Effect of Rehabilitation Intervention for Lifestyle Improvement of Spinal Cord Injury: Systematic Review of Randomized Controlled Trials and Meta-Analysis (척수 손상 환자의 라이프 스타일 개선을 위한 재활 중재 효과: 무작위 대조군 연구의 체계적 고찰 및 메타분석)

  • Objective : The purpose of this study was to investigate the effectiveness and basis of rehabilitation intervention in patients with spinal cord injury by systematic review and meta-analysis of randomized controlled studies. Methods : Two researchers independently searched and selected a study published in an academic journal using a search term in an international thesis database. A total of 21 studies met the selection criteria, and qualitative evaluation of the study was conducted using the PEDro Scale. Meta-analysis was performed using Comprehensive Meta-Analysis 3.0 program. Results : A total of 713 subjects were included. The results of the meta-analysis showed a score of 0.406 (95.0% confidence interval: 0.221 ~ 0.591) for intervention using physical activity and 0.505 (95.0% confidence interval: 0.449 ~ 1.528) for electronic stimulation therapy, which showed medium effect; educational intervention had a 0.248 (95.0% confidence interval: 0.033 ~ 0.464), and mixed intervention 0.280 (95.0% confidence interval: 0.122 ~ 0.438). It was shown that the effect of small (small). There was a significant heterogeneity in the statistical heterogeneity test, and thus the random effects model was selected and analyzed. Conclusion : The results showed that rehabilitation interventions were effective for patients with spinal cord injury. During the rehabilitation of spinal cord injury patients, clinicians are expected to contribute to the development of programs to improve their lifestyles.

Evaluation of Crystalline Silica Exposure Level by Industries in Korea (국내 업종별 결정형 유리규산 노출 평가)

  • Objectives: The major aim of this study is to construct the database of retrospective exposure assessment for crystalline silica through reviews of literatures in South Korea. Methods: Airborne concentrations of crystalline silica were collected using an academic information search engine, Research Information Service System(RISS), operated by the Korea Education & Research Information Service(KERIS). The key words used for the literature search were 'silica', 'crystalline silica', 'cristobalite', 'quartz' and 'tridymite'. A total number of 18 published documents with the information of crystalline silica level in air or bulk samples were selected and used to estimate retrospective exposures to crystalline silica. Weighted arithmetic mean(WAM) calculated across studies was summarized by industry type. Industries were classified according to Korea Standard Industrial Classification(KSIC) using information provided in the literature. Results: A total of 2,131 individual air sampling data measured from 1987 to 2012 were compiled. Compiled individual measurement data consisted of 827 respirable crystalline silica (RCS), 31 total crystalline silica(TCS), 24 crystalline silica(CS), 778 respirable dust(RD) and 471 total dust(TD). Most of RCS measurements(68.9%) were collected from 'cast of metals(KSIC 243)'. Comparing industry types, 'mining coal and lignite(KISC 051)' showed the highest WAM concentration of RCS, $0.14mg/m^3$, followed by $0.11mg/m^3$ of 'manufacture of other non-metallic mineral products(KSIC 239)', $0.108mg/m^3$ of 'manufacture of ceramic ware(KSIC 232)', $0.098mg/m^3$ of 'heavy construction(KSIC 412)' and $0.062mg/m^3$ of 'cast of metals(KSIC 243)'. In terms of crystalline silica contents in airborne dust, 'manufacture of other non-metallic mineral products(KSIC 239)' showed the highest value of 7.3%(wt/wt), followed by 6.8% of 'manufacture of ceramic ware(KSIC 232)', 5.8% of 'mining of iron ores(KSIC 061)', 4.9% of 'cast of metals(KSIC 243)' and 4.5% of 'heavy construction(KSIC 412)'. WAM concentrations of RCS had no consistent trends over time from 1994 ($0.26mg/m^3$) to 2012 ($0.12mg/m^3$). Conclusion: The data set related RCS exposure level by industries can be used to determine not only the possibility of retrospective exposure to RCS, but also to evaluate the level of quantitative retrospective exposure to RCS.

  • Development of a new design guide which is based on empirical-mechanistic concept for pavement design is in action. It is called AASHTO 2002 Design Guide in USA and the KPRP(Korean Pavement Research Project) in Korea. The material characteristic of hot mix asphalt is a key role in the design guide. Therefore it is urgent to get a proper materials database, especially the dynamic modulus of hot mix asphalt. In this research, dynamic modulus test, which is based on aged asphalt binder, has been carried out and proposed the predicted equation of dynamic modulus. Nine different hot mix asphalt with three different asphalt binder have been used for the dynamic modulus test. Short-term aging, which is covers the time for the production of asphalt plant, transportation, lay-down, and compaction, can be simulated at $135^{\circ}C$ with 2 hour curing. Long-term aging has been carried out for a performance period of asphalt pavement. The dynamic modulus of asphalt pavement increases with aging time. As the nominal aggregate size increases, the change of dynamic modulus is not big.

