일부 노인의 보철장착에 따른 구강건강행태와 구강관리 요구도 조사 (A study of the oral health status and the need of oral management of elderly)

  • Objectives : The purpose of this study was identified about oral health status and oral health needs of elderly. Moreover this study would be based further research of development of oral health in elderly. Methods : This study was undertaken to determine the oral health status and the need of oral management of over 65years elderly who were with dental prosthesis in elderly welfare center and Kyung Ro Dong in Kyung Bok and Kyung Nam. The results were as follows. Results : The sample was 200 participants. The demographical characteristic were that 74.0% of participant was female, 29.0% of participant was 75~79 years old, 42.5% of participant was under elementary, and 45.5% of participant was living alone. 43.7% of participant was pain from muscle-skeletal disease, 37.1% of participant was cardiac-vascular disease, and 31.1% of participant was ophthalmic disease. In oral health status, 54.5% of participant was partly artificial tooth and 45% of participant was whole artificial tooth. 78.0% of participant used under 10 years with partly artificial tooth and only 27.4% were satisfaction with artificial tooth. 83.5% of participant used under 10 years with whole artificial tooth and only 26.4% were satisfaction with artificial tooth. In the oral health status of partly artificial tooth, the average of toothbrush was 2 times, 42.2%. The majority method of toothbrush was 'their own freely' 55.0%. The time of toothbrush was 'after meal' 81.8%. The study result showed that majority participant didn't experience of tongue brush, regular examination, scaling, and oral health education. In the need of oral management with the whole artificial tooth, the participant who used longer artificial tooth was significantly higher about artificial irrigation and the method of management(p<.05). The need of participant who answered 'don't gum massage', was higher of xerostomia treatment(p<.01) and halitosis treatment(p<.05). In the need of oral management with the partly artificial tooth, the participant who used longer artificial tooth was significantly higher about regular examination(p<.01), oral cavity massage(p<.05), scaling(p<.05), dental caries treatment(p<.01). The need of participant who answered 'don't satisfaction with artificial tooth' was higher of regular examination and scaling(p<.05). Conclusions : Oral management needs of elderly who kept artificial tooth or denture required were regular dental examination, xerostomia management, management of artificial tooth and irrigation, and management of bad breath. This results meaned the oral health intervention program for elderly was developed regularly.

3D 프린터로 출력된 치과용 레진의 자동바렐연마공정에 따른 표면 거칠기 및 연마도 관찰 (Observation of surface roughness and grinding angle by automatic barrel finishing process of dental 3D printed resin)

  • Purpose: This study aimed to confirm the applicability of gloss polishing using automatic barrel finishing with respect to three-dimensional (3D)-printed resin specimens. The surface roughness and grinding angle of the 3D-printed resin specimens were observed with respect to gloss polishing time using automatic dental barrel finishing. Methods: Herein, experiments were conducted on four types of 3D-printed resin specimens. The specimens, with a thickness of 100 ㎛ each, were printed using a 3D printer. Subsequently, light polymerization was performed on these specimens for 15 min. Post this surface treatment, the specimens underwent grinding for 25 min. This process was followed by gloss polishing at 5-min intervals for up to 25 min using automatic dental barrel finishing. The specimens were photographed using a 3D optical microscope, and their surface roughness and grinding angle were measured. Results: The Ra (centerline average roughness) values of all the specimens, except for crown & bridge 10 group and those in the control group that were not polished using automatic barrel finishing, were <0.2 ㎛. However, polishing time needs to be controlled to realize the desired surface roughness and grinding amount considering the hardness of the resin used. Conclusion: Gloss polishing of 3D-printed resin can be realized using automatic dental barrel finishing. Trends in behavioral management techniques for dental treatment of patients with autism spectrum disorder: a 10-year retrospective analysis

  • Background: Patients with autism spectrum disorder (ASD) present challenges in dental treatment cooperation owing to deficits in communication skills and social interaction. Behavioral guidance, sedation, and general anesthesia may be employed to ensure the quality of dental care for individuals with ASD. This study aimed to examine the trends in dental treatment for patients with ASD who visited the Department of Pediatric Dentistry at Dankook University Jukjeon Dental Hospital, an oral health center for the disabled in the Gyeonggi region, over the past 10 years. Methods: This study utilized the order communication system to gather data on sex, age, cooperation level, number of quadrants treated, and administration of sedation or general anesthesia for patients with ASD who visited the Department of Pediatric Dentistry at Dankook University Jukjeon Dental Hospital between January 2013 and December 2022. Results: The total number of patients with ASD increased annually, possibly due to an increase in ASD prevalence and the hospital's designation as a center for disabled oral health. General anesthesia was predominant before 2017, with a shift towards N2O-O2 sedation. The most common age group for sedation or general anesthesia was 6-9 years, with a higher prevalence in males than in females. Notably, N2O-O2 and midazolam sedation resulted in better cooperation and fewer treated teeth than general anesthesia. Conclusion: This study highlights the evolving trends in dental treatment for individuals with ASD, indicating a shift towards outpatient methods, particularly N2O-O2 sedation. The sex distribution aligns with national statistics, emphasizing a higher prevalence of ASD in males than in females. These findings underscore the need for further research to establish evidence-based guidelines for optimal dental care strategies tailored to the unique needs of individuals with ASD.

