  • This study aimed to develop an observation scale for assessing the quality of day care programs and to examine reliability and validity of the scale. Instruments included 3 self-assessment scales, the Evaluation Scale for Early Childhood Programs and an observation scale completed by 8 trained observers in 89 day care sites. Item analysis indicated that most items were acceptable in terms of item discrimination and item response distribution. Cronbach ${\alpha}^{\prime}s$ and inter-observer reliability were relatively high. Validity of the scale was evidenced by correlations between the observation scale and the criterion measure and inter-correlations among sub-areas of the observation scale. Scores on the observation scale were validated by quality ratings from self-assessment scores.

Two 30-minute visits a day were allowed to the control group according to current policy, while four 30-minute visits a day were allowed to the experimental group. Patients' state anxiety was measured at the first day of ICU admission, and on the third day of ICU admission patients' anxiety and family satisfaction were measured. For the infection rate, comparison was made between the experimental and control data-collecting periods. Results: The patients' state anxiety significantly decreased in the experimental group. Family satisfaction of experimental group was significantly higher than that of control group. There was no significant difference in the infection rate. Nurses positively evaluated extension of visiting hours because it could stabilize patients, reduce the number of arrangements for additional visits, and help establish trust relationship with families. Conclusion: Extended family visiting hours in the ICU reduced patients' anxiety and improved family satisfaction but had no effect on the infection rate. To achieve these research goals, the present study 1) analysed the differences in the mother's parenting guilt as the socio-demographic variables of the mother and child, and 2) examined the relative effect size of influences on the mother's parenting guilt with their parenting efficacy, satisfaction on the day-care center, emotional and informational support from teachers, and mother's active interaction with day-care teachers. The participants of the study were 350 mothers whose infants and toddlers were attending day-care centers in Gwangju and Jeollanamdo. The results of this study are as follows. First, the mothers with infants who were younger than 3 years old reported more parenting guilt than the mothers with toddlers. Second, the mothers whose children were the first child reported more parenting guilt than the mothers with children who were the second or the third child. Third, the emotional support from the day-care teacher was the most influential factor on the mother's parenting guilt. This study suggests that the mutual cooperation between mother and day-care teacher are very important to reduce the mother's parenting guilt.

  • Purpose: The purpose of this study is to develop a day care center model focused on public health institutions for the elderly residing in their homes. Method: Research design for this study was a mult-level research, which consisted of a related literature review, an Internet search for knowledge of the current situation at home and abroad, on-site interviews, questionnaires collected from a sample of residents in a rural area, and a key-informants approach. Results: 1) The subjects of service - Generalized service should be provided to the elderly, 65 years and older, regardless of their assets. 2) The contents of service - Providing pre-health oriented and post-social welfare service that can integrate and satisfy a wide variety of public health and welfare needs of the elderly would strengthen the health care service of a day care center for the elderly. 3) Delivery system - Basic-level local self-governments should become a central operating body, and establishing a properly adjusted delivery system to a rural area after considering the efficiency and the access of vulnerable rural areas is needed based on modification of 'a Special Law for Agricultural and Fishery Areas' (rural public health center>rural health sub-center ${\rightarrow}$ unified health sub-center ${\rightarrow}$ public health hospital (public health center) ${\rightarrow}$ public welfare office). 4) Facility - Public health facilities such as public health centers and sub-centers should be located in areas that can easily access the facilities. 5) Funding - For day care center for the elderly in local self-government, the central government should modify a relevant implementation of subsidy in and provide some facilities and service regardless of the degree of self reliance of local self-government. 6) Human resources - It is needed to guarantee the period of workers of a day care center for the elderly, at least 3 to 5 years, with considering their specialty on aged care and avoiding circulation based positions. Furthermore, appropriate specially trained personnel such as medical workers and social workers should be placed to take care of both health service and welfare through strengthening of 'rules of law of elderly welfare,' Conclusion: future research is needed to test the model through a demonstration study using a model which may be developed in the future and to standardize the appraisal criteria of people hoping to enter a day care center for the elderly.
