치수생활력이란 치수내의 혈관의 상태를 가리킨다. 치수생활력 측정을 위해서 사용되어져 온 온도검사나 전기치수 검사는 치수신경의 반응에 기초하여 간접적으로 치수의 생활력을 판단하는 방법이다. 따라서 치수생활력의 판단을 위해서 치수생활력의 기본인 치수혈류를 파악할 필요가 있어 laser Doppler flowmeter를 이용하여 치수 혈류를 측정하는 방법이 연구되었다. 본 연구에서는 성인치아에서 치수생활력 검사를 위해 laser Doppler flowmeter를 이용하여 치수혈류를 측정 할 경우 생활력의 판단 근거로 참고할 수 있는 정상혈류 측정치의 기초자료를 마련할 목적으로, laser Doppler flowmeter를 이용하여 성인의 상악 중절치에서부터 제1대구치에 이르기까지 각 정상 치아의 치수혈류를 측정하고, probe 고정용 splint 제작 방법 및 생활치수와 실활치수에 따른 치수혈류 측정치의 차이를 비교, 연구하였다. 22세에서 30세까지 성인 남녀 79명을 대상으로 상악 중 절치에서부터 제1대구치에 이르기까지 laser Doppler flowmeter를 사용하여 치수혈류를 측정하였고, 생활치아에 대한 비교군으로는 실활치아라고 판단되는 치아를 가진 성인 남녀 자원자 5명의 치아 13개도 함께 검사하였다. 혈류 측정 probe고정용 splint제작을 위한 고무인상시 직접법에서는 환자 구강내에서 직접 고무인상을 채득하였고, 간접법에서는 구강내에서 일차 인상을 채득하여 만든 석고모형상에서 probe 위치를 표시한 다음 석고모형상에서 이차적으로 고무인상을 채득하였다. 고무인상에서 치관부에 probe 고정을 위한 hole을 형성하고 probe를 삽입, 고정한 다음 probe를 치아 협면에 적용하였다. Laser Doppler flowmeter에서 안정된 신호가 나타날 때까지 피검자를 10분 이상 동안 안정시켰으며 안정상태 도달 후 각 치아마다 5 분간 지수혈류를 기록하였다. 측정된 치수혈류는 'Perisoft'(Perimed Co., Sweden) 프로그램을 이용하여 컴퓨터에 저장한 후 측정치를 계측하였다. 각 치아간의 실험치 평균의 차이는 일원 변량 분석법(one-way ANOVA) 및 Duncan's Multiple Range test를 이용하여 분석하였으며, 다음과 같은 결과를 얻었다. 1. 상악 중절치에서부터 제1대구치에 이르기까지 시험대상 전 치아에서의 치수혈류 측정치 평균은 9 - 16 Perfusion Unit 사이로 나타났는데, 상악 측절치에서 가장 높게 나타났고, 제1소구치, 제2소구치, 견치, 중절치, 제1대구치의 순으로 높게 나타났다(p<0.01). 2. 상악 6전치에서는 간접인상법으로 제작된 splint 적용군이 직접인상법으로 제작된 splint 적용군에 비해 높은 측정치를 나타내었으나(p<0.01). 소구치 및 대구치에서는 측정치에 있어서 차이를 나타내지 않았다(p>0.05) 3. 생활치아에서는 실활치아에 비해 현저히 높은 측정치를 나타내었고(p<0.01). 측정선의 양상은 심장 박동과 일치하는 파동성 및 율동성을 나타내었다.
