Haddad 증후군 환아의 외래 전신마취 하 치과 치료 (Dental Treatment of a 3-Year-Old Male Patient with Haddad Syndrome under Outpatient General Anesthesia)

  • The co-occurrence of congenital central hypoventilation syndrome (CCHS) and Hirschsprung's disease (HD) is termed Haddad syndrome, which is an extremely rare discorder. It was reported first by Haddad in 1978 and there are approximately 60 cases reported in the worldwide literature. Recent studies described that congenital central hypoventilation syndrome had deep relation to the mutation of the PHOX2B gene in its diagnosis and phenotype. This article presents a case report: Dental treatment of a 3-year-old male patient with Haddad syndrome under outpatient general anesthesia. The special considerations of dental care, especially caries theatment of the patient with Haddad syndrome are discussed.

Treatment of severe pain in a patient with complex regional pain syndrome undergoing dental treatment under general anesthesia: A case report

  • Complex regional pain syndrome (CRPS) is rare, characterized by pain from diverse causes, and presents as extreme pain even with minor irritation. General anesthesia may be required for dental treatment because the pain may not be controlled with local anesthesia. However, treatment under general anesthesia is also challenging. A 38-year-old woman with CRPS arrived for outpatient dental treatment under general anesthesia. At the fourth general anesthesia induction, she experienced severe pain resulting from her right toe touching the dental chair. Anesthesia was induced to calm her and continue the treatment. After 55 minutes of general anesthesia, the patient still complained of extreme toe pain. Subsequently, two administrations for intravenous sedation were performed, and discharge was possible in the recovery room approximately 5 h after the pain onset. The pain was not located at the dental treatment site. Although the major factor causing pain relief was unknown, ketamine may have played a role.

한국의료패널의 치과외래 이용 및 의료비 지출에 대한 4년간(2008-2011)의 변화추이 (Dental utilization and expenditures in Korea Health Panel Survey, 2008-2011)

  • Korea Health Panel Survey (KHPS) has been conducted to provide nationally representative estimates of health care use and expenditures for non-institutionalized population in Korea since 2008. The aim of this study was to present estimates from KHPS on dental utilization and expenditures from 2008 to 2011. While weighted estimates were presented for each year from 2008 to 2011, the focus of this study was on the 2011 estimates and changes in dental utilization and expenditures between 2008 and 2011. Weighted estimates were obtained using population weights and statistical significances were analysed by independent t-test or ANOVA test using SPSS version 20 for Windows. In 2011, 21.1% of Korean population obtained outpatient dental services from dentists. From 2008 to 2011, the percent of people with a dental visit in each year increased slightly. In 2011, the mean number of dental visits was 4.46. While the mean number of dental visits remained stable by 2010, it increased slightly in 2011. The total annual dental expense in 2011 for people with a visit was about 460,000 Korean Won and 10% higher in 2008. In summary, this study results highlight low rates of dental utilization and high dental expenses in Korean population.

  • This study aims to decide optimized size of a dental care unit. The efficient treatment room makes its user so comfortable that treatment can be easier and faster. Therefore, an operating room is the most important part in the dental hospital. This study focused on the outpatient care unit of dental hospital. The hospital for the case study is selected recently rebuilt dental university hospital. This study is to survey the behavior of all user in the sector and treatment unit. As the result, optimized plan type and size for each cases are proposed.

정신지체 환아의 치과치료를 위한 외래마취관리 -증례보고- (Anesthetic Management of a Mentally Retarded Child during Dental Treatment -A case report -)

  • General anesthesia is often required for mentally retarded children undergoing extensive dental treatment. We experienced a case of dental treatment under general anesthesia in a 14-year-old boy with mental retardation. He was treated on an outpatient basis. He was diagnosed of Noonan syndrome and received heart surgery when he was six years old. Induction using thiopental and vecuronium was uneventful and nasotracheal intubation were carried out. General anesthesia was maintained with sevoflurane for 2.5 hours. After monitoring the patient for 2 hours and confirming his recovery, he was discharged from the day care unit. In summary, we report this successful anesthetic management of a mentally retarded child during dental treatment in as an out-patient.

