De Quervain's Disease after Delivery Treated with SBV Herbal-acupuncture : Clinical Study on 3 Cases (Sweet Bee Venom 약침을 이용한 산후 De Quervain's disease 치험 3례)

  • Purpose: To investigate effect of Sweet Bee Venom on postpartum's De Quervain's Disease. Methods: We investigated 3 patients diagnosed as De Quervain's Disease. Sweet Bee Venom Herbal-acupuncture was used for treatment of De Quervain's Disease, once every two days, 5 times. Pain, satisfaction at treatment and function of wrist were measured with VNS, Baumgaertner's nine point scale and dynamometer. Results: Pain reduction, functional improvement, increased grip power were found out in cases treated with Sweet Bee Venom Herbal-acupuncture. In every cases, they represent high subjective satisfaction with treatment. Conclusion: Clinical results show that Sweet Bee Venom Herbal-acupuncture has a good analgesic effect in De Quervain's Disease.

  • De Quervain's disease is a stenosing tenosynovitis that affects the synovial sheath of the tendons of the abductor pollicis longus and extensor pollicis brevis; it can be considered a work-related disease, since it frequently is associated with highly repetitive movements in jobs using the hands. The usual sign and symptom of de Quervain's diseases are positive Finkelstein's sign and pain and tenderness around the radial styloid process. The majority of de Quervain's disease has a good results to conservative treatment, but surgery can be performed in case of recurrence or pain last more than 6 months without improvement in spite of conservative treatment. We diagnosed a patient with atypical de Quervain's disease by ultrasonography, treated by local steroid injection and pain was relieved. We report a case of atypical de Quervain's disease evaluated by ultrasonography with brief of literatures.

  • De Quervain's disease is caused by stenosing tenosynovitis of the first dorsal compartment of the wrist. The first dorsal compartment at the wrist includes the tendons of the abductor pollicis longus (APL) and the extensor pollicis brevis (EPB). Patients with De Quervain's usually report pain at the dorsolateral aspect of the wrist with referral of pain toward the thumb and/or lateral forearm. This condition responds well to nonsurgical treatment. Objective : The aim of this study was to find out the therapeutic effect of the Cervus elaphus Herbal-acupuncture in the treatment of patients with De Quervain's Disease. Design : A prospective study of Cervus elaphus Herbal-acupuncture was conducted. Setting : The study was conducted in the Kang-nam Oriental Hosp. of Dong-Guk University. Patients : The study comprised 4 patients with De Quervain's Disease. Methods : We investigated 4 cases patients the De Quervain's Disease. We evaluated wrist function that before and after Cervus elaphus Herbal-acupuncture treatment by VAS score, tenderness and Finkelstein test. the treatment is operated every other day. Results : Pain reduction(VAS) was found out in the case treated with Cervus elaphus Herbal-acupuncture. Conclusions : Clinical results show that Cervus elaphus Herbal-acupuncture has a good analgesic effect in De Quervain's Disease.

봉약침으로 치료한 드꾀르벵 병 1례 보고

  • Objective : The aim of this study was to find out the therapeutic effect of the Korean Bee Venom theraphy in the treatment of patient with De Quervain's disease. Methods : As the effect of Bee Venom acupuncture about various articulation diseases observed, we tried to treat a case that was diagnosed De Quervain's disease by using Korean Bee Venom theraphy. Results : After Korean Bee Venom theraphy, 'VNRS' value decreased quite noticeably, and other symptoms have improved as well. Conclusions : Korean BeeVenom theraphy have shown fairly effective to De Quervain's disease. Furthermore, it shows considerable potential in its effectiveness to be utilized other fields of medicine practices.

  • Bee Venom acupuncture has anti-inflammatory and analgesic effect that facilitates natural healing. So it is used in treatment of several musculoskeletal diseases. Recently, we experienced a case of De Quervain's disease that was not responded to steroid injection. It was successfully treated with intra-synovial Bee Venom acupuncture for two weeks. It is thought that Bee Venom acupuncture can be used in tenosynovitis. And Bee Venom injection techniques like intra-joint and subcutaneous method are need to be developed.

  • This study reports the effectiveness of Korean medicine and Chuna manual therapy on patients with De Quervain's tenosynovitis. Two patients were treated with Chuna manual therapy, electro-acupuncture, moxibustion therapy. We used numeric rating scale (NRS), Finkelstein's test, sonography to measure changes during treatment. After each treatment, NRS, Finkelstein's test, sonography outcome were improved. Korean medicine including Chuna manual therapy can be effectively used to improve De Quervain's tenosynovitis. This study may suggest that korean medicine including Chuna manual therapy can be effective for De Quervain's tenosynovitis.

