Influence of abutment materials on the implant-abutment joint stability in internal conical connection type implant systems

  • PURPOSE. This study evaluated the influence of abutment materials on the stability of the implant-abutment joint in internal conical connection type implant systems. MATERIALS AND METHODS. Internal conical connection type implants, cement-retained abutments, and tungsten carbide-coated abutment screws were used. The abutments were fabricated with commercially pure grade 3 titanium (group T3), commercially pure grade 4 titanium (group T4), or Ti-6Al-4V (group TA) (n=5, each). In order to assess the amount of settlement after abutment fixation, a 30-Ncm tightening torque was applied, then the change in length before and after tightening the abutment screw was measured, and the preload exerted was recorded. The compressive bending strength was measured under the ISO14801 conditions. In order to determine whether there were significant changes in settlement, preload, and compressive bending strength before and after abutment fixation depending on abutment materials, one-way ANOVA and Tukey's HSD post-hoc test was performed. RESULTS. Group TA exhibited the smallest mean change in the combined length of the implant and abutment before and after fixation, and no difference was observed between groups T3 and T4 (P>.05). Group TA exhibited the highest preload and compressive bending strength values, followed by T4, then T3 (P<.001). CONCLUSION. The abutment material can influence the stability of the interface in internal conical connection type implant systems. The strength of the abutment material was inversely correlated with settlement, and positively correlated with compressive bending strength. Preload was inversely proportional to the frictional coefficient of the abutment material.

Rotational tolerances of a titanium abutment in the as-received condition and after screw tightening in a conical implant connection

  • PURPOSE. The success of an implant-prosthetic rehabilitation is influenced by good implant health and an excellent implant-prosthetic coupling. The stability of implant-prosthetic connection is influenced by the rotational tolerance between anti-rotational features on the implant and those on the prosthetic component. The aim of this study is to investigate the rotational tolerance of a conical connection implant system and its titanium abutment counterpart, in various conditions. MATERIAL AND METHODS. 10 preparable titanium abutments, having zero-degree angulation (MegaGen, Daegu, Korea) with an internal 5-degree conical connection, and 10 implants (MegaGen, Daegu, Korea) were used. Rotational tolerance between the connection of implant and titanium abutments was measured through the use of a tridimensional optics measuring system (Quick Scope QS250Z, Mitutoyo, Kawasaki, Japan) in the as-received condition (Time 0), after securing with a titanium screw tightening at 35 Ncm (Time 1), after tightening 4 times at 35 Ncm (Time 2), after tightening one more time at 45 Ncm (Time 3), and after tightening another 4 times at 45 Ncm (Time 4). RESULTS. The group "Time 0" had the lowest values of rotational freedom (0.22 ± 0.76 degrees), followed by the group Time 1 (0.46 ± 0.83 degrees), the group Time 2 (1.01 ± 0.20 degrees), the group Time 3 (1.30 ± 0.85 degrees), and the group Time 4 (1.49 ± 0.17 degrees). CONCLUSION. The rotational tolerance of a conical connection is low in the "as received" condition but increases with repetitive tightening and with application of a torque greater than 35 Ncm.

Effect of abutment types and resin cements on the esthetics of implant-supported restorations

  • PURPOSE. The aim of the study was to evaluate the optical properties of new generation (3Y-TZP) monolithic zirconia (MZ) with different abutment types and resin cement shades. MATERIALS AND METHODS. A1/LT MZ specimens were prepared (10 × 12 × 1 mm, N = 30) and divided into 3 groups according to cement shades as transparent (Tr), yellow (Y) and opaque (O). Abutment specimens were obtained from 4 different materials including zirconia (Group Z), hybrid (Group H), titanium (Group T) and anodized yellow titanium (Group AT). MZ and abutment specimens were then cemented. L*, a*, and b* parameters were obtained from MZ, MZ + abutment, and MZ + abutment + cement. ∆E001* (between MZ and MZ + abutment), ∆E002* (between MZ and MZ + abutment + cement) and ∆E003* (between MZ + abutment and MZ + abutment + cement) values were calculated. Statistical analyses included 2-way ANOVA, Bonferroni, and Paired Sample t-Tests (P < .05). RESULTS. Abutment types and resin cements had significant effect on L*, a*, b*, ∆E001*, ∆E002*, and ∆E003* values (P < .001). Without cementation, whereas zirconia abutment resulted in the least discoloration (∆E001* = 0.68), titanium abutment caused the most discoloration (∆E001* = 4.99). The least ∆E002* = 0.68 value was seen using zirconia abutment after cementation with yellow shaded cement. Opaque shaded cement caused the most color change (∆E003* = 5.24). Cement application increased the L* values in all groups. CONCLUSION. The least color change with/without cement was observed in crown configurations created with zirconia abutments. Zirconia and hybrid abutments produced significantly lower ∆E002* and ∆E003* values in combination with yellow shaded cement. The usage of opaque shaded cement in titanium/anodized titanium groups may enable the clinically unacceptable ∆E00* value to reach the acceptable level.

