Usage of the rubber dam has been advocated by countless number of dentists. The advantages of the rubber dam such as the following are well-known 1. Moisture control. 2. Improved field of vision. 3. Ease of approach. 4. Soft tissue retraction and Injury prevention. 5. Prevention of aspiration of materials or instruments. 6. Shortened chair time. 7. Induction of nasal breathing during administration of $N_2O-O_2$ sedation. Recent reports indicate the rubber dam can protect the dental staffs from the infection when treating HBV or HIV positive patients. Also, improved moisture control and freeing of both hands allowed by the rubber dam makes it very useful when bonding orthodontic brackets. This case study presents the various clinical application of the rubber dam on patients visiting SNUDH dept. of pediatric dentistry to emphasize the importance of its use in pediatric dentistry.
Objectives : This study examined the influence of socio-demographic, health-related, and oral health-related characteristics on adolescent DMFTs. Methods : The subjects in this study were 1,129 adolescents selected from the 6th national health and nutrition examination survey data. To determine the factors influencing dental caries, a regression analysis using a complex-sample generalized linear model was conducted after adjusting for confounding factors. Results : DMFTs were smaller among boys than girls, and smaller in the "13-15 age group" than in the "16-18 age group." In terms of household income, DMFTs were larger in the "lower," "lower-middle," and "upper-middle" income brackets than in the "upper" income bracket. DMFTs were smaller among adolescents with a history of smoking than those who had never smoked. Conclusions : In order to improve the oral health of adolescents, oral health-promotion programs should be provided for girls, high school students, students from low-income families, smokers, and those who consider themselves to have poor oral health.
Objective: With an increasing demand for esthetic orthodontic appliances, the interest in polycarbonate brackets has also increased. However, polycarbonate bracket wings are prone to fracture. The purpose of this study was to evaluate the clinical usefulness of a newly-developed polycarbonate bracket by measuring the fracture strength of bracket tie wings. Methods: Alice (K.M. Daicom, Seoul, Korea) and Spirit MB (Ormco, Glendora, CA, USA) brackets were used as an experimental and control bracket respectively. Each bracket group was divided into halves. One half was untreated and the other half was treated with 2,000 times of thermocycling between $5^{\circ}C\;and\;55^{\circ}C$. The fracture strength of the wing was measured by a universal testing machine. Results: Alice bracket wings showed significantly higher strength than Spirit MB bracket wings in both untreated and thermocycled bracket cases. Conclusion: Alice brackets may be used clinically in terms of the strength of bracket wings.

Objective: To test the null hypothesis that no difference exists between the effects of at-home bleaching and in-office bleaching on shear bond strength (SBS) with bracket bonding at 4 different time intervals after dental bleaching. Methods: Ninety extracted human premolars were randomly divided into 9 groups (n = 10) according to the bleaching methods used (at-home bleaching and in-office bleaching) and the storage time in artificial saliva (30 min, 1 day, 2 weeks, and 3 weeks before bonding). The control group was stored in artificial saliva for 7 days. Brackets were bonded with the Transbond XT adhesive system, and SBS testing was performed. The adhesive remnant index (ARI) was used to assess the amount of resin remaining on the enamel surfaces after debonding. The SBS data were analyzed by analysis of variance (ANOVA) and the Tukey test. For the ARI, the Kruskal-Wallis test was performed. Significance for all statistical tests was predetermined to be p < 0.05. Results: The SBS of the unbleached group was significantly higher (p < 0.05) than that of the bleached groups (except for the group bonded 30 min after at-home bleaching). Conclusions: The null hypothesis was not totally rejected. All bleaching groups tested had decreased SBS of the brackets to the enamel, except for the group bonded 30 min after at-home bleaching. The SBS returned to values close to those of the unbleached enamel within 3 weeks following bleaching.
A device called incisor inclination indicator to control the axial inclinations of the incisors in the diagnostic setup is introduced. It is used to control the retraction of the maxillary and mandibular incisors. In this article, we describe the use of the incisor inclination indicator to prescribe adequate torque into the anterior lingual brackets and evaluate the results of treatment in a bimaxillary dentoalveolar protrusion case that underwent orthodontic treatment with the pretorqued anterior lingual brackets. Retraction of the maxillary and mandibular incisors was achieved with careful control of the axial inclination. It is indicated that the use of the incisor inclination indicator is an effective adjunctive laboratory procedure for anterior torque control during retraction in lingual orthodontic treatment.
Wear and friction properties of the alumina-glass dental ceramics were evaluated in exact in vitro environment (artificial saliva at 37$^{\circ}C$), using a ball-on-disc apparatus, at contact loads from 49 N to 196 N and with the disc rotating at 120 rpm up to 10$^{6}$ cycles to investigate the wear behavior. As the load increased from 49 N to 196 N, the friction coefficient of 0.025 was maintained, however, the volumetric wear rate rose from 2.18 ${\times}$10$^{-9}$TEX>$mm^{3}$/Nㆍm to 2.35 ${\times}$10$^{-6}$TEX>$mm^{3}$/Nㆍm. Experimental results indicated that the wear behavior of the alumina-glass composites with a sliding distance of 4.4 km was a typical adhesive wear, which was applicable for orthodontia ceramic brackets.
Objectives: This study evaluated the effects of different surface conditioning methods on the bond strength of orthodontic brackets to glazed full-zirconia surfaces. Materials and Methods: Glazed zirconia (except for the control, Zirkonzahn Prettau) disc surfaces were pre-treated: PO (control), polishing; BR, bur roughening; PP, cleaning with a prophy cup and pumice; HF, hydrofluoric acid etching; AA, air abrasion with aluminum oxide; CJ, CoJet-Sand. The surfaces were examined using profilometry, scanning electron microscopy, and electron dispersive spectroscopy. A zirconia primer (Z-Prime Plus, Z) or a silane primer (Monobond-S, S) was then applied to the surfaces, yielding 7 groups (PO-Z, BR-Z, PP-S, HF-S, AA-S, AA-Z, and CJ-S). Metal bracket-bonded specimens were stored in water for 24 hr at $37^{\circ}C$, and thermocycled for 1,000 cycles. Their bond strengths were measured using the wire loop method (n = 10). Results: Except for BR, the surface pre-treatments failed to expose the zirconia substructure. A significant difference in bond strengths was found between AA-Z ($4.60{\pm}1.08MPa$) and all other groups ($13.38{\pm}2.57-15.78{\pm}2.39MPa$, p < 0.05). For AA-Z, most of the adhesive remained on the bracket. Conclusions: For bracket bonding to glazed zirconia, a simple application of silane to the cleaned surface is recommended. A zirconia primer should be used only when the zirconia substructure is definitely exposed.

