Objectives: This study aimed to investigate the regional difference of chronic periodontal care services in Korea by the analysis of 2010 raw data from Health Insurance Review and Assessment Service. Methods: The subjects were the chronic periodontitis patients over 35 years old from dental care facilities in Korea. The study population was 278,319 including 264,994 claims made by dental clinics, 8,084 by dental hospitals, 3,509 by general hospitals, and 1,732 by tertiary hospitals. Results: There was a significant difference in medical care cost benefit between the provinces(p<0.0001). The age groups showed a clear difference in the patient charge, cost of insurance, and medical care cost benefit(p<0.0001). In consideration of the first visit or revisit, there were differences in the rate of prescription, dental examination, and surgical procedures of the chronic periodontal patients from dental facilities. The radiographic use rate in the tertiary hospitals was 2.6 times higher than that of the dental clinic in the treatment of the new chronic periodontal patients. Conclusions: The use of dental services in the periodontitis is influenced by the types of medical services facility, cost of medical insurance, and patient charge. In consideration of cost benefit analysis, prevention is the most important care for the periodontitis. Regional difference in peridontitis is cause by the use of medical services and quality of treatment.
Objectives: The objective of the study was to infection control by analyzing the influence of working environment on infection control activities in the dental hygienists. Methods: The cross-sectional study was based on a survey on a total of 377 dental hygienists working in dental settings. Multiple linear regression analysis was performed to examine the relationship of general characteristics and infection control activities. All statistical analyses were performed using the SPSS for Windows version 20.0, and p<0.05 was considered to be significant. Results: Predictive powers(=Adjusted $R^2$ of the investigated factors such as operation room, sterilization disinfection laundry, dental unit waterline, staff individual, infection control system, personal protective equipment, medical waste, hand hygiene, oral surgical procedures, clinical contact surfaces were adjusted $R^2=0.394$, 0.306, 0.277, 0.244, 0.241, 0.177, 0.165,, 0.154, 0.134, 0.124 respectively. Conclusions: In order to enhance infection control activities, the program development and implementation for the aseptic procedure is very important. The program should include periodic reinforcement of infection control education and regular monitoring of infection control activities.

This systematic review evaluates current evidence regarding the feasibility of using needleless jet injection instead of a conventional local anesthetic needle. EBSCO, ProQuest, PubMed, and Scopus databases were used to identify relevant literature published in English from 2005 to 2020. Ten studies were selected. Five of them were randomized clinical trials, 3 case-control studies, and 2 equivalence trials. Using the Critical Appraisal Skills Program checklist, 6 studies scored between 67% and 100%, and 4 studies scored between 34% and 66%. According to Jadad's scale, 2 studies were considered strong, and 8 studies were considered moderate in quality. The results of the 10 studies showed differences in patient preference for needleless jet injection. Needleless injection technique has been found to be particularly useful in uncooperative patients with anxiety and needle phobia. Needleless jet injection is not technique sensitive. However, with needleless jet anesthesia, most treatments require additional anesthesia. Conventional needle anesthesia is less costly, has a longer duration of action, and has better pain control during dental extraction. Needleless jet anesthesia has been shown to be moderately accepted by patients with a fear of needles, has a faster onset of action, and is an efficient alternative to conventional infiltration anesthesia technique.
The purpose of this study is to analyze and compare dental hygine curriculum of three countries--Korea, U.S.A., and Japan--, focusing on the ways of organizing content. In this study, 30 curriculum documents of dental hygienist in U.S., Japan, and Korea were analyzed and discussed by a framework of analysis which was constructed referring to Tylerian curriculum model. Using this framework, three kinds of key components of curriculum document were compared according to specific elements of each component. More specifically, comparing those curriculum documents, we must consider the dimension of content organization. In this study, the problem of course organization was viewed on three dimensions: continuity, sequence, and integration of curriculum content. Based on the results of this study, the conclusions are as follows. 1. Courses of dental hygienist education should be organized so that the curriculum foster the cumulative and continuous learning. It will be accomplished by considering the essential procedures or sequence of dental hygiene activities. 2. When organizing curriculum elements, we should consider the continuity dimension of learning content. Continuity deals with the vertical manipulation or repetition of curriculum components. In other words, it accounts for the reappearance in the curriculum of certain major ideas or skills. Therefore, courses should be based on the level of the dental hygienist activities in order to ensure a continuity of learning experiences. 3. Dental hygienist curriculum must be organized in a way to integrate the logic of dental hygiene as a discipline with the process of dental hygiene performance. Integration refers to the linking of all types of knowledge and experiences contained within the curriculum design. It emphasizes horizontal relations among various content topics and themes involving all domains of knowledge recognized. Thus, courses of dental hygiene curriculum should be integrated to allow college students to obtain a unified view of knowledge and an in-depth meaning of the dental hygiene as a profession.

