외상성 상악동 안구탈출의 치험례: 증례보고 (Traumatic Displacement of the Globe into the Maxillary Sinus: Case Report)

  • Purpose: Globe displacement due to a blowout fracture is a rare clinical phenomenon. The authors present reduction of a globe displacement to the maxillary sinus due to trauma suffered in a fall and the reconstruction of a large defect left in the medial and inferior orbit. Methods: A 39-year-old male patient was unable to open his left eye after being struck on the periorbital area by a metal edge. Laceration was not noted in that area but we were unable to observe the intraorbital globe. A facial computed tomography (CT) scan showed that the globe was displaced through the maxillary sinus. A transconjunctival approach was used to access the infraorbital margin and the globe entrapped in the inferior margin of the orbit was successfully reduced. A large defect in the medial and inferior orbit was reconstructed using a graft from the iliac bone. Results: In 5 months after the operation, no atrophy of the globe was seen. Both sides retained a similar shape. A satisfactory functionality outcome in terms of improved extraocular muscle movement, and a satisfactory aesthetical outcome were achieved. Conclusion: The authors report the reduction of a globe displaced to the maxillary sinus following a fall and the reconstruction of the large defect left in the medial and inferior orbit.

소규모 조직을 위한 오픈 소스 소프트웨어 기반의 결함 관리 프로세스 (A Defect Management Process based on Open Source Software for Small Organizations)

  • 고품질의 소프트웨어를 만들기 위해서는 발생하는 결함을 발견하고 해결해야 한다. 결함 관리 작업의 소홀함은 후에 재작업을 요구하게 되고, 결국 납기 지연 등 프로젝트 실패로 이어질 수 있다. 그러므로 조직들은 결함 관리 프로세스를 정립하고 내재화해야 한다. 관련 표준 모델들은 결함 관리 프로세스를 프로젝트 모니터링 및 제어 영역에서 다루고 있다. 하지만, 소규모 조직에서는 표준 문서 기반의 프로세스 구현에 어려움을 느끼고, 적용하지 못하고 있는 것이 현실이다. 본 논문에서는 소규모 프로젝트 참여 인원과 기간에 적합한 결함 관리 프로세스와 체계적인 지원을 위한 Redmine과 Subversion, Maven, Jenkins 등의 오픈 소스 소프트웨어 기반의 SW Visualization 구축 방안을 제시한다. 또한, 결함 데이터 베이스 구축과 이를 기반으로 결함 데이터 분석 및 제어 활동을 위한 방안을 제시한다. 제안한 프로세스와 Tool Chain을 중소기업에 적용하여 그 효율성을 입증하였다.

  • 결함을 초기 단계에 발견하여 제거함으로써 품질이 좋은 소프트웨어를 만들 수 있다 이를 위해 결함을 효과적으로 추적하고 관리하는 결함 추적 시스템(Defect Trucking System)의 도입이 필요하다. 기존의 결함 추적 시스템은 한꺼번에 많은 종류의 데이터를 모음으로써 이를 도입한 조직에서 수많은 데이터를 왜 모아야 하는지에 대해 인식이 부족하고 결함 관리프로세스 없이 도구만 적용함으로써 결함 추적 시스템 도입에 어려움을 겪고 있다. 또한 CMMI 성숙도 수준 2, 3에서는 결함 관리 활동을 수행하는 프로세스 영역이 없기 때문에 CMMI 성숙도 수준 2, 3에 해당하는 조직이 결함 추적 시스템을 도입하는데 문제가 되고 있다. 이에 본 논문에서는 CMMI 기반의 조직이 기존의 결함 추적 시스템을 도입하였을 때 생기는 문제점을 해결하고 CMMI 각 성숙도마다 어떤 결함 데이터를 모아야 하는지, 어떻게 결함 관리를 해야 하는지에 대한 가이드라인을 제공하고자 한다. 그리고 검토 및 테스팅을 통해 도출된 결함 데이터에 대하여 각 담당자 별로 업무를 할당, 추적할 수 있게 하며, 결함에 관한 각종 상태정보와 통계 정보 등을 제공하는 결함 분석 및 통제 시스템을 개발하고자 한다.

