A morphologic evaluation of defects created by a piezoelectric ultrasonic scaler on casting gold alloy (압전방식초음파치석제거기의작업조건에따른치과주조용합금의삭제결손부 양상에 관한 고찰)

  • Purpose: In this study we evaluated the morphologic aspects of defects created by a piezoelectric ultrasonic scaler with scaler tip on casting gold alloy using scanning electron microscope (SEM) images and defect surface profiles. Methods: 54 blocks of type III casting gold alloy (Firmilay, Jellenko Inc, CA, USA) were scaled by a piezoelectric ultrasonic scaler (P-MAX, Satelec, France) with scaler tip (No. 1 tip) on a sledge device. 2-dimensional profiles of defects on all samples were investigated by a surface profilometer (a-Step 500, KLA-Tencor, CA, USA). The selected working parameters were lateral force (0.5 N, 1.0 N, 2.0 N), mode (P mode, S mode), and power setting (2, 4, 8). SEM images were obtained. Defect surface profiles were made on Microsoft Excel program using data obtained by a surface profilometer. Results: Among P mode samples, there were similarities on defect surface profiles and SEM images regardless of lateral force. The defects created in P mode were narrow and shallow although the depth and the width increased as power setting changed low (2) to high (8). In P mode samples, the defect depth was the greatest when lateral force of 0.5 N was applied. However all the depths were smaller than 1 m. SEM images of Lateral force of 0.5 N, S mode, power setting 2 and 4 were similar to that of P mode, but the other SEM images of S mode showed discernible changes. Defect depth of S mode samples was the greatest when lateral force of 1.0 N was applied. Conclusions: Within the limitations of this study, it can be concoluded that removing capability of piezoelectric scaler with scaler tip becomes maximized as power level becomes higher but the capability is restricted when excessive lateral force is applied on scaler tip.

Comparative evaluation of roughness of titanium surfaces treated by different hygiene instruments

  • Purpose: The use of appropriate instruments to clean surfaces with minimal change, is critical for the successful maintenance of a dental implant. However, there is no consensus about the type and methodology for such instruments. The aim of this study was to characterize changes in the roughness of titanium surfaces treated by various scaling instruments. Methods: Thirty-seven identical disks (5 mm in diameter) were investigated in this study. The specimens were divided into eight groups according to the types of instrumentation and the angle of application. Ultrasonic scaling systems were applied on a titanium disk to simulate standard clinical conditions. The equipment included a piezoelectric ultrasonic scaler with a newly developed metallic tip (NS group), a piezoelectric ultrasonic scaler with a conventional tip (CS group), a piezoelectric root planer ultrasonic scaler with a conventional tip (PR group), and a plastic hand curette (PH group). In addition, the sites treated using piezoelectric ultrasonic scaler systems were divided two sub-groups: 15 and 45 degrees. The treated titanium surfaces were observed by scanning electron microscopy (SEM), and the average surface roughness (Ra) and mean roughness profile depth (Rz) were measured with a profilometer. Results: SEM no significant changes in the titanium surfaces in the NS group, regardless of the angle of application. The PH group also showed no marked changes to the titanium surface, although some smoothening was observed. All CS and PR sites lost their original texture and showed irregular surfaces in SEM analysis. The profilometer analysis demonstrated that the roughness values (Ra and Rz) of the titanium surfaces increased in all, except the PH and NS groups, which showed roughness decreases relative to the untreated control group. The Ra value differed significantly between the NS and PR groups (P<0.05). Conclusions: The results of this study indicated that changes in or damage to titanium surfaces might be more affected by the hardness of the scaler tip than by the application method. Within the limitations of this study, the newly developed metallic scaler tip might be especially suitable for peri-implant surface decontamination, due to its limited effects on the titanium surface.

