Job satisfaction of caregivers is very important factor in the quality of care service. In addition, as perception of human rights is heightened, human rights behavior in nursing homes is also recognized as an important factor. The purpose of this study is to investigate the structural relationship between job satisfaction and human rights behavior, job stress, and social support of dementia workers who have significant influence on quality of care service. For this purpose, 210 questionnaires were collected from 300 dementia workers working in the elderly nursing home, day night care center, and domiciliary visit care center in Gyeonggi province. After excluding 13 questionnaires, the questionnaires were analysed by AMOS and SPSS 21.0. The results of the analysis are following. It was found that job stress significantly affects the social support directly (-.276). Also, social support significantly affects job satisfaction (.315). On the other hand, human rights advocacy has a direct effect on job satisfaction (.175) and an indirect effect (.102), showed a total effect of (.277). This shows that social support partially mediates human rights advocacy and job satisfaction. Job stress had a direct effect on job satisfaction(-.217) and an indirect effect(-.095), indicating a total effect of (-.312), and social support partially mediated job stress and job satisfaction Could know. The results of this study suggest that in order to improve the job satisfaction of the dementia workers, it is necessary to increase the human rights advocacy behavior and reduce the job stress in an environment that recognizes the social support.

  • Purpose: The purpose of this study was to investigate factors related to use of restraints for older adults with dementia in long-term care settings. Methods: This study analyzed secondary data from the Nationwide Survey on Dementia Care in Korea (NaSDeCK). Demographic characteristics, disease characteristics, and functional characteristics were analyzed. Data were analyzed with χ2 test, t-test, ANOVA, and logistic regression analysis. Results: Among participants, 8.2% experienced use of restraints. 'Strap' (78.3%) was the most commonly used restraint and 'Behavioral problem' (51.3%) was a major reason for the restraint. Persons who have low levels of daily activity and cognition deficits, higher levels of dependence, histories of falls within the past 3 months, and living in long-term hospitals have greater probability of restraint experience. Conclusion: Developing interventions for older adults who have a high risk of being restrained is essential to minimize unnecessary restraint use among older adults with dementia in long-term care settings. Further study exploring the interacting roles of organizational and staff factors related to use of physical restraints would provide more comprehensive perspectives in understanding this phenomenon.

  • Purpose: The aim of this study was to understand the nurses' experiences of caring for demented older persons in geriatric in care facilities. Methods: The subjects were ten nurses employed in geriatric care facilities. Open in-depth interviews were used to collect data from January, 2010 to October, 2010. The Colaizzi phenomenological method was used for data analysis. Results: The experiences of nurses' caring for patients with dementia were divided into the following six categories: 'new encounter', 'understanding of intention and satisfying', 'dilemma of caring', 'contriving of comfort', 'comprehensive coordinator', and 'being present'. There were 15 clusters of themes. The subjects stated that the best caring for the demented older persons were 'encounter, knowing, and being with them'. Conclusion: Nurses' caring could improve the quality of life for demented older patients in geriatric care facility. These results suggest that institutional, administrational strategies and schemes need to cluster around the relationship with patients who would include encounter knowing and being with them.

  • This study was performed to evaluate the existing pain assessment methods including the tools developed for use with nonverbal older adults with dementia, and to suggest recommendations to clinicians based on the evaluations. Computerized literature searches published after year 2000 using databases - Google scholar, RISS, KoreaMed, Medline, ScienceDirect, CINAHL - were done. Searching keywords were 'pain', 'pain assessment', and 'cognitive impairment/dementia'. The pain assessments for non-communicative dementia patients who are unable to self-report their pains are often made using the assessment tools relying on the observation of behavioral indicators or alternatively the strategy of surrogate reporting. While several tools in English version and only one in Korean are suggested for the pain assessments based on the observation of behavioral indicators, none are commonly used. In this review, we selectively evaluated those tools known to show relatively higher degree of validity and reliability for nonverbal older adults with dementia, namely, CNPI, DOLOPLUS 2, PACSLAC, PAINAD, and DS-DAT. It is hoped that the present review of selected tools for assessing pain in those vulnerable population and the general recommendations given be useful for clinicians in their palliative care practice. And future studies should focus on enriching the validation of the useful tools used to observe the nonverbal patient's behavioral indicators for pain in Korean.

  • The purpose of this study is to have a preliminary study in order to identify changes in occupational performance skills and visual perception of patients with mild dementia, who have been provided with a group occupational therapy program, within community day-care center. The subjects of this study were 9 people with mild dementia living in a community. The period of administration was for 60 minutes per session three times a week during four weeks. We assessed their visual perception function(MVPT) and occupational performance(AMPS). One group pre-post test design was implemented as an experimental study to compare the results between before and after applying the group occupational therapy program within sensory, physical, creative activity. While a statistically significant difference between before and after the program of motor skills(occupational performance skill) is not shown, the visual perception function and occupational performance of patients with mild dementia improved significantly after receiving occupational therapy program(p<0.01, p<0.05). Therefore, it is possible to conclude that the group occupational therapy program is helpful to manage the occupational performance skills and visual perception of patients with mild dementia in community day-care center.

  • Purpose: This study was done in order to examine the differences in gender respect to care burden, fatigue, and life contentment of family caregivers of elderly with dementia. Methods: The setting of the study was a community-based dementia care center providing daycare services. One hundred ninety four caregivers were recruited for the survey. A structured questionnaire was used which included demographic information, care burden, fatigue, and general contentment scales. Caregivers were classified into 4 groups by gender relationship with care recipients. Results: The majority of the caregivers were middle-aged and elderly. More than twice as many were female rather than male caregivers. The results showed that women caregivers expressed more fatigue than men overall (p < .05). In the gender relationships, a different combination of man-woman appeared to have more burdens than that of a woman-woman group (p < .05). For fatigue, the man caregiver-woman elderly group showed a higher fatigue score than a woman-man group (p < .05). However, there was no significant difference in life contentment among groups. Conclusion: These results suggest that caregiver's gender and gender relationship with elders could be considerable factors when the nurses make a plan in the community for dementia nursing management services.