치과용 임플란트 시스템의 기계적 가공오차에 관한 연구 (Machining Tolerance of Various Implant Systems and their Components)

  • 연구목적 : 본 연구에서는 rotational freedom을 측정할 수 있는 기구를 개발하여 시중에서 유통되고 있는 국산 임플란트 및 다양한 국적의 임플란트들의 기계적인 가공오차들을 측정하여 다양한 임플란트 시스템의 component간의 기계적인 안정성을 평가하고자 한다. 또한 본 연구에서는 더 나아가 최근에 임플란트 abutment로 각광을 받고 있는 각종 ceramic abutment의 절삭 가공오차에 관한 항목을 측정하여 임플란트 제조사 및 임상의들에게 올바른 정보를 제공하고자 한다. 연구재료 및 방법 : 국내에서 유통되는 외부연결구조의 외국산 임플란트 시스템(Nobel Biocare, Anthorgyr)과 국산 시스템(Neobiotec)과 내부연결구조의 임플란트 시스템(외국산:Nobel Biocare, Anthorgyr, Straumann, Frident Dentsply, 국산:Dentium) 별로 임플란트 fixture, abutment, analog를 서로 교차 연결하여 회전각도측정기(rotational angle measuring device)로 freedom of rotational angle을 측정하였다. 국산 외부연결구조의 지르코니아 abutment(ZirAce)를 외부연결구조의 임플란트 시스템(Neobiotec, Nobel Biocare, Anthorgyr)의 fixture와 analog와 교차연결하여 freedom of rotational angle을 측정하였다. 연구결과 : 국산 외부연결구조의 임플란트 시스템은 약 2.67도(fixture와 abutment 연결시), 내부연결구조의 임플란트는 약 4.3도(fixture와 abutment 연결시)의 rotational freedom을 보였다. 국산 지르코니아 abutment는 외국산 및 국산 외부연결구조 임플란트 시스템과 상관없이 3도 이하(fixture와 연결시)의 결과를 보였다. 결 론 : 시제품으로 제작된 디지털 회전각도측정기는 높은 분해능을 갖고 있었으며, 국산 임플란트의 기계적 가공오차는 외국산 임플란트와 거의 유사했다. 국산 세라믹 abutment의 기계적 가공오차는 fixture 제조회사별로 다르게 나타났지만 같은 회사의 절삭가공된 금속 abutment와 비교시 가공오차가 더 적었다.

Influence of preparation depths on the fracture load of customized zirconia abutments with titanium insert

  • PURPOSE. This study evaluated the fracture load of customized zirconia abutments with titanium insert according to preparation depths, with or without 5-year artificial aging. MATERIALS AND METHODS. Thirty-six identical lithium disilicate crowns (IPS e.max press) were fabricated to replace a maxillary right central incisor and cemented to the customized zirconia abutment with titanium insert on a $4.5{\times}10$ mm titanium fixture. Abutments were fabricated with 3 preparation depths (0.5 mm, 0.7 mm, and 0.9 mm). Half of the samples were then processed using thermocycling (temperature: $5-55^{\circ}C$, dwelling time: 120s) and chewing simulation (1,200,000 cycles, 49 N load). All specimens were classified into 6 groups depending on the preparation depth and artificial aging (non-artificial aging groups: N5, N7, N9; artificial aging groups: A5, A7, A9). Static load was applied at 135 degrees to the implant axis in a universal testing machine. Statistical analyses of the results were performed using 1-way ANOVA, 2-way ANOVA, independent t-test and multiple linear regression. RESULTS. The fracture loads were $539.28{\pm}63.11$ N (N5), $406.56{\pm}28.94$ N (N7), $366.66{\pm}30.19$ N (N9), $392.61{\pm}50.57$ N (A5), $317.94{\pm}30.05$ N (A7), and $292.74{\pm}37.15$ N (A9). The fracture load of group N5 was significantly higher than those of group N7 and N9 (P<.017). Consequently, the fracture load of group A5 was also significantly higher than those of group A7 and A9 (P<.05). After artificial aging, the fracture load was significantly decreased in all groups with various preparation depths (P<.05). CONCLUSION. The fracture load of a single anterior implant restored with lithium disilicate crown on zirconia abutment with titanium insert differed depending on the preparation depths. After 5-year artificial aging, the fracture loads of all preparation groups decreased significantly.

