Quantitative Light-Induced Fluorescence의 이해와 치위생 과정에서의 활용방안 (Quantitative Light-Induced Fluorescence: A Potential Tool for Dental Hygiene Process)

  • Recently, there have been improvements in diagnostic methods for the assessment of early caries lesions. The reason is that dental professionals are seeking methods to reliably detect incipient dental caries and to remineralize them. This review examines the literature on principles, theoretical background, and history of the Quantitative Light-Induced Fluorescence (QLF) system (Inspektor Research Systems BV, The Netherlands). Furthermore, this paper discusses the potential application of QLF system to clinical practice for educational purpose, enabling dental hygiene students to perform oral health assessment using the QLF system. In addition, the clinical application of QLF system can motivate patients by providing additional visual information about caries and bacterial activity. The evidences on validity and reliability of the QLF system for detection of longitudinal changes in de/remineralization and caries were examined. The QLF system is capable of monitoring and quantifying mineral changes in early caries lesions. Therefore, it can be used to assess the impacts of caries preventive measures on the remineralization and reversal of the caries process. And the QLF system is a very promising equipment to assess educational effectiveness for dental hygiene students in their learning process. In conclusion, the QLF system is the most effective technology for more sensitive staging of caries and treatment without surgical intervention.

임상 치과위생사의 근무환경에 따른 직무스트레스 (Job stress according to the working environment of clinical dental hygienists)

  • Objectives: The aims of this study were to analyze the job stress of clinical hygienists according to their positional system and to confirm its relevance. Methods: Seven hundred seventy six clinical dental hygienists participated in this survey. The questionnaire items consisted of general characteristics, working environment, and job stress. The data analysis was performed using IBM SPSS statistics ver.23.0 software. Results: Among the job stresses, the stress on the job demands was the highest with 63.4 points, and the stress on interpersonal conflict was the lowest with 34.6 points. The total score of job stress was the highest in the 26-30 year-old age group, and major stress factors in this group were job insecurity, organizational system, and lack of reward. Team members were more stressful about insufficient job control, organizational system, and lack of reward than team managers, while team managers were more likely to score interpersonal conflict. The dental hygienists in the hospitals that did not have the positional system got more stressed in the insufficient job control, organizational system, and lack of reward area. Conclusions: The presence of positional system was related to job demand, insufficient job control, organizational system, and lack of reward. Structural efforts such as establishing a proper positional system in dental clinics will be needed to control job stress in the clinical dental hygienists.

  • Dental caries has been widely prevalent with presence of cavitation on teeth. For the last several decades, the prevalence of dental caries in developed countries has rapidly decreased so there has been needed a new and detailed diagnostic guideline to differentiate the severity of dental caries, especially for early status of caries. The cariology specifically requires the development of an integrated definition of dental caries and uniform systems for measuring the caries process in the fields of clinical diagnosis and treatment, epidemiological researches, and dental education and so forth. The international Caries Detection and Assessment System (ICDAS) optically measures the enamel surface changes and potential histological depth of carious lesions by relying on surface characteristics of teeth. ICDAS is a visual classification system that was developed to diagnose the subtle changes of enamel surface, predict the progress direction of early caries, allow standardized data collection in relation to caries in different settings, and to enable better comparison of oral health between countries worldwide and research studies.

미국치과위생사의 전문직화에 대한 고찰 (The Professionalization of American Dental Hygienist)

  • 2013년, 치과위생사 역사의 100주년을 맞이하였다. 1913년, Dr. Fones가 Bridgeport Junior College에 2년 과정의 치위생교육프로그램을 개설하며, 구강간호가 아닌 구강질환의 예방과 보건학을 전문으로 하는 '치과위생사' 라는 직종명칭을 사용한 것이 그 시초다. 미국에서 최초로 치과위생사 제도를 최초로 도입한 만큼, 미국치과위생사의 발전과정을 되짚어 보는 것은 국제적인 치위생계 발전과정을 확인하고, 각국의 제도를 비교할 수 있는 근간이 될 것이다. 이 연구는 지난 100년간 미국치과위생사가 전문직 역할을 수립해 나가는 과정을 '치과위생사 조직의 발전과정', '치위생학 교육제도의 발전과정', '치과위생사 면허 및 관련 자격제도의 발전과정', '치과위생사 관련 법령과 윤리' 및 '미국치과위생사의 역할 및 업무범위의 발전과정' 요건에 따라 고찰하는 것이다. 미국치과위생사는 전문직 요건을 형성하고 있고 사회적 분위기 역시 전문직으로 인식하고 있다. 이에 미국치과위생사가 오늘날의 모습으로 전문직화해 온 과정과 그 특징을 규명하는 데 연구목적을 두고, 이 목적을 달성하기 위해 요건 별 문헌자료를 수집하고 분석하였다.

