Cervicofacial Lymphatic Malformations: A Retrospective Review of 40 Cases

  • Background Lymphatic malformation (LM) is a form of congenital vascular malformation with a low incidence. Although LM has been studied, no consensus has emerged regarding its cause or treatment. Methods In this study, we retrospectively evaluated 40 patients who visited our vascular anomalies center for the treatment of cervicofacial LM, which is a common manifestation of LM. The medical records of patients over a period of 12 years were reviewed and analyzed for commonalities regarding the diagnosis and the results of treatment. Results Suspected cervicofacial LM was confirmed through imaging studies. No difference in incidence was observed according to sex, and 73% of patients first presented with symptoms before the age of two years. The left side and the V2-V3 area were most commonly affected. No significant differences in incidence were observed among the macrocystic, microcystic, and combined types of LM. A total of 28 out of 36 patients received sclerotherapy as the first choice of treatment, regardless of the type of lesion. Complete resolution was achieved in only 25% of patients. Conclusions LM is important to confirm the diagnosis early and to choose an appropriate treatment strategy according to the stage of the disease and each individual patient's symptoms. When treatment is delayed or an incorrect treatment is administered, patient discomfort increases as the lesion gradually spreads. Therefore, more so than is the case for most other diseases, a team approach on a case-by-case basis is important for the accurate and appropriate treatment of LM.

악성 골 및 연부조직 종양에서 Tl-201 SCAN의 진단적 효능 - Tc-99m-MIBI 및 Tc-99m-MDP scan과의 비교 - (Thallium-201 Scan in Bone and Softtissue Sarcoma - Comparison with Tc-99m-MIBI and Tc-99m-MDP Scan -)

  • PURPOSE : The purpose of this study is to know the ability of detecting malignant tumor tissue by Tl-201 scan, and to compare with that of Tc-99m-MIBI and Tc-99m-MDP scan. MATERIAL AND METHODS : Between February 1994 and December 1995,38 unselected patients with various bone pathologies were studied prospectively. Eighteen had malignant bone and soft tissue pathologies, while twenty had benign. All patients were studied with Tl-201, Tc-99mMIBI and Tc-99m-MDP scan prior to surgical biopsy. PICKER Prism 2000 gamma camera with high resolution parallel hole collimator was used for scanning. To avoid the interaction of isotope, the early(30min.) and delayed phase(3hrs.) of Tl-20l scan was performed first and Tc-99m-MIBI scan was performed after 30 minutes, and then Tc-99m-MDP scan 48 hours later. The scan images were visually evaluated by a blinded nuclear medicine physician. We could find true positive, true negative, false positive and false negative by the comparison of results with those of biopsy. We calculated positive and negative predictive value(%), sensitivity(%), specificity(%) and diagnostic accuracy(%) of each scan. RESULT : The results of each scan were 85.7, 100, 100, 85, 92.1% in Tl-201, 81, 94.1, 94.4, 80, 86.8% in Tc-99m-MIBI and 50, 66.7, 88.9, 20, 52.6% in Tc-99m-MDP scan. As a conclusion, Tl-201 scan was the most specific and accurate method for detecting malignant tumor tissue. Tc-99m-MIBI scan was also good for malignant tumor searching. CONCLUSION : With our results, we can use Tl-201 scan to differentiate benign from malignant tumor, and to evaluate the response of preoperative chemotherapy or radiotherapy, and to determine the residual tumor or local recurrence. For the better result, we need to have a more detail information about false positive cases and a more objective and quantitative reading technique.

객혈의 병소를 확인하기 위한 굴곡성 기관지경 검사의 시행시기 (The Optimal Time of Fiberoptic Bronchoscopy to Locate the Bleeding Site in Patients with Hemoptysis)