임플란트 지대주 종류와 동적하중이 고정체와 지대주의 미세간극에 미치는 영향 (The Effect of Types of Abutment and Dynamic Loading on Microgap between Implant Fixture and Abutment)

  • 이전부터 사용된 티타늄, 금합금 지대주 및 최근 사용이 급증한 지르코니아 지대주와 임플란트의 계면에 대한 비교 연구와 동적하중이 계면에 미치는 영향에 관한 연구가 많지 않은 실정이다. 본 연구에서 지대주 종류와 동적 하중에 따른 미세간극의 차이를 주사전자현미경 분석을 통하여 비교평가 하였다. USII ${\phi}3.75{\times}11.5mm$ (Osstem Co., Seoul, Korea) 티타늄 임플란트에 티타늄 지대주, 지르코니아 지대주, 주조 금합금 (UCLA plastic) 지대주를 각 군당 7개씩, 30Ncm의 조임회전력으로 연결하였다. 25-250N의 sine형 동적하중을 장축 방향에서 $30^{\circ}$ 경사로 $10^5$회 시행하였다. 하중 전후에 계면의 미세간극을 순측, 구개측, 근심측, 원심측 부위에서 측정하였고, 그 결과 동적 하중으로 인한 통계적 유의성은 없었다. 지대주 종류에 따른 미세간극을 비교한 결과, 동적하중 시행 전후 모두 UCLA 군과 티타늄 군, UCLA 군과 지르코니아 군 사이에 통계적으로 유의한 차이를 보였다(p<0.05). 티타늄 군과 지르코니아 군 사이에는 유의한 차이를 보이지 않았다. UCLA 지대주는 정밀 연삭된 티타늄, 지르코니아 지대주 보다 더 큰 미세간극을 보였고 $10^5$회 동적하중은 미세간극에 영향을 미치지 않았다.

  • 연구목적 : 본 연구는 임플랜트 지대주에 동적하중을 가하기 전, 후의 풀림토크 변화를 측정함으로써 수종의 지대주에 가해진 단기간의 교합력이 임플랜트-지대주의 안정성에 영향을 미치는지를 연구하고자 하였다. 재료및 방법 : 임플랜트 고정체는 외부육각구조를 사용하였고, 티타늄합금 지대주, 지르코니아 지대주, 주조 금합금 지대주인 UCLA plastic abutment를 사용하였다. 초기 풀림회전력을 측정하고, 하중을 $10^5$ 회 가한후, 풀림회전력을 측정하였다. 초기 풀림회전력 상실률, 하중후 풀림회전력 상실률, 동적하중에 의한 풀림회전력 상실률을 계산하였다. 결과 : 초기 풀림회전력, 동적 하중후 풀림회전력, 초기 풀림회전력 상실률, 동적 하중후 풀림회전력 상실률, 동적 하중에 의한 풀림회전력 상실률은 각 지대주간 유의한 차이를 나타내지 않았다. 또한, 각 지대주 내에서 동적 하중 전후의 풀림회전력은 통계적으로 유의한 차이를 나타내지 않았다. 결론 : Titanium, zirconia, UCLA 지대주의 초기 나사풀림은 지대주간 유의한 차이가 없음을 알 수 있었고, 초기의 저작력은 하중전후 나사풀림에 큰 영향을 주지 않는다고 생각된다. 또한, UCLA, zirconia, titanium 지대주의 선택시 초기 나사풀림과 관련하여 특정 종류의 지대주 선택을 기피할 필요는 없다고 생각된다.

내측 연결 임플란트에서 지대주 내부길이가 나사 풀림에 미치는 영향 (Influence of internal connection length on screw loosening in internal connection implants)

  • 목적:본 연구에서는 long internal connection 형태의 임플란트 지대주를 내부 연결 길이에 변화를 주어 임플란트-지대주 결합부의 안정성을 비교 평가해 보고자 하였다. 재료 및 방법: Long internal connection의 임플란트(Replus system, $4.7{\times}11.5mm$)를 각각 지대주의 길이에 따라 4개의 군(1, 2, 3, 4 mm 군)으로 나누었고 총 20개의 시편을 사용하였다. 시편을 레진에 매몰하여 고정시키고 100 N의 힘으로 임플란트 장축에 대해 30도의 각도에서 $1.0{\times}10^6$ 번의 반복하중을 가한 후 하중 전 후의 풀림회전력의 차이를 계산하여 95% 유의수준에서 Kruskal-Wallis 검정 방법을 통해 통계 분석하였다. 결과:지대주 내부 길이에 따른 풀림 회전력의 통계적 유의성은 나타나지 않았으며 (P > .05) 어떤 시편에서도 완전한 나사 풀림이나 나사 파절은 관찰되지 않았다. 결론: 내측 연결 임플란트에서 지대주 내부길이에 따른 나사 풀림의 정도는 차이가 나지 않았다.

