임플란트 지지 고정성 치과 보철물 유지방식에 따른 고정체의 직경과 길이 분포 비교 연구 (A comparative study of the distribution of implant fixtures according to length and diameter by retained type of implant-supported fixed prosthesis)

  • The Purpose of this study was to compare the distribution of implant fixtures according to length and diameter between screw-retained and cement-retained implant-supported fixed prosthesis and to asses whether prosthesis retained types affected the selection of size of implant fixtures. This study presents a follow-up 2,416 implant-supported fixed type prosthesis that have been screw retained or cemented retained for about 10 years in 14 dental clinics. Included in the study were 458 men and 397 women and implant fixtures used in this study were screw retained type 1,057 and 1,359 of cemented retained type. The statistical results among the diameter types of fixture by prosthesis retained type was no significant difference noted (P= 0.809) and there was significant differences was enough to among the lengths of fixture by prosthesis retained type (P= 0.020). However there were no significant difference among the fixture diameter types and length by prosthesis retained type (P= 0.486). So there was not affected to prostheis fixation mechanism for the size of implant fixtures.

A Digitally Designed All-on-4 Restoration with Screwmentable Concept

  • An all-on-4 restoration allows edentulous patients to use a fixed prosthesis with a minimum number of implants. These implant-supported fixed complete dentures have traditionally been fabricated as screw-retained or cement-retained prostheses. However, it is difficult to passively fit the long-span full-arch prosthesis using the screw-retained type restoration, and predictable retrievability is not obtained with the cement-retained type. This case report describes a prosthesis fabricated using a combination of the two retention types. The screwmentable method allows the implant-supported fixed complete denture to achieve a passive fit at the connection with retrievability. In addition, a framework with an optimized size was designed by using digital dental technology.

임프란트 지지 국소의치의 임상적 연구 (Clinical study of implant supported removable partial dentures)

  • OBJECTIVE The purpose of clinical study was to evaluate the survival of dental implants used in restoring patients with implant supported removable partial dentures (ISRPDs) of different configurations. MATERIALS AND METHODS The sample consisted of 20 consecutively treated partially edentulous patients, who, between 2003 and 2008, had a total of 84implants placed in different arch sites and who were treated with ISRPDs. The mean age was 57 years. Mean follow up time from delivery of ISRPDs was 3 years 2 months (range, 1 to 6 years). Osseointegration failure, postoperative complication of dental prosthesis, and the success rate of ISRPDs were retrospectively evaluated using clinical and radiographic examination. RESULT The overall implant survival rate was 100%. During follow up, the one clasp of removable partial denture was broken in 1 year11 months after prosthesis delivery. All patients were satisfied with their prosthesis. CONCLUSIONS Implant supported removable partial dentures could serve as favorable prognosis. Careful patient selection, with an appropriate maintenance and recall system, is recommended to obtain satisfactory results.

Effect of cement washout on loosening of abutment screws and vice versa in screw- and cement- retained implant-supported dental prosthesis

  • PURPOSE. The purpose of this study was to examine the abutment screw stability of screw- and cement-retained implant-supported dental prosthesis (SCP) after simulated cement washout as well as the stability of SCP cements after complete loosening of abutment screws. MATERIALS AND METHODS. Thirty-six titanium CAD/CAM-made implant prostheses were fabricated on two implants placed in the resin models. Each prosthesis is a two-unit SCP: one screw-retained and the other cemented. After evaluating the passive fit of each prosthesis, all implant prostheses were randomly divided into 3 groups: screwed and cemented SCP (Control), screwed and non-cemented SCP (Group 1), unscrewed and cemented SCP (Group 2). Each prosthesis in Control and Group 1 was screwed and/or cemented, and the preloading reverse torque value (RTV) was evaluated. SCP in Group 2 was screwed and cemented, and then unscrewed (RTV=0) after the cement was set. After cyclic loading was applied, the postloading RTV was measured. RTV loss and decementation ratios were calculated for statistical analysis. RESULTS. There was no significant difference in RTV loss ratio between Control and Group 1 (P=.16). No decemented prosthesis was found among Control and Group 2. CONCLUSION. Within the limits of this in vitro study, the stabilities of SCP abutment screws and cement were not significantly changed after simulated cement washout or screw loosening.