  • 연구배경 : 객혈은 호흡기 질환으로 입원하는 환자의 11%를 차지하는 비교적 흔한 호흡기 증상으로 가급적 출혈부위를 확인하여 신속하고 적절한 치료를 시행하여야 위험한 상황을 예방할 수 있다. 출혈부위를 찾기 위하여 기관지경검사를 하는 경우 그 시행시기가 객혈의 원인질환을 밝히거나 치료방침을 결정하는데 영향을 미치지는 못하는 것 같다. 그러나 출혈부위를 확인할 수 있다면 재출혈시 체위를 취하는데 도움이 되고 수술적 폐절제나 동맥색전술을 시행하는데 중요한 정보가 될 것이다. 방법 : 1989년 8월부터 1992년 8월까지 객혈로 입원하였던 환자 63명의 병록을 검토하여 원인질환, 객혈의 양, 객혈 지속기간, 기관지경검사의 시행시기등에 대하여 조사 하였다. 결과 : 1) 객혈의 주된 원인질환은 폐결핵(52.4%), 기관지확장증(27.0%), 폐암(11.1%) 이었다. 2) 41.3%(26/63) 에서 기관지경검사에 의하여 출혈부위가 확인 되었다. 3) 출혈부위의 확인율은 객혈의 양이나 기간과 관계가 없었다. 4) 출혈부위의 확인율은 객혈도중에 검사한 경우 61.8%(21/34), 객혈이 멈춘 후 24시간이내에 검사한 경우 18.2%(4/22), 그 이후에 시행한 경우 14.3%(1/7)이었다. 결론 : 방사선학적, 임상적 소견으로 원인 및 객혈부위의 확인이 어렵고, 객혈부위나 출혈병변을 확인하는 것이 치료방침을 결정하는데 중요할 것으로 판단될 경우 굴곡성 기관지경검사는 가능하면 조기에 특히 지혈되기 전에 시행하는 것이 좋을 것으로 사료된다.

Periodontal and endodontic pathology delays extraction socket healing in a canine model

  • Purpose: The aim of the present exploratory study was to evaluate extraction socket healing at sites with a history of periodontal and endodontic pathology. Methods: The mandibular 4th premolar teeth in 5 adult beagle dogs served as experimental units. Periodontal and endodontic lesions were induced in 1 premolar site in each animal using wire ligatures and pulpal exposure over 3 months (diseased sites). The contralateral premolar sites served as healthy controls. The mandibular 4th premolar teeth were then extracted with minimal trauma, followed by careful wound debridement. The animals were sacrificed at days 1, 7, 30, 60, and 90 post-extraction for analysis, and the healing patterns at the healthy and diseased extraction sites were compared using radiography, scanning electron microscopy, histology, and histometry. Results: During the first 7 days of healing, a significant presence of inflammatory granulation tissue was noted at the diseased sites (day 1), along with a slightly accelerated rate of fibrin clot resolution on day 7. On day 30, the diseased extraction sites showed a greater percentage of persistent fibrous connective tissue, and an absence of bone marrow formation. In contrast, healthy sites showed initial signs of bone marrow formation on day 30, and subsequently a significantly greater proportion of mature bone marrow formation on both days 60 and 90. Radiographs exhibited sclerotic changes adjoining apical endodontic lesions, with scanning electron microscopy showing collapsed Volkmann canals protruding from these regions in the diseased sites. Furthermore, periodontal ligament fibers exhibited a parallel orientation to the alveolar walls of the diseased sites, in contrast to a perpendicular arrangement in the healthy sites. Conclusions: Within the limitations of this study, it appears that a history of periodontal and endodontic pathology may critically affect bone formation and maturation, leading to delayed and compromised extraction socket healing.

$^{99m}Tc-HMPAO$를 이용한 자가백혈구표지 및 그를 이용한 염증병소의 스캔 (Inflammation Scan Using $^{99m}Tc-HMPAO$ Labelled Leukocytes)

