영적 안녕감과 죽음 준비도 그리고 내세관이 죽음의 태도에 미치는 영향 (Effect on Influence the Attitude of Death of the Old Ages for Afterlife View and Death Preparation and Spiritual Wellbeing)

  • 본 연구는 노년기 영적안녕감과 죽음 준비도 그리고 내세관이 죽음에 대한 태도에 미치는 영향을 알아보고자 하였다. 또한 죽음에 대한 올바른 인식과 태도를 형성하여 죽음불안을 경감시키고, 만족스러운 노년의 삶을 살아갈 수 있도록 실천적 개입기술을 개발하고자 하는데 목적이 있다. 연구방법은 원주시에 거주하는 65세 이상의 노인들에게 설문지법으로 조사하여 SPSS 통계프로그램으로 결과를 도출하였다. 주요결과는 다음과 같다. 첫째, 영적안녕감과 죽음준비도 그리고 내세관이 죽음에 대한 태도에 미치는 영향을 분석한 결과 유의미한 (p<.001) 차이가 나타나 영향이 있는 것으로 나타났다. 둘째는 내세관, 죽음준비도, 영적 안녕감이 죽음에 대한 태도에 미치는 영향의 상대적 중요도 차이를 분석한 결과는 (P<.001)로 99.9% 신뢰수준 이상에서 통계적으로 유의한 영향관계가 있는 것으로 나타났다. 즉 죽음준비도, 영적 안녕감 순으로 죽음에 대한 태도에 영향을 미치며, 내세관은 다른 두변수가 영향을 미칠 경우 유의한 영향을 미치지 않는 변수로 나타났다. 셋째는 인구사회학적 변인 포함 후 죽음준비도, 내세관, 영적 안녕감이 죽음에 대한 태도에 미치는 영향을 분석한 결과 경제수준차이에서만 (p<.05) 통계적으로 유의한 음의 영향을 미치고 있는 것으로 나타났다. 따라서 죽음 준비도, 영적 안녕감은 노인의 삶의 질을 좌우하는 부분으로서 죽음불안을 해소하는 데 큰 영향이 있음을 본 연구를 통해 확인하였다. 따라서 본 연구의 의미는 죽음에 대한 불안을 긍정적으로 해소할 수 있다는 점을 검증할 수 있었다는데 의미가 있다.

  • 본 논문은 고령사회를 맞이한 우리 사회가 안고 있는 노인의 문제에 주목하여 노년의 삶을 이해하고 노후준비와 죽음준비의 필요성을 인식하도록 하기 위한 것을 목적으로 한다. 영화 '죽여주는 여자'의 주인공 소영의 서사를 중심으로 작품 속 맥락에 따라 문학 치료 관점으로 리뷰 분석한다. 영화 속 작품 서사에는 빈곤, 질병, 치매, 외로움, 죽음의 두려움 등 노인들이 겪는 고통의 문제를 잘 보여준다. 초고령 시대를 맞이하여 국가적 차원의 소외된 노인에 대한 제도적 보완장치 마련과 사회적 관심과 안전망이 확보되어야 하며, 개인적으로는 노후설계를 통한 구체적인 노후준비가 필요하다. 나아가 노후준비 및 죽음을 준비하기 위한 노후준비 교육 프로그램과 죽음 교육 프로그램의 연구와 교육을 위한 지원이 지속적으로 이루어져야 할 것이다.

예비노인의 사전연명의료의향서 작성 여부에 따른 죽음불안과 품위있는 죽음태도의 차이 (Death Anxiety and Dignified Death Attitudes of Pre-Elderly Who Opt for Signing Advance Directives)

  • Purpose: This study attempts to provide basic data for establishing and implementing an advanced directive (AD) system that helps identify dignified death attitudes and reduces death anxiety of the preliminary, through preparation of an AD. Methods: Data were collected from 135 preliminary elderly aged 55 to 64 years, recruited from health centers and welfare facilities. All data were analyzed using SPSS 22.0. Results: Overall scores of the participants were 2.26±0.64 for death anxiety scale, and 3.16±0.44 for dignified death attitude. Scores of individuals who prepared an AD were determined to be 2.19±0.64 for death anxiety, and 3.34±0.44 for dignity of death. The difference in scores obtained for dignified death attitude was significantly higher for the group that signed an AD, as compared to the group with no AD (F=14.81, p<0.001). Conclusions: Results of this study reveal that preliminary elderly who sign an AD have a higher dignified death attitude score as compared to subjects who do not sign an AD. Additionally, the former group of participants desire a dignified end to their life. This indicates a necessity to promote public campaigns for ADs, and to develop educational programs that assist the elderly to prepare for a dignified death and make autonomous decisions.