The Effect of a Piezoelectric Ultrasonic Scaler with Curette Tip on Root Substitute Removal in Vitro (큐렛형 팁을 장착한 압전방식 초음파 치석제거기가 치근대체물 삭제에 미치는 영향)

  • Based on current evidence in the literature, it is known that endotoxin is a weakly adherent surface phenomenon and that power-driven instruments can be used to accomplish definitive root detoxification and maximal wound healing without overinstrumentation of root and without extensive cementum removal. And one of the newly developed curette tips used with low power of piezoelectric ultrasonic scaler, is effective to remove calculus and not to remove the excessive cementum. The purpose of this study is therefore, to assess the influence of ultrasonic power and various working parameters on root substitute removal when instrumentation is performed with the curette tip on piezoelectric ultrasonic scaler. This study assessed defect depth, width and area resulting from instrumentation using a piezoelectric ultrasonic scaler with a curette type tip in vitro to acrylic resin block as a root substitute. The working parameters was standardized by the sledge device which controls lateral force(0.5 N, 1 N, 2 N) and instrumentation time(5 sec, 10 sec, 20 sec) and power setting was adjusted 0,2,4,8 in P mode. Power setting had the greatest influence on defect depth compared to lateral force and instrumentation time(standardized regression parameter estimates${\pm}$standard error, $0.37{\pm}0.02$, $0.19{\pm}0.02$, $0.07{\pm}0.02$). The effects on defect area also greatest for power setting($0.57{\pm}0.03$) compared to lateral force and instrumentation time($0.33{\pm}0.03$, $0.12{\pm}0.03$). The effect of the power setting on the defect width($0.15{\pm}0.01$) is not so great as defect depth or defect area compared to lateral force($0.12{\pm}0.01$) and effect of instrumentation time is minimal($0.02{\pm}0.01$). It could be concluded that the power setting has the greatest influence on the defect depth and area in curette type tip with low power of piezoelectric ultrasonic device. Many parameters can be adjusted in various situation in clinical use of piezoelectric ultrasonic scaler but the power setting is the first parameter to be adjusted.

Cytotoxicity(MTT) evaluation of dental instruments made of polymers (치과용 폴리머 기구의 세포독성(MTT) 평가)

  • In order to assess the cell toxicity of 10 instruments made of polymers, the MTT assay which utilizes the L-929 cell was selected. Specimens were eluted at a temperature of 37℃ for 24 hours at a rate of 4g per 20mL, RPMI 1640, and then was positively and negatively contrasted with a control test solution, in accordance with the Notification No. 2020-12 Protocols of Medical Apparatus Biological Safety from the Ministry of Drug and Food Safety. As a result of 24 hours of incubation in 37℃, 5% CO2 Incubator and assessment using an ELISA reader, the results of Intraoral camera indiciated a cellular viability of more than 70% at a 50% eluate. But, the Plastic impression tray, 3D printing tweezer, Impression disposable syringe, Dental floss holder, Hand implant scaler, Surgical retractor, Oral scanner tip, Dental mirror, and the Water pick tip all reported a cellular viability of more than 70% at a 100% eluate, which indicates that do not exhibit cytotoxicity, thus allowing it to be used in contact with the mucous membrane of the oral cavity.

A Study on Temperature Changes during Bone Scaling and Cutting of Dental Ultrasonic Scaling/Surgery System (치과용 초음파 스케일러/수술기 통합 시스템의 스케일링 및 절삭 시 온도 변화에 관한 연구)

  • While dental clinics still use the ultrasonic scaling/surgery tool for teeth scaling and cleaning the tool's use is expanding steadily to include treatment of damaged teeth and bone tissue. In this study, a handpiece moving system (HMS) was developed to evaluate bone scaling and cutting in the field of dentistry. The HMS, through a scaling test of bone using a scaler tip, it was able to identify surface damage. Additionally, a thermos-graphic camera was used to observe the temperature distribution that occurred during the bone scaling and cutting process. Consequently, we found that increasing the working load increased the amount of surface damage. Changes in temperature distribution occurred slowly and were maintained within safety bounds for 10 minutes. Going forward, we will compare the HMS performance on scaling and cutting with other devices.