  • Inflammation scan using radiolabelled leukocytes has high sensitivity and specificity. Several methods for labelling leukocytes have been evaluated using P-32 diisopropyl fluorophosphate (DFP-32), H-3 thymidine, Cr-51 chromate, Ga-67 citrate and Tc-99m-sulfur colloid. In-111-oxine has proved so far to be the most reliable agent for labelling leukocytes. In-111-oxine is, however, expensive, not easily available when needed, and its radiation dose to leukocytes is relatively high. Moreover, resolution of the resultant image is relatively poor. Tc-99m is still the agent of choice because of, as compared with the indium, its favorable physical characteristics, lower cost and availability. Now the technique for labelling the leukocytes with technetium is successfully obtained using the lipophilic HAPAO with higher efficiency for granulocytes than for other cells. With this technique it is possible to label leukocytes in plasma to improve the viability of the leukocytes. Inflammation scan using Tc-99m-HMPAO has been evaluated in several laboratories, and difference in methods for separation and labelling accounts for difference in efficiency, viability and biodistribution of the labelled leukocytes. We performed inflammation scan using leukocytes labelled with Tc-99m-HMPAO in three dogs 24 hours after inoculation of live E. Coli and A. Aureus in their right abdominal wall. We separated mixed leukocytes by simple sedimentation using 6% hetastarch (HES) and labelled the leukocytes with Tc-99m-HMPAO in 20% cell free plama diluted with phosphate buffer solution(Fig. 1). Uptake was high in the liver and spleen but is was minimal in the lungs on whole body scan. Kidneys and intestine showed minimal activity although it was high in the urinary bladder(Fig. 2). Uptake of labelled leukocytes in the inflammation site was do(mite on 2 hour-postinjection scan and abscess was clearly delineated on 24 hour-delayed scan with high target-to-nontarget ratio(Fig. 3, 4). Inflammation scan using mixed leukocytes labelled with Tc-99m-HMPAO is very sensitive and specific in early detection of inflammation.

국방무기체계 획득영향요인 상호관계 분석 : 함정획득지연 사례 및 요인 분석을 중심으로 (A Methodology for Analyzing on the Correlation of Factors Affecting the Acquisition of Military Weapons Systems : Focused on Factor Analysis of Naval Ship Acquisition Delay Cases)

  • Naval weapons systems of the Republic of Korea are acquired through the Defense Planning Management System. Recently, acquisition of some naval ships have been delayed, and the causes of the delays have been recognized as inappropriate project management at the Execution Phase. However, we argue that the delay problem in naval ships acquisition should be approached, with due regard for the entire Defense Planning Management System. That is, We should try to investigate from Planning Phase to those of Programming, Budgeting and Execution Phases. Therefore, in this study, we investigated the actual cases of the delay in naval acquisition at all phases of the Defense Planning Management System. Based on the investigation, we tried to identify the naval ship Acquisition Delay Factors and find out the Weights of those factors. As the next step, we calculated the Influence Measures on the naval missions, including the Cost of Naval Capability Gap derived from the delays in acquisition of naval ships. As a final step, we calculated the Acquisition Delay Measures based on the interrelationship between the Acquisition Delay Factors and the Influence Measures. Then we evaluated and analyzed what the results stand for. Finally, we made suggestions for future improvement. The improvement suggestions we made for preventing delay in acquisition of naval ships in this study are as follows. First, we need a shift in perception. It is necessary to measure the Acquisition Delay Factors in acquiring naval ships and manage them from the Planning Phase. Second, resolution must be concerted efforts. All relevant agencies, not just a few, should work together to resolve the problems of acquisition delay. Third, analysis must be based on the accumulation of data. This allows the elaborating of naval ship Acquisition Delay Factors and Delay Measures. If this research method is applied to other military weapons systems in the future, we may be able to not just identify the Acquisition Delay Factors in acquisition of other military weapons systems, but also pursue improvement in those cases.

패치 특징 코어세트 기반의 흉부 X-Ray 영상에서의 병변 유무 감지 (Leision Detection in Chest X-ray Images based on Coreset of Patch Feature)