하지에 발생한 연부 조직 종양의 광범위 절제술 후 재건술에서 전외측 대퇴부 유리 피판술의 유용성 (The Signification of Anterolateral Thigh Free Flap for Reconstruction of Soft Tissue Defect in Malignant Soft Tissue Tumor of Lower Extremity)

  • Purpose: The purpose of this study was to evaluate the clinical results of anterolateral thigh free flap on soft tissue defect following wide excision in malignant soft tissue tumor of lower extremities. Methods: Between February 2005 to April 2010, we followed up 19 cases who were undergoing anterolateral thigh free flap because of soft tissue defect following wide excision of malignant soft tissue tumor in lower extremity, including 9 cases were heel, 5 cases in foot, 3 cases in ankle, 2 cases in knee and leg. We observed that of implanted area's color, peripheral circulation at 3, 5, 7 days after operation and evaluated operating time, amount of hemorrhage, implanted skin necrosis, additional operations, complications. And we also evaluated the oncologic results, including local recurrence, metastasis, and morbidity. Results: Average operation time of wide excision and anterolateral thigh free flap was 7 hour 28 minutes. 18(94.7%) of total 19 cases showed successful engraftment, on the other hand, failure of engraftment due to complete necrosis of flap in 1 case. In 18 cases with successful engraftment, reoperation was performed in 4 cases. Among them, removal of hematoma and engraftment of flap after bleeding control was performed in 3 cases, because of insufficient circulation due to the hematoma. In the remaining 1 case, graft necrosis due to flap infection was checked, and grafted after combination of wound debridement and conservative treatment such as antibiotics therapy, also skin graft was performed at debrided skin defect area. Skin color change was mainly due to congestion with hematoma, flap was not observed global congestion or necrosis except 4 cases which shows partial necrosis on margin that treated with conservative therapy. Conclusions: Anterolateral thigh free flap could be recommended for reconstruction of soft tissue defect following wide excision of malignant soft tissue tumor in lower extremity.

악골 낭종의 적출술 후 골재생에 대한 파노라마 촬영과 컴퓨터 단층촬영의 비교 분석 (Spontaneous bone regeneration after enucleation of jaw cysts: a comparative study of panoramic radiography and computed tomography)

  • Introduction: A cyst is a closed pathologic sac containing fluid or semi-solid material in central region. The most common conventional treatment for a cyst is enucleation. It was reported that spontaneous bone healing could be accomplished without bone grafting. We are trying to evaluate bone reconstruction ability by analyzing panorama radiograph and computed tomography (CT) scan with retrograde studying after cyst enucleation. In this way we are estimating critical size defect for spontaneous healing without bone graft. Materials and Methods: The study comprised of 45 patients who were diagnosed as cysts and implemented enucleation treatment without bone graft. After radiograph photo taking ante and post surgery for 6, 12, 18, 24 months, the healing surface and volumetric changes were calculated. Results: 1. Spontaneous bone healing was accomplished clinically satisfying 12 months later after surgery. But analyzing CT scan, defect volume changes indicate 79.24% which imply incomplete bone healing of defect area. 2. Comparing volume changes of defect area of CT scan, there are statistical significance between under $5,000mm^{3}$ and over $5,000mm^{3}$. The defect volume of $5,000mm^{3}$ shows $2.79{\times}1.91$cm in panoramic view. Conclusion: Bone defects, which are determined by a healed section using a panoramic view, compared to CT scans which do not show up. Also we can estimate the critical size of defects for complete healing.

한국인 전상환자의 악안면결손에 대한 임상적연구 (THE CLINICAL STUDY OF THE MAXILLOFACIAL WAR INJURIES IN KOREAN)

  • Maxillofacial war injures is specific representative of severe hard and soft tissue defect. This type of injuries were different from the trauma because it may be fatal. The purpose of this study was to evaluate the injury type base on the retrograde medical record in the 104 patients from Feb. 1991 to Aug. 1992 in Korea Veterans Hospital. The obtained results were as follows. 1. Among 104 cases, 51 cases(49.0%) were classified as mandibular defects only, and 53 cases (51.0%) were classified as maxillary with mandibular defect (combined). 2. The etiologic factors of injury were gunshot, artillery and grenade or shell: 33 cases, 14cases, and 10 cases respectively in Korea War, 19 cases, 5 cases, and 8 cases respectively in Vietnam War. 3. In 57 cases of mandibular partial defect, the angle area showed the highest frequencies, 18 cases(31.6%). 4. In 42 cases of mandibular segmental defect, the area between the 1st molar and the ascending ramus showed the highest frequencies. 17 cases(40.4%), and almost all cases were unilateral defect (40 cases, 95.0%) 5. Reconstruction method performed for segmental mandibular defect were wire or plate(15 cases, 35.7%) and soft tissue closure only(12 cases, 28.7%), respectively.

