Analysis and Consideration of Factors for Predicting Cooperation Levels in Pediatric Dentistry

  • This study aimed to evaluate various factors related to cooperation levels in pediatric dentistry and determine their correlation with children's cooperation during dental treatments. This study included one hundred children and their guardians who visited the dental hospital at the Chonnam National University. Children and their guardians completed surveys regarding dental fear, dental caries experience, dental treatment experience, temperament, and guardians' dental anxiety, as well as the background characteristics of the children. Based on these data, factors associated with children's cooperation during dental treatment were investigated. Dental fear, caries experience during the primary and mixed dentition stages, and temperament traits such as shyness and negative emotionality significantly impacted children's cooperation during dental visits, with higher levels of these factors corresponding to lower cooperation. The extent of dental experience also modestly influenced children's cooperation, with higher levels of cooperation observed in children with greater dental experience. Additionally, children's dental fear was strongly correlated with guardians' dental anxiety, increasing as guardians' anxiety levels increased. Twelve-year-old children exhibited significantly lower levels of dental fear compared to other age groups, and regardless of cooperation levels, injections (shots) were identified as the primary factor inducing dental fear among the children. To improve children's cooperation in pediatric dentistry, strategies should focus on alleviating their fears and adopting an individualized approach that consider their oral health status and temperamental traits.

Minimal sedation using oral sedatives for multi-visit dental treatment in an adult patient with dental phobia

  • In adult patients with dental phobia, dental treatment may be difficult, or may not be possible. Depending on the level of fear or anxiety, non-pharmacological or pharmacological behavior management techniques are used in the dental treatment of such patients. Among the pharmacological behavior management techniques, minimal sedation, that is, the lowest depth of sedation, can be easily obtained in adult patients using oral sedatives, does not require special equipment or tools, and does not affect ventilatory and cardiovascular function. Diazepam is an anxiolytic drug belonging to the benzodiazepine family that, in addition to relieving anxiety, produces muscle relaxation, and is a representative drug used in adult patients with fear of dental treatment. Herein, we report the case of a 50-year-old woman with severe dental fear who successfully underwent long-term dental treatment in approximately 20 visits with minimal sedation using oral diazepam. In addition, we reviewed the considerations for the use of benzodiazepines for minimal sedation.

The relationship between subjective oral health and dental fear in Korean adolescents

  • Background: This study is aimed to evaluate the level of fear and to reduce the overall fear, thereby enabling patients to receive treatment via timely visits. Methods: In a survey conducted by 460 South Korean middle school students, we used 453 data that faithfully responded to the survey. Multiple regression analysis was conducted to investigate the factors influencing subjective oral health and dental fear. The significance level used for statistical significance was ${\alpha}=0.05$. Results: The level of fear was higher for upper grade, female students. The factors affecting dental fear were higher for gingival bleeding and dental pain. Regarding factors for dental fear affecting subjective oral health, lower fear of puncture needle and tooth removal tool resulted in higher subjective oral health. Conclusions: The study found that adolescents had higher fear of dental care when they had gingival bleeding and tooth pain. Gingival bleeding is a symptom of early gingival disease and dental pain is likely due to advanced dental caries. These results suggested that it is necessary to have a program to reduce dental fear and anxiety as well as a program to prevent dental diseases through regular periodic screening and education.

일부 고등학생의 치과불안도 평가 연구 (Dental clinic fear in high school students)

  • Objectives : The aim of this study is to investigate dental clinic fear and anxiety in high school students. Methods : The subjects were 311 high school students aged 19 years. They completed self-reported questionnaire including general characteristics, dental clinic experiences, dentist or dental hygienist confidence, and dental fear. Data were analyzed using SPSS 18.0. Results : Poor oral health status showed negative attitude towards dental clinic visit. Girl students tended to have more fear than boy students. Muscle tension was the most common experience and was closely related to anesthetic needles and drills. Dental anxiety was caused by lack of confidence to dentists and dental hygienists. Students were more afraid of dentists than dental hygienists. Higher score of dental anxiety seemed to be associated with infrequent visit to dental clinic. Conclusions : Frequent regular dental checkup may reduce dental fear and anxiety. Dental clinic staff should try to minimize dental anxiety in performing treatment.

