Factors Affecting On Caregiving Self-efficacy among Dementia Caregivers (치매노인 주부양자의 자기효능감에 영향을 미치는 요인)

  • Purpose: The study evaluated the determinants of caregiving self-efficacy among dementia caregivers. Methods: Data were collected through a structured questionnaire survey from dementia sufferers and caregivers by nurses or social workers caring for dementia sufferers at health centers during July to September 2007. Multiple stepwise regression analysis using SAS Version 9.1 was performed to examine the determinants of caregiving selfefficacy. Results: Factors affecting caregiving self-efficacy were behavioral and psychological symptoms of dementia (BPSD), activities of daily living (ADL), and age of dementia sufferer. Conclusion: To increase dementia caregivers' self-efficacy, there is a need to reduce difficulties of dementia caregivers in caring BPSD and increasing the ADL level of dementia sufferers by providing guidelines of care and intervention programs for BPSD and ADL management.

A Q-methodology Study on Subjective Attitudes toward Dementia in Nursing Students (간호대학생의 치매에 대한 주관적 태도: Q 방법론적 연구)

  • Purpose: Nursing students are key personnel to take care of people with dementia in the nursing field in future. The purpose of this study was to identify subjective attitudes toward dementia in nursing students. Methods: Q methodology, which is a research method to study people's subjective points of views, was used. Thirty seven Q-statements (Q-sample) were derived from the Q population (Concourse) and were arranged in rank order by each of 42 participants into a normal distribution grid (from -4 to +4). A QUANL program was used to analyze the collected data. Results: Three types of attitudes toward dementia were identified: 'self-guided dementia management', 'national-guided dementia management' and 'facility-guided dementia management.' Conclusion: The results of this study suggest that nursing strategies to improve understanding of dementia and actively cope with dementia should be considered.

  • In modern society, dementia patients are increasing due to busy daily life. Due to the nature of dementia, family members are having a hard time with their caring activities. Dementia diseases reduce the quality of life not only of the patient, but also of the family, and nursing activities lead to social problems such as unemployment or frequent turnover. This study aims to reduce the difficulty and stress of the nursing process by supporting daily life of dementia patients and nursing activities through smart care platform. Ultimately, we will reduce the cost of treatment for people with dementia and delay the worsening of symptoms through a systematic care program for people with dementia. The smart dementia care platform features smart technology to help the dementia patients' daily lives and support their caregivers.

Space Design Based on the Behavioral Characteristics of the Elderly with Dementia (치매노인의 행동특성을 고려한 공간디자인)

  • This study was intended to suggest the design guidelines of the space for the elderly with dementia. To achieve this, the behavioral characteristics of the elderly with dementia were grasped, and the design cases which were considered and reflected of the behavioral characteristics of the elderly with dementia were examined. Then, the effective space design guidelines which met the behavioral characteristcs of the elderly with dementia were suggested. The behaviors of the elderly with dementia were categorized into cognitive, emotional, and functional behavior. The positive influences of space design on dementia symptom were founded in the planned living environments, accessible and visible location of common space such as living room, soft and homelike interior finishes and lighting, and healing garden. Therefore, the space design for the elderly with dementia should be done on the basis of the dementia symptoms and healing effects.

New Proposal of Private Insurance Program for Dementia Patients: Design of Sustainable Private Insurance Program in Korea

  • The purpose of this study is to examine interventions and supporting systems by dementia stage, take a look at dementia insurance policies in Korea and the United States, and present Korean private insurance programs for dementia patients. According to the study, our suggestions of a design of private insurance products for Korean dementia patients are as follows. First, the products should support people aged 80 and older. Second, new products should include the mild stage dementia in the insurance coverage. Third, non-pharmacological treatments, such as the cognitive stimulation, the cognitive training, and exercises need to be covered through the new private insurance. Fourth, the private insurance should be contained home health care services in its coverage. These suggestions can reduce the dependence of the public insurance, help people choose appropriate treatments for themselves, and give people a good opportunity to improve the effect of dementia treatment and to increase the satisfaction of patients and their families.

