Prediction of Suitable Site to Measure Abdominal Skin Fold Thickness and Correlation among Obesity Indicators in Patients with Type 2 Diabetes Mellitus (제2형 당뇨병 환자에서 피부두겹두께의 측정부위 예측 및 비만지표들 간의 관련성)

  • Purpose: The purpose of this study was to predict measuring site suited for abdominal skin fold thickness (ASFT) by measuring the distribution of abdominal subcutaneous fat thickness (AScFT) and ascertain the correlations among obesity indicators. Methods: The size of analysis materials was 124 secondary data measured by ultrasonic device, bioelectrical impedance analyzer and caliper. Data were analyzed using t-test, and Pearson's correlation. Results: The average of AScFT was 10.63± 6.79mm with its range 1.39-36.16 mm, and AScFT of female and of central parts were thicker than those of male and outer parts in the abdomen. The average of ASFT was 29.26±12.59 mm. Site 5 on Figure 1 was most similar to the average of AScFT in both sexes. Body mass index (BMI) and waist hip ratio (WHR) were 23.65±3.98 and 0.88±0.05 respectively. The body weight, BMI, WHR, visceral fat, ASFT vs AScFT revealed in significant correlation (r= .29, r= .55, r= .39, r= .33. r= .29). Conclusion: BMI and WHR seem more useful than other obesity indicators, when obesity control is necessary for Type 2 diabetes patients. Site 5 on Figure 1 is most suitable site to measure ASFT.

Convergence between Dental Caries and Obesity in Adolescents: The 6th Korea National Health and Nutrition Examination Survey (청소년 비만과 치아우식증과의 융합연구: 제6기(2013-2015년) 국민건강영양조사를 바탕으로)

  • This study aims to assess the relationship between obesity and dental caries in adolescents. Data were used from the 6th Korean National Health and Nutrition Examination Survey (2013-2015). Data analysis was performed using the SPSS Ver 22.0 program, and complex sample multivariate logistic regression analysis was performed to analyze relationship dental caries and obesity according to gender in adolescents. The subjects of this study were 1,243 adolescents aged 13 to 18 years. The relationship between obesity and dental caries in adolescents according to gender was not confirmed after adjusting for confounding variables. Therefore, obesity prevention programs and oral care programs for adolescents will be necessary.

  • Purpose: This study is to investigated the eating behavior, physical activities and obesity degree of the adolescents. Methods: The subjects of research were 1,092 third year students in middle school and high school randomly chosen of N city area and L gun area in Chonbuk province. The data was collected by an anonymous questionnaire. A survey tool developed in Ministry of Health and Welfare(2005) was used to investigate eating behavior, physical activities and obesity degree of subjects. The data was analyzed with the SPSSWIN 15.0 Programs. Results: 37.2% of adolescents only had positive eating behavior. 43.4% of adolescents did regular physical exercise, 10.8% did moderate physical exercise, and 43.3% did active physical exercise. 76.6% of adolescents did muscular strength exercise. So, the majority of adolescents were doing physical exercise. The percentage of adolescents who had normal weight range was 90.6%. The percentage of over weight range was 5.2%, which was higher than the percentage of under weight range, 4.2% Conclusion: This study is a descriptive research for investigating the eating bahavior, physical activities and obesity degree of adolescents. The results provided the baseline data for improving eating behavior, and improving physical activities and relieving obesity degree in adolescence. Therefore an effective national policy must study and develope a program to establish the positive eating behavior, and to promote the physical activities, and to prevent the adolescents' obesity.

