추간원판의 퇴행성 변화에 관한 고찰 (A review of degenerative changes in the intervertebral disc)

  • The intervertebral disc is a cartilaginous structure that resembles articular cartilage in its biochemistry, but morphologically it is clearly different. It shows degenerative and ageing changes earlier than does any other connective tissue in the body, It is believed to be important clinically because there is an association of disc degeneration with back pain. Degenerative changes in the intervertebral disc are thought to develop as aging, mechanical stress and metabolic factors. Genetic factors may also play a part in the onset or progress of the degenerative process. They, together with environmental factors, may act as determinants of the structural characteristics of the intervertebral disc and produce a tendency to generation, In this short review we outline the morphplogy and biochemistry of normal intervertebral disc and the changes that arise during degeneration. Therefore this study will review degeneration of intervertebral disc, so we will have knowledge about low back pain associated with degenerative change in the intervertebral disc.

Correlation between pain and degenerative bony changes on cone-beam computed tomography images of temporomandibular joints

  • Background: The aim of this study was to assess correlation between pain and degenerative bony changes on cone-beam computed tomography (CBCT) images of temporomandibular joints (TMJs). Methods: Two hundred eighty-three temporomandibular joints with degenerative bony changes were evaluated. Pain intensity (numeric rating scale, NRS) and pain duration in patients with degenerative joint disease (DJD) were also analyzed. We classified condylar bony changes on CBCT into five types: osteophyte (Osp), erosion (Ero), flattening (Fla), subchondral sclerosis (Scl), and pseudocyst (Pse). Results: Degenerative bony changes were the most frequent in the age groups of 10~19, 20-29, and 50~59 years. The most frequent pain intensity was "none" (NRS 0, 34.6%) followed by "annoying" (NRS 3-5, 29.7%). The most frequent condylar bony change was Fla (219 joints, 77.4%) followed by Ero (169 joints, 59.7%). "Ero + Fla" was the most common combination of the bony changes (12.7%). The frequency of erosion was directly proportional to NRS, but the frequency of osteophyte was inversely proportional. The prevalence of Ero increased from onset until 2 years and gradually decreased thereafter. The prevalence of Osp, Ero, and Pse increased with age. Conclusions: Osp and Ero can be pain-related variables in degenerative joint disease (DJD) patients. "Six months to 2 years" may be a meaningful time point from the active, unstable phase to the stabilized late phase of DJD.

추간판의 퇴행성 변화와 생활기능의 관련성 분석 (Analysis of the Degenerative Disc Change and Its Relationship to Living Function)

  • Purpose: Changes of degenerative disc have been analyzed or with the Magnetic Resonance Imaging (MRI) to assign appropriate meaning, and the relationship between the degenerative changes of the discs and its living function has been evaluated through the living function evaluation scores collected by functional rating index. Methods: in April, 2006, a physical examination was conducted for physical laborers to be employed as on-site laborer in a steel industrial complex located at a region. A month later, these 20 laborers who participated in lumbar vertebra MRI tests have been investigated for one-to-one mobile phone functional rating index test. Excluding 3 of 14 respondents whom scored 0 in functional rating index, the rest of respondents' degenerative disc grade changes shown in MRI and its relationship to living functional rating index have been performed. Results: The Dabbs method of measuring disc height resulted to show significant increase as the disc height changes from L1-2 level to L5-S1 level (p<0.01). However, there was no statistical significance with a relationship between disc height and functional rating index, and disc height average. The Magnetic Resonance Imaging analysis regard on the degree of disc degeneration and its relationship to living functional rating proved to have significant relationship (p<0.05). Conclusion: The degenerative changes monitored by Magnetic Resonance Imaging show significant relationship (p<0.05) to living function. However, this relationship could vary depend upon the characteristics of study population. So, it suggested that the future studies should be performed by considering population's age and job career.