  • 현대에도 일부 소외된 지역에서는 의료 인력의 부족으로 인해 위·중증 환자에 대한 치료가 지연되는 경우가 많다. 의료 데이터에 대한 분석을 자동화하여 의료 서비스의 접근성 문제 및 의료 인력 부족을 해소하고자 하는 연구가 계속되고 있다. 컴퓨터 비전 기반의 진료 자동화는 훈련 목적에 대한 데이터 수집 및 라벨링 작업에서 많은 비용이 요구된다. 이러한 점은 희귀질환이나 시각적으로 뚜렷하게 정의하기 어려운 병리적 특징 및 기전을 구분하는 작업에서 두드러진다. 이상 탐지는 비지도 학습 전략을 채택함으로써 데이터 수집 비용을 크게 절감할 수 있는 방법으로 주목된다. 본 논문에서는 기존의 이상 탐지 기법들을 기반으로, 흉부 X-RAY 영상에 대해 이상 탐지를 수행하는 방법을 다음과 같이 제안한다. (1) 최적 해상도로 샘플링된 의료 영상의 색상 범위를 정규화한다. (2) 무병변 영상으로부터 패치 단위로 구분된 중간 수준 특징 집합을 추출하여 그 중 높은 표현력을 가진 일부 특징 벡터들을 선정한다. (3) 최근접 이웃 탐색 알고리즘을 기반으로 미리 선정된 무병변(정상) 특징 벡터들과의 차이를 측정한다. 본 논문에서는 PA 방식으로 촬영된 흉부 X-RAY 영상들에 대한 제안 시스템의 이상 탐지 성능을 세부 조건에 따라 상세히 측정하여 제시한다. PadChest 데이터세트로부터 추출한 서브세트에 대해 0.705 분류 AUROC를 보임으로써 의료 영상에 대한 이상 탐지 적용의 효과를 입증하였다. 제안 시스템은 의료 기관의 임상 진단 워크플로우를 개선하는 데에 유용하게 사용될 수 있으며, 의료 서비스 접근성이 낮은 지역에서의 조기 진단을 효율적으로 지원할 수 있다.

원형 머리 모델을 이용한 머리 전달 함수의 보간 (HRTF Interpolation Using a Spherical Head Model)

  • 본 논문에서는 머리 전달 함수에 대한 새로운 보간 기법을 제안하였다. 제안된 기법은 각 방위각에 대한 머리 전달 함수의 충격파 응답이 인접 방위각에 대한 시간 지연된 충격파 응답의 선형 보간으로 주어진다고 가정하였다. 각 방위각에 대한 충격파 응답의 시간 지연은 방위각, 머리의 물리적 형태, 음원과 머리의 거리 정보를 이용하여 추정될 수 있는 귀와 음원간의 전파시간과 최소 자승 오차를 갖도록 하는 교정값의 합으로 주어진다. 또한 제안된 모델에서는 보간 시 방위각의 간격을 고정 간격이 아닌 가변 간격으로 하였으며 본래 충격파 응답과 보간된 충격파 응답이 본래의 충격파 응답과 비교하여 청취 상으로 큰 차이가 느껴지지 않고, 보간에 필요한 충격파 응답의 재수가 최소화되는 조건을 만족하도록 결정하였다. 제안된 보간 모델의 유용성을 검증하기 위하여 더미 헤드 및 3명의 사람으로부터 측정된 머리 전달 함수에 대해 제안된 보간 모델을 적용하였다 머리 전달 함수는 0도의 고도각을 갖는 수평면을 5도 간격의 방위각으로 분할한 총 72개가 사용되었으며, 실험 결과 전체 머리 전달 함수 중 단지 $30\sim40%$ 만을 사용하고 나머지는 보간에 의해 얻어진 머리전달 함수를 사용하더라도 청취상의 음원의 위치가 변동되지 않음을 알 수 있었다.

가습기살균제 참사의 진행과 교훈(Q&A) (Questions and Answers about the Humidifier Disinfectant Disaster as of February 2017)

