전문대학 보건계열학과 전공학생의 NCS 기반 교과내용 개발 적용에 따른 직무능력 단위요소의 전공교육 적합성 평가 (A Study on Fitness Evaluation for Major Education of Competency Unit Element by the Development and Application of Subject Contents Based on NCS of Health Majoring Students at Junior Colleges)

  • 본 연구는 전문대학 보건계열학과의 NCS 기반 신규 교과 개발적용에 따른 전공교육에의 적합성 여부를 밝히는데 목적을 갖고 2016년 3월 11일부터 3월 30일까지 전문대학 보건계열학과 학생 300명을 대상으로 설문한 내용을 바탕으로 통계분석 하였다. 연구결과, 보건계열 전문대학생의 NCS에 대한 인지도와 통합취지 및 교과내용 이해도가 기존 교육과의 연계접목에 대한 적합성에 유의한 영향을 미치는 것으로 나타났으며 이는 새로 개발된 NCS 기반 직무능력 단위요소의 전공교육에 대한 직무, 일, 자격, 훈련 표준화가 잘 연계됨을 의미한다. 또한 이를 받아들이는 학생들이 새로 도입된 교과내용에 대체적으로 빠르게 동화되고 있음을 알 수 있었으며 특히 기존교재와 다르게 3개 전공학과 간 표준화된 교과의 통합개발에 따른 이해도가 높은 것으로 평가될 수 있다. 결론적으로 지식 통합 교육의 높은 인지도와 이해도로 인해 보건계열 교과 적용수업 효과가 매우 긍정적이며 높게 나타나는 것으로 기대되며 향후 보건계열 NCS 체제의 조기 정착을 시사하고 NCS 개발내용의 현장 적합성을 높일 수 있을 것으로 기대할 수 있다.

Multiplex PCR Using Conserved and Species-Specific 16S rDNA Primers for Simultaneous Detection of Fusobacterium nucleatum and Actinobacillus actinomycetemcomitans

  • This study was undertaken to develop PCR primers for the simultaneous detection of Fusobacterium nucleatum and Actinobacillus actinomycetemcomitans, using two species-specific reverse primers in combination with a single conserved forward primer. These primers target the variable and conserved regions of the 16S rDNA. The primer specificity was tested against (i) four F. nucleatum and three A. actinomycetemcomitans strains and (ii) seven representatives of the different species of oral bacteria. The primer sensitivity was determined by testing serial dilutions of the purified genomic DNA of F. nucleatum and A. actinomycetemcomitans. The data indicate that species-specific amplicons could be obtained for all the F. nucleatum and A. actinomycetemcomitans strains tested, which were not found in the seven other species. The multiplex PCR could detect as little as 4 fg of chromosomal DNA of F. nucleatum and A. actinomycetemcomitans simultaneously. These findings suggest that these PCR primers are highly sensitive and are suitable for applications in epidemiological studies, diagnosis, and monitoring F. nucleatum and A. actinomycetemcomitans after the treatment of periodontitis.

고양이 삼차신경꼬리핵에서 저역치기계자극수용기 유래 들신경종말의 연접이전종말이 함유하는 신경전달물질에 대한 정량적 분석 (Quantitative Analysis of Neurotransmitters in the Endings Presynaptic to Vibrissa Afferent Terminals in the Cat Trigeminal Caudal Nucleus)

  • 이 연구에서는 삼차신경꼬리핵 제 3~4층에서 저역치기계자극정보를 전달하는 일차들신경섬유의 종말과 연접하는 연접이전종말(presynaptic ending; p-ending)들이 어떤 억제성 신경전달물질을 함유하는 지를 분석하고자 하였다. 이를 위해 전기생리학적으로 동정된 고양이콧수염유래 일차들신경종말을 단일 축삭내 HRP주입법으로 표식하였고, GABA와 glycine에 대한 항혈청으로 포매후금입자면역염색법을 시행한 후, 정량적 분석을 실시하였다. 표식종말과 연접하는 16개 p-ending들 중 8개(50%, 8/16) p-ending들은 GABA만을 함유하였으며, 나머지 8개(50%, 8/16) p-ending들은 GABA와 glycine 모두를 함유하는 집단으로 분류할 수 있었다. 또한, 이 두 집단의 p-ending 사이에는 유의한 평균체적의 차이가 보이지 않았으며, 각 p-ending이 함유하는 GABA와 glycine의 상대적 함량은 서로 달랐다. 이러한 결과들은 삼차신경꼬리핵에서 콧수염유래 일차들신경섬유에 의해 전달되는 저역치기계자극정보는 GABA 및 glycine에 의해 연접이전제어(presynaptic modulation)를 받으며, 그 연접이전제어는 각 일차들신경섬유의 종말마다 다르게 나타날 것 이라는 점을 제시한다.