  • A total of 2,510 police personnel aged 20-59 were examined to determine the prosthodontic demands. The results were as follows. 1. Prosthodontic demands increase with aging. 2. Periodontic problem is the major cause of the prosthodontic needs in old personnel. 3. Trauma is the more popular cause of the prosthodontic need than periodontal problem in policeman. 4. The major cause of the untreatment is the lacks of time. 5. The younger persons have more fear of dental treatment. 6. Prostodontic demands show declining trends.

  • The high success rate of dental treatment is dependent on the cooperation of pediatric patients during procedures. Dental treatment often causes pain, particularly in children. The factors in providing treatment to pediatric patients include the characteristics and location of the tooth, profoundness of the anesthesia including the type of local anesthetic, and cooperation of the patient. Previous studies have examined several techniques to successfully achieve profound pulpal anesthesia in maxillary permanent teeth. The dentist should select the injection technique to be used based on patient needs. In children, either buccal with palatal injections or buccal with intra-septal injections may be used to anesthetize the permanent maxillary first molar. Buccal with palatal injections are commonly used prior to routine maxillary dental procedures. Currently, there are only a few studies on the employment of buccal with intra-septal injections to anesthetize permanent maxillary first molars in pediatric patients. This review will focus on efficacy of buccal with palatal versus buccal with intra-septal pulpal anesthesia of the permanent maxillary first molars in pediatric patients and aim to determine which technique should be used during routine dental procedures.

자폐증 환자에서 함치성 낭종과 연관된 매복치의 처치 (MANAGEMENT OF THE IMPACTED TOOTH ASSOCIATED WITH DENTIGEROUS CYST IN AUTISTIC YOUNG PATIENTS)

  • 자폐증을 진단 받은 16세 남환이 하악 좌측 부위의 낭종 및 매복치를 주소로 내원하였다. 해당낭종은 함치성 낭종으 로 최종 진단 되었으며 일일 입원 및 전신 마취 하에 낭종의 적출 및 매복치의 발거와 자가 이식을 시행 후 치근단 형성 술 진행하였다. 2년 6개월 경과 관찰한 현재 주소 증상은 사라지고 재발 성향은 없으나, 치근단 염증 소견 및 발거 부 위 공간 소실 등의 합병증이 관찰된다. 저자는 본 증례를 통 해 다음과 같은 지견을 얻었다. 1. 함치성 낭종의 치료는 일반적으로 골 결손이 적고 매복치의 자발적 맹출을 유도할 수 있는 조대술이 추천된다. 2. 매복치의 자발적 맹출 가능성은 매복 깊이, 치축 각도, 환자의 나이, 치근 성숙도 등을 주의 깊게 고려하여 판단해야 한다. 3. 본 증례의 경우 매복치의 자발적 맹출 가능성이 낮으며 자폐증으로 인한 구강위생 불량, 저조한 협조도 등 을 고려하여 적출술을 시행하였다. 4. 치료 계획 설정 시 일반적 요인뿐만 아니라 환자의 의과 적, 전신적 병력에 따른 특별한 요구도 고려해야 한다.

Miniscrew를 고정원으로 이용한 교정치료 (The use of miniscrew as an anchorage for the orthodontic tooth movement)

  • 교정치료에 있어서 anchorage는 진단 및 치료계획에서부터 치료 종료 단계까지 교정의사가 항상 염두에 두면서 치료를 진행 해야 하는 중요한 요소이다. 환자의 적극적인 협조를 필요로 하는 전통적인 anchorage 조절법 보다는 miniscrew 같은 skeletal anchorage 가 좀더 효과적인 방법으로 제시되어 지고 있어, miniscrew의 교정적 이용 시 고려사항에 대하여 전반적인 고찰과 증례보고를 통해서 niniscrew의 다양한 임상적 적용에 대해서 살펴보았다. 또한 midpalate부위가 miniscrew의 식립 부위로서 가지는 장점과 식립 시의 주의사항을 알아보았으며 skeletal anchorage 가 교정에 도입되면서 치료개념과 방법에 있어서의 변화를 요약하면 다음과 같다. 1. 절대적인 개념의 Anchorage가 도입 되었다. 2. 생역학적 면에서 치아의 치체이동이 쉬워지고 determinate system이 설계되어 질수 있다. 3. 기존의 수술로만 가능했던 치료들 중 일부는 교정 치료만으로 치료가 가능하게 되었다.

