The purpose of this study is to investigate the potential of a novel tissue engineering approach to regenerate intervertebral disc. In this study, thermosensitive scaffold (chitosan-Pluronic hydrogel) and nanofiber were used to replace the nucleus pulposus (NP) and annulus fibrosus of a degenerated intervertebral disc, leading to an eventual regeneration of the disc using the minimally invasive surgical procedure and organ culture. In preliminary study, disc cells were seeded into the scaffolds and cellular responses were assessed by MTT assay and scanning electron microscopy (SEM). Based on these results, we could know that tissue engineered scaffolds might provide favorable environments for the regeneration of tissues. Organ culture was performed in fresh porcine spinal motion segments with endplates on both sides. These spinal motion segments were classified into three groups: control (Intact), injured NP (Defect), and inserting tissue engineered scaffolds (Insert). The specimens were cultivated for 7 days, subsequently structural stability, cell proliferation and morphological changes were evaluated by the relaxation time, quantity of DNA, GAG and histological examination. In these results, inserting group showed higher relaxation time, reduced decrement of DNA contents, and accumulated GAG amount. Consequently, the tissue engineered scaffolds used in this study seen to be a promising base scaffolds for regenerative intervertebral disc due to its capacity to absorb external dynamic loading and the possible ideal environment provided for disc cell growing.
The study examined the changes in the decreased facial exposure dose for radiological technologists depending on increased distance between the workers and the X-ray tube head during intraoral radiography. First, the facial phantom similar to the human tissues was manufactured. The shooting examination was configured to the maxillary molars for adults (60kVp, 10mA, 50msec) and for children (60kVp, 10mA, 20msec), and the chamber was fixed where the facial part of the radiation worker would be placed using the intraoral radiography equipment. The distances between the X-ray tube head and the phantom were set to 10cm, 15cm, 20cm, 25cm, 30cm, 35cm, and 40cm. The phantom was radiated 20 times with each examination condition and the average scattered doses were examined. The rate at the distance of 40cm decreased by about 92.6% to 7.43% based on the scattered rays radiated at the distance of 10cm under the adult conditions. The rate at the distance of 40cm decreased by about 97.6% to 2.58% based on the scattered rays radiated at the distance of 10cm under the children conditions. Protection from the radiation exposure was required during the dental radiographic examination.

Objective: The aim of the present study was to evaluate the changes in tooth mobility following orthodontic treatment and to obtain information regarding the guideline of retainer wear duration during the post-treatment period. Methods: The sample consisted of twenty patients who had been treated with edgewise appliances. The mobility of the maxillary teeth from the central incisor to the first molar was measured bilaterally by way of the $Periotest^{(R)}$, a non-invasive, electronic device that provides an objective measurement of the reaction of the periodontium to a defined impact load. Tooth mobility was monitored at the time of the removal of the orthodontic appliances and subsequently at three-month intervals during the two years following appliance removal. Results: Tooth mobility decreased rapidly for the first six months and then decreased at a slower rate during the next six months; no statistically significant decrease in mobility was observed during the second year following appliance removal. Conclusions: The results of the present study suggest that adequate tooth stabilization is critical during the first six months following appliance removal and that continued wearing of retainers is recommended at least until twelve months after the completion of orthodontic treatment.

The purpose of this study was to examine the differences in representation of a globular radiopaque mass on the pantomograms and Waters' views and to compare the efficacy of periapical radiograms, pantomograms and Waters' views in detection of defects on the internal walls of the maxillary sinus. This study was performed with dried human skull. For the study of difference of radiopaque mass shadow in the two views, rubber ball with a diameter of 10㎜ was used as the experimental lesion. It was placed successively on the internal wall of the anterior, posterior, medial, lateral walls and floor of the maxillary sinus. To examine the detectability of defects for radiographic techniques, defects were formed in the anterior, posterior, medial, lateral walls, and floor of the maxillary sinus. They were formed with 0.5㎜, 0.75㎜, 1.0㎜, 2.0㎜ and 3.0㎜ sized steel round burs with a slow speed dental handpiece. By subsequently plugging the holes with zinc oxide eugenol paste, radiopaque defects were produced. After that the periapical radiograms, the pantomograms and the Waters' views were taken each and every defect. The obtained results were as follows: 1. Rubber balls placed on each internal wall of the maxillary sinus were correctly depicted on the posterior wall and the floor in case of the pantomogram, and on the anterior wall and the medial wall in case of the Waters' view. 2. On the detectability of defects for each radiographic technique, radiolucent defects were detected in different places for each technique. Periapical radiogram could detect 1.0㎜ defect on the floor of the maxillary sinus, pantomogram could detect 2.0㎜ defect on every internal wall of the maxillary sinus, and Waters' view could detect 3.0㎜ defect on the anterior wall of the maxillary sinus. 3. On the detectability of defects for each radiographic technique, radiopaque defects were detected in different places for each technique. Periapical radiogram could clearly detect 0.5㎜ defect on the floor of the maxillary sinus, pantomogram could detect 0.5㎜ defect on every internal wall of the maxillary sinus, and finally Waters' view could detect 0.5㎜defect on the anterior wall of the maxillary sinus but 0.75㎜ defect on the anterior wall, lateral wall and floor of the maxillary sinus. As the result, the periapical radiogram is the most simple and satisfactory method for investigating in the maxillary sinus. The pantomogram is suitable method for screening of changes in the maxillary sinus. And the Waters' view is available for detect of lesion in the anterior wall of the maxillary sinus. For the purpose of accurate diagnosis and evaluation of lesion in the maxillary sinus, these techniques supplement each other.

