled water at $37^{\circ}C$ for 24 hours and the other half was stored at atmosphere for 24 hours respectively. Dentin surface was etched with 10% $H_3PO_4$ for 15 seconds and luting cement(Variolink II, Vivadent, Liechtenstein) was applied by manufacturer's recommendation. Shear bond strength for each group was then measured. To examine the failure patterns after shear bond test and to observe the change after surface treatment of Targis. Specimens were fabricated and observed under the SEM. Statistical analysis was performed by One Way ANOVA test and t-test. The results were as follows ; 1. The shear bond strength of the groups stored in water significantly lower than that of groups stored at atmosphere (P<0.05). 2. There was no significant difference in shear bond strength in groups stored in water (P>0.05). 3. The shear bond strength without surface treatment of Targis were lowest among all experimental groups in atmosphere condition(P<0.05). 4. There was no significant difference in bond strength between groups using the silane or not(P>0.05). 5. The groups treated by blasting, hydrofluoric acid and silane sequentially showed highest bond strength than that of other groups in atmosphere condition, but there was no significant difference(P>0.05). 6 The proportions of the specimens showing the mixed fracture failure were 20% in HF etching group and blasting + HF group, 40% in blasting + HF + silane group in atmosphere condition. All the specimens stored in water showed adhesive fracture failure.