본 연구의 목적은 다양한 MTA 재료(ProRoot MTA, RetroMTA, Endocem Zr)에 대한 유치 치수 세포의 생활력 및 분화능을 비교 평가하는 것이다. 유치 치수세포는 각 재료별로 경화된 원형 디스크를 이용하여 직접법 및 간접법으로 세포 생활력을 관찰하였다. 또한 재료별 추출물을 이용하여 pH를 측정하였으며, alkaline phosphatase(ALP) 활성도 및 Alizarin Red S 염색법을 통하여 세포의 분화능을 관찰하였다. 직접법에서 유치 치수세포는 ProRoot MTA와 RetroMTA에서 Endocem Zr에 비해 높은 세포 생활력을 보였으나, 반면 간접법에서는 Endocem Zr에서 다른 재료에 비해 높은 세포 생활력이 관찰되었다. pH의 경우 Endocem Zr가 다른 두 재료에 비해 낮은 알칼리성을 나타냈다. 모든 재료에서 ALP 활성도는 대조군에 비해 증가하지 않았으며, Alizarin Red S 염색결과 유치 치수세포의 분화능이 대조군에 비해 낮았다. 본 실험에서 재료별 차이는 있었으나 모든 재료에서 어느 정도의 세포 독성이 관찰되었으며, 유치 치수세포의 생활력과 분화능을 증진시키지 못하였다. 하지만 Endocem Zr의 경우 ProRoot MTA나 RetroMTA에 비해 낮은 알칼리성과 높은 생활력을 보였다.
PURPOSE. To evaluate the cytotoxicity of temporary luting cements on bovine dental pulp-derived cells (bDPCs). MATERIALS AND METHODS. Four different temporary cements were tested: Rely X Temp E (3M ESPE), Ultratemp (Ultradent), GC Fuji Temp (GC), and Rely X Temp NE (3M ESPE). The materials were prepared as discs and incubated in Dulbecco's modified eagle's culture medium (DMEM) for 72 hours according to ISO 10993-5. A real-time cell analyzer was used to determine cell vitality. After seeding $200{\mu}L$ of the cell suspensions into the wells of a 96-well plate, the bDPCs were cured with bioactive components released by the test materials and observed every 15 minutes for 98 hours. One-way ANOVA and Tukey-Kramer tests were used to analyze the results of the proliferation experiments. RESULTS. All tested temporary cements showed significant decreases in the bDPCs index. Rely X Temp E, GC Fuji Temp, and Rely X Temp NE were severely toxic at both time points (24 and 72 hours) (P<.001). When the cells were exposed to media by Ultratemp, the cell viability was similar to that of the control at 24 hours (P>.05); however, the cell viability was significantly reduced at 72 hours (P<.001). Light and scanning electron microscopy examination confirmed these results. CONCLUSION. The cytotoxic effects of temporary cements on pulpal tissue should be evaluated when choosing cement for luting provisional restorations.

The purpose of this study was to investigate the influence of NK1 receptor antagonists on the pulpal blood flow (PBF) when applied iontophoretically through the dentinal cavity of the teeth in order to understand whether iontophoretically applied NK1 receptor antagonists can control the pulpal inflammation. Eleven cats were anesthetized with alpha-chloralose and urethane, and substance P (SP) was administered to the dental pulp through the catheterized lingual artery in doses that caused PBF change without the influence of systemic blood pressure. NK1 receptor antagonists were applied iontophoretically to the prepared dentinal cavity of ipsilateral canine teeth of the drug administration, and PBF was monitored. Data were analyzed statistically with paired t-test. PBF increase after iontophoretic application of the NK1 receptor antagonists followed by the intra-arterial administration of SP was significantly less than PBF increase after iontophoretic application of the 0.9% saline followed by the intra-arterial administration of SP as a control (p < 0.05). Iontophoretic application of the NK1 receptor antagonists (0.2~3.4 mM) following the intra-arterial administration of SP resulted in less increase of PBF than the iontophoretic application of the 0.9% saline following the intra-arterial administration of SP as a control (p < 0.05). Therefore. the results of the present study provide evidences that the iontophoretic application is an effective method to deliver drugs to the dental pulp. and that iontophoretically applied NK1 receptor antagonists block SP-induced vasodilation effectively. The above results show the possibility that the iontophoretical application of NK1 receptor antagonists can control the neurogenic inflammation in the dental pulp.