독거노인돌보미의 고독사 인식에 관한 주관성 연구 (A Study on the Subjectivity of the Elderly who Live Alone Caregivers in Perception of Lonely Death)

  • Purpose: This study was to analyzed the types of subjectivity that elderly who live alone caregivers have about the lonely death. Methods: This study objectified the subjective area by applying the Q methodology. It studied 24 caregivers in H region, who responded to 34 Q sample statements. The method of forced distribution was practiced regarding the results on a 9 points Q sample distribution chart. The collected data was analyzed using PC QUANL program. Results: The caregivers' perception of lonely death has been classified into four types: Type 1 ('type of fate-receptive invitation of lonely death'), Type 2 ('type of afterlife-expectantly invitation of lonely death'), Type 3 ('type of death preparation invitation of lonely death'), and type 4 ('type of rejection of lonely death due to attachment with life'). Conclusion: The present study attempts to provide basic resources for the development of nursing-intervention program to solve the problems with lonely death by grasping and understanding the types of the caregivers' perception of lonely death through which it also aims to yield information necessary to improve the quality of life in their remaining years.

고등학생의 호스피스완화의료 인식 영향요인 (Factors Influencing Perception of Hospice Palliative Care in High School Students in Korea)

  • 목적: 본 연구는 고등학생을 대상으로 영적안녕, 죽음에 대한 태도, 호스피스 인식 간의 상관관계를 파악하고, 호스피스 인식에 영향을 미치는 요인을 규명하고자 한다. 방법: 대상자는 B 광역시에 소재한 고등학교인 실업계(남자, 여자) 2곳, 인문계(남자, 여자) 2곳에 재학 중인 1, 2학년 학생 229명이었고, 자료수집 기간은 2015년 5월 1일부터 동년 5월 31일까지였다. 자료는 SPSS Win 18.0 program을 이용하여 기술통계, t-test, ANOVA, Pearsion correlation coefficients, Stepwise multiple regression으로 데이터를 분석하였다. 결과: 고등학생의 호스피스 인식은 성별(t=-3.377, P=0.001), 주관적 건강상태(t=3.334, P=0.001), 주관적 학교생활만족도(t=3.233, P=0.001), 부직업(t=2.393, P=0.018), 호스피스 사전인지경험(t=-7.181, P<0.001), 호스피스, 죽음 관련 교육경험(t=-2.120, P=0.035)에 따라 유의한 차이가 있었다. 사후 내 몸에 대한 태도와 죽음준비에 대한 태도가 긍정적일수록 호스피스 인식에 대한 태도가 긍정적이었다. 호스피스 인식에 영향을 미치는 주요 요인은 호스피스 사전 인지 경험, 죽음준비에 대한 태도, 주관적 학교생활 만족도, 성별로 총 설명력은 34.5%였다. 결론: 고등학생의 호스피스 인식을 증진하기 위해서는 죽음준비에 대한 태도와 호스피스에 대한 인지를 높여줄 수 있는 좋은 죽음 및 호스피스 교육프로그램 개발과 시행이 필요하다고 본다.

노인병원 환자 죽음에 대한 간호사의 경험 (Nurses' Experiences of the Death of Patients in Geriatric Hospitals)

  • Purpose: The purpose of this study was to identify and understand nurses' experiences of the death of patients in geriatric hospitals. Methods: Van Manen's hermeneutic phenomenological analysis was applied in the interpretation of experiential descriptions of seven nurses who had experienced the death of patients in a geriatric hospital. Results: The essential subjects derived from the experience of the nurses on the death of patients in a geriatric hospital are covered in the following 7 themes. 'Placed in death site', 'Difficult repetition of death and farewell', 'Emotional waves that rushes in after farewell', 'Dilemmas in a place with no preparation to greet expected death', 'Getting dull from continually being struck with sorrow', 'Being together with living death', and 'Showing courtesy for a good farewell and living well'. Conclusion: The results of this research will contribute to the development of policy on all the deaths of patients in geriatric hospitals and suggest basic data that need to be applied in real practice and directions to introduce plans for realistic improvements in nursing care of deathbed patients in geriatric hospitals.