Development of a Network Expert System for Safety Analysis of Structures Adjacent to Tunnel Excavation Sites (터널굴착 현장에 인접한 지상구조물의 안전성 평가용 전문가 시스템의 개발)

  • Ground settlements induced by tunnel excavation cause the foundations of the neighboring superstructures to deform. An expert system called NESASS was developed to analyze the structural safety of such superstructures. NESASS predicts the trend of ground settlements to be resulted from tunnel excavation and carries out a safety analysis for superstructures on the basis of the predicted ground settlements. Using neural network techniques, NESASS learns a data base consisting of the measured ground settlements collected from numerous actual fields and infers a settlement trend at the field of interest. NESASS calculates the magnitudes of angular distortion, deflection ratio, and differential settlement of the structure and, in turn, determines the safety of the structure. In addition, NESASS predicts the patterns of cracks to be formed on the structure using Dulacskas model for crack evaluation. In this study, the ground settlements measured from the Seoul subway construction sites were collected and sorted with respect to the major factors influencing ground settlement. Subsequently, a database of ground settlement due to tunnel excavation was built. A parametric study was performed to verify the reliability of the proposed neural network structure. A comparison of the ground settlement trends predicted by NESASS with the measured ones indicates that NESASS leads to reasonable predictions. An examples is presented in this paper where NESASS is used to evaluate the safety of a structure subject to deformation due to tunnel excavation near to the structure.

Effect of Acupuncture on Sciatica in Rat Models: Systematic Review and Meta-analysis (좌골신경통 백서모델에서 침치료의 효과: 체계적 문헌고찰 및 메타분석)

  • Objectives While sciatic neuropathy is one of the common symptoms which have the lifetime incidence of 13~40%, still there is no consensus about the standardized and the most effective conservative treatment. In addition, the importance of systematic review and meta-analysis of preclinical are growing as they could suggest possible effective treatment strategy for future studies. Therefore, we conducted systematic review and meta-analysis to estimate analgesic effect of acupuncture on sciatica in rat models. Methods Systematic search was conducted for all controlled comparative preclinical trials which assessed analgesic effect of acupuncture in sciatica rat models. Database of PubMed, EMBASE, Web of Science, CNKI and 6 Korean databases were used. The primary outcome was pain, which is evaluated by stimulus behavior tests in rat models. We assessed the methodological quality with Systematic Review Centre for Laboratory Animal Experimentation's risk of bias tool. RevMan 5.3 was used for meta-analysis and subgroup analysis was conducted according to treatment site, acupuncture point, treatment period and frequency used in electroacupuncture. Results 14 studies were finally included following our inclusion criteria. The data from meta-analysis indicated that the acupuncture significantly improved the result values of behavior tests for pain evaluation, compared to no-treatment control group in animal models (standardized mean difference=4.43, 95% confidence interval 3.16 to 5.69, Z=6.84, p<0.00001; χ2=68.02, p<0.00001; I2=82%). The results of subgroup analysis indicate that acupuncture treatment of unilateral site, distal acupoints, longer treatment period and applying 2/100 Hz frequency in electroacupuncture could be more effective. Conclusions Systematic review and meta-analysis of animal studies are getting important for the future clinical studies and the improvement of heatlh care. Therefore the results of the study would provide evidence and better design for the forthcoming studies.

On the Homotoneity of Species Composition in the Phytosociologically Synthesized Community Tables (식물사회학적 식생자료의 종조성 균질성에 대하여)

  • Securing the species compositional integrity (typicalness and representativeness) is the essential prerequisite for an integrated management of vegetation resources using the phytosociological $relev\acute{e}s$ and plant communities of the Z.-M. school. This study is intended to develop a tool for qualitative and quantitative evaluation of species compositional homotoneity of a set of $relev\acute{e}s$ per syntaxon. The new homotoneities, actual homotoneity ($H_{act}$), and optimal homotoneity ($H_{opt}$) taking into account the heterogeneous factors of $relev\acute{e}s$ are proposed. The correlations between the floristic variables such as the vegetation type, the new homotoneities, and the previously studied homogeneous measures (e.g. Pfeiffer's homogeneity, basic homotoneity-coefficient, corrected homotoneity-coefficient, and mean floristic similarity) are analyzed by using Spearman's rank correlation coefficient. $H_{act}$ and $H_{opt}$ are effective in determining the difference of inter-synthesized units and of inter-$relev\acute{e}s$, respectively. $H_{act}$ is the homotoneity that is the most independent of the number of $relev\acute{e}s$. On actual vegetation with long-term human impact in the Korean Peninsula, $H_{opt}$ has become an aid to the more precise understanding of $H_{act}$ as substantive homogeneousness of species composition of syntaxa. It is expected that $H_{act}$ and $H_{opt}$ can be used for the selection of a sort of homogeneous vegetation data to build a phytosociological $relev\acute{e}$-database with consistency and objectiveness for national vegetation resources.

