주성분분석을 이용한 C[11]-PIB imaging 영상분석 (Principal component analysis in C[11]-PIB imaging)

  • 주성분분석(Principal component analysis, PCA)은 고차원 변수들 사이의 복잡한 상관성 구조를 더 낮은 차원으로 단순화하여 상관성의 구조를 쉽게 설명하기 위한 다변량분석기법으로 뇌영상 분석에서 자주 사용되는 방법이다. 주성분분석의 기본개념은 서로 직교하는 변수들의 선형결합을 통해서 원래의 뇌영상 자료가 가지고 있는 전체정보를 최대한 설명할 수 있는 서로 독립적인 새로운 변수들을 유도하는 것이다. 뇌영상분석에서 주성분분석의 효율성과 유용성을 알아보기 위해서 C[11]-PIB 영상을 이용하여 분석하였다. 대상 및 방법으로는 평균나이가 같은 9명의 정상인, 10명의 알츠하이머/경도인지장애환자들의 C[11]-PIB 영상을 이용하였다. PET-CT 장비로는 Biograph 6 Hi-Rez (Siemens-CTI, Knoxville, TN)를 영상을 획득하였고 9.6 MBq/kg C[11]-PIB를 정맥주사 한 후 40분 후에 20분 동안 3D acquisition mode로 방출영상을 얻었다. Attenuation map은 X-ray CT scan을 이용하여 재구성하였다(130 kVp, 240 mA). PIB template을 만들기 위해서 정상인에서 3T MRI T1-weighted 영상을 동시에 얻었다. 주성분분석을 위한전처리과정으로서 공간정규화 및 공간편평화를 SPM8을 이용하여 실시하였고 주성분분석은 Matlab2012b를 이용하여 분석하다. 결과는 주성분분석을 통해서 서로 독립적인 주성분영상들을 얻을 수 있었다. 주성분분석을 통해서 얻어진주성분영상은 C[11]-PIB brain PET 영상의 패턴을 몇 개의 주성분으로 단순화 할 수 있었으며 주로는 neocortex를 변동 나타내는 영상, white matter의 변동을 나타내는 영상 그리고 pons등 deep brain의 변동을 나타내는 영상 등으로 단순화되었다. 결론으로는 주성분분석은 C[11]-PIB brain 영상을 단순화하여 영상의 패턴을 해석하는데 매우 유용하였다. 이러한 주성분분석은C[11]-PIB영상 분석뿐만 아니라 뇌의 포도당 대사를 측정하는 FDG-PET 또는 뇌기능영상등의 다변량분석 방법으로서 그 적용범위가 클 것으로 기대된다.

  • Subcortical dementia may occur as a complication of cerebral malaria via deep white matter demyelination with encephalitis and diffuse small vessel vasculitis. In subcortical dementia, impairment in attention and frontal executive function are predominant. Patients are often inert, indifferent, and uninterested. They appear characteristically 'slowed up' with a marked deficit in the retrieval of information. Changes in mood, personality, and social conduct are very common. We describe a case of subcortical dementia, who has definite changes in brain MRI after cerebral malaria.

소아에서 발생한 연필에 의한 관통 뇌손상 (Penetrating Orbitocranial Injury of a Pencil in a Pediatric Patient)

  • Pencils are common instruments for children to use and play with. This report describes an unusual penetrating orbitocranial injury in a 5-year-old girl who was struck in her facewith a pencil. She was holding it at a desk, and her friend pushed her back. The pencil penetrated the left lower eyelid and went deep into the right frontal lobe through the base of the skull. It was removed at the emergency room, after which brain CT was performed to detect the development of an intracranial hematoma. No complications occurred after conservative management with antibiotics and an antiepileptic drug. Pencils can be hazardous to children, and a penetrating head injury with a pencil may be managed without cranial surgery.

Effect of Subchronic 3-Monochloro-1,2-propanediol Exposure on the Expression of Inducible Nitric Oxide Synthase in Rat Brain

  • 3-Monochloro-1,2-propanediol (3-MCPD) is a contaminant of acid-hydrolyzed vegetable protein. Several reports have suggested that chronic exposure to 3-MCPD could produce neurotoxicity in vitro or neurobehavioral effects inaspects of experimental animals. Disturbance of the nitric oxide signaling pathway by chronic exposure to 3-MCPD may be a causal factor of neurological disorders in rats. In order to investigate the relationship between 3-MCPD administration and expression of inducibal nitric oxide synthase (iNOS), the numbers and distribution patterns of iNOS-immunoreactive neurons were examined. At the all three bregma level examined, the optical density of iNOS-postive neurons was significantly increased following exposure to 3-MCPD. The change was more severe in the upper layer than in deep layer of the cortex. These data suggest that 3-MCPD toxicity may be mediated through disturbances to the nitric oxide signaling pathway.