Trend analysis of prosthodontic treatment modality between 2005 and 2008 in Seoul National University Dental Hospital

  • PURPOSE. The aim of this article is to analyze the preference for treatment modality of dentists. MATERIAL AND METHODS. Data of 20,038 patients was involved. Data analysis were done by distribution according to the various kinds of prosthesis, including complete denture, removable partial denture, fixed partial denture, implant-supported dental prosthesis as well as distribution according to the professional titles of the dentists finishing the treatment, including resident and professors. RESULTS. The number of cases of dental prosthesis increased year by year. 61.06% of the patients accepted fixed partial denture restoration. The number of patients who accepted implant supported restoration is also increasing year by year. The number of complete denture, implant-supported dental prosthesis finished by professors was larger than that done by residents, while it was contrary for removable partial denture, fixed partial denture, and the difference was statistically significant (P < .05). CONCLUSION. Professors and residents have some difference in the categorization of prosthesis finished. Fixed partial denture and implant-supported dental prosthesis are preferred.

Does the prosthesis weight matter? 3D finite element analysis of a fixed implant-supported prosthesis at different weights and implant numbers

  • PURPOSE. This study evaluated the influence of prosthesis weight and number of implants on the bone tissue microstrain. MATERIALS AND METHODS. Fifteen (15) fixed full-arch implant-supported prosthesis designs were created using a modeling software with different numbers of implants (4, 6, or 8) and prosthesis weights (10, 15, 20, 40, or 60 g). Each solid was imported to the computer aided engineering software and tetrahedral elements formed the mesh. The material properties were assigned to each solid with isotropic and homogeneous behavior. The friction coefficient was set as 0.3 between all the metallic interfaces, 0.65 for the cortical bone-implant interface, and 0.77 for the cancellous bone-implant interface. The standard earth gravity was defined along the Z-axis and the bone was fixed. The resulting equivalent strain was assumed as failure criteria. RESULTS. The prosthesis weight was related to the bone strain. The more implants installed, the less the amount of strain generated in the bone. The most critical situation was the use of a 60 g prosthesis supported by 4 implants with the largest calculated magnitude of 39.9 mm/mm, thereby suggesting that there was no group able to induce bone remodeling simply due to the prosthesis weight. CONCLUSION. Heavier prostheses under the effect of gravity force are related to more strain being generated around the implants. Installing more implants to support the prosthesis enables attenuating the effects observed in the bone. The simulated prostheses were not able to generate harmful values of peri-implant bone strain.

개선된 설측 고정 나사 시스템(T-screw system)을 이용한 임플란트 보철 수복 증례 (Clinical cases of implant-supported fixed dental prosthesis using modified lingual screw system (T-screw system))

  • 임플란트 보철물은 나사 유지형 보철물과 시멘트 유지형 보철물로 나눌 수 있다. 각 방법은 장점과 함께 임플란트를 유지 관리하는데 불리한 단점을 가지고 있다. 이러한 단점을 극복하기 위해 개선된 설측 고정 나사 시스템(T-screw system)이 개발되었다. T-screw system은 설측 방향의 나사를 이용하여 임플란트 보철물을 유지하는 방법으로, 보철물의 탈부착이 쉽고, 수동적 적합이 가능하며, 심미적이고 기능적인 교합면을 형성할 수 있다는 장점이 있다. 기존의 수평 나사를 이용한 보철은 기공 과정이 어려워 다수 유닛의 보철물의 경우 제작이 쉽지 않았고, 완전 도재를 이용한 보철물에서는 사용할 수 없다는 한계를 가지고 있었다. 본 증례에서 T-screw system을 이용하여 임플란트 보철물을 제작함으로써 유지 관리가 용이하고 심미적, 기능적으로 우수한 결과를 얻었다. 또한 다수 유닛의 임플란트 보철 및 완전 도재를 이용한 임플란트 보철에서 T-screw system을 이용하는 방법을 보고하고자 한다.