The effect of copper alloy scaler tip on the surface roughness of dental implant and restorative materials (구리 합금 초음파 스케일러 팁이 치과 임플란트 및 수복 재료 표면에 미치는 영향)

  • Purpose: This study is designed to investigate the various impacts of different types of scaler tips such as cooper alloy base tip and the others on the surface roughness of teeth and implant by the method which is currently in clinical use. Materials and methods: Four different types of disc shaped porcelain, titanium, zirconia, and Type III gold alloy dental materials sized 15 mm diameter, 1.5 mm thickness were used for the experiment. Plastic hand curette (Group PS), cooper alloy new tip (Group IS), and stainless steel tip (Group SS) were used as testing appliances. A total of 64 specimens were used for this study; Four specimens for each material and appliance group. Surface roughness was formed with 15 degree angle in ultrasonic scaler tip and with 45 degree angle in hand curette of instrument tip and the specimen surface with 5 mm long, one horizontal-reciprocating motion per second for 30 seconds by 40 g force. To survey the surface roughness of each specimen, a field emission scanning electron microscope, an atomic force microscope, and a surface profiler were used. (Ra, ${\mu}m$). Results: According to SEM, most increased surface roughness was observed in SS group while IS groups had minimal roughness change. Measurement by atomic force microscope presented that the surface roughness of SS group was significantly greater than those of PS, IS and control groups in the type III gold alloy group (P<.05). IS group showed lesser surface roughness changes compared to SS group in porcelain and gold alloy group (P<.05). According to surface profiler, surface roughness of SS group showed greater than those of PS, IS and control groups and IS group showed lesser than those of SS group in all specimen groups. Type III gold alloy group had large changes on surface roughness than those of porcelain, titanium, zirconia (P<.05). Conclusion: The result of this study showed that newly developed copper alloy scaler tip can cause minimal roughness impacts on the surface of implant and dental materials; therefore this may be a useful alternative for prophylaxis of implant and restored teeth.

A study on the state of infection control in dental clinic (치과진료실에서의 감염관리 실태 조사)

  • The purpose of this study was to examine the state of infection control provided to members of Korean Dental Hygienists Association. The subjects in this study were dental hygienists who attended a symposium on July 1. 2006. after a survey was conducted, the answer sheets from 489 participants were analyzed, and the findings of the study were as follows: 1. Possession of disinfection room was being(72.7%), and person of infection control was zero(52.9%). Number of sterilizer was one(62.2%). 2. As a repetition choice, type of sterilizer was autoclave(97.9%), UV sterilizer(67.4%) and EO gas sterilizer(21.4%). As a repetition choice, infection materials was ethanol(84.1%). 3. Water tube of unit and chair was using of sterilized water(42.9%). Sterilizing of compressed air was no(69.0%). 4. Re-using of disposal was not using(62.5%), re-using disposal was suction tip(28.2%)(repetition choice) 5. In sterilization of instruments, hand-piece was every using time(28.4%), and reamer-file, bur, mirror, pincette, explorer, hand scaler and ultrasonic scaler were high in every using time. 6. Individual protection was high of using, cleaning of hands before treatment was every treatment(87.0%). Type of soap was liquid type in dental clinic(48.2%), infection soap in dental hospital(41.2%) and solid soap in public health center(50.6%). Answered that they need regular oral health education, and 82.9% respondents answered that they need oral health technicians in school. And 87.8% respondents needed individual oral health education for the benefit of better oral health.

Influence of scaling procedures on the integrity of titanium nitride coated CAD/CAM abutments

  • PURPOSE. To determine the extent of treatment traces, the roughness depth, and the quantity of titanium nitride (TiN) removed from the surface of CAD/CAM abutments after treatment with various instruments. MATERIALS AND METHODS. Twelve TiN coated CAD/CAM abutments were investigated for an in vitro study. In the test group (9), each abutment surface was subjected twice (150 g vs. 200 g pressure) to standardized treatment in a simulated prophylaxis measure with the following instruments: acrylic scaler, titanium curette, and ultrasonic scaler with steel tip. Three abutments were used as control group. Average surface roughness (Sa) and developed interfacial area ratio (Sdr) of treated and untreated surfaces were measured with a profilometer. The extent of treatment traces were analyzed by scanning electron microscopy. RESULTS. Manipulation with ultrasonic scalers resulted in a significant increase of average surface roughness (Sa, P<.05) and developed interfacial area ratio (Sdr, P<.018). Variable contact pressure did not yield any statistically significant difference on Sa-values for all instruments (P=.8). Ultrasonic treatment resulted in pronounced surface traces and partially detachment of the TiN coating. While titanium curettes caused predominantly moderate treatment traces, no traces or detectable substance removal has been determined after manipulation with acrylic curettes. CONCLUSION. Inappropriate instruments during regular plaque control may have an adverse effect on the integrity of the TiN coating of CAD/CAM abutments. To prevent defects and an increased surface roughness at the transmucosal zone of TiN abutments, only acrylic scaling instruments can be recommended for regular maintenance care.