치위생학과 구강위생교육실에 내원한 대상자의 치과공포 원인과 구강건강관리행태 (Dental fear cause and oral health care behavior in the dental hygiene clinic clients)

  • Objectives : This study is aimed to seek how to promote oral health care by leading the clients with dental fears to change their behaviors in oral hygiene care based on the analysis of the relationships between the dental fears and oral health care behaviors of the clients who visit the Dental Hygiene Education Room. Methods : This study analyzed the 361 dental hygiene records of the subjects with finished treatment and accurate records among a total of 370 clients who had received dental hygiene care at the Dental Hygiene clinic in Y University from March 2007 till June 2009. According to their general features, the study analyzed whether they had any dental fears and why such fears occurred and conducted Chi-square test to compare their oral hygiene care experiences and behaviors with dental fears. Data analysis was made using the SPSS 12.0K for Window, with level of significance set at 0.05. Results : 1. In terms of dental fear or non-fear, the comparison by gender showed that women had a higher level than men, with 76 women(47.2%) answering yes; the comparison by age showed that the age group of 30 years old or older had a higher level than other groups. 2. In terms of dental fear reasons, "the past pain experiences" recorded 34.2%, showing the highest rate. 3. In addition, dental fears had influences on regular dental examination, scaling and dental hygiene education and the subjects without dental fear showed higher levels in terms of tooth-brushing frequency and time. Conclusions : To control "the past pain experiences", future dental treatment should place priority on preventive treatment and get clients to cope with pains through the preliminary education with pains. Besides, to make an effective oral health care of the clients with dental fears, dental hygienists should control the fears of the clients and perform an appropriate oral health care for them using the communication techniques to make friendly and trustworthy impressions.

예방처치경험이 진료만족도 및 치과공포도에 미치는 영향 (The Effect of Preventive Treatment Experience on Treatment Satisfaction and Dental Fear)

  • 본 연구는 성인의 예방처치경험 여부가 진료만족도 및 치과공포도에 미치는 융복합적인 영향을 확인하고자 수도권에 거주하는 20세 이상 성인 292명을 대상으로 2017년 1월부터 4월까지 설문조사를 실시하였다. 수집된 자료는 SPSS 22.0 프로그램을 이용하여 $x^2-test$, t-test(검증), 다중회귀분석(multiple regression analysis)을 실시하였다. 성인의 예방처치경험은 불소도포가 가장 많았으며, 불소도포경험이 있는 경우 진료만족도가 높게 나타났다. 한편 예방적 스케일링경험이 있는 경우 치과 공포도는 낮게 나타났다. 다중회귀분석을 한 결과 진료만족도에 영향을 미치는 변수는 불소도포경험이었으며, 치과공포도에 영향을 미치는 변수는 구강건강상태, 예방적 스케일링경험으로 나타났다. 본 연구의 결과는 예방처치경험이 있는 경우 진료만족도는 높아지고 치과공포도는 낮아지는 것을 확인하였으므로 구강건강교육을 통한 예방처치의 중요성을 국민에게 인식시키고 치과예방처치 서비스를 제공함으로써 구강건강증진 향상을 기대할 수 있을 것이다.

일부 성인의 개인적 기질 및 치과 방문경험과 치과공포와의 상관성 (Correlation between personal temperament, dental visit, and dental fear among adults)

  • Objectives: The purpose of the study was to investigate the correlation between personal temperament, dental visit, and dental fear among adults. Methods: A self-reported questionnaire was completed by 400 adults visiting dental clinics from April to June, 2016. The questionnaire consisted of general characteristics of the subjects, dental fear, and temperament. Data were analyzed by t test, chi-square test, chi-square test, and pearson's correlation analysis using SPSS v. 23.0. Results: The first impression of dental clinics had an impact on dental fear (p<0.001). The negative image on the dental clinics increased dental fear of the visitors. There was a significant difference between the purpose of dental visit and dentla fear. Conclusions: The positive image of the dental clinics can solve the dental fear due to treatment in the adults. The dental consumers should try to have the comfortable attitude toward dental clinic in case of visit.

치과내원환자의 주관적 구강건강인식과 치과공포의 관련성 (Relationship between Subjective Oral Health Recognition and Dental Fear in Dental Clinic Patients)

  • 본 연구는 부산 경남지역의 치과내원환자를 대상으로 주관적 구강건강인식과 상태 그리고, 치과진료 분야에 치과공포수준을 파악하여 추후 치과진료 시 치과공포를 줄일 수 있는 방안을 마련하여, 구강건강의 향상과 더불어 삶의 질 향상에 기초자료로 개발하고자 한다. DFS의 전체 문항에서 100점 만점에 남자(44.9점)보다 여자(53.1점)에서 공포감이 모두 높게 나타났다. 치료종류에 따른 공포수준은 임플란트 치료 치료회피요인(p=0.015), 교정치료 치료회피요인(p=0.002), 생리적 반응요인(p=0.009), 보철치료 치료회피요인(p=0.014), 신경치료 치료회피요인(p=0.005), 생리적 반응요인(p=0.017), 치주치료 치료회피요인(p=0.013), 생리적 반응요인(p=0.004), 정기검진을 받지 않는 경우 치료회피요인(p=0.027)이 높았다. 구강증상에 대하여 인지하고 있는 개수가 많을수록 치료회피요인(p=0.001), 자극반응요인(p=0.002), 생리적반응요인(p=0.005)로 높았다. 이상의 결과에서 치과공포를 줄이기 위해서는 정기검진과 예방적 처치를 위한 다양한 프로그램개발 및 구강보건교육을 실시해야 할 것이다.