Development of Structural Equation Model for Causal Relationships Among the Risk Factors of Arteriosclerosis (동맥경화증 위험요인들간의 인과관계에 대한 구조모형 구축)

  • The purpose of this study was to clarify the dynamic relationships among risk factors of arteriosclerosis and to develop and examine a model which could explain this relationship clearly. Data were collected from medical records of 400 male clients who visited a university hospital located in Inchon for physical examinations, from May 1996 to December 1996. Data were analyzed using the LISREL (Linear Structural Relationship) 8 program. To test the fitness of the hypothesized model, chi-square, RMSR (root mean square residual), GFI (goodness of fit index), CN (critical number) and Q-plot were used. Most of the fitness measurements, except the chi-square showed that the hypothesized model complimented the real data. According to the results, there were trends that obesity and hyperlipidemia were prevalent in heavier smokers, higher alcohol intakers, and groups who excercised less. Also, hypertension was more prevalent in older age, higher alcohol intaker, and higher serum lipid level groups. In contrast to the hypothesis, alcohol intake did not significantly affect serum lipid levels. This might be due to the serum lipid measurements (total cholesterol and trigryceride) used in this study to estimate hyperlipidemia. The direct effect of smoking on hypertension was not significant. However, the total effect of smoking on the hypertension was significant since indirect effects of smoking on hypertension, such as obesity and hyperlipidemia, were significant. The total effect of obesity on hypertension was significant since the indirect effect of obesity on hypertension via hyperlipidemia was significant, although the direct effect of smoking on hypertension was not significant. The degree of explaining hyperlipidemia with smoking, exercise, and obesity was high (60%), however, the degree of explaining obesity with age, smoking, alcohol intake, and exercise was very low (7%). On the basis of these results, high risk factors of arteriosclerosis such as hypertension, hyperlipidemia, or obesity are either directly or indirectly correlated each other. Therefore, it is difficult to predict outcomes for increasing or decreasing the risk factors by simply modulating a factor. Smoking, alcohol, and exercise both directly and indirectly affected major risk factors of arteriosclerosis. Therefore, correcting these variables is required to decrease risk factors. Finally, the relationship among other risk factors which have been known to be related with arteriosclerosis (diet, stress or hereditary) should be clarified in further studies.

Correlation between sodium intake and obesity with related factors among Koreans: a cross-sectional study on dietary intake and eating habits

  • Purpose: Sodium is essentially required for homeostasis and physiological functions, but excessive sodium consumption increases the risk of obesity and other chronic disorders. Korean studies on the sodium-obesity relationship are limited, and thus, this study was undertaken to determine the nature of the relationship between sodium intake and obesity in Korean adults. Methods: Forty-two participants were divided into 2 groups according to body mass index (BMI, non-obese BMI < 25 kg/m2, obese BMI ≥ 25 kg/m2). Dietary intakes and eating habits were analyzed using 3-day food records and a food frequency questionnaire. Anthropometric data were obtained from bioimpedance results, and fasting glucose and lipid levels were measured. Results: Mean weight, BMI, waist and hip circumferences, and body fat mass were greater in the obese group than in the non-obese group for men and women. Skeletal muscle mass and body fat mass were higher in obese women than in non-obese women. Biochemical data were no different in these two subgroups except triglycerides (TGs), which were higher in obese women. Nutrient intakes were not significantly different in obese and non-obese groups. However, obese men consumed excessive sodium, while obese women consumed slightly more than non-obese women. Obese men preferred salty foods and tended to overeat. Positive correlations were found between sodium intake and weight in men and percent body fat mass (PBFM) in women. Correlation analysis (adjusted for energy intake) of the relation between sodium intake and obesity-related factors showed sodium intake was positively correlated with PBFM and TG in women. Conclusion: This anthropometric and biochemical data analysis emphasizes the need for awareness and interventions to mitigate the health risks of elevated sodium consumption. Our findings should aid future studies on the relationship between sodium and obesity and contribute to preventing and managing this metabolic condition.