The aim of this study was to compare the species and biotypes of mutans streptococci isolated from dental plaques sampled from the interfaces between the bracket aid tooth surface and smooth tooth surfaces In orthodontic patients. Dental plaque was collected from the interfaces between brackets aid teeth (test group), and from smooth tooth surfaces distant from brackets by more that 2mm (control group). The dental plaque collected by a sterilized curette was transferred into a vial of 1 X PBS. The sample in the vial was vigorously vortexed for 1 min and plated ou mitis-salivarius bacitracin (MSB) agar plate using cotton tips. The agar plates were incubated at $37^[\circ}C$ in a candle jar for 2 days, and again incubated for 1 more day at anambient temperature Individual colonies were cultured in TH broth at $37^[\circ}C\;CO_2$ incubator. The PCR-RFLP based on dextranase gene was performed for the identification of mutans streptococci at the species-level For biotyping of mutans streptococci, biochemical tests were performed There was no significant difference of the species of mutans streptococci isolated from both test and control groups However, the biotypes of the mutans streptococci isolated from test and control groups were different. These results may offer the basic data to verify the relationship between the mutans streptococci biotype and enamel decalcification or dental caries in orthodontic patients with fixed appliances.
The purpose of this study was to evaluate the effect of oral environment on the strength of resin bracket wings by comparing fracture mode according to water immersion and thermocycling. Seventy-five resin brackets(Spirit MB, Ormco, California) were divided into three groups and treated for six months as follows; (1) untreated, (2) water immersion in distilled water at $37^{\circ}C$, (3) water immersion in distilled water at $37^{\circ}C$ with total 2,100 times of thermocycling taken 350 times each month. Fracture mode of the wing was tested on universal testing machine. In addition to resin brackets, 25 metal brackets were used as controls. Through the statistical analyses, following results were obtained. 1. Resin bracket wings showed significantly lower fracture strength than metal brackets(p<0.001). 2. Water immersion and water immersion with thermocycling groups showed significantly lower fracture strength than open air condition group(p<0.001). 3. Water immersion with thermocycling group showed significantly lower fracture strength than water immersion group(p<0.001). The above results suggest that the mechanical property of resin bracket wing nay be influenced by oral environment and further research is needed to improve the strength of the wing in the resin bracket.

Purpose: The purpose of this study is to compare the shear bond strength of resin cement for orthodontic brackets without applying an adhesive primer, to the case of applying an adhesive primer. Materials and Methods: The specimens were divided into three experimental groups, Transbond XT, GC Ortho Connect and Orthomite LC, and the enamel surface was divided into two sections, one with 37% phosphoric acid and the other with 37% phosphoric acid and an adhesive primer or universal adhesive. Each of three types of cement was applied to orthodontic bracket, and after bonding, the shear bond strength was measured. Results: Transbond XT and Orthomite LC significantly increased shear bond strength when orthodontic brackets were bonded after applying an adhesive primer and universal adhesive, respectively. Conclusion: It is expected that application of an adhesive primer or universal adhesive after acid etching will improve shear bond strength of orthodontic brackets in Transbond XT and Orthomite LC.