  • The purpose of this research is to introduce the best practice of the Japanese national dementia strategy and explore implications to the Korean national dementia strategy. Interview was conducted among professions those who is in charge of Kumamoto dementia care practice in Kumamoto province, upon review of related literature and public documents. The Kumamoto model is implemented by the department of neuropsychiatry in public university hospitals, which can offer dementia-specialized medical services. Medical centers for dementia in public university hospitals play a leading role for managing practice and training local dementia centers specialist, coordinating medical services among medical institutions and community welfare facilities. In reference to the Kumamoto model, the Korean national dementia strategy can find implications in the direction of current system, specifically its approaches toward policy governance.

  • Dementia is a clinical syndrome characterized by progressive impairment of cognitive and functional abilities. As currently applied treatments for dementia can only delay the progression of dementia and cannot fundamentally cure it, much attention is being paid to reducing its incidence by preventing the associated risk factors. Cardiovascular and metabolic diseases are well-known risk factors for dementia, and many studies have attempted to prevent dementia by treating these risk factors. Growing evidence suggests that sex-based factors may play an important role in the pathogenesis of dementia. Therefore, a deeper understanding of the differences in the effects of drugs based on sex may help improve their effectiveness. In this study, we reviewed sex differences in the impact of therapeutics targeting risk factors for dementia, such as cardiovascular and metabolic diseases, to prevent the incidence and/or progression of dementia.

  • Dementia is classified into Alzheimer's disease, vascular dementia and secondary dementia by its causes. Alzheimer's disease is hard to expect cognitive improvement, whereas vascular dementia and secondary dementia are reversible by medical treatments Dementia can be categorized as Chi-Mae(痴?), Geon-Mang(健忘), Huh-Ro(虛勞), Guang(狂), in the oriental medicine and treated by 'purgation therapy' when it is classified into excessive syndrome. We observed some significant improvements in the cognitive function from three patients diagnosed vascular dementia and secondary dementia by herbal medications of $\ulcorner$Dong-Eui-Su-Sheh-Bo-Won(東醫壽世保元)$\lrcorner$. We report the process of the treatment.

Experience of a Special Rating Dementia Service for Family Caregivers of Elderly People with Dementia (치매노인 가족의 치매특별등급서비스 이용 경험)

  • Purpose: This study tries to explore experiences with a special rating dementia service among family caregivers of elderly people with dementia. Methods: The participants were 11 family caregivers of elderly people with dementia and had used a day-care service from the special rating dementia service for more than 3 months. Data were collected through individual in-depth interviews and analyzed using content analysis. Results: Three categories emerged: "reminiscence of an entangled life", "feeling relieved from a stifled life", and "hope for a better life through help and support" with 8 sub-categories and 23 codes. By using the special rating dementia service, the participants experienced relief from the burden of caring for their elderly family member with dementia and restored their social relations or family relationships. They experienced a positive transition in which their caring for elderly family members with dementia was integrated into their daily lives, and they expressed their desires for better policies. Conclusion: These findings imply the importance and necessity of the special rating dementia service. Practical and systematic family support programs are necessary and proactive publicity activities by the government and public agencies are suggested to ensure that family caregivers can easily use the special rating dementia system.

The Effects of a Support Program for Family Caregivers of Elderly with Dementia on Empowerment and Attitudes toward Dementia (치매노인 가족 지지프로그램이 치매가족의 임파워먼트와 치매태도에 미치는 효과)

  • Purpose: This research was conducted to identify the effects of providing support programs to families of elderly with dementia on family empowerment and attitudes toward dementia. Methods: A nonequivalent control group pre- and post-test design was used. Participants were 49 family caregivers, experimental (24) and control (25), recruited from families through a dementia support center in Y district. This program implemented Haearim, a support program for family caregivers of demented elders developed by the National Institute of Dementia in 2016. Outcome measures were empowerment and attitudes toward to dementia. Data were analyzed with a ${\chi}^2-test$, independent t-test, Fisher's exact probability test, paired t-test, and repeated measures ANOVA with SPSS/PC version 20.0. Results: Family empowerment (F=6.84, p=.002) and family caregivers' attitudes (F=16.48, p<.001) toward dementia in the experimental group improved significantly more than that of the control group after intervention. Conclusion: Results indicate that support programs for families of elderly with dementia are effective in positively changing empowerment and attitudes toward dementia among family caregivers.