Objectives: This study aimed to determine whether collagen triple helix repeat containing-1 (CTHRC1), which is involved in vascular remodeling and bone formation, can stimulate odontogenic differentiation and angiogenesis when administered to human dental pulp stem cells (hDPSCs). Materials and Methods: The viability of hDPSCs upon exposure to CTHRC1 was assessed with the WST-1 assay. CTHRC1 doses of 5, 10, and 20 ㎍/mL were administered to hDPSCs. Reverse-transcription polymerase reaction was used to detect dentin sialophosphoprotein, dentin matrix protein 1, vascular endothelial growth factor, and fibroblast growth factor 2. The formation of mineralization nodules was evaluated using Alizarin red. A scratch wound assay was conducted to evaluate the effect of CTHRC1 on cell migration. Data were analyzed using 1-way analysis of variance followed by the Tukey post hoc test. The threshold for statistical significance was set at p < 0.05. Results: CTHRC1 doses of 5, 10, and 20 ㎍/mL had no significant effect on the viability of hDPSCs. Mineralized nodules were formed and odontogenic markers were upregulated, indicating that CTHRC1 promoted odontogenic differentiation. Scratch wound assays demonstrated that CTHRC1 significantly enhanced the migration of hDPSCs. Conclusions: CTHRC1 promoted odontogenic differentiation and mineralization in hDPSCs.
Background: This study evaluated the efficacy of virtual reality (VR) distraction and counter-stimulation (CS) on dental anxiety and pain perception to local anesthesia in children. Methods: A prospective, randomized, single-blinded interventional clinical trial with a parallel design was used. Seventy children 7-11 years old who required local anesthesia (LA) for pulp therapy or tooth extraction were recruited and allocated to two groups with equal distribution based on the intervention. Group CS (n = 35) received CS and Group VR (n = 35) received VR distraction with ANTVR glasses. Anxiety levels (using pulse rate) were evaluated before, during, and after administration of local anesthesia, while pain perception was assessed immediately after the injection. Wong-Baker faces pain-rating scale (WBFPS), visual analog scale (VAS), and Venham's clinical anxiety rating scale (VCARS) were used for pain evaluation. Student's t-test was used to test the mean difference between groups, and repeated measures ANOVA was used to test the mean difference of pulse rates. Results: Significant differences in mean pulse rates were observed in both groups, while children in the VR group had a higher reduction (P < 0.05), and the mean VCARS scores were significant in the VR group (P < 0.05). Mean WBFPS scores showed less pain perception to LA needle prick in the CS group while the same change was observed in the VR group with VAS scores. Conclusions: VR distraction is better than CS for reducing anxiety to injection in children undergoing extraction and pulpectomy.
After orthognathic surgery, postoperative complications are studied by many clinician. The complications include sensory disturbance, jaw fracture, excessive bleeding, condylar positional changes and loss of pulp vitality. Few surgical procedures are as satisfying for the surgeon and patient as a well-done orthognathic surgery. On the other hand, the patient is more satisfied with the result than who are treated with only orthodontic treatment especially in severe deformity case. There are problems that patient overcome but it is not serious complications. One of these, the problem about loss of pulp vitality can't influence function but give a lot of discomfort to the patient. From September 1997 to January 1998, 7 patients who are treated for dentofacial deformity via Le Fort I osteotomy or anterior segmental osteotomy were examined pulp sensitivity using digital pulp tester. This preliminary study have a focus on the investigation of recovery of pulp vitality. The electric pulpal test were used at preoperative, postoperative, at intervals. And we report some results acquired from this study. Follwing result are obtained 1. In anterior segmental ostetomy case (1 case), total 12 teeth were examined. Background: This study aimed to evaluate the efficacy of external vibrating devices and counterstimulation on a child's dental anxiety, apprehension, and pain perception during local anesthetic administration. Methods: This was a prospective, randomized, parallel-arm, single-blinded interventional, clinical trial. One hundred children aged 4-11 years, requiring pulp therapy or extraction under local anesthesia (LA), were recruited and allocated equally into two groups (1:1) based on the interventions used: Group BD (n = 50) received vibration using a Buzzy® device {MMJ Labs, Atlanta, GE, USA} as a behavior guidance technique; Group CS (n = 50) received counterstimulation for the same technique. Anxiety levels [Venham's Clinical Anxiety Rating Scale (VCARS), Venham Picture Test (VPT), Pulse oximeter {Gibson, Fingertip Pulse Oximeter}, Beijing, China)] were assessed before, during, and after LA administration, while pain perception [Wong-Baker Faces Pain Rating Scale (WBFPS), Visual Analogue Scale (VAS)] was evaluated immediately after injection. Statistical analysis was performed using the Student's t-test to assess the mean difference between the two groups and the repeated measures ANOVA for testing the mean difference in the pulse rates. Statistical significance was set at P < 0.05. Results: Significant differences in mean pulse rate values were observed in both groups. In contrast, the children in the BD group had higher diminution (P < 0.05), whereas the mean VCARS and VPT scores were conspicuous (P < 0.05). Based on the mean WBFPS and VAS scores, delayed pain perception after LA injection was more prominent in the BD group than in the CS group. Conclusion: External vibration using a Buzzy® device is comparatively better than counterstimulation in alleviating needle-associated anxiety in children requiring extraction and pulpectomy.
