The introduction of osseointegrated dental implants in dentistry brought about a new era in everyday dental practice. For the past 50 years, prosthetic restoration with implant-supported prosthesis has developed into a viable and predictable treatment option. Alongside the increasing use of dental implants is the occurrence of many complications during implant placement (surgery), in the mechanical or prosthetic problem, and in the biological aspect. In particular, abutment or screw fracture as one of the mechanical complications can put the dentist in a tight spot in a clinical situation. It is hard to remove the fractured abutment and screw to restore it properly. Therefore, it is very important that clinicians consider possible complications in advance and make an appropriate treatment plan. We discuss cases of abutment fracture and mechanical/prosthetic complications together with the causes and solutions.
Patients with severe mental challenges are often subjected to extensive dental problems. Their impaired communication and lack of cooperation unable to receive timely dental treatments, which deteriorates their adverse oral conditions. In spite of a limited time frame, well-planned comprehensive treatments can be delivered in an one-day operation setting under general anesthesia. This multidisciplinary treatment regimen primarily comprises of single-visit endodontics, direct bonded restoration, periodontal treatment, and surgical extraction. Further, one or two additional visits for prosthetic procedures are able to accomplish functional reestablishment and esthetic enhancement. This article discusses on clinical managements for severely mentally challenged patients under a single-day operation.
Tested the flexural strength and maximum load of two types of dental 3 D printed temporary tooth materials of the DLP method. The average flexural strength was 206.98 MPa in the test group and 139.77 MPa in the control group. The average flexural strength of the experimental group was 67.21 MPa higher than that of the control group. In the maximum load experiment, an average of 44.16N in the experimental group and 37.31N in the control group were measured. The average value of 6.85N was higher in the experimental group, and the durability of the artificial tooth restoration was improved.
Cavities of Class V are caused by heavy occlusal loads due to bruxism and clenching habit. It is general to restore abfraction lesions with dental filler materials to reduce stress concentration. A material should be selected from various dental products based on long term clinical experiences or personal preference concerning filler methods. A quantitative criterion is necessary to make an evaluation of the results as dentists decide treatment methods and dental materials relying on their clinical experiences. The purpose of this study is to find an optimal restoration method and material for noncarious cervical lesions using the finite element method. An objective function was defined to minimize the sum of tensile and compressive stresses. Several models with different combinations of resins were suggested and compared in terms of the values of objective function. An optimal solution was to fill TetricFlow inside the lesion and Z100 in the remaining region with a thickness ratio of 0.125.

So I studied on the design waxup technique for mandible molar occlusion. The results of the study were as follows; 1. The dam wax up method can restore axial contour of teeth very easy and make short working time of wax pattern. 2. The height of dam must be same with cusp of adjacent teeth. 3. Automatically the contour of tooth is appeared if the contour of dam is relationship with cuspid line of adjacent teeth. 4. The height of contour of buccal, lingual surface is formed natural curve to add fluid wax by gravitation. 5. The development groove of mandible first premolar is appeared V form. 6. The development groove of mandible second premolar is appeared Y form. 7, The development groove of mandible first molar is appeared M form. 8. The development groove of mandible second molar is W form. 9. The embrasure is formed to carve around contact point area as round convex. It affects to axial form of tooth. 10. The buccal, lingual groove of molar is formed parallel with direction of teeth arrangement.

Objectives: Patients with excessive tooth wear should first be diagnosed for the etiology of the tooth wear. Causes of tooth wear include bruxism, clenching, and taking medications for systemic diseases. After identifying the cause of tooth attrition, the final prosthesis should be restored with an appropriate vertical dimension. Methods: A 79-year-old man with worn out teeth desired a whole dental treatment. He was on medications for high blood pressure and asthma. The treatment proceeded with a consultation with a medical doctor. The medications for asthma evoked multiple teeth wear and a loss of the vertical dimension. After recovery of 3 mm of vertical dimension, 2 months of evaluation was followed by an interim prosthesis. Results: The increased vertical dimension caused no problem in function and esthetics, and the final restoration was performed with a full monolithic zirconia crown. Group function, adequate anterior guidance, and the occlusal plane were determined. Conclusions: After the final restoration, the patient was both esthetically and functionally satisfied, and a night guard splint was delivered to prevent prosthesis fracture. The patient was informed about the potential tooth wear associated with asthma drugs and educated to visit the clinic regularly.