조선소 근로자들의 구강보건교육경험 유무에 따른 구강보건인식의 차이 (Relationship of Oral Health Education Experience to Oral Health Awareness among Shipbuilding Workers)

  • 작업공정 특성상 작업환경이 열악하다고 판단되는 조선소 근로자의 구강보건증진과 구강보건교육계획, 산업구강보건정책 및 제도 개선에 기여하고자 편의표본추출법에 의해 선정한 전남 지역 조선소 및 협력업체 10곳에 근무하는 조선소 근로자를 대상으로 구강보건교육 경험에 따른 구강보건인식의 차이를 알아보기 위하여 조사한 결과는 다음과 같다. 연구대상자 310명 중 구강보건교육경험이 없는 근로자는 89.4%이었고, 구강보건교육 무경험자는 구강보건 지식정도, 인식정도, 관심정도, 중요정도가 낮은 것으로 나타났으며 구강보건 지식정도, 인식정도, 관심정도, 중요정도 간에는 상관관계가 높은 것으로 나타났다. 구강보건교육 무경험자가 구강보건교육을 받지 못하는 이유는 기회가 없었기 때문인 것으로 나타났고 선호하는 교육내용은 치아우식증예방 및 치료법, 선호하는 교육방법은 치의사의 강연인 것으로 나타났으며 구강보건교육 경험이 있는 근로자일수록 구강보건교육 경험이 없는 근로자보다 구강보건교육에 참여할 의향이 있는 것으로 나타났다.

보존적 치료를 이용한 TMD 환자에 관한 연구 (Study on Temporomandibular Disorder Patients with Conservative Treatment)

  • 2012년 7월부터 2012년 9월까지 부산대학교치과병원 구강내과에 내원하여 측두하악장애로 진단받은 후 행동치료, 약물치료, 물리치료, 교합안정장치 등 보존적 치료를 시행한 환자 137명을 대상으로 초진 시와 치료 3개월 후의 증상 및 임상소견을 비교, 평가하여 다음과 같은 결과를 얻었다. 1. 측두하악장애는 행동치료, 물리치료, 약물치료, 교합안정장치 등의 보존적 치료에 의해 통증, 관절잡음, 개구제한 및 최대편이개구량이 현저히 개선되었다. 2. 보존적 치료에 따른 결과는 성별 및 급, 만성별로 유의한 차이 없이 개선되었다. 3. 측두하악장애의 치료에서 물리치료와 약물치료만 시행한 경우보다 교합안정장치를 병행한 경우에서 임상 증상들의 개선이 현저하였다. 4. 치료 3개월 후 근육장애군은 통증과 개구제한이 유의하게 개선되었고, 관절원판변위군은 통증, 개구제한 및 관절잡음이 유의하게 개선되었다. 골관절염군에서는 통증, 관절잡음, 개구제한 및 최대편이개구량이 모두 개선되었다.

집토끼 하악골에서 신연 골형성술시 BMP-2,-4의 발현 (EXPRESSION OF BMP-2, -4 DURING DISTRACTION OSTEOGENESIS IN THE RABBIT MANDIBLE)

  • Distraction osteogenesis(DO) is defined as a gradual mechanical process of mechanical stretching two vascularized bone surface apart with a critical rate and rhythm such that new bone forms within the expanding gap, reliably bridges the gap, and ultimately remodels to normal structure. DO has become a mainstay in bone tissue engineering and has significantly improved our armamentarium for reconstructive craniomaxillofacial procedures. But the molecular and biological mechanisms that regulate the formation of new bone during distraction osteogenesis are not completely understood. BMPs are potent osteoinductive agents. Our hypothesis was that BMPs, especially BMP-2 and BMP-4, might play an importent role in the signaling pathways that link the mechanical forces created by distraction to biological responses and in promting new bone formation. Using a rabbit's mandible, we investigated the expression of BMP-2, -4 at different time points during distraction osteogenesis. The purpose of this study is to research the pattern of expression of BMP-2, -4 in new bone formation during distraction osteogenesis of the rabbit mandible. The experimental group was applied gradual distraction (0.7mm a day by twice a day, 4.9mm in total, for 7 days) and the control group was carried out osteotomy alone. They were examined clinically, histologically, and by RT-PCR analysis. On 3 days after osteotomy, the high level of expression of BMP-2, -4 was detected. But, the expression of BMP-4 was decreased during latency period. As distraction was started, its expression was increased and maintained till postoperative 28days. In control group, the expression of BMP-4was remarkably decreased till postoperative 14 days. On the other hand, the expression of BMP-2 was no difference between experimental group and control group. The expression of BMP-4 was maintanined at high level during the entire experimental period in both group. These findings suggested that excellent bone formation during distraction osteogenesis is associated with enhanced expression of BMP-4 genes by mechanical tension stress.

