Dental sedation plays a pivotal role in alleviating patient anxiety during various procedures. Remimazolam, a benzodiazepine derivative, stands out for its distinctive attributes, particularly its rapid onset of sedation coupled with a brief duration, making it an invaluable option for dental applications. The patient was admitted for the extraction of impacted third molars via patient-controlled sedation and not only demonstrated stable vital signs but also expressed a high level of satisfaction with the procedure. An in-depth analysis of plasma remimazolam concentrations and changes in the Patient State Index revealed negative correlation patterns, highlighting the inherent potential of remimazolam in achieving effective sedation. This expanded research scope aims to provide a more nuanced understanding of the pharmacological responses to remimazolam in dental sedation scenarios. This case report offers valuable insights into the evolving landscape of dental sedation methodologies and paves the way for a more informed and evidence-based approach to the use of remimazolam in patient-controlled sedation.
Chronic periodontitis is one of the most common diseases in clinical dentistry, which requires various surgical interventions to treat the moderately to severely destructed supporting periodontium. Most patients have anxiety and fear to these surgical procedures and dentists often have problems dealing with these patients. By applying the conscious sedation technique in outpatient units, periodontists have become able to manage their patients successfully with less anxiety or fear. Also, we have experienced the increased level of patients' satisfaction. Generally, periodontal treatments are time consuming procedures and patients are usually reluctant to the periodontal instruments. This study is focused on the sedation procedure with intravenous midazolam infusion performed in Department of Periodontology of Ewha Womans University Hospital. Using questionnaires, we evaluated 80 randomly selected patients for the anxiolytic effect of intravenous midazolam. Anxiety control using IV sedation was very helpful in performing various periodontal reconstructive and advanced surgical procedures in implant dentistry.
The advent of osseointegration and advances in biomaterials and techniques have contributed to increased application of dental implants in the restoration of maxillary partial edentulous patients. Often, in these patients, soft and hard tissue defects result from a variety of causes, such as infection, trauma, and tooth loss. These create an anatomically less favorable foundation for ideal implant placement. Reconstruction of the atrophic maxillary alveolar bone through a variety of regenerative surgical procedures has become predictable; it may be necessary prior to implant placement or simultaneously at the time of implant surgery to provide a restoration with a good long-term prognosis. Regenerative procedures are used for horizontal and vertical ridge augmentation. Many different techniques exist for effective bone augmentation. The approach is largely dependent on the extent of the defect and specific procedures to be performed for the implant reconstruction. It is most appropriate to use an evidenced-based approach when a treatment plan is being developed for bone augmentation cases. The cases presented in this article clinically demonstrate the efficacy of using a autogenous block graft, guided bone regeneration, ridge split, immediated implant placement technique on the atrophic maxillary area.
Objective: The objectives of this study were to measure pediatric organ and effective doses of cone-beam computed tomography (CBCT) for orthodontic analysis and to compare them to those of panoramic and lateral cephalometric radiography, the conventional radiography for orthodontic analysis. Materials and Methods: Alphard VEGA for CBCT, Planmeca Proline XC for panoramic radiography and Orthophos CD for cephalometric radiography were used for this study. Thermoluminescent dosimeter (TLD) chips were located at 24 anatomic sites of 10-year-old anthropomorphic phantom and exposed during CBCT (C-mode; $200{\times}179mm$ FOV), panoramic and lateral cephalometric radiographic procedures at the clinical exposure settings for 10-year-old patient. Pediatric organ and effective doses were measured and calculated using ICRP 2007 tissue weighting factors. Results: Effective doses of CBCT, panoramic radiography and lateral cephlometric radiography in pediatric clinical exposure settings were $292.5{\mu}Sv$, $19.3{\mu}Sv$, and $4.4{\mu}Sv$ respectively. The thyroid gland contributed most significantly to the effective dose in all the radiographic procedures. Conclusion: Effective dose of CBCT was about 12 times to conventional radiographic procedures for orthodontic analysis in pediatric patient. The use of CBCT for orthodontic analysis should be fully justified over conventional radiography and dose optimization to decrease thyroid dose is needed in pediatric patients.