최근 치과 임상 영역에 디지털이 접목되면서 디지털을 활용한 임상 진료가 점차 일반화되어가고 있다. 현재까지의 디지털 치과진료는 구강스캔 등의 자료채득, 가이드를 활용한 수술, CAD CAM을 사용한 보철물 제작 등의 직접적인 부분에 다소 집중된 감이 있으나, 치과 진료의 시작인 진단 및 치료계획 수립 단계에서도 디지털의 활용은 큰 효용을 가지고 있다. 이번 증례에서는 전치부 수복을 위해 내원한 환자에게 구치부를 포함한 상악 전체의 수복을 이해시키고, 교합고경을 재설정하여 수복하는 과정의 가이드라인으로 사용한 digital mockup의 활용 예를 보고하고자 한다.
Temporomandibular disorders have been defined as a collective term embracing a number of clinical problems that involve the temporomandibular joint, the masticatory nuscles, and associated structures. There have been many different contributing factors of TMDs which were traumatic, occlusal, pathophysiological and psychosocial. Among there factors, the effect of occlusion on TMDs have been a controversy for a long time. The purpose of this study was to investigate the effect of occlusal factors and oral habits on TMDs. In this study, 140 subjects with signs and symptoms of TMDs and diagnosed of TMD in the Orofacial Pain clinic of Yonsei University Dental Hospital though March to July 2004 were selected for the TMDs group and 50 subjects without any signs and symptoms of TMDs as the control group. The subjects were evaluated clinically in TMDs' Occlusal and Prosthodontic Restoration examinations. TMDs' examination was composed of the TMJ pain, sound, locking, temporal or masseter muscle palpation, mandibular movement, oral habits and headache. Occlusal examination was made of overjet, overbite, lost teeth number, nonfunctional interference, midline shift, then pattern of lateral movement and attrition. prosthodontic restoration examination had the existence of restoration, placement, then number of crown or bridge and Satisfiable index which estimated the quality of occlusal state of prosthodontic restorations. Following results were obtained : 1. The prevalence of TMDs was higher in their 20s & 30s, female of the TMD patients group. 2. The clenching frequency in the TMDs group(40.71%) was higher than those in the control group(18.00%), and there was a significant statistical difference(p<0.05). 3. The frequency of Nonfunctional interference in the TMDs group(10.00%) was higher than those in then Control group(2.00%), and there was a significant statistical difference(p<0.05). The result of this study indicated TMDs prevalence was higher in their 20s, 30s, female group of TMDs patients similar to the previous studies. Clenching and nonfunctional interference were estimated as the contributing factors of TMDs.
건강에 대한 관심과 평균 수명 증가로 보철수복을 위해 치과에 내원하는 고령 환자의 수도 증가하고 있다. 고령 환자는 다양한 만성 질환을 가지고 있는 경우가 많다. 특히 골다공증은 고령 환자에서 매년 증가하는 추세로 치과에 내원하는 골다공증 환자 수는 더욱 증가할 전망이다. 현재 비스포스포네이트 제재가 골다공증 치료에 가장 많이 사용되며 장기간 경구복용 하거나 주사제를 투여받은 환자에서 악골괴사가 부작용으로 보고되었다. 이러한 약제관련 악골괴사는 외과적인 치과치료가 원인이 되는 경우가 많고 발병하면 치유가 어려워 예방이 중요하다. 보철 수복을 위한 구강형성 과정에서 외과적 치료가 필요할 수 있으며, 골다공증 환자의 성공적인 치료를 위해 치과의사는 질환에 대해 충분히 이해하고 관심을 가져야 한다.