포토보이스를 활용한 베이비부머 은퇴자의 죽음준비 인식의 연구 (Using Photovoice A Study on the Perception of Death Readiness in Babyboomer Retirees)

  • 한국 베이비부머 세대의 은퇴는 대량의 인구가 중년층에서 벗어나 노년층으로 들어서게 되면서 고령화 사회의 주요인이 되었다. 또한 30년 이상 구조화된 직장에서 바쁘게 일을 하다 은퇴 후 비구조화 된 환경에 적응하지 못해 우울감을 가지며 자살의 위험 등 사회적 문제로까지 이어지게 되었으며, 이러한 사회적 문제의 해결 및 예방적 차원의 정책적 검토를 위한 연구가 필요하였다. 본 연구는 죽음에 이르기까지 준비된 삶을 살고자 하는 은퇴자들의 죽음준비에 대한 인식은 어떠한가 라는 연구문제를 포토보이스를 활용하여 심층 탐구하였고, 은퇴자들의 노년기 죽음준비에 대한 사회복지 정책적 제언을 하고자 함이 본 연구의 목적이다. 연구 참여자는 베이비부머 은퇴자 7명이며, 자료는 2개월 동안 수집하였고, 직접 촬영한 사진과 설명과 심층면담 내용을 주제 분석법으로 분석한 결과 도출된 인식은 순리로 받아들이는 죽음을 맞이하며 준비하기 위한 교육의 필요성이었다. 본 연구의 논의에서 베이비부머 은퇴자들을 도울 수 있는 죽음준비 교육프로그램 개발이 시급하며, 프로그램을 담당할 지역 기관의 협력이 필요함을 제안하고자 한다. 본 연구는 베이비부머 은퇴자의 죽음준비의 인식을 통한 은퇴 이후 노년을 위한 사회복지 정책적 방안을 마련함에 있어 기초자료를 제시한 점에서 그 의의가 있다.

대학생의 죽음에 대한 주관성 연구 (Subjectivity toward Death among College Students)

  • The purpose of this study was to classify the subjectivity type toward death among college students and to understand the characteristics of attitudes and orientations toward death of each type. Since attitudes and orientations toward death is very subjective and unique, Q-methodology was employed in this study. Q-methodology explains the respondent's subjectivity by objectifying his subjectivity for himself. The P-sample consisted of 63 students of K university in D city. Forty statements concerning attitudes and orientations toward death, which developed by Yeun(1999) were utilized for Q-sample. Forty Q-statements were sorted according to the level of agreement or disagreement by forced normal distribution. The Q-sorts by each subjects were coded and analysed with the PC-QUANL program. Five types of subjectivity toward death were identified and labeled. Type 1 'the death- preparation' think frequently about his own death and talk openly about the problem of death with a positive attitude. Type 2 'the life-esteemed' respect the dignity of life most of all. Type 3 'the realty-oriented' do not believe the afterlife and is very concerned about the present life itself instead of thinking about death. Type 4 'the ambivalent' think importantly the aspect of present life and orient the afterlife at the same time. This type reveals opposite attitude of preparing and scaring the death at the same time. Type 5 'the destiny-recipient' attribute death to the destiny and deny suicide. On the basis of this result, the studies about applying and developing educational program on death and hospice care for nurses who care terminal cancer patients and families are needed.