청소년의 치과치료와 관련된 공포감에 영향을 미치는 요인에 관한 공변량 구조모형 (Structural Equational Modeling of Fear Factors Associated with Dental among Teenagers)

  • 본 연구에서는 15세에서 17세의 청소년들을 대상으로 치과치료에 대한 공포감의 일반적인 경향과 관련변인들에 따라 공포감이 어떻게 차이가 나며, 이들 변인들이 치과치료에 대한 공포감에 영향을 미치는 정도와 인과관계를 밝히고자 하였다. DFS, DBS 및 GFS를 포함한 설문도구와 청소년의 특성에 관한 문항들로 구성된 설문지를 이용하였고, SPSS를 이용하여 단변량 분석을 시행한 후 LISREL 8.12를 사용하여 공변량 구조모형을 분석하였다. DFS (0.957), DBS (0.916) 및 GFS (0.910)의 Cronbach 신뢰도 계수가 모두 0.9이상으로 높게 나타났고, 자신의 치아상태가 건강하지 않다고 생각할수록, 최근 구강 내 동통을 경험했을 경우, 치과치료 시 동통을 강하게 느꼈을 경우, 그리고 치료 시 동통을 자주 경험했을 경우와 치과약속을 어긴적이 있을 경우에 그렇지 않은 경우보다 치과치료에 대한 공포감의 수준이 통계적으로 유의하게 높아지는 추세로 나타났다. 구조방정식 모형의 적합도는 양호하게 나타났으며, 치과치료시 동통정도와 동통 횟수, 치과약속을 어긴 횟수, 치과의사에 대한 불신감 그리고 일반적인 공포감이 치과치료와 관련된 공포감에 정(+)의 직접효과가 있었다.

일부 남자 고등학생의 치과치료 공포감과 구강건강 삶의 질에 관한 연구 (Fear of dentist care and quality of life in dental health in male high school students)

  • Objectives: The purpose of this study was to find out the fear of dentist care, subjective recognition of dental health, and quality of life in the male high school students and to analyze the influencing factors on dental health care. Methods: A self-reported questionnaire was filled out by 243 special high-school in Deagu province from March 3 to March 14, 2014. The questionnaire consisted of general characteristics of the subjects(5 questions), subjective recognition of health and activities to improve health(6 questions), dental fear(20 questions), oral health related quality of life(16 questions). The instrument for dental fear was adapted from measured by Berggren Dental Fear Survey(DFS). A total of 20 DFS questions included treatment avoidance(8 questions), stimulus reaction(6 questions), and physiological reaction(5 questions) and score by Likert 5 scale. Cronbach alpha was 0.974 in the study. Oral health related quality of life was measured by 16 questions of CPQ11-14 for the adolescents by Lau. CPQ11-14 consisted of oral symptoms(4 questions), functional restriction(4 questions), and emotional wellbeing(4 questions). The instrument was score by Likert 5 scale and Cronbach alpha was 0.9354 in the study. Data were analyzed using SPSS 18.0 program for ANOVA and multiple regression analysis. Results: Fear of dentist care showed significant differences in treatment avoidance factor(p<0.001), stimulus reaction factor (p<0.05), and physiological reaction factor(p<0.001). The factors depended on subjective recognition of health and health-improving activities and differences in treatment avoidance factor(p<0.05) and physiological reaction factor(p<0.01). The dental symptoms factors showed significant differences in health recognition(p<0.001), interest in health(p<0.001), alcohol drinking status(p<0.001) and regular meal(p<0.001). While function limit factors showed differences in health recognition (p<0.001), interest in health(p<0.001), smoking(p<0.001), alcohol drinking(p<0.001) and regular meal(p<0.001). Mental and social stabilities factors showed significant differences in health recognition(p<0.001), interest in health(p<0.001) and alcohol drinking status(p<0.001). Among the factors influencing on the quality of life in dental health, interest in health(p<0.005), alcohol drinking(p<0.005) and physiological reaction in the midst of fear of dentist care(p<0.001) were the significant impact factor. Conclusions: It is necessary to develop a continuous and systematical program of dental health and dental care by experts so that the students can reduce the fear of dentist care by regular dental checkup and preventive treatment and care.