  • TFT-LCD 자동 검사 시스템에서 결함 검출을 위한 영상은 라인 스캔 카메라(line scan camera)나 에어리어 스캔 카메라 (area scan camera)에 의해서 획득하게 된다. 그러나 임펄스 잡음과 가우시안 잡음, CCD 혹은 CMOS 센서의 한계, 조명등의 영향으로 열화된 영상이 획득되며, 한도성 결함 영역을 인간의 육안으로 구분하기 어렵게 된다. 본 논문에서는 효율적인 결함 검출을 위해 특징 추출 방법과 결함 검출 방법을 제안한다. 특징 벡터로 웨버의 법칙을 이용한 결함 영역과 주변 배경 영역의 평균 밝기 차와 주변 배경 영역의 밝기 변화를 이용한 표준편차를 이용하며, 결함 영역 검출를 위해 추출된 특징 벡터를 이용하여 비선형 SVM을 적용한다. 실험 결과는 제안한 방법이 다른 방법들 보다 성능이 우수함을 보여준다.

유도초음파를 활용한 격납건물 라이너 플레이트 상시감시 모니터링 검사를 위한 토모그래피 영상화 (Tomographic Imaging for Structural Health Monitoring Inspection of Containment Liner Plates using Guided Ultrasonic)

  • Large-scale industrial facility structures continue to deteriorate due to the effects of operating and environmental conditions. The problems of these industrial facilities are potentially causing economic losses, environmental pollution, casualties, and national losses. Accordingly, in order to prevent disaster accidents of large structures in advance, the necessity of diagnosing structures using non-destructive inspection techniques is being highlighted. The defect occurrence, location and defect type of the structure are important parameters for predicting the remaining life of the structure, so continuous defect observation is very important. Recently, many researchers have been actively researching real-time monitoring technology to solve these problems. Structure Health Monitoring Inspection is a technology that can identify and respond to the occurrence of defects in real time, but there is a limit to check the degree of defects and the direction of growth of defects. In order to compensate for the shortcomings of these technologies, the importance of defect imaging techniques is emerging, and in order to find defects in large structures, a method of inspecting a wide range using guided ultrasonic is effective. The work presented here introduces a calculation for the shape factor for evaluation of the damaged area, as well as a variable β parameter technique to correct a damaged shape. Also, we perform research in modeling simulation and an experiment for comparison with a suggested inspection method and verify its validity. The curved structure image obtained by the advanced RAPID algorithm showed a good match between the defect area and the shape.

STI--CMP 공정에서 Torn oxide 결함 해결에 관한 연구 (A Study for the Improvement of Torn Oxide Defects in Shallow Trench Isolation-Chemical Mechanical Polishing (STI-CMP) Process)

  • STI(shallow trench isolation)-CMP(chemical mechanical polishing) process have been substituted for LOCOS(local oxidation of silicon) process to obtain global planarization in the below sub-0.5㎛ technology. However TI-CMP process, especially TI-CMP with RIE(reactive ion etching) etch back process, has some kinds of defect like nitride residue, torn oxide defect, etc. In this paper, we studied how to reduced torn oxide defects after STI-CMP with RIE etch back processed. Although torn oxide defects which can occur on trench area is not deep and not severe, torn oxide defects on moat area is not deep and not severe, torn oxide defects on moat area is sometimes very deep and makes the yield loss. Thus, we did test on pattern wafers which go through trench process, APECVD process, and RIE etch back process by using an IPEC 472 polisher, IC1000/SUVA4 PAD and KOH base slurry to reduce the number of torn defects and to study what is the origin of torn oxide defects.

  • 본 논문에서는 항공기용 터보 축 엔진의 결함진단 알고리즘으로 지지 벡터 장치(Support Vector Machine) 과 인공신경망(Artificial Neural Network) 을 복합으로 이용하였다. 인공신경망 알고리즘의 특성상 데이터 수에 따라 정확성과 수렴속도 등에서 차이가 나므로 탈설계 영역에서의 효용성여부를 판단하기 위해서 연료유량과 마하수에 따른 탈설계 영역 진단 결과를 지상정지 상태와 비교하였다.