A Guideline for Construction of Building Disaster Information System (건축물방제시스템 구축${\cdot}$운영방안 연구)

  • While occasional disasters have occurred recently in urban facilities such as bridges, buildings, subways, roads and others, the systematic safety management of urban facilities is greatly needed to prevent the disaster or to minimize the damages. This study proposes the way of setting-up and managing an building disaster information system so that this experience can be applied to other urban facilities to minimize trials and errors. With such a purpose, this study presents a building safety management information system and utilizes it for the building disasten information system. This study includes research agendas as follows : building maintenance and management infonnation system, setting-up and utilization the building safety management infonnation system, building disaster infonnation system using GIS. The results suggest that the building safety management infonnation system which can be related to the urban disaster infonnation system should be constructed in advance in order to utilize the building disaster infonnation system, and that, under this condition, it proved to be easily connected to the related database, especially GIS. And, to utilize such an infonnation system, the application programs such as safety evaluation program, cost estimation program and documents management program should be readily available. In addition, other functional programs related with maintenance and management should be designed and established. The schemes or devices for practical usage should be supplemented through a standardization and a pilot project in central government or Seoul Metropolitan Government level in order to set up and utilize the building safety management infonnation system, and the related laws or regulations should be prepared or amended to support the system.

The study of difference between HIVD group and simple low back pain group by Current Perception Threshold test (요추추간판탈출증군과 단순요통군간의 Current Perception Threshold상의 차이에 관한 연구)

  • Objective : The aim of this study is to assess the ability of the CPT test to dignosis radiculopathy in Korean with NeuvalTM CPT database. Method : Electrical stimulation(at 2000, 250, and 5Hz) using Neurometer was applied to the 19 patients who felt back and sciatic pain with herniated intervertevral disc(HIVD) of L-spine, dignosisd by lumbar CT or MRI, and the 33 patients who felt only back or hip or leg pain without HIVD, stenosis, spondylolisthesis which causing radiculopathy. The test sites were toe1, toe3 and toe5 related to L4, L5 and S1 nerve roots. Results : The mean values of the CPT of HIVD group was stiatically lower than LBP group at toe1-250Hz, toe3-2000Hz, 250Hz and toe5-250Hz, 5Hz. The grading CPT score of HIVD group was stastically higher than LBP group at the toe3(L5). The VAS of HIVD group was stastically higher than LBP group. Conclusion : These results suggest that the CPT test can be a valuable testing for diagnosing radiculopathy in Korean. In using CPT test further study is needed for the diagnosis and evaluation of sensory nerve dysfunction in the musculoskeletal disease.

An Analysis of the Characteristics of Standard Work and Design Information on Estimating Environmental Loads of PSC Beam Bridge in the Design Phase (PSC Beam 교량의 설계단계 환경부하량 산정을 위한 공종 및 설계정보 특성 분석)

  • As many environmental pollution problems have arisen, various studies related to the environmental evaluation have been carried out in the construction industry. However, there is no methodology for estimating the environmental load quickly for design alternatives of civil facilities in the design phase. This study aim to establish criteria of works information and designed parts which can efficiently estimate environmental loads of PSC beam bridge based on standard quantity at the early design phase. For this purpose, a detailed environmental loads database was constructed by performing Life Cycle Assessment (LCA) based on detailed design data of 25 bridges. In addition, major work with high impact on environmental load were selected, and the analysis of characteristics of environmental load according to the required materials and 8 impact categories were conducted. As a result, the superstructure accounted for 42.91%. In the superstructure, remicon of the material base and PSC beam work occupied 53.13% and 31.25%. In the substructure, remicon, rebar, and cement, which are material base, accounted for more than 93%. It is expected that this major work and material information for each part of bridge can be utilized in the construction of the model, which can estimate the approximate environmental load, reflecting the characteristics of the structure in the design phase.

Scheduling Method of Real-Time Mobile Transaction Manager considering Value of Transactions and Validity of Real-Time Data (트랜잭션의 중요도와 데이터의 유효성을 고려한 실시간 이동 트랜잭션 관리자의 스케줄링 기법)

  • In this paper, we present a scheduling method for real-time mobile transaction manager in mobile computing environment. The proposed method checks whether a transaction is executable or not. It is able to by considering not only the deadline of real-time data in mobile hosts. And then, it schedules the real-time mobile transactions by making optimal execution window based on the priority queue, while considering transaction value and deadline. Disconnection with mobile hosts is monitored in selecting the transaction for execution. Using the proposed method reduces the number of restarting times after transaction aborts which is caused by the violation of the validity constraints of real-time data. And in has merits of maximizing the sum of values of real-time mobile transactions which meet the deadline. The performance evaluation demonstrates that the number of committed real-time transactions within the deadline is improved by 20%. This method can be used in real-time mobile transaction manager is such environments as cellular communications, emergency medicine information system and so on.
