  • We report three cases of children who underwent definitive conversion to the Fontan circulation using a new surgical technique, Extracardiac Epicardial Lateral Tunnel. This new procedure allows the operation to be performed as a totally extracardiac operation [especially in ventricular dysfunction] and allows it to be performed in a very small atrium and in cases with unsuitable pulmonary venous drainage.Our data suggest that this procedure may achieve satisfactory hemodynamics of the total cavopulmonary connection.

  • When the perforation of intrathoracic esophagus occurs in the presence of preexisting esophageal stricture, aggressive and definitive therapy often provides the only chance for patient salvage. Two adults suffering from intrathoracic esophageal perforation with underlying stricture underwent transhiatal esophagectomy. The perforations were due to esophageal instrumentation. Restoration of alimentary continuity with a primary cervical pharyngogastric anastomosis was carried out in one patient. Another patient underwent a cervical esophagostomy and had a subsequent colonic interposition 3 months later.

Two Cases of Embryonal Rhabdomyosarcoma Originating from Chest Wall (흉벽에 발생한 배아성 횡문근육종 2례 보고)

  • Rhabdomyosarcoma is relatively rare in general pediatric population. Furthermore, the primary site in the thorax is one of the least sites. Because most patient, when first seen, are extensively advanced and bad in prognosis, an adequate work-up prior to any definitive therapy must be undertaken. Author experienced 2 cases of embryonal rhabdomyosarcoma originating from chest wall. Both 2 cases are included in Group III [Rhabdomyosarcoma-Intergroup Protocol Grouping], one case is treated with radical surgery and radiation therapy, and the other case is treated with radiation therapy and chemotherapy. So author present 2 cases of embryonal rhabdomyosarcoma with reference.

  • Regardless of the type of impression being made, the tray is the most important part of the impression-making procedure for completely edentulous patients. Dentists have to make use of a combination of rigid, thermoplastic, and resilient materials and control step-by-step procedure from irreversible hydrocolloid impression to definitive cast fabrication. For successful edentulous impression, some considerable clinical tips were guided.

  • Subcutaneous emphysema of the head and neck are well-known sequealae of surgery and trauma to the chest airway. This occurs when air is introduced into the tissue. This can happen as a complication during, or immediately after surgery. Among the causes subcutaneous emphysema due to complication of tonsillectomy, has rarely been described. Definitive treatment will depend on the cause. Recently. we have experienced two cases of subcutaneous emphysema without respiratory and circulatory insufficiency as a complication of tonsillectomy in 38-year-old male and uvulopalatopharyngoplasty in 41- year-old male.

  • This report is an analysis of 183 cases of mediastinal tumors which were experienced in the Department of Thoracic and Cardiovascular Surgery, Yonsei Medical Center from January 1960 to June 1985. In this series, teratoma and neurogenic tumors were found to be the most frequent tumors [24.0%] histopathologically. Male to female sex distribution was 1.2 to 1 with the male predominant. The main clinical symptom was dyspnea, and there was no definitive symptom in 10.9% of all cases. In operating, all of the benign tumors were removed. The most frequent complication was wound infection. [13.7%].

  • Gastric volvulus in neonates is an extremely uncommon disorder, which is challenging to diagnose because of its non-specific clinical manifestations. Early diagnosis of gastric volvulus is important to avoid life-threatening complications, such as gastric ischemia, necrosis, and perforation. A definitive diagnosis could be made with radiological upper gastrointestinal series. In this report, we present two cases of neonate gastric volvulus, which were confirmed by radiological upper gastrointestinal series, and the patients underwent surgical treatment.

  • Primary extranodal bone lymphoma involving the peripheral extremities is extremely rare. Here, we report a definitive case of diffuse large B-cell lymphoma involving the phalangeal bone of the 3rd finger. Systemic evaluation revealed the lesion as the only site of lymphoma involvement.

  • A fistula between the right pulmonary artery and the left atrium is a very rare congenital anomaly, for which there is no definitive embryogenetic explanation. Patients present with cyanosis or clubbing, and the treatment strategy is to close the fistula, which can be done by an open surgical technique or by percutaneous intervention. Although rare, this condition should be considered in the differential diagnosis when evaluating a patient with central cyanosis.