  • 치아의 치석제거 및 치골절삭에 사용되는 초음파 스케일러는 일반적으로 마그넷형과 압전형으로 구분할 수 있으며, 최근에 들어 소형화, 저전력, 정밀성, 저비용 등의 장점으로 인해 압전형이 주로 사용되고 있다. 국내의 대부분의 치과에서 한 대 이상 운용되는 초음파 스케일러는 대부분 유럽 제품으로며, 국내에서는 이를 대체하기 위한 제품이 출시되고 있으나 아직까지 유럽 제품에 비해 출력강도, 정밀도 등에서 성능이 모자란 현실로 시장에서 외면 받고 있다. 본 연구에서는 앞서 언급한 압전 초음파 스케일러에 대한 체계적인 연구를 진행하여 외국에 비해 상대적으로 성능이 떨어지는 초음파 스케일러의 성능을 개선하고자 하였다. 이를 위하여 스케일러의 진동 발생부, 즉 압전 세라믹과 SUS 재질의 head, tail 부로 구성된 진동발생부의 최적구조톨 도출하기 위하여 유한요소 해석을 실시하였으며, 스케일러의 중심주파수 28kHz에서 최대 출력이 발생할 수 있는 구조를 도출하였다. 스케일러의 Head 와 Tail 부문의 두께와 직경, 길이 변화에 따른 중심주파수 및 출력 변위의 경향분석을 실시하였으며, 이상의 결과를 바탕으로 실제 스케일러를 제작하여 시뮬레이션의 유효성을 검증하였다. 이상의 과정으로 거쳐 개발된 압전 초음파 스케일러는 다양한 Tip 종류의 영향을 최소화할 수 있으며, 중심주파수는 28~30kHz 에서 뛰어난 성능을 나타내어 기종 유럽제품의 성능을 앞지르는 특성을 확보할 수 있었다.

Current conditions regarding dental infection management recognition of students in the department of dental hygiene (치위생(학)과 학생의 치과감염관리에 관한 인식현황)

  • This research was performed to provide basic data for the development of infection related dental hygiene studies by surveying the current condition of recognition among students in the department of dental hygiene toward hospital infection management while receiving the following results by using a personal self-administered survey method targeting 303 students in the department of dental hygiene from certain areas. 1. With the current condition of recognition on the sanitization and sterilization of instruments among students in the department of dental hygiene(study), the rate at which surgical instruments are to be sterilized with autoclaves was 79.9% which was relatively higher than other instruments while it was shown that prosthetic instruments for treatment was 56.4%, conservative instruments for treatment was 51.8%, and ultrasonic scaler tip was 51.1% while the way syringe tips(36.1%) and the dental anesthetic apparatus(27.9%) were revealed to require sanitization by alcohol. 2. The 'hand wash' area was the highest with 4.71 while the 'materials and environment management' area and 'equipment management' area appeared high respectively with 4.43 and 4.41. 3. With the current condition of recognition on equipment management, 'equipments used for contagious patients are separately washed after a one-time use and must be sterilized or separated-and-discarded' was the highest with 4.82 while 'sterilization equipments with humidity or water on it are considered contaminated and are not used' showed the lowest recognition level with 3.90. 4. Regarding the current condition of materials and environment management, 'contagious and general trash are separated and discarded' was the highest with 4.70 while 'the refrigerator for medicine storage is cleaned on a regular basis once a month' was revealed as the lowest with 4.11. 5. With the current condition of recognition on hand washing, 'one must wash their hands after coming in contact with contagious patients, was the highest with 4.90 while washing hands after taking off gloves' appeared as the lowest with 4.51 point. To conclude department of dental hygiene there is to infection management and necessary about organization disinfecting and pasteurization to strengthen an education in order raising a stamp helping practical ratio about the infection management which whole, is from presence at a sickbed and connection does and about the infection management which is substantial and educational program development leads feed with the fact that deepening studying which is continuous must become accomplished becomes.