A Review of Research on Acupuncture and Moxibustion Treatment for De Quervain's Stenosing Tenosynovitis

  • Objectives : This study was performed to review articles concerning acupuncture and moxibustion treatment for De Quervain's Stenosing Tenosynovitis. Methods : On-line databases including Cochrane Library, Pubmed, CNKI, NDSL and OASIS were searched to identify articles concerning acupuncture and moxibustion treatment for De Quervain's Stenosing Tenosynovitis (DQST). Several duplicated articles and those not relevant to this topic were excluded, as were review articles and commentaries. Results : Fifty-one studies were identified, which included 28 clinical case studies with 1,227 patients and 23 randomized controlled trials with 2,040 patients. In these studies, acupuncture, acupotomy, moxibustion, pharmacopuncture, and laser acupuncture were used as DQST interventions. Although DQST is a common disease seen in clinical practice, only four of the 51 studies we identified in our search were published in Korean academic journals, all of which used pharmacopuncture. Conclusion : Although the results of the studies to date provide evidence that acupuncture and moxibustion are effective treatments for De Quervain's Stenosing Tenosynovitis, the absence of a standard, objective evaluation tool, and a lack of reporting on the negative side-effects associated with treatment remain important factors that should be addressed in future studies.

First extensor compartment morphology and clinical significance: a cadaver series study

  • The first extensor compartment of the wrist is a distinctly variable anatomical area. Anatomical variations in this region contribute to the pathophysiology and treatment failure of de Quervain's disease, which is a kind of tenosynovitis that develops in the first extensor compartment of the wrist. We aim to describe the first extensor compartment morphology, to evaluate the septum frequency, location of the septum, and the number of tendons of abductor pollicis longus (APL) and extensor pollicis brevis muscles (EPB). First extensor compartment of 87 wrists of 45 cadavers were dissected. The presence or absence of septum and number of tendon slips of APL and EPB revealed. The proximal and distal widths of the compartments were measured. Septums were detected in 60.9% (n=53) of the wrists. Incomplete (distal) and complete (proximal) septa were present in 35.6% (n=31) and 25.3% (n=22) of the cases. Only 26.4% of the wrists had a single slip of APL tendon. The Remaining had multiple slips. The median inner width of the proximal and distal compartments in all wrists were calculated as in the order of 9.11±1.14 mm and 8.55±1.12 mm. We believe that understanding the anatomy of the first extensor compartment in the Turkish population would be helpful to surgeons, radiologists, and physiotherapists to diagnose and manage de Quervain's disease.

Effects of Kinesio Taping Therapy in Patients with Stenosing Tenosynovitis (협착성 건초염에 대한 키네시오 테이핑치료의 효과)

  • Purpose: Stenosing tenosynovitis is a stenosing condition of the sheath of the abductor pollicis longus and extensor pollicis brevis tendons at the radial styloid process. The purpose of this study was to investigate the effects of kinesio taping on the pain and grip power in patients with de Quervain's disease. Method: 20 female with de Quervain's disease was recruited. This procedure was performed with or without taping procedure. Pain levels were assessed using a visual analogue scale, and spherical and pinch grip powers were assessed using a dynamometer. Results: 1. Resting pain, Finkelstein test pain, and Tenderness using VAS scale was showed significantly reduced after taping application(p<0.01). 2. Spherical grip power using dynamometer was showed significantly increased after taping application(p<0.01). 3. Pinch grip power using dynamometer was showed significantly increased after taping application(p<0.01). Conclusion: So we thought that kinesio taping therapy had effects on the pain release and grip power increasing to the small or local muscle groups and joints.

Research trend of Korean Medicine for Musculoskeletal Disorders of the Hand and Wrist (국내 의료기관에서 시행된 손/손목 근골격계 질환의 한방치료 연구동향)

  • Objectives The purpose of this study is to investigate the research trend of Korean traditional medicine for musculoskeletal disorders related to wrist and hand under Korean healthcare facilities. Methods We collected data from search engines for research including foreign and domestic online database using the keywords ligament tendon, wrist sprain, tendinitis, trigger finger, carpal tunnel, ganglion cyst, de Quervain's and Raynaud. Results A total of 45 articles were about ligament tendon, wrist sprain, tendinitis, tenosynovitis, trigger finger, carpal tunnel syndrome, ganglion cyst, de Quervain's tenosynovitis and Raynaud disease. Out of 45 articles, 20 articles were about carpal tunnel syndrome and the most commonly used measurement was visual analog scale. Conclusions This study shows the research trend of musculoskeletal disorders related with the wrist and hand. Through the collected data, the treatment methods and specific information of treatments were organized. This study can be used in clinical environments and will contribute for further study in musculoskeletal disorders related with the wrist and hand.