치과의료기관 유형별 감염관리 이행실태 (Infection control by type of dental institution)

  • Objectives: The purpose of this study was to investigate the infection control system and actual conditions according to the type of dental medical institution. Methods: From April 1st to May 10th, 2020, dental institutions were recruited through stratified random sampling. Each item constituting the infection control system by dental institution type was analyzed using the chi-square test and fisher's exact test. Results: The infection control system and operation level of each dental institution type was 89.0 points for university-general hospitals, 75.6 points for dental hospitals, and 34.4 points for dental clinics. The environment management levels were 76.1, 72.5, 73.0, and 74.0 points for university-general hospitals, similar to 77.2, 75.1, 71.0, and 73.8 points for dental hospitals, while dental clinics had 61.1, 40.0, 37.0, and 45.6 points. Prevention and management of staff infection exposure, wearing personal protective equipment, and hand hygiene levels were 90.7, 75.5, 88.5 points for university-general hospitals, 79.8, 79.5, 80.4 points for dental hospitals, and 50.2, 88.0, 61.5 points for dental clinics, respectively. Conclusions: Efforts are required to bring about improvement in the areas of insufficient infection control in order to raise the overall infection control levels, especially the management of dental clinics is urgently needed.

치과위생사의 한국형 전문치과위생사제도 도입에 대한 견해 (Opinions of Korean dental hygienists on the advanced dental hygienist system in Korea)

  • Objectives: The purpose of this study was to investigate the opinions of Korean dental hygienists on the Korean advanced dental hygienist system in Korea. Methods: The survey was conducted on 328 dental hygienists working at dental clinics and hospitals in Seoul, Gyeonggi-do, Incheon, and Chungcheong-do. Collected data were analyzed with the frequency analysis, independent-samples t-test, and one-way analysis of variance after their reliability and validity were verified through the reliability, exploratory factor, and confirmatory factor analyses. Results: Respondents preferred sectoral advanced dental hygienists to integrated advanced dental hygienists, but 47.0% of respondents did not respond to this questionnaire item. To become an advanced dental hygienist, 'more than 5 years but less than 8 years' of clinical experience was appropriate according to 45.7% of the respondents and 'more than 30 h but less than 40 h' of training period was appropriate according to 25.9% of the respondents. Respondents thought that the duration of the training period for advanced dental hygienists would be directly proportional to expectations from the effect of advanced dental hygienists (p=0.023). Further, respondents who were usually interested in advanced dental hygienists showed higher expectations from the role (p<0.001) and effect (p<0.001) of advanced dental hygienists than those who were not interested. Conclusions: This study suggests various implications of the introduction of the advanced dental hygienist system in Korea.

Use of a gesture user interface as a touchless image navigation system in dental surgery: Case series report

  • Purpose: The purposes of this study were to develop a workstation computer that allowed intraoperative touchless control of diagnostic and surgical images by a surgeon, and to report the preliminary experience with the use of the system in a series of cases in which dental surgery was performed. Materials and Methods: A custom workstation with a new motion sensing input device (Leap Motion) was set up in order to use a natural user interface (NUI) to manipulate the imaging software by hand gestures. The system allowed intraoperative touchless control of the surgical images. Results: For the first time in the literature, an NUI system was used for a pilot study during 11 dental surgery procedures including tooth extractions, dental implant placements, and guided bone regeneration. No complications were reported. The system performed very well and was very useful. Conclusion: The proposed system fulfilled the objective of providing touchless access and control of the system of images and a three-dimensional surgical plan, thus allowing the maintenance of sterile conditions. The interaction between surgical staff, under sterile conditions, and computer equipment has been a key issue. The solution with an NUI with touchless control of the images seems to be closer to an ideal. The cost of the sensor system is quite low; this could facilitate its incorporation into the practice of routine dental surgery. This technology has enormous potential in dental surgery and other healthcare specialties.