Objectives: Implants connect the internal body to its external structure, and is mainly supported by alveolar bone. Stable osseointegration is therefore required when implants are inserted into bone to retain structural integrity. In this paper, we present an implant with a "wing" design on its area. This type of implant improved stress distribution patterns and promoted changes in bone remodeling. Materials and Methods: Finite element analysis was performed on two types of implants. One implant was designed to have wings on its cervical area, and the other was a general root form type. On each implant, tensile and compressive forces ($30N/m^2$, $35N/m^2$, $40N/m^2$, and $45N/m^2$) were loaded in the vertical direction. Stress distribution and displacement were subsequently measured. Results: The maximum stresses measured for the compressive forces of the wing-type implant were $21.5979N/m^2$, $25.1974N/m^2$, $29.7971N/m^2$, and $32.3967N/m^2$ when $30N/m^2$, $35N/m^2$, $40N/m^2$, and $45N/m^2$ were loaded, respectively. The maximum stresses measured for the root form type were $23.0442N/m^2$, $26.9950N/m^2$, $30.7257N/m^2$, and $34.5584N/m^2$ when $30N/m^2$, $35N/m^2$, $40N/m^2$, and $45N/m^2$ were loaded, respectively. Thus, the maximum stresses measured for the tensile force of the root form implant were significantly higher (about three times greater) than the wing-type implant. The displacement of each implant showed no significant difference. Modifying the design of cervical implants improves the strength of bone structure surrounding these implants. In this study, we used the wing-type cervical design to reduce both compressive and tensile distribution forces loaded onto the surrounding structures. In future studies, we will optimize implant length and placement to improve results. Conclusion: 1. Changing the cervical design of implants improves stress distribution to the surrounding bone. 2. The wing-type implant yielded better results, in terms of stress distribution, than the former root-type implant.
Anatomical changes in the facial and alveolar bones occur after multiple teeth are extracted. In the maxilla, the alveolar bone is absorbed in the direction and inclination of the root, and the remaining alveolar bone becomes shorter, reducing the diameter of the arch. In addition, as the nasolabial angle increases, the support of the lips and the aesthetics of the face are lost. This case reports a functional and aesthetically satisfactory results of full mouth rehabilitation with the implant-supported fixed prosthesis using a zirconia framework.