Isolated Sphenoid Sinus Mucocele Presenting as Third Nerve Palsy

  • A sphenoid mucocele often presents late due to its deep seated anatomical site. And it has varied presentation due to its loose relationship to the cavernous sinus and the base of the skull. We describe a case of large sphenoid sinus mucocele. A middle aged old man suddenly developed third cranial nerve palsy. Brain imaging study revealed an isolated sphenoid sinus mucocele, compressing right cavernous sinus. Endoscopic marsupialization of the mucocele via transnasal approach led to complete resolution of the third cranial nerve palsy. Involvement of the third cranial nerve in isolated mucocele is rare but important neurosurgical implications which must be excluded. In addition, proper and timely treatment must be performed to avoid permanent neurologic deficit.

분절성 대상포진마비와 동시에 발병한 급성 뇌운동피질경색 1예 (A Case of Acute Cerebral Motor Cortical Infarction Coincidentally Occurred in Segmental Zoster Paresis)

  • Segmental zoster paresis (SZP) is a clinically rare complication of herpes zoster. But it has not been reported that acute cerebral motor cortical infarction coincidentally occurred in SZP. A 86-year-old woman was admitted due to pain, tingling sensation, and weakness of left arm. She had an acute onset of pain and tingling sensation in left arm at first day, shoulder weakness at second day, and multifocal vesicles at fourth day. Deep tendon reflexes of left arm were decreased than right. Electromyography showed an axonal polyneuropathy at superior trunk level of left brachial plexus. Median and ulnar sensory evoked potential tests were normal. Brain MRI showed a high signal in right primary motor cortex on diffusion weighted image. We report a case of acute cerebral motor cortical infarction coincidentally occurred in SZP.

까다로운 만성불면증의 원인과 치료 (The Causes and Treatment of Complicated Chronic Insomnia)

  • The causes of complicated chronic insomnia are very various and interact with vicious circle. Patient with this insomnia has generally a strong fear and frustration about failing to control of sleep and a deep mistrust toward doctor. To solve this complicated problems detailed history taking and sleep questionnaires are needed with objective polysomnography. Through these procedures, doctor should clarify causes of insomnia and explain them to patient in details and kindly. This process would be very helpful to restore the mistrustful relationship between patient and doctor and reduce patient's vague fear for insomnia. In treatment of complicated chronic insomnia, it is most important for patient to understand his problems and participate in the treatment schedule actively with assurance. Also doctor should encourage patient persistently not to be drop out. Most important factor for prognosis is patient's personality. Causes of complicated chronic insomnia are like these, overdose of hypnotics and sedatives, daily drinking alcohol with hypnotics, insomnia associated depression, delayed sleep phase syndrome, sleep state misperception, marked fear for insomnia, hyperarousal at bed, insomnia associated periodic leg movement and sleep apnea, chronic hypnotic insomnia, and immature personality. And possible treatments of these insomnias were discussed.

치과치료를 위한 진정자가조절법 (Patient-Controlled Sedation for Dental Treatment)

  • This article discusses the issues of benefit and possibility of application of patient-controlled sedation (PCS) for the dental treatment. The purpose of administering PCS for patients is to diminish anxiety and to provide cooperative and effective dental treatment. But there are a lot of barriers to application of PCS such as knowledge about pharmacokinetics of sedatives, expensive high speed and patient-controllable syringe pump, and well educated sedationist. And there could be risk of deep sedation, airway obstruction and hypoxic brain damages, etc. In order to decrease incidence of complication, appropriate drug selection, selection of right bolus dose, lock-out time is much important.

Neuromodulation for Trigeminal Neuralgia

  • Clinical studies on neuromodulation intervention for trigeminal neuralgia have not yet shown promising results. This might be due to the fact that the pathophysiology of chronic trigeminal neuropathy is not yet fully understood. Chronic trigeminal neuropathy includes trigeminal autonomic neuropathy, painful trigeminal neuropathy, and persistent idiopathic facial pain. This disorder is caused by complex abnormalities in the pain processing system, which is comprised of the affective, emotional, and sensory components, rather than mere abnormal sensation. Therefore, integrative understanding of the pain system is necessary for appropriate neuromodulation of chronic trigeminal neuropathy. The possible neuromodulation targets that participate in complex pain processing are as follows : the ventral posterior medial nucleus, periaqueductal gray, motor cortex, nucleus accumbens, subthalamic nucleus, globus pallidus internus, anterior cingulate cortex, hypothalamus, sphenopalatine ganglion, and occipital nerve. In conclusion, neuromodulation interventions for trigeminal neuralgia is yet to be elucidated; future advancements in this area are required.

Lesch-Nyhan syndrome: a case report

  • Lesch-Nyhan syndrome (LNS) is a rare X-linked recessive disorder caused by a mutation in the hypoxanthine phosphoribosyltransferase 1 (HPRT1) gene. This syndrome is characterized by excessive production of uric acid, mental retardation, self-mutilation, choreoathetosis, and spasticity. The most distinctive symptom is compulsive self-mutilation. For patients with LNS, different methods have been tried to reduce self-biting behaviors including restraints, behavioral treatment, medications, deep brain stimulation, tooth extraction and botulinum toxin A injection. In this report, we present a case of LNS undergoing cheiloplasty due to self-mutilation and tooth extraction of the left deciduous maxillary canine.