Prosthetic misfit of implant-supported prosthesis obtained by an alternative section method

  • PURPOSE. Adequate passive-fitting of one-piece cast 3-element implant-supported frameworks is hard to achieve. This short communication aims to present an alternative method for section of one-piece cast frameworks and for casting implant-supported frameworks. MATERIALS AND METHODS. Three-unit implant-supported nickel-chromium (Ni-Cr) frameworks were tested for vertical misfit (n = 6). The frameworks were cast as one-piece (Group A) and later transversally sectioned through a diagonal axis (Group B) and compared to frameworks that were cast diagonally separated (Group C). All separated frameworks were laser welded. Only one side of the frameworks was screwed. RESULTS. The results on the tightened side were significantly lower in Group C ($6.43{\pm}3.24{\mu}m$) when compared to Groups A ($16.50{\pm}7.55{\mu}m$) and B ($16.27{\pm}1.71{\mu}m$) ($P$ <.05). On the opposite side, the diagonal section of the one-piece castings for laser welding showed significant improvement in the levels of misfit of the frameworks (Group A, $58.66{\pm}14.30{\mu}m$; Group B, $39.48{\pm}12.03{\mu}m$; Group C, $23.13{\pm}8.24{\mu}m$) ($P$ <.05). CONCLUSION. Casting diagonally sectioned frameworks lowers the misfit levels. Lower misfit levels for the frameworks can be achieved by diagonally sectioning one-piece frameworks.

임시 의치와 이중 디지털 스캐닝 기법을 활용한 전악 고정성 임플란트 수복 증례 (Full mouth rehabilitation with fixed implant-supported prosthesis using temporary denture and double digital scanning technique: a case report)

  • 임플란트 고정성 보철물 제작 시 디지털 시스템을 이용하여 수복할 때, 3차원적인 임플란트 위치의 구내 스캔과 임시 의치 혹은 임시 보철물 장착 후 구내 스캔을 이용한 이중 디지털 스캐닝(double digital scanning) 기법이 필요하다. 구내 스캔 시, 스캔 바디를 안정적인 랜드마크로 이용하면 다지털 인상의 정확도 향상과 기공과정이 효율적이고 단순화될 수 있다. 본 증례는 완전 디지털 시스템을 활용하여 맞춤형 치과용 임플란트 지대주 및 임시 보철물, 최종 보철물을 계획하고 제작하였다. 임플란트 식립 후, 임플란트에 체결한 구내 스캔 바디와 임시 의치 조직면에 인기 된 구내 스캔 바디의 스캔 영역을 중첩하였다. 중첩된 파일에서 임시 의치의 수직 고경에 맞는 맞춤형 치과용 임플란트 지대주와 임시 보철물을 제작하여 환자에게 시적 하였고, 임시 보철물을 토대로 최종 보철물을 제작하였다. 임시 의치 제작 시 설정한 수직 고경, 중심위 등을 최종 보철물까지 활용하여 기능 및 심미적으로 만족할 만한 결과를 얻어 이를 보고하고자 한다.

O-ring abutment를 이용한 Implant over denture의 제작 (Implant supported over denture with O-ring abutment)

  • The purpose of this study is to introduce how to make implant supported over denture with Oring. Many kinds of attachments have used to dental restorations. The application of attachment has widely increased implant fixed prosthesis and implant supported over denture. In order that implant supported over denture have properly retention, generally used O-ring, magnetic, bar attachment. O-ring give us an advantage that is required more minimum vertical dimension than bar-type and easily replace with new part. When we make these prosthesis using O-ring, Bar, Ball attachment, we should following procedures. Strong occlusion force leads to fracture of over denture because part of functional mechanism as implant abutment or attachment is spaced. Clips are regularly activated. O-ring and springs are changed every year. The pattern of resorption should be carefully monitored and compensated for by relining procedures. If the over denture appears to rest on the bar or the ball attachments, relining should be performed and clips/caps should be changed.