The Effect of the Impacted Position of Palatally Inverted Mesiodens on the Selection of Sedation Method

  • Purpose: Hyperdontia is a developmental disorder of the oral cavity. Mesiodens refers to the hyperdontia located between the maxillary central incisors. During the surgical procedure, the anesthetic method for pain control should be considered along with factors related to the surgery itself. The purpose of this study was to evaluate the effect of the impacted position of the mesiodens on the selection of sedation method and to suggest incisive foramen as a brief reference. Materials and Methods: This study included 126 patients who were scheduled for extraction of mesiodens. The selection criteria included patients with one palatally impacted inverted mesiodens accessible from the palatal gingival margin, and those with good cooperation potential in order to control for clinical information. Using cone beam computed tomography, vertical, horizontal, and palatal positional factors were measured, and the anesthetic method was determined by two examiners. The patients were grouped into vertical and horizontal groups based on the position of the incisive foramen. Data were statistically analyzed using the Mann-Whitney test, the chi-square test, and logistic regression analysis. Result: All positional factors differed between the outpatient and inpatient anesthetic groups. The vertical minimum distance from the alveolar ridge to the mesiodens (Va) and the minimum distance from the palatal surface to the crown tip of the mesiodens (Tc) were factors affecting the choice of anesthetic method. The distribution of the vertical and horizontal positional groups differed between the outpatient and inpatient anesthetic groups. Conclusion: The incisive foramen can be used as a brief reference to determine the appropriate anesthetic method. Referral for inpatient anesthesia may be a priority if they are in the V2H2 group with Va ≥5 mm, and Tc ≥6 mm, and outpatient sedation may be considered if they are in the V1H1 group with Va ≤1.5 mm, and Tc ≤2.5 mm.

불안조절과 치주수술 (Anxiety Control and Periodontal Practice)

  • Chronic periodontitis is one of the most common diseases in clinical dentistry, which requires various surgical interventions to treat the moderately to severely destructed supporting periodontium. Most patients have anxiety and fear to these surgical procedures and dentists often have problems dealing with these patients. By applying the conscious sedation technique in outpatient units, periodontists have become able to manage their patients successfully with less anxiety or fear. Also, we have experienced the increased level of patients' satisfaction. Generally, periodontal treatments are time consuming procedures and patients are usually reluctant to the periodontal instruments. This study is focused on the sedation procedure with intravenous midazolam infusion performed in Department of Periodontology of Ewha Womans University Hospital. Using questionnaires, we evaluated 80 randomly selected patients for the anxiolytic effect of intravenous midazolam. Anxiety control using IV sedation was very helpful in performing various periodontal reconstructive and advanced surgical procedures in implant dentistry.

치주임상에서의 정주진정법의 적용 (Intravenous Sedation in Periodontal Practice)

  • Chronic periodontitis is one of the most common disease in clinical dentistry, which needs various surgical interventions to treat the moderate to severe destruction of supporting periodontium. Most patients have fear and anxiety to these surgical procedures and some dentists also have problems to deal with these patients. Applying the conscious sedation technique in outpatient units, periodontists have become to manage their patients successfully without any fear and anxiety and increased the level of patients' satisfaction. Particularly, periodontal treatments require relative long chair-time and patients are usually reluctant to the periodontal instruments used in the oral cavity. This study is focused on the sedation procedure with intravenous midazolam infusioon performed in Department of Periodontology, Ewha Womans University Hospital. The whole procedure of intravenous midazolam sedation is briefly reviewed from patient selection to patient discharge.

  • To analyze the outpatient of the Dental Infirmary Yonsei Medical Center, anthor ruled out the charts of 1,622 patients which were sampled from 18.921 patients among the selected years 1973.1977 and 1981. The results are as follows : 1. By the age-group the twenties and thirties were about 50% of the total patients. 2. According to the analysis by the district, the distance from residence was most influencial to select the hospital. 3. The frequency of the patient by the service departments, many patients wanted oral sugery tretment, and operative & periodontic tretments followed, And there were some necessities of dental health education for the early care and the protective tretment. 4. Of the season, there were more patients in Summer and Winter, especially in July and August than Spring and Autumn. It might be related with the vacations. 5. The ratio of professor's and general patients, were decreasing compared with the data of three selected years, and the fact might be studied to improve for the better quality of service, even the application of the medical and dental insurance is getting wider.

  • Pelizaeus-Merzbacher disease (PMD) can be defined as an X-linked recessive leukodystrophy that is caused by a mutation in the proteolipid protein gene on chromosome Xq22. PMD is one of a group of progressive, degenerative disorders of the cerebral white matter known as the leukodystrophies. Due to the progressive nature of the disorders and their devastating effects on the central nervous system, these children frequently require anesthesia during imaging procedures such as MRI or during various surgical procedures. Anesthetic concerns in theses cases include high prevalence of seizure disorders, gastroesophageal reflux with the risk of aspiration, airway complications related to poor pharyngeal muscle control and copious oral secretions, and mental retardation. We report a successful anesthetic management in a patient with PMD for dental procedures.