보육교사의 영유아 구강보건 지식과 교육 요구도 (A study on the knowledge and educational needs of infant oral health in childcare teachers)

  • Objectives: In this study, existing knowledge and needs of childcare teachers for developing oral health education plans for infants and young children were investigated. Methods: From March to April 2023, an online questionnaire survey led to the collection of data from 150 childcare teachers. Data were analyzed using the SPSS 22.0 program. Results: Importance-Performance Analysis (IPA) revealed that importance and knowledge of oral health education, importance of oral health, and prevention of oral diseases were priority areas, and treatment, symptoms, and causes were priority areas that needed improvement . According to the IPA on the importance of and demand for oral health education support, the importance and demand for an educational plan were both high. Expert education based on educational goals, information sharing, educational environment support, evaluation system, and monitoring was low in importance but high in demand. Conclusions: Oral care habits during infancy are important for maintaining oral health throughout life. Oral care practice should be improved by developing an oral health education plan for childcare teachers to implement.

치과적 장애환자의 치과치료를 위해 시행한 깊은 진정 : 전신마취와 비교한 통계적 고찰 (CLINICO-STATISTICAL ANALYSIS OF DEEP SEDATION COMPARED WITH GENERAL ANESTHESIA AT THE CLINIC FOR THE DISABLED)

  • Background : To do dental treatments successfully for dentally disabled patients who are unable to cooperate to procedures needs deep sedation (DS) or general anesthesia (GA). But there are some difficulties in selecting DS because of some disadvantages such as airway problem etc. But, if we select appropriate cases, DS would be better than GA. Methods : We reviewed total 238 cases of patients who had received dental treatments under GA or DS at the clinic for the disabled in Seoul National University Dental Hospital from November 2007 to February 2009. To compare anesthesia condition between DS and GA, we reviewed preanesthesia evaluation sheet, anesthesia or sedation records and PACU sheets retrospectively. Results : The number of DS cases was 25 (11%) and that of GA was 218 (89%). To maintain DS, intravenous propofol was infused with syringe pump (100%), and sevoflurane (134 cases) or propofol (13 cases) were used for sedation induction. Mean total treatment time for DS was 36 min and 2 hour 25 min for GA. The recovery time at PACU was 44 min for DS and 80 min for GA. There were no severe complications in DS, but 18 cases showed nausea and vomiting in GA. Conclusion : Deep sedation for disabled dental patients should be selected for effective behavioral control in conjunction with general anesthesia, considering duration and pain-evoking potentials of dental treatment and type and severity of patients' disabilities altogether.

Tonsillectomy as prevention and treatment of sleep-disordered breathing: a report of 23 cases

  • Background: The paradigm of tonsillectomy has shifted from a treatment of recurrent throat infection to one of multi-discipline management modalities of sleep-disordered breathing (SDB). While tonsillectomy as a treatment for throat problems has been performed almost exclusively by otorhinolaryngologists, tonsillectomy as a part of the armamentarium for the multifactorial, multidisciplinary therapy of sleep-disordered breathing needs a new introduction to those involved in treating SDB patients. This study has its purpose in sharing a series of tonsillectomies performed at the Seoul National University Dental Hospital for the treatment and prevention of SDB in adult patients. Methods: Total of 78 patients underwent tonsillectomy at the Seoul National University Dental Hospital from 1996 to 2015, and 23 of them who were operated by a single surgeon (Prof. Jin-Young Choi) were included in the study. Through retrospective chart review, the purpose of tonsillectomy, concomitant procedures, grade of tonsillar hypertrophy, surgical outcome, and complications were evaluated. Results: Twenty-one patients diagnosed with SDB received multiple surgical procedures (uvulopalatal flap, uvulopalatopharyngoplasty, genioglossus advancement genioplasty, tongue base reduction, etc.) along with tonsillectomy. Two patients received mandibular setback orthognathic surgery with concomitant tonsillectomy in anticipation of postoperative airway compromise. All patients showed improvement in symptoms such as snoring and apneic events during sleep. Conclusions: When only throat infections were considered, tonsillectomy was a procedure rather unfamiliar to oral and maxillofacial surgeons. With a shift of primary indication from recurrent throat infections to SDB and emerging technological and procedural breakthroughs, simpler and safer tonsillectomy has become a major tool in the multidisciplinary treatment modality for SDB.