7세 남아가 맹출 중인 상악 영구 중절치의 외상을 주소로 내원하였다. 환아의 우측 중절치는 아탈구와 치근 파절을 보였고, 좌측 중절치는 함입성 탈구와 치근 파절을 보였다. 두 치아는 정복 후 선부자에 의해 고정하였다. 이후 주기적인 검진을 시행하였다. 4년이 지난 현재, 우측 중절치는 방사선 사진에서 파절편 사이에 방사선 투과성이 보이지 않고 정상적인 치조백선을 보여 경조직으로 치유되는 양상을 보였다. 좌측 중절치는 파절편이 분리되어있고 근관이 점차 폐쇄되는 소견을 보이고 있어 골과 결합조직의 개입에 의한 치유 양상을 보이고 있다. 임상 검사에서 정상적인 동요도와 타진 반응을 보였고, 전기치수검사에 양성 반응을 보였다. 외상을 받은 치아의 예후는 외상의 유형, 환자의 연령, 치근의 발육단계, 변위의 정도 등에 의해 영향을 받는다. 본 증례는 개방근관을 가진 치아가 외상 후 우측 중절치는 석회화 조직으로 치유, 좌측 중절치는 골과 결합조직의 개입으로 치유되고 있는 양상을 보이고 있어 이를 보고하는 바이다.
Objectives: This study aimed to evaluate the bleaching efficacy and hydrogen peroxide permeability in the pulp chamber by the at-home bleaching gel in protocols applied on different dental surfaces. Materials and Methods: Forty premolars were randomly into 4 groups: control group no bleaching, only application on the buccal surface (OB), only application on the lingual surface (OL) and application in buccal and lingual surfaces, simultaneously (BL). At-home bleaching gel (White Class 7.5%) was used for the procedure. The bleaching efficacy was evaluated with a digital spectrophotometer (color change in CIELAB [ΔEab] and CIEDE 2000 [ΔE00] systems and Whitening Index for Dentistry [ΔWID]). The hydrogen peroxide permeability in the pulp chamber (㎍/mL) was assessed using UV-Vis spectrophotometry and data were analyzed for a 1-way analysis of variance and Tukey's test (α = 0.05). Results: All groups submitted to bleaching procedure showed bleaching efficacy when measured with ΔEab and ΔE00 (p > 0.05). Therefore, when analyzed by ΔWID, a higher bleaching efficacy were observed for the application on the groups OB and BL (p = 0.00003). Similar hydrogen peroxide permeability was found in the pulp chambers of the teeth undergoing different protocols (p > 0.05). Conclusions: The application of bleaching gel exclusively on the OB is sufficient to achieve bleaching efficacy, when compared to BL. Although the OL protocol demonstrated lower bleaching efficacy based on the ΔWID values, it may still be of interest and relevant in certain clinical scenarios based on individual needs, requiring clinical trials to better understand its specificities.

제 19 조 (관할 법원)
서비스 이용으로 발생한 분쟁에 대해 소송이 제기되는 경우 민사 소송법상의 관할 법원에 제기합니다.

1. (시행일) 이 약관은 2016년 9월 5일부터 적용되며, 종전 약관은 본 약관으로 대체되며, 개정된 약관의 적용일 이전 가입자도 개정된 약관의 적용을 받습니다.