소설과 말기 암환자를 통해 본 한국인의 죽음의 의미

  • Every one experiences death one day, however no one can knows exactly what it is because people can not experience death until it comes, it is therefore impossible to judge correctly on the phenomenon of the death. On the whole, man experiences indirect death through the mass communications such as TV drama, fiction, magazine etc because those methods can easily access by every one. In addition to this, people usually acquire the negative awareness of death through the dramatic change of story like dying of cancer for dramatic effect by giving scare and fear to the cancers. The purpose of this study is to provide basic information on the spiritual care that enables the facing death patients to accept death as a part of life and divert hope from scare about after death by comparing and analyzing of two aspects of death meaning I.e, Korean fiction and the end stage cancer patients. Additionally, for medical staff to understand the facing death cancer patients by making to aware patients correctly and provide the better quality of care. The study was performed from September 28, 2002 to February, 28 2003. The materials of this study were collected by direct data obtained from observation, interviews, note and diary of end stage of cancer patients and written materials acquired from Korean contemporary fiction. Participants of this study were 4 end stage cancer patients including 2 lung cancer patients, 1 liver cancer patient and 1 esophagus cancer patient. The methodology used in this study was divided into two types; Huberman & Miles methodology was used for fiction to find and categorize subject, and Colaizzi, one of phenomenological methodology was used for end stage cancer patients to find the major meaning, subject and categorization. 1.The death investigated in the fiction, was found as a progress of negative emotion, acceptance and sublimation, life related subjects in the negative emotion were tenacity for life, anxiety, lingering attachment, responsibility, abandonment and death related subjects were shock, isolation, fear, scare and rejection. Acceptance related subjects were acceptance, destiny, secularism, preparation and arrangement, and sublimation related subjects were sublimation through Christian and Buddhism. 2.The death showed in the participants was negative emotion, acceptance and sublimation, life related subjects were repentance, anxiety, responsibility and hopelessness, and death related subjects were dejection, solitude, anger, fear and scare. The acceptance was a type of religious acceptance that admitted instantly by reaching an understanding with the God, and death was accepted as a progress of preparation, arrangement, acceptance and hope. Sublimation related subjects were Christian sublimation and relief or destiny incurred from self-reflective sublimation through communications and thoughts. 3.The death in view of fiction and participants were positively accepted both death and negative emotion, and the study disclosed the fact that death was sublimated dependent on religion. 4.The progress of negative emotion, acceptance and sublimation was disclosed more complicated and various in the real end stage cancer patients and acceptance only found in the patients on the form of religious acceptance, according to the results compared with fiction and real end stage cancer patients. The death showed in the fiction was standardized, gradated and similar progress with psychological status of Kubler-Ross. However, death in the participants was showed complex and various feelings simultaneously, and sometimes they accepted death positively. The sublimation through religion was found in Buddhism and Christian in the fiction and mostly Christian in the participants due to a number of Hospice patients. It was found that negative emotion various types of death was more found in the participants than fiction. It is therefore necessary to study on the response of death in various types. In the participants death was incurred more systematic and variously, we knew that nursing practice focused on experience of participants is required and reality on death is much profound than we analyzed and presented, lots of situations and reactions should be premised because we can not completely rule out the negligence possibility of care mediation of participants. In caring for the facing death patients, we discovered and confirmed again through this study that the spiritual care should be needed as a mediation method.

영적간호 교육이 간호학생들의 죽음에 대한 태도변화에 미치는 영향 (The Influences of Spiritual Care Nursing Education Towards Death and Dying)

  • In order to care the persons who are dying a nurse should first solve her / his own conflicts about death. and be aware of their own concepts of death and dying. In order to find out patient's spiritual needs and to give better spiritual nursing care. a nurse should know her / his own spiritual needs and be aware of their own concepts of spiritual nursing problems. To improve nurse's understanding towards death and dying and nurse's knowledge towards spiritual needs and spiritual nursing care. 14 weeks(two hours a week) spiritual nursing care education was given to 3th grade baccalaureate nursing college student. Before and after spiritual nursing care education. 30 items of prepared questionare focused on the attitudes toward death and dying was asked. Pre and post results are as follow ; 1. The dying patient's emotional and physical needs. There was no significant difference between pre and post educated groups. Both of the situations. they agreed upon$(69.64\%)$ that the dying patients have high emotional and physical needs to solve. 2. Telling the truth of dying process. There was no significant difference between pre educated group$(53.33\%)$ and post educated group$(55.95\%)$. 3. Attitudes of medical personnels. There was no significant difference between pre$(51.49\%)$ and post educated groups $(53.87\%)$. These responses indicate that nursing college student didn't have enough experiences on dying patients care. 4. General attitudes on death and dying. Number of nursing students who were thinking positively toward death and dying were Increased (pre $39.68\%$. post $45.44\%$) and who were thinking negatively toward death and dying were also decreased (pre $37.30\%$. post $33.93\%$). 5. Attitudes toward mechanical assistance for life-expanding of helpless patient. There was a significant difference between pre and post educated groups. About $34.13\%$ of them approved upon mechanical assistance for life and about $33.14\%$ of them disapproved. 6. Attitudes of family members of dying patient. There was no significant difference between pre and post educated groups. About $45.24\%$ of both groups, agreed upon that the family members feel annoyed with dying patients and about $22.42\%$ of both groups disagreed. Whether they received the spiritual nursing education or not, they were aware of that the family members feel annoyed with dying patients. 7. Special facility and educational preparation for dying patient. There was a significant difference between pre$(82.14\%)$ and post$(90.87\%)$ educated groups. These responses indicated that after they received the education, they felt more about the necessity of special facility and educational preparation for the death and dying patients. 8. Special facility and welfare system for the old. There was a significant difference between pre$(58.33\%)$ and post$70.64\%$ educated groups. There responses indicated that after they received the education, they felt more about the necessity of special facility and welfare systems for the old.