치기공학 전공 대학생들의 전문치과기공사 제도의 인식에 관한 조사 (A survey on perception about Certified Dental Technician(CDT) system of students majoring in dental laboratory technology)

  • Purpose: This study was aimed to know the perception about Certified Dental Technician system of students majoring in dental laboratory technology in Korea. Methods: A survey was conducted for four months from August through November 2011 with questionnaires. The responses from 1148 students majoring in dental laboratory technology in Korea were analyzed with SPSS 12.0 program, and frequency analysis and chi square test were implemented. Results: Total students who responded to the questionnaire were 1148 (100%). The population characteristic of this survey is that 691 (60.2%) are male. The respondents believed that the job skill could be expanded by implementing the system (85.6%). Also, they responded that they would be willing to acquire the Certified Dental Technician qualifications in the future (95.5%). Conclusion: Students majoring in dental laboratory technology in Korea believed that the execution of the system of Certified Dental Technician would expand the dental technology industry.

한국 내 의과대학병원 내 치과의 공간구성특성에 관한 연구 - 서울지역 의과대학병원을 중심으로 (A Study on the Spatial Configuration Characteristics of Dental Department in Medical Center in Korea - Focused on the Medical Center in Seoul Area)

  • Purpose: Analysis on the spatial configuration characteristics of dental department in medical center through examining outpatient department of medical center is necessary for the development of architectural planning of the dental healthcare system in Korea. This study has been performed to provide data for the planning of hospital architecture. Methods: Literature review of dental healthcare system and investigation on current status of dental department in medical center have been conducted. The plan and spatial configuration of seven medical centers in Seoul area have been analyzed. Results: The result of this study can be summarized in four points. The first one is that the clinical dental spaces are classified by dental school's dental hospital, dental department in medical center, dental hospital, private dental clinic, and public dental healthcare center in Korea. The second one is that the dental department in medical center is a result from medical law regulation and it is specified and subdivided with dental specialist system. The third one is that the types of the dental department in medical center are divided into independent type or comprehensive type according to the relationship with main outpatient department. The fourth one is that the spatial configuration of dental department in medical center is planned with 5-7 specialized departments and they are allocated in the dental department. Implications: In addition to the spatial configuration of dental department in medical center, it is necessary to analyze the other factors like circulation, relationship with other facilities in medical center to develop the dental healthcare system.

Possibility of a Standardized System for Task-Related Hierarchal Positions of Dental Hygienists

  • Background: Despite the demand for greater competence of dental hygienists in recent times, and the consequent standardization of systems in the organizations, there is a lack of a proper system for hierarchal positions of dental hygienists. The aim of this study was to identify the tasks performed by Korean dental hygienists in various work settings and hierarchal positions to suggest a standardized system for task-related hierarchal positions. Methods: A survey was conducted by sending questionnaires to 966 dental hygienists, and responses from 757 participants were included in the final analysis. The survey comprised questions regarding participants' general characteristics, tasks, current positions, and the internal policies or standards of the respective organizations to define these positions. The participants' positions were analyzed as frequency and cross-sectional analysis with respect to their general characteristics, their place in the organizational charts, the size of their workplaces, and differences in the tasks performed. Results: Data showed that 82.6% of the hospitals and clinics in Korea have organizational systems to determine the positions, and there was a significant difference in the systems according to the size of the health facility (p<0.05). The standards or policies used to define positions within general or university hospitals were determined by internal regulations or bylaws, whereas work experience was the major determinant of positions in smaller dental hospitals or clinics. The main tasks performed by dental hygienists in Korea were dental assistance (81.2%), routine dental hygiene tasks (53.4%), and administrative tasks, such as reception and billing (46.1%). Conclusion: Most dental hygienists in Korea are actively involved in dental assistance. Therefore, their specific tasks and responsibilities should be standardized more effectively. The data obtained from this study can be used to establish a standardized position system.