Incidence of Obesity-related Diabetes Mellitus in Adults with Prediabetes: Use of Data from a Prospective Cohort Study (당뇨전단계 성인의 비만이 당뇨병 발생 위험에 미치는 영향: 전향적 코호트 자료의 활용)

  • Purpose: The purpose of this study was to identify the impact of obesity on the incidence of diabetes mellitus in adults with pre-diabetes. Methods: This study employed a longitudinal study design and utilized secondary data drawn from the Korean Genome and Epidemiology Study. This study used data from a sample of 3,693 adults with prediabetes who were followed every two years from 2001 to 2018. Statistical data analysis for frequency, number of cases per 1,000 person-years, log-rank test, Kaplan-Meier curve, and Cox's proportional hazards regression analysis was performed using IBM SPSS statistics version 26. Results: During the observation period, there were 1,309 (35.4%) patients with diabetes, and the total number of person-years was 35,342. The incidence of diabetes was higher in the obese group compared to the normal weight group (body mass index [BMI]: hazard ratio=1.57, 95% confidence interval [CI]=1.40~1.77, waist: hazard ratio=1.55, 95% CI=1.38~1.76, waist to hip ratio [WHR]: hazard ratio=1.53, 95% CI=1.24~1.89, body fat [BF] (%): hazard ratio=1.42, 95% CI=1.27~1.61). Conclusion: An increase in BMI, waist circumference, and WHR, which are indicators of obesity, can exacerbate the risk factors for diabetes. Thus, a decrease in BMI, waist circumference, and WHR is necessary to prevent pre-diabetes. In particular, health care professionals should provide individualized weight management program interventions, including adult obesity programs and obesity counseling in partnership with local health departments, to reduce BMI and waist circumference in people at high risk for diabetes.

Prediction of Obesity by Sasang Constitutions and Enneagram in University Students (사상체질과 에니어그램 성격유형에 따른 대학생의 비만 예측)

  • Objectives Identification of individuals predisposed to obesity is an important issue for prevention and control of the obesity. It was reported that a high prevalence of obesity appeared in Taeumin among Sasang constitutions, but Enneagram personality-types has not been used to classify the patterns of obesity. These two classification methods were employed in combination in the current study, and it was analyzed whether the morbidity pattern of obesity can be characterized in further detail. Methods The subjects were 125 University students(62 males and 63 females) who answered both questionnaires for Sasang constitutions and Enneagram personality types. The obesity of students was classified by the obesity index and BMI. Results Only Taeumin group of Sasang constitutions was overweight, and the male of the group was overweight or obese. Analysis of the obesity index and BMI according to the Enneagram personality types showed significant differences(p<.05) between the types in the female group. These values were highest at the type 3 and lowest at the type 4. The physical indices according to both the centers of Enneagram and Sasang constitutions showed that only Taeumin female group had significant differences(p<.05) in the obesity index and BMI. Taeumin male group was overweight or obese in all centers of Enneagram and Taeumin female group was overweight or obese only in heart-center. Conclusions Diagnosis of the present data suggest that the classification of obesity patterns using Enneagram personality types in addition to Sasang constitutions is very useful to prevent and control the obesity.

Comparison of Diet Quality and Diversity according to Obesity Type among 19-64 year old Korean Adults (한국성인의 비만유형에 따른 식사의 질과 다양성 비교)

  • Objectives: This study was performed to compare the diet quality and diversity according to types of obesity categorized by body mass index and waist circumference among Korean adults aged 19-64 years. Methods: This study used the data of the 5th Korea National Health and Nutrition Examination Survey (KNHANES-V) and included 11,081 study participants. Type of obesity was categorized into 4 groups (Type 1: BMI obesity + abdominal obesity; Type 2: BMI obesity only; Type 3: abdominal obesity only; Type 4: Normal). To compare the diet quality and diversity according to obesity type, ANCOVA (Analysis of covariance) was used with stratification of age groups (19-44 years, 45-64 years). Results: With regard to comparative analysis of diet quality, there were significant differences between diet qualities in energy, protein, thiamin, riboflavin, niacin, phosphorous and iron and type of obesity in the 19-44 age group, while there were significant differences between diet qualities on protein, vitamin C, phosphorous and type of obesity in the 45-64 age group. There was no significant difference between diet diversity score and type of obesity in Korean adults. Conclusions: This study showed that in Korean adults, diet qualities of some nutrients were different among obesity types, while diet diversity was not. These observations should be considered in an effort to improve intake of over-and deficient nutrients and in further studies to evaluate the effects of nutrient quality on obesity.