Bone remodeling is a process controlled by the action of two major bone cells; the bone forming osteoblast and the bone resorbing osteoclast. In the process of osteoclastogenesis, stromal cells and osteoblast produce RANKL, OPG, and M-CSF, which in turn regulate the osteoclastogenesis. During the bone resorption by activated osteoclasts, extracellular $Ca^{2+}/{PO_4}^{2-}$ concentration and degraded organic materials goes up, providing the hypertonic microenvironment. In this study, we tested the effects of hypertonicity due to the degraded organic materials on osteoclastogenesis in co-culture system. It was examined the cellular response of osteoblastic cell in terms of osteoclastogenesis by applying the sucrose, and mannitol, as a substitute of degraded organic materials to co-culture system. Apart from the sucrose, mannitol, and NaCl was tested to be compared to the effect of organic osmotic particles. The addition of sucrose and mannitol (25, 50, 100, 150, or 200 mM) to co-culture medium inhibited the number of tartrate-resistant acid phosphatase (TRAP) positive multinucleated cells induced by 10 nM $1{\alpha},25(OH)_2vitaminD_3$ ($1{\alpha},25(OH)_2D_3$). However, NaCl did exert harmful effect upon the cells in this co-culture system, which is attributed to DNA damage in high concentration of NaCl. To further investigate the mechanism by which hypertonicity inhibits $1{\alpha},25(OH)_2D_3$-induced osteoclastogenesis, the mRNA expressions of receptor activator of nuclear factor (NF)-kB ligand (RANKL) and osteoprotegerin (OPG) were monitored by RT-PCR. In the presence of sucrose (50 mM), RANKL mRNA expression was decreased in a dose-dependent manner, while the change in OPG and M-CSF mRNA were not occurred in significantly. The RANKL mRNA expression was inhibited for 48 hours in the presence of sucrose (50 mM), but such a decrement recovered after 72 hours. However, there were no considerable changes in the expression of OPG and M-CSF mRNA. Conclusively, these findings strongly suggest that hypertonic stress down-regulates $1{\alpha},25(OH)_2D_3$-induced osteoclastogenesis via RANKL signal pathway in osteoblastic cell, and may playa pivotal role as a regulator that modulates osteoclastogenesis.

Introduction: As dental implant surgery is becoming increasingly popular, it has become one of the causes for the hypesthesia of the inferior alveolar nerve, along with other surgical procedures, such as a third molar extraction. In addition, it tends to cause legal problems between the operator and patient. Therefore, there must be a proper method that is reliable, objective and economical to assess the nerve impairment. For this reason, an attempt was made to use an Electric Pulp Tester to assess inferior alveolar nerve block anesthesia. Materials and Methods: Thirty patients were tested. Electric pulp testing of the lower jaw skin was performed at the three different times, before anesthesia, at the onset of sensory changes and after 15 minutes waiting from the onset, and on the 10 points of the chin, which produced 10 sections on the skin area. Results: Twenty seven patients (90%) could feel the electric stimulus on the chin at all 10 points before local anesthesia and the scores represent the statistical differences between the right and left points except R4 and L4. After anesthesia, the difference between the right and left points (L3-R3, L4-R4, L5-R5) increased significantly with time but two points (L2, R2) showed no significant difference. The scores on the left chin (L3, L4, L5) increased, whereas the other points (R1-R5, L1, L2) showed no significant differences. Conclusion: This study highlights the potential clinical use of an electric pulp tester for an assessment of inferior alveolar nerve impairment.
Statement of problem: Modification of titanium implant surface has potential to ensure clinically favorable performance that several surface modification technologies have been introduced. Among the methods. anodizing method and sol-gel hydroxyapatite coating method have gained much interest due to its roughness and chemical composition of the coating layer, but more of its biocompatibility result is required. Purpose : The purpose of this study was to compare bone-implant interface shear strength of four different surface treated implants as time elapsed. Resonance frequency analysis(RFA) and removal torque measurement methods were employed to measure implant stability at one week and six week after implantation. Material and method: A total of 80 screw-shaped implant [20 machined, 20 resorbable media blasted(RBM), 20 anodized, and 20 anodized+hydroxyapatite sol-gel coated] were prepared, and one of each group was implanted in the tibia of a New Zealand white rabbit that total 20 of them were used. In order to test the implant stability and implant-tissue interface contact changing in the bone bed, each 10 rabbit were sacrificed 1 week and 6 week later while resonance frequency and removal torque were measured. One-way analysis of variance and the Tukey test were used for statistical analysis. Results : The results were as follows. 1. There was no statistically significant difference of implant stability quotients(ISQ) value in RFA between individual groups after 1 week of implantation and 6 weeks(p>0.05). But, there was statistically significant increase of ISQ value in 6 weeks group compared to 1 week group(p<0.05). 2. There was no statistically significant difference in removal torque analysis between individual groups after 1 week of implantation and 6 weeks(p>0.05). but there was statistically significant increase in all 4 groups after 6 weeks compared to 1 week later(p<0.05). 3. There was no statistically significant difference in removal torque analysis between anodized group and HA coating after anodic oxidation 6 weeks later(p>0.05), but significant difference was appeared in both groups compared to RBM group and smooth-machined group(p<0.05). Conclusions : It can be suggested that changes in surface characteristics affect bone reactions. Anodized and anodized+hydroxyapatite sol-gel coating showed significantly improved bone tissue response to implants, but further study on the effect of hydroxyapatite dissolution is needed.