백서에서 난소절제가 티타늄 임플랜트의 골 유착에 미치는 영향 (THE EFFECTS OF OVARIECTOMY ON TITANIUM IMPLANT OSSEOINTEGRATION IN RAT'S TIBIA)

  • The purpose of this study was to observe the titanium implant osseointegration in the osteoporosis-induced animal model. Seventy rats, 11 weeks of age, were divided into two groups : an ovariectomized group and a control group. Titanium screw implants(diameter, 2.0mm; length, 3.5mm) were placed into left tibias of 70 rats, 35 in the control group and 35 in the experimental group. The rats were sacrificed at different time interval (1, 2, 3, 4, 6, 8, and 12 weeks after implantation) for histopathologic observation, histomorphometric analysis and immunohistochemistry with fibronectin and CD34 antibody. The results obtained from this study were as follows: 1. Histopathologically findings, newly formed bone was seen at 3 weeks and became lamellar bone at 8 weeks, and mature trabecullar bone was seen at 12 weeks control group. In experimental group, thickness of regenerated bone increased till 8 weeks gradually and mature trabecullar bone was seen at 12 weeks. 2. By histomorphometric analysis, marrow bone density and contact ratio of marrow bone to implant decreased significantly from 8 to 12 weeks in experimental group compared to control group and also total bone to implant contact ratio decreased significantly from 8 to 12 weeks in experimental group. 3. Fibronectin immunoreactivity was strong at 3 weeks control group and reduced after 8weeks gradually. But it was continuously strong from 3 to 8 weeks in experimental group. 4. CD34 immunoreactivity was very strong in the newly formed osteoblasts from 3 to 8 weeks control group. But it reacted minimally later. While in experimental group, it reacted continuously strong from 3 to 12 weeks. The results of this study suggest that osteoporosis is not an absolute contraindication to dental implantation if sufficient period suggested after fixture installation till second stage surgery.

칼슘과 비타민 D 섭취가 티타늄 임플랜트 주위의 골 형성에 미치는 영향 (EFFECT OF CALCIUM AND VITAMIN D SUPPLEMENTATION ON BONE FORMATION AROUND TITANIUM IMPLANT)

  • The purpose of this study was to observe the effect of calcium and vitamin D to the titanium implant osseointegration in animal model. 32 rats, 10 weeks of age, were divided into two group: additional calcium and vitamin D supplementation group and a control group. Titanium screw implant(diameter, 2.0mm; length, 3.5mm; pitch-height 0.4mm) were placed into tibia of 32 rats, 16 in the control group and 16 in the experimental group. The rats were sacrificed at different time interval(1, 2, 4, and 8 weeks after implantation) for histopathologic observation, histomorphometric analysis and immunohistochemistry with osteocalcin and osteopontin antibody. Histopathologically findings, newly formed bone was seen at 1 weeks and became lamellar bone at 2 weeks, and mature trabecullar bone was seen at 4 weeks experimental group. In control group, thickness of regenerated bone increased till 4 weeks gradually and trabecullar bone was seen at 8 weeks. By histomorphometric analysis, bone marrow density was increased significantly at 1 and 2 weeks in experimental group compared to control group. Osteocalcin immunoreactivity was strong at 1 week experimental group and reduced after 4 weeks gradually. But it was continuously weakly from 1 to 4 weeks in control group. Osteopontin immunoreactivity was very strong in newly formed bone from 2 to 8 weeks experimental group. And the amount of osteopontin expression was more abundant in experimental group. The results of this study suggest that calcium and vitamin D supplementation promotes bone healing around dental implants

구강 편평 세포암 이환견에서 Carboplatin과 Meloxicam의 병용투여 증례 (Combined Therapy with Carboplatin and Meloxicam for Oral Squamous Cell Carcinoma in a Dog)