  • 'The worstest environment disaster', 'World's first biocide massacre', 'Home-based Sewol ferry disaster' are all phrases attached to the recent humidifier disinfectant disaster. In the spring of 2011, four of 8 pregnant women including 1 adult man passed away at a university hospital in Seoul due to breathing failure. Epidemiologic investigation conducted by the Korean CDC soon revealed the inhalation of humidifier disinfectant, which had been widely used in Korea during the winter, to be responsible for the disease. As well as lung fibrosis hardening of the lungs, other diseases including asthma, rhinitis, skin disease, liver disease, fetal disease or cancers have been researched for their relation with exposure to the products. By February 9, 2017, 5,342 cases had registered for health problems and 1,131 of them were already dead (20.8% mortality rate). Based on studies by government agencies and a telephone survey of the general population by Seoul National University and civic groups, around 20% of the general public of Korea has used these products. Since the market release of the first product by SK Chemical in 1994, over 7.1 million items from around 20 brands were sold up to 2011. Most of the products were manufactured by well-known large conglomerates such as SK, Lotte, Samsung, Shinsegye, LG, and GS, as well as some European companies including UK-based Reckitt Benckiser and TESCO, the German firm Henkel, the Danish firm KeTox, and an Irish company. Even though this disaster was unveiled in 2011 by the Korean government, the issue of the victims was neglected for over five years. In 2016, an unexpected but intensive investigation by prosecutors found that Reckitt Benckiser manipulated and concealed animal tests for its own brand and brought several university experts and company employees to court. The matter was an intense social issue in Korea from May to June with a surge in media coverage. The prosecutor's investigation and a nationwide boycott campaign organized by victims and environmental groups against Reckitt Benckiser, whose product had been used by more than 70% of victims, led to the producer's official apology and a compensation scheme. A legislative investigation organized after the April 2016 national election revealed the producers' faults and the government's responsibility, but failed to meet expectations. A special law for the victims passed the National Assembly in January 2017 and a punitive system together with a massive environmental epidemiology investigation are expected to be the only solutions for this tragedy. Sciences of medicine, toxicology and environmental health have provided decisive evidence so far, but for the remaining problems the perspectives of social sciences such as sociology and jurisprudence are highly necessary, similar to with the Minamata disease and Wonjin Rayon events. It may not be easy to follow this issue using unfamiliar terminology from medical and chemical science and the long, complicated history of the event. For these reasons the author has attempted to write this article in a question and answer format to render it easier to follow. The 17 questions are: Q1 What is humidifier disinfectant? Q2 What kind of health problems are caused by humidifier disinfectant? Q3 How many victims are there? Q4 What is the analysis of the 1,112 cases of death? Q5 What is the problem with the government's diagnostic criteria and the solution? Q6 Who made what brands? Q7 Has there been a recall? What is still on sale? 단독 태아 경뇌실확장증의 임상적 예후 (The Developmental Outcome of Fetal Mild Isolated Ventriculomegaly)

  • 목 적 : 객관적이고 표준화된 검사(BSID-II)를 통해 단독 태아 경뇌실확장증의 임상적 예후를 알아보고자 하였다. 방 법 : 2001년 6월부터 2002년 5월까지 본원에서 출생한 만삭아중 산전 초음파에서 다른 동반 기형이 없는 10-15 mm 정도의 단독 경뇌실확장증의 소견은 107례에서 관찰할 수 있었으며, 이 중 외래에서 추적 관찰이 가능하였던 40명과, 성별, 연령, 임신주수, 출생체중이 유사한 대조군 34명을 대상으로 발달 검사를 시행하였고 그 결과를 비교 분석하였다. 결 과 : 단독 경뇌실확장증군 40명 중 남아가 21명, 여아가 19명이었고, 12 mm 미만은 35명, 12 mm 이상은 5명이었다. 산전 혹은 출생 후 초음파 추적 관찰시 감소되거나 그대로인 경우가 35명, 진행된 경우가 5명이었고, 일측성 뇌실확장이 25명, 양측성인 경우가 15명이었다. 단독 경뇌실확장군 중 발달지연은 11명(27.5%)이었고 이 중 경미한 발달지연은 10명(25%), 의미있는 심한 발달지연은 1명(2.5%)이었고 대조군 중 경미한 발달지연은 5명(14.7%), 심한 발달지연은 1명(2.9%)이었으며 발달지연을 보인 단독 경뇌실확장군의 91%와 대조군의 80%가 경미한 발달지연이었다. 단독 경뇌실확장군의 발달 검사 평균점수는 대조군과 유의한 차이를 보이지 않았다; MDI($92.75{\pm}12.95$ vs $94.71{\pm}14.14$, P=0.47), PDI($100.33{\pm}14.14$ vs $101.27{\pm}10.70$, P=0.75). 뇌실확장이 소실되거나 진행되지 않는 경우, 뇌실 크기가 12 mm 이하, 그리고 여아인 경우 발달 검사 평균점수는 더 높았으나 통계적으로 유의하지 않았고, 양측성과 일측성의 여부는 발달정도와 관련이 없었다. 결 론 : 산전 초음파 검사에서 단독 경뇌실확장증을 보인 경우 발달 검사에서 정상 대조군과 비교하여 유의한 차이를 보이지 않았으나 단독 경뇌실확장증이 있는 경우 경미한 발달지연의 위험성을 보일 경향이 더 높은 것으로 생각된다.