Vacor 유발 당뇨 모래쥐의 심장신경절과 과립함유세포의 미세구조 (Ultrastructural Changes in the Ganglion and Granule-Containing Cells in the Heart of Vacor-Induced Diabetic Mongolian Gerbil)

  • The ultrastructural changes of the cardiac ganglion and granule-containing cells in the heart of vacor-induced diabetic Mongolian gerbils were studied by electron microscopy. After one month of vacor-induced diabetes the ganglion cells showed increase in numbers of dense bodies and mitochondria compared with the normal cardiac ganglion. Most of the satellite cells were filled with numerous phagosomes containing digested debris. Both electron-dense and lucent types of degenerating axon terminals were observed. The former was characterized by clusters of agranular vesicles and numerous mitochondria. The electron lucent type of degenerating axon terminal contained a few agranular vesicles and swollen mitochondria. Degenerating unmyelinated and myelinated axons contained large numbers of dense bodies, lamellar bodies, and mitochondria. Numerous macrophages containing phagosomes were reveled in the interstitial spaces. Some of the granule-containing cells in the heart showed a variety of degenerative changes and a decreased number of dense-cored vesicles. After three months of vacor-induced diabetes the unmyelinated and myelinated axons showed degenerative changes, whereas no structure changes could be demonstrated in intraatrial ganglion and granule containing cells. The satellite cells containing engulfed debris were observed in the cardiac ganglion cells. These results suggest that the degenerative changes occur in the cardiac ganglion cells of vacor-induced diabetic Mongolian gerbils as well as atrial granule-containing cells.

Do partial glenohumeral degenerative changes in patients undergoing arthroscopic rotator cuff repair influence clinical outcomes?

  • Background: The prognostic factors for patients with full-thickness rotator cuff tears (RCTs) include tear size, muscle atrophy and fatty infiltration. However, the influence of early coexisting degenerative changes on RCT outcomes is unappreciated. The purpose of this study was to calculate the impact that pre-existing partial glenohumeral cartilaginous changes have on patients undergoing arthroscopic RCT repair. Methods: A study of 54 patients undergoing arthroscopic RCT repair was undertaken. The presence of co-existing patches of glenohumeral degenerative cartilaginous changes and RCT size was recorded at surgery. Pre- and postoperative outcomes were assessed using traditional (Oxford Shoulder Score [OSS], 5-level EuroQol-5D [EQ-5D-5L] questionnaire and EuroQol visual analog scale [EQ-VAS]) and patient-centric re-formatted prisms. Outcomes were assessed as an entire dataset, and sub-group analysis was performed according to the grade of co-existing arthritis and tear size. Results: Significant improvements (p<0.05) in clinical outcomes were recognized when assessed using either the traditional or reformatted prisms (average % improvements in OSS, EQ-5D-5L and EQ-VAS were 47%, 33% and 43%, respectively; average improvements in pain, function, and psychological well-being were 48%, 33% and, 29%, respectively). Positive gain was noted in all sub-groups of arthritic grading and tear size. Conclusions: Good clinical outcomes can be achieved following RCT repair even in the presence of local partial degenerative cartilage changes and advancing tear size. These benefits are patient-centered but require RCT repairability.

  • Epigallocatechin gallate (EGCG) was reported to exert weak cytotoxicity against normal healthy cells such as C3H10T1/2 cells, but profound inhibitory effects on the initiation or promotion stage of chemical carcinogenesis in mammary gland, blood and mouse skin. This study was carried out to develop antitumor agents with weak side effects and strong antitumor activity. Human skin melanoma cells (HBT 69) and human oral epitheloid carcinoma cells (OCL 17) were cultured in RPMI-1640 media containing 10% fetal bovine serum, antibiotic, and fungizone. After incubation for 24 hrs, the cells were treated with various amounts of (EGCG) for 48 hrs. The growth inhibitory effects of EGCG in human oral epitheloid carcinoma cells were evaluated by the 3- (4,5-djmethylthiazol-2-yl)-2,5-diphenyl-2H-tetrazolium bromide (MTT), neutral red (NR), and sulforhodamine B protein (SRB) assays of colorimetric methods. The light microscopic study was also carried out to observe morphological changes of the treated cells. These results obtained were as follows; 1. Significantly inhibitory effects of EGCG against cultured human oral epithelioid carcinoma cells. 2. Significantly inhibitory effects against cultured human skin melanoma cells treated with 50 $\mu$M EGCG, but decreased inhibitory effects in 100 $\mu$M EGCG. 3. Degenerative changes against cultured human oral epitheloid carcinoma cells. 4. Degenerative changes against human skin melanoma cells treated with 50 UM EGCG, but recovered degenerative changes in 100 $\mu$M EGCG.