  • 체중이 4.3 kg인 중성화된 16세의 암컷 말티즈견이 약 2주 동안 누런색의 비강과 구강 분비물, 식욕부진과 기면 증상으로 내원하였다. 신체검사에서 구강내 왼쪽 혀 밑 부위에 약 $3{\times}3$ cm 크기의 궤양성 종괴, 심한 치은치주염, 구취, 중등도의 치석, 발열과 아래턱의 연조직 부종을 관찰하였다. 방사선 사진상에서 위턱과 아래턱의 앞 부위에서 골융해 소견을 볼 수 있었다. 총혈액검사와 혈액화학검사에서 혈소판수치, $NH_3$, AST와 ALP의 증가를 관찰할 수 있었으며, 요검사에서 혈뇨와 단백뇨가 나타났다. 조직검사결과 구강 편평 세포암으로 진단되었다. 축주의 거부로 외과적 처치는 실시하지 않았으며, 약물요법으로 carboplatin 주사와 piroxicam 구강내 투여를 병용하였다. 처음 약물투여 후 5일에 지속적인 구토 증상이 나타났으며, 이에 piroxicam을 meloxicam으로 대체하였으며 구토증상은 소실되었다. Meloxicam의 종양에 대한 치료효과에 대한 보고는 흔하지 않지만, 위장관계에 대한 부작용 발생은 piroxicam에 비해 낮음을 알 수 있었다. 이환견은 총 3회의 carboplatin 주사를 실시하였으나, 세 번째 주사 투여 후 5일에 심한 기면증상, 구토와 혈변 증상으로 내원하였고, 검사결과 심한 신기능부전 소견을 보였으며, 축주의 요구에 의해 안락사하였다.

아말감 수복시(修復時) Cavity varnish와 Bonding agent 도포(塗布)에 따른 접합성(接合性)에 관(關)한 주사전자현미경적(走査電子顯微鏡的) 연구(硏究) (A SCANNING ELECTRON MICROSCOPIC STUDY ON THE MARGINAL ADAPTIBILITY IN APPLYING THE CAVITY VARNISH AND DENTIN BONDING AGENT IN AMALGAM RESTORATIONS)

  • The purpose of this study was to evaluate the marginal adaptability of the amalgam restorations in applying the cavity varnish (Copalite$^{(R)}$) and dentin bonding agent (Scotchbond 2$^{(R)}$) under the scanning electron microscope. For this study, eighteen sound extracted human molars were selected. Class I cavities in 12 teeth and class V cavities in 6 teeth were prepared using an air turbine with No. 701 tungsten carbide bur and finished using a low speed handpiece with No. 557 fissure bur. The prepared specimens were then divided into three groups including 4 class I cavities and 2 class V cavities in each group and restored as follows ; Group I. All the prepared cavities were restored with amalgam only (Control). Group II. Two layers of Copalite$^{(R)}$ cavity varnish were applied to the cavities with a gentle stream of air after each application and cavities were restored with amalgam. Group III. The enamel cavity margins were etched with 37% phosphoric acid gel for 60 sec., rinsed for 30 sec. and dried. One layer of visible lightcured Scotchbond Dental Adhesive$^{(R)}$ was applied and immediately cured for 20 seconds with visible light-cure unit and cavities were restored with amalgam. All the specimens were cut at the neck of the teeth and the occlusal halves of specimens were sectioned buccolingually in the longitudinal axis centering the amalgam restorations, using the disk. The cut specimens were ground with sandpapers (400, 600, 800, 1000 grit), and cleaned for 5 minutes in the ultrasonic cleaner (Brason Co. U.S.A.). In the cut surfaces, the amalgam - tooth interfaces were examined under the scanning electron microscope (JSM, 35C type, JEOL). The obtained results were as follows ; 1. The amalgam-tooth interfaces were reduced more significantly in the Copalite$^{(R)}$ and Scotchbond 2$^{(R)}$ application group than in the control group. 2. In the class I cavities, the Scotchbond 2$^{(R)}$ application group showed the findings similar to the Copalite$^{(R)}$ application group in the cavity floor, and the marginal adaptability was better in the side wall than in the cavity floor. 3. In the class I cavities, the Scotchbond 2$^{(R)}$ application group showed better marginal adaptability in the occlusal margin than in the gingival margin. 4. The marginal adaptability was in the order of the Scothbond 2$^{(R)}$ application group, the Copalite$^{(R)}$ application group and the control group.