Aspergilloma of the maxillary sinus is considered rare in immunocompetent patients, but a considerable increase has recently been seen in the incidence of reported cases. Dental procedures involving the antral region are thought to predispose individuals to this form of aspergillosis. Because aspergilloma shares similar clinical features with other sinus pathologies, its diagnosis may be delayed. Thus, an early diagnosis confirmed by a histopathological examination plays a crucial role in the adequate management of aspergilloma. This article provides a concise review of the reported cases of aspergilloma associated with dental procedures and reports 2 new cases of aspergilloma in middle-aged female patients, with a presentation of their cone-beam computed tomographic findings.
Relapse following rotational movement of the tooth is a common problem in orthodontic practice. To overcome such relapse, many procedures have been advocated: prolonged retention, permanent retention, over-rotation, rotation of teeth at an early age, surgical procedures such as gingivectomy, redressement force, septotomy, et cetra. A 23-year-old woman presented with Angle's Class I malocclusion and extreme rotation of maxillary central incisors. After 15 months' active therapy, septotomy was performed on maxillary central incisors and Howley retainer was applicated for the purpose of overcoming rotational relapse. During the 1 year post-operative observation, negligible, if any, rotational relapse occurred.
Purpose: The development of the dental technology industry has digitized the dental process. Definition of Digitalization of Dental Implantation Digitalization is the process of model making and prosthodontic processing using 3D CAD and CAM. Currently, it is becoming popular due to the spread of various devices. However, precision evaluation at this stage is an important factor in precision-based dental procedures. Therefore, in this study, we want to analyze the precision of the processed body made with digital dental machine. Methods: The accuracy of digital dental pore devices was evaluated. Specimens were fabricated with 5 types of 3D printers. The external shape was measured with the prepared specimen. The surface roughness was measured. Results: As a result, precision was excellent in order of EP2 specimen, EP1 specimen, and EA2 specimen. The precision of EP3 specimen and EA1 specimen is not excellent. And the precision of the specimen processed with polymer 3D printer is excellent. The accuracy of LCPS type polymer 3D printers is considered to be excellent. Conclusion : 1. Observation of the shape The 3D printer for LCPS system and the 3D printer for SLM $40{\mu}m$ system were found to be precisely processed. 2. Surface roughness results The LCPS system polymer 3D printer has been shown to have a precise surface.

Population aging and the usage of small devices in implant prosthetic procedures have led to many incidents of dental aspiration and ingestion. Various preventive measures have been introduced to prevent these accidents. However, accidents can occur at any time. Dental aspiration and ingestion lead to fatal consequences if the issue is not promptly and appropriately dealt with. Preparing a collaborative system for dealing with accidents before they occur can prevent further sequelae. This study involves ingestion and aspiration accidents that occurred during dental treatment: two ingestion cases and one aspiration case. All dental foreign bodies were removed according to the guidelines presented in our review. With the cooperation of other medical departments, the issues were quickly resolved. Simple and accurate protocols should be provided to all dentists and dental staff to respond to such dental emergencies. In addition, collaboration among other medical departments should be established before any accidental ingestion and aspiration events occur.
In dentistry, the term 'fusion' is used to describe a developmental disorder of dental hard tissues. In the permanent dentition, fusion of a normal tooth and a supernumerary tooth usually involves the incisors or canines. However, a few cases of fusion involving premolars have also been reported to date. We present a rare case in which fusion of the maxillary left second premolar and a supernumerary tooth in a 13-year-old girl was diagnosed using cone beam computed tomography (CBCT, Alphard-3030, Asahi Roentgen Ind. Co., Ltd.). The tooth was bicuspidized after routine nonsurgical root canal treatment, and the separated teeth underwent appropriate restoration procedures. The second premolar and supernumerary tooth remained asymptomatic without any signs of inflammation after a follow-up period of 9 years. Identification of anatomical anomalies is important for treatment in cases involving fusion with supernumerary tooth, and therefore the microscopic examinations and CBCT are essential for the diagnosis. Fused teeth can be effectively managed by the comprehensive treatment which includes both endodontic and periodontal procedures.