임상가를 위한 특집 3 - 치과진료에서 악관절 골관절염의 중요성 (Clinical importance of TMJ Osteoarthritis in Dental Practice)

  • Osteoarthritis is one of the most common degenerative disease in the temporomandibular joints(TMJ). Structural changes in the osseous structure is associated with destructive changes such as erosion, flattening and other bony changes. Destructive degenerative changes quite often cause shortening of the condyles which eventually produces the changes in the occulsion and the facial profile which require orthorgnathic surgery and/or orthodontic treatment by the dental professional. The dentists need to understand the nature and the pathophysiology of the osteoarthritis in the TMJ for the better dental treatment, especially in orthodontic and prosthodontic treatment. The possibility of serious complication can not be avoided after any dental treatment is given to the patient if osteoarthritic changes in the TMJ is under progression as undiscovered.

미세전류치료가 퇴행성 무릎관절염 환자의 통증과 균형에 미치는 영향 (Micro-current Treatment Effects on Pain, Balance of the Degenerative Knee Arthritis)

  • Purpose: The purpose of this research was to ascertain the effect of Micro-current on a pain, balance, of knee joint in the patients with degenerative arthritis. Method: The 30 subjects who had micro-current in this research and randomly assigned into two group, an experiment group and a control group. micro-current was applied to 15 subject in the experiment group after general physical therapy. experiment group was applies general therapies and micro-current stimulation. The measurement were analyzed by using SPSS(V.20). In order to compare the post to the amount of pre-changes and post-changes of each group, the paired t-test was used. The difference between the experiment group and the control group was analyzed by using the analysis of convariance. Results: The results of this study were as follows; 1) In VAS measures, the pain point was significantly decreased in both the experimental and the control group. 2) Mc Gill Pain Questionnaire(MPQ), the pain point was significantly decreased in both the experimental and the control group. 3) TUG measures, the second was significantly decreased in both the experimental and the control group. 4) Weight distribution measures, the authority was significantly decreased in both the experimental and the control group. Conclusion: The micro-current is considered a degenerative knee arthritis that can be presented as an effective physical therapy intervention.

퇴행성 추간판 질환의 최신 지견 -종판의 중요성- (Current Concepts of Degenerative Disc Disease -A Significance of Endplate-)

  • 퇴행성 추간판 질환은 전통적으로 수핵과 섬유륜의 변화에 따른 요통으로 생각되어 왔으나, 최근 연구들에서 추간판을 이루고 있는 상·하 종판의 변화가 추간판의 퇴행을 유발하고, 그 결과로 기계적 압박과 염증 반응에 의해 요통이 유발된다고 한다. 또한 최근에는 추체 골수-종판-수핵과 섬유륜을 하나의 단위로 생각하고, 그 연관성을 설명하고 있다. 종판이 손상되면 결국 추체, 수핵과 섬유륜의 퇴행성 변화를 가중시킨다. 이 과정에서 섬유륜에 가해지는 압박력이 증가하고, 염증성 매개 물질들에 의한 염증 반응이 일어나서, 동척추 신경(sinuvertebral nerve)과 기저추 신경(basivertebral nerve)이 자극을 받으면서 요통을 발생시킨다. 이런 변화가 만성화되면, 추체 내 골수에도 Modic 변화 같은 퇴행성 변화가 발생하게 된다. 결국, 퇴행성 추간판 질환은 추체 골수-종판-수핵과 섬유륜을 하나의 단위로 연관지어서 생각해야 할 필요가 있다. 그러므로 만성 요통 환자 진료 시에는 수핵과 섬유륜의 변화 소견뿐 아니라, 종판의 병변에 관심을 가질 필요가 있다고 생각된다.

Degenerative joint disease with systemic mycoses in a oriental white stork

  • Rehabilitation Research was presented to Veterinary Medical Center of Chungbuk National Universitywith anorexia and lameness for 5 days. Bilateral intertarsal joint swellings were observed in physicalexamination. The radiographic findings indicated degenerative changes of joint cartilage and suroundingbones. In cytologic examination of synovial fluids, mononuclear leukocytic inflammation was identified.on Sabroud dextrose agar. From all of examinations, this patient was diagnosed as a degenerative jointdisease with systemic mycoses.
