Complete Transposition of the Great Arteries with Atrial Septal Defect -One Case Report- (대혈관 전위증 1예 보고)

  • The most common abnormal relationship between the great arteries and ventricle is transposition of great arteries, among which complete D-transposition is a common and lethal malformation. Without appropriate treatment, the infant born with transposition of the great arteries rarely survives the first year of life. In contrast to the grim hopelessness of only a few years ago, modern aggressive palliative and corrective surgical interventions can provide such infants with considerable hope for adolescent and adult life. Up until this time, intraatrial baffle technique for re-direction of venous return, which was proposed by Mustard originally, has been successfully applied to infants and children in many foreign clinics with decreasing trend of the operative risk. In this report, we present one case of a 4 year-old girl having complete D-transposition of the great arteries with atrial septal defect, and reviewed the relevant literatures.

  • A defect on the product safety display is defined by a failure to prevent the damage caused by the inappropriate explanation, direction, and caution to warn the users of the related risks. In other words, it lacks the sufficient instructions or caution for proper use and is unable to furnish the possible danger of the product and the instructions to deal with it. The defect is the responsibility of the manufacturers who are supposed to provide the adequate information so as to prevent injuries of the users. However, in order to prove the product safety display defective, there should be a comprehensive consideration in many different ways such as social norms (such as property and expectation of the product), usage form, indication of the expected risk, and awareness of consumer about the harm and possibility of avoidance of danger. The purpose of this research is to propose the content and types of standardized indication of safety in the industrial product by collecting and analyzing domestic safety information.

  • The scalp is the thickest skin in the body and protects the intracranial structures. The coverage of a large scalp defect is a difficult surgical procedure, the full details of which must be considered prior to the procedure, such as defect size and depth, and various factors related to the patient's general condition. Although a free flap is the recommended surgical procedure to cover large scalp defects, it is a high-risk operation that is not appropriate for all patients. As such, other surgical options must be explored. We present the case of a patient with an ulcer on the scalp after wide excision and split-thickness skin graft for squamous cell cancer. We successfully performed a reverse temporalis muscle flap for this patient.

Open Techniques for Bone Defect in Anterior Shoulder Instability (골 결손이 동반된 전방 견관절 불안정성에서 개방적 수술 술기)

  • Purpose: An osseous defect in the glenoid and humeral head is closely associated with recurrence of anterior shoulder instability. The purpose of this article is to describe the open surgical techniques and introduce our experiences with anterior instability with a significant osseous defect. Materials and Methods: We reviewed the articles that have focused on and/or mentioned the affect of osseous defects on anterior shoulder instability. The open surgical techniques and its related pearls are summarized in this review. Results: Accurate evaluation for the size and location of the osseous defect is critical for preventing recurrence after restoration of the anterior capsulolabral structure. The glenoid bone restoration techniques include the coracoids transfer (the Bristow procedure and the Latarjet procedure) and a structural iliac bone graft. Rotational humeral osteotomy and an osteoarticular allograft could be used for repairing a significant posterosuperior humeral defect (Hill-Sachs lesion). Shoulder arthroplasty may be tried for treating a humeral bone defect, but more study on this is needed. Conclusion: Open surgical restoration decreases the risk of recurrence anterior shoulder instability that is combined with a significant osseous defect. Arthroscopic surgery currently has limitations for treating an osseous defect, but it will become useful in proportion to the development of arthroscopic instruments and techniques in the future.

Development of real-time defect detection technology for water distribution and sewerage networks (시나리오 기반 상·하수도 관로의 실시간 결함검출 기술 개발)

  • The water and sewage system is an infrastructure that provides safe and clean water to people. In particular, since the water and sewage pipelines are buried underground, it is very difficult to detect system defects. For this reason, the diagnosis of pipelines is limited to post-defect detection, such as system diagnosis based on the images taken after taking pictures and videos with cameras and drones inside the pipelines. Therefore, real-time detection technology of pipelines is required. Recently, pipeline diagnosis technology using advanced equipment and artificial intelligence techniques is being developed, but AI-based defect detection technology requires a variety of learning data because the types and numbers of defect data affect the detection performance. Therefore, in this study, various defect scenarios are implemented using 3D printing model to improve the detection performance when detecting defects in pipelines. Afterwards, the collected images are performed to pre-processing such as classification according to the degree of risk and labeling of objects, and real-time defect detection is performed. The proposed technique can provide real-time feedback in the pipeline defect detection process, and it would be minimizing the possibility of missing diagnoses and improve the existing water and sewerage pipe diagnosis processing capability.

A Study on Reliability Analysis and Quantitative Risk Analysis for Liquefied Petroleum Gas Station (LPG 충전시설에 대한 신뢰도 분석과 정량적 위험성 분석에 관한 연구)

  • For a Liquified Petroleum Gas(LPG) station, the reliability analysis, such as Fussell-Vesely importance, risk decrease factor and risk increase factor, was carried out and the risk ranks of events were determined. In order to confirm the degree of the risks identified in the reliability analysis, the quantitative risk analysis was done for the equipments which had the large values of risk ranks. As a result of the importance analysis for the LPG station, the external event was identified as the most riskful event. The defect of construction structure and the pipe corrosion were riskful as well. The result of quantitative risk analysis showed that the length of 46.3 meters were estimated to damage the process equipments by the thermal flux from the catastrophic rupture of storage tank in Boiling Liquid Expanding Vapor Explosion.

Aortic Translocation for Complete Transposition of the Great Arteries with a Ventricular Septal Defect and Pulmonic Stenosis (심실 중격 결손과 폐동맥 협착을 동반한 완전 대혈관 전위에서 대동맥 전위술)

  • The Rastelli operation has been a standard procedure for repairing complete transposition of the great arteries combined with a ventricular septal defect and pulmonary stenosis. Yet this procedure has several shortcomings, including the risk of incurring left ventricular outflow tract obstruction on long-term follow-up. In this regard, aortic translocation has recently been regarded as a potent alternative to Rastelli's operation. We report here on a case of complete transposition of the great arteries that was combined with an inlet-extended perimembranous ventricular septal defect and pulmonary stenosis in a 2-year-old boy. All the problems were successfully repaired using the aortic translocation technique. Postoperative echocardiography showed a straight and wide left ventricular outflow tract.

  • From 1982 to 1987, six patients underwent left ventricular aneurysmectomy with concomitant myocardial revascularization. Simultaneous repair of postinfarction ventricular septal defect was performed in 3 patients and left ventricular thrombectomy in two. There was no hospital mortality and late mortality during 220 months* follow up period-.[Mean 36.7 months, range 13 to 72 months] Their condition was improved in all. They live in physical condition of NYHA class I-II. We believe early surgical intervention can be life-saving, and can be done without undue surgical risk, even in cases of postinfarction VSD and LV aneurysm with failing heart.

  • Recently, we experienced one case of multiple ribs fracture with large chest wall defects. This patient was treated with internal fixation of ribs by use of Judet`s struts and reconstruction of chest wall defects by use of Teflon mesh. Postoperative outcome was satisfactory result and its advantages were reduced duration of operation, prevention of pulmonary herniation and reduced risk of postoperative infection.