A study on practice of obesity control by school nurses (보건교사의 비만관리실태 조사연구)

  • This study was a research study to find out the real picture and practice of student obesity control by primary, middle and high school school nurses. The study sampled 196 school nurses working in the Busan City and Northern Gyungsang Province, who agreed on the purpose of the study. When it comes to the study process, at the meeting of the whole school nurses, the researcher explained the purpose of the study and asked them to complete questionnaires and return on the spot. The study is designed to provide basic data for school nurses in field to systematically and efficiently control student obesity by examining perception of school obesity control and the real picture or practice of obesity control as well as its problems. The study used SPSS 10.0 to analyze data. The results are as follows: Annual plans about obesity control appeared significant at the primary school, 89.3%, higher than 77.3% of the middle and high school(p<0.05). Annual project evaluation showed a significant difference by regions, 56.0% of metro cities vs. small cities(p<0.01). The number of measuring weight of obese students was higher at the primary school than at the middle and high school(p<0.05). Also small cities conducted the weight measurement more often than metro cities(p<0.001). Among the health education methods for obesity, counseling appeared statistically significant(p<.01). The health education showed a significant difference in more experienced school nurses. Among the educational media, environment postings were used at the primary school, 24.0%, higher than 5.3% of the middle and high school. It showed a significant difference(p<.001). OHP or epidiascopes were used at the primary school, 36.4%, significantly higher than the middle and high school(p<.001). In addition, computer was used at the primary school, 21.5%, higher than the middle and high school(p<.05). Problems of obesity education data included a lack of textbooks and programs and a short of educational media. However it didnt show a significant difference in schools or in regions. Counseling activities for obesity showed a significant difference in schools(p<.01) and in regions(p<.001). When parents with obese students were in counseling, those in their 20s used home correspondence and those in their 30s and 40s used phone call(p<.01). The rate of equipment to measure obesity was higher at the middle and high school than the primary school, showing a significant difference(p<.001). Therefore, regular health education curriculums for school nurses should be systematically created to promote student health. Furthermore, it needs to make standard textbooks for health education by grades as well as by levels and to develop various educational programs and educational media.

Health Behavior Factors Related Type 2 Diabetes by Obesity for Health Promotion in Adolescents (청소년 건강증진교육을 위한 비만여부에 따른 당뇨병 관련 건강행태)

  • Purpose: Several health behavior factors affect the incidence of type 2 diabetes. Especially, obesity, which causes insulin resistance, is the most important determinant of diabetes. Therefore, we expect the risk factors associated with insulin resistance and type 2 diabetes are affected by obesity and, additionally, the related factors with diabetes caused by obesity can be controlled. Methods: This study used data collected from the 2001 Korea National Health and Nutrition Examination Survey (KNHANES). A stratified multistage probability sampling method was applied and the final sample included 5,500 subjects over 30 years old who had completed necessary health examinations and health behaviors survey. Results: The risk factors associated with type 2 diabetes are affected by obesity. According to logistic regression model stratified by body mass index (BMI) and sex, abdominal obesity and age were the significant risk factors of diabetes regardless of sex and BMI. However, drinking, smoking, total energy consumption, and protein consumption were risk factors for women with normal BMI, while carbohydrate consumption was a risk factor for man with normal BMI. Sleeping hours affected diabetes for women with obesity and fiber consumption was a risk factor for both women and men with obesity. In addition, statistically the family history of diabetes was a significant risk factor only in the group with normal weight, not in the group with obesity. Conclusion: The study results will provide information for implementing a regional initiative of type 2 diabetes prevention by BMI.