COMPARATIVE ACCURACY OF THE SPLINTED AND UNSPLINTED IMPRESSION METHODS FOR INTERNAL CONNECTION

  • Statement of problem. Accurate impression is essential to success of implant prostheses. But there have been few studies about the accuracy of fixture-level impression techniques in internal connection implant systems. Purpose. The purpose of this study was to compare the accuracy of two fixture-level impression techniques in two conditions (parallel and divergent) and to assess the effect of tightening sequences and forces on stresses generated on superstructures in internal connection implant system (Astra Tech). Material and methods. Two metal master frameworks made from two abutments (Cast-to Abutment ST) each and a corresponding, passively fitting, dental stone master cast with four fixture replicas (Fixture Replica ST) were fabricated. Ten dental stone casts for each impression techniques (direct unsplinted & splinted technique) were made with vinyl polysiloxane impressions from the master cast. Strain gauges for each framework were fixed midway between abutments to measure the degree of framework deformation on each stone cast. Pairs of strain gauges placed opposite each other constituted one channel (half Wheatstone bridge) to read deformation in four directions (superior, inferior, anterior, and posterior). Deformation data were analyzed using one-way ANOVA and the Tukey test at the .01 level of significance. And the effect of tightening sequences (right-to-left and left-to-right) and forces (10 Ncm and 20 Ncm) were assessed with ten stone casts made from parallel condition by the splinted technique. Deformation data were analyzed using paired t-test at the .01 level of significance. Conclusions. Within the limitations of this study, the following conclusions could be drawn. 1. Frameworks bent toward the inferior side on all casts made by both direct unsplinted and splinted impression techniques in both parallel and divergent conditions. 2. There was no statistically significant difference of accuracy between the direct unsplinted and splinted impression techniques in both parallel and divergent conditions (P>.01). 3. There was no statistically significant difference of stress according to screw tightening sequences in casts made by the splinted impression technique in parallel condition (P>.01). 4. Greater tightening force resulted in greater stress in casts made by the splinted impression technique in parallel condition (P<.01).

변형된 임플란트 임시 지대주의 물성에 대한 연구 (The Study on the Physical Property of Provisional Prosthesis using Modified Temporary Abutment)

  • Statement of problem: Damping of the peak force transmitted to implants has been reported by in vitro studies using impact forces on resin-veneered superstructures. Theoretical assumptions suggest that use of acrylic resin for the occlusal surfaces of a prosthesis would protect the connection between implant and bone. Therefore, the relationship between prosthesis materials and the force transmitted through the implant system also needs to be investigated under conditions that resemble the intraoral mechanical environment. Purpose: The purpose of this study was to analyze the fracture strength and modes of temporary prosthesis when a flange or occlusally extended structure were connected on the top of the abutment. Material and method: Modified abutments of winged and bulk design were made by casting the desired wax pattern which is made on the UCLA type plastic cylinder. Temporary crowns were made using templates on the modified abutments, and its fracture toughness and strain were compared to the traditional temporary prosthesis. To evaluate the effect of aging, 5.000 times of thermocycling were performed, and their result was compared to the 24hours specimen result. Results: The following conclusions were drawn from this study: 1. In the fracture toughness test, temporary crown's fracture line located next to the screw hole while modified designs with metal support showed fracture line on the metal and its propagation along the metal-resin interface. 2. Wing and bulk structure didn't show significant difference in the fracture toughness (p>0.05), but wing structure showed stress concentration on the screw hole area compared to bulk structure which showed even stress distribution. 3. In the fracture toughness test after thermocycling, wing and bulk structure showed increased or similar results in metal supported area while off-metal area and temporary crown showed decreased results. 4. In the strain measurement after thermocycling, its value increased in the temporary and bulk structure. However, wing structure showed decreased value in the loading point while increased value in the screw hole area. Conclusion: Wing type design showed compatible result to the bulk type that its application with composite resin prosthesis to the implant dentistry is considered promising.