Maxillary implants are inserted in the upward direction, meaning that they oppose gravity, and achieving stable support is difficult if the alveolar bone facing the maxillary sinus is thin. Correspondingly, several sinus-lifting procedures conducted with or without bone graft materials have been used to place implants in the posterior area of the maxilla. Even with these procedures available, it has been reported that in about 5% of cases, complications occurred after implantation, including acute and chronic sinusitis, penetration of the sinus by the implant, implant dislocation, oroantral fistula formation, infection, bone graft dislocation, foreign-body reaction, Schneiderian membrane perforation, and ostium plugging by a dislodged bone graft. This review summarizes common maxillary sinus pathologies related to implants and suggests an appropriate management plan for patients requiring dental implantation.

Background: The vascularity of the peri-implant tissue is a very important parameter in establishment and maintenance of a healthy tissue after dental implant insertion. Objective: The purpose of this study was to compare the vascularity of the peri-implant mucosa between flap and flapless implant surgeries by using a canine mandible model. Study Design: In six mongrel dogs, bilateral, edentulated, flat alveolar ridges were created in the mandible. After 3 months of healing, two implants were placed in each side by either the flap or flapless procedures. After another healing period of 3 months, biopsies were obtained, prepared for light microscopy and exposed to morphometric measurements. Results: The supracrestal connective tissue lateral to the implant was found to be more richly vascularized in the flapless group than in the flap group. Conclusion: These results suggest that the flapless procedure may have an effect on increasing the vascularity of the peri-implant mucosa.

The inferior alveolar nerve (IAN) block is the most frequently used mandibular injection technique for achieving local anesthesia for restorative and surgical procedures. However, The IAN block does not always result in successful anesthesia, especially pulpal anesthesia. Lidocaine is used as a "standard" local anesthetic for the inferior alveolar nerve. Articaine recently joined Korean market as a form of dental cartridge. It has an advantage of superior diffusion through bony tissue. A variety of trial was performed to improve the success rate of inferior alveolar nerve block. In this review, the recent update related with inferior alveolar nerve block anesthesia will be discussed on the anatomical consideration, anesthetic agent, technique, and complications.
Implant treatment has long been established as the main stream for the recovery of lost teeth. Implant therapy, which began to be practiced under the concept of osseointegration, was performed on the completely healed bone, but implant placement immediately after extraction, which began to be introduced in the 1970s, began to become a widely used treatment modality since the 2000s. However, as with all other procedures, immediate implant placement is not omnipotent. If you are obsessed with the obsession that you need to provide quicker implant treatment to the patients, and if you do it as if you are being chased by time, it is the immediate implant placement that can lead to various embarrassing situations. In this article, to reduce complications, the author will look at some issues that need to be considered when placing implants immediately after extraction.
Treatment of immature permanent teeth with irreversibly damaged pulp has been challenging in dental practice because of the lack of apical constriction, thin dentinal walls, and short roots. This may lead to the extrusion of filling materials, and fracture of the root due to its more fragile feature during shaping of the root canal. Apexification with calcium hydroxide or MTA is one of the treatment options for these cases. Although favorable results of apexification have been reported, these treatment procedures do not guarantee the increase of root length and/or width even after a long term period. Thus, treated teeth are still prone to fractures. Recently, pulp revascularization has been proposed as an alternative treatment for immature teeth with necrotic pulp and periapical pathosis. Pulp revascularization allows the stimulation of the apical development and the root maturation. There have been many treatment protocols using various materials such as antibiotics and calcium hydroxide medicament. In this case report, literature review about pulp revascularization and two related cases are presented.

Oromandibular dystonia (OMD) is a rare focal form of dystonia caused by prolonged muscles spasms in the mouth, face, and jaw. OMD can develop after dental treatment, as poorly aligned dentures or multiple tooth extraction may cause an impairment of proprioception in the oral cavity, leading to the subsequent development of dystonia. These repetitive involuntary jaw movements may interfere with chewing, swallowing, and speaking. We report here two cases of OMD after dental procedures.
