각막 압평을 이용한 로봇 바늘 삽입법: 심부표층각막이식수술에의 적용 (Robotic Needle Insertion Using Corneal Applanation for Deep Anterior Lamellar Keratoplasty)

  • This paper describes a robotic teleoperation system to perform an accurate needle insertion into a cornea for a separation between the stromal layer and Descemet's membrane during deep anterior lamellar Keratoplasty (DALK). The system can reduce the hand tremor of a surgeon by scaling the input motion, which is the control input of the slave robot. Moreover, we utilize corneal applanation to estimate the insertion depth. The proposed system was validated by performing the layer separation using 25 porcine eyes. The average depth of needle insertion was 742 ± 39.8 ㎛ while the target insertion depth was 750 ㎛. Tremor error was reduced from 402 ± 248 ㎛ in the master device to 28.5 ± 21.0 ㎛ in the slave robot. The rate of complete success, partial success, and failure were 60, 28, and 12%, respectively. The experimental results showed that the proposed system was able to reduce the hand tremor of surgeons and perform precise needle insertion during DALK.

Individualized Therapy to Manage Quadriplegic Spastic Cerebral Palsy with Unani Treatment Modalities

  • Objective: Cerebral palsy (CP) is a neurodevelopment disorder attributed to an insult or injury to the developing brain with abnormalities in muscular tone, movement and motor skill. Improvement in quality of life and ameliorating symptoms can be achieved. Therefore, this case report details a distinctive approach to treating a 5-year-old male child with quadriplegic spastic cerebral palsy utilizing Unani treatment modalities. Methods: The treatment regimen commenced with 'Habb Ayarij for constipation followed by Sharbat Ustukhuddus administered orally. Notably, Sharbat Ustukhuddus was combined with Melia Azedarach L. leaves vapour bath. Subsequently, Roghan Babunna douche was performed followed by Dalk Layyin andcontinued until symptomatic improvement was observed. Majun Falasfa, Khamira Marwareed and Khameera Gauzaban were administered for 30 days. The therapeutic outcome included anthropometrical measurements, developmental milestones, spasm/reflex scale, and muscle power grading. Results and conclusion: Over the course of a 2-year follow-up, several clinical findings emerged. These included notable improvements in anthropometric measurements, developmental milestones such as improved head control and sitting ability, and a reduction in spasticity of the upper limbs, along with decreased muscle spasms. The therapeutic outcome of this individualized and holistic approach is potentially due to the multifaceted properties of medicinal plants (Musakkin wa Muharrik wa Muqawwi-i- A'sab wa Dimāgh, Munawwim, Dafi-i-Tashannuj, Muqawwi-i-Qalb-i-Ruh). Furthermore, the use of Dalk and Naṭūl was instrumental in providing nourishment to musculoskeletal cells and initiating intracellular signaling cascades. While these findings are encouraging, further research in the form of case series andrandomized controlled trials is warranted to validate the efficacy of this unique holistic approach.

A Simultaneous NIRS-EEG Study of Seizure in the Mouse Brain

  • We measured hemodynamic responses of seizure in the mouse brain using frequencydomain near infrared spectroscopy (NIRS) and electroencephalogram (EEG). We adapted microfabricated optical holder for consistent contact of the optical fiber to the mouse brain. Our results show that the cerebral oxygenation and hemodynamics of mice can be stably monitored with EEG in the mouse brain.

Hemodynamic Responses of Rat Brain Measured by Near-infrared Spectroscopy During Various Whisker Stimulations

  • NIRS (Near-infrared spectroscopy) is a relatively, new, non-invasive, and non-ionizing method of measuring hemodynamic responses in thick biological tissues such as the cerebral cortex. In this study, we measured the hemodynamic responses of the rat barrel cortex to whisker stimulation by using a frequency-domain NIRS system. We designed multiple optical probes comprising multi-mode optical fibers and manipulating arms, both of which can be easily applied to small animals. Various electrical stimulations were applied to rat whiskers at different voltage levels and stimulation frequencies. Our results show that the hemodynamic responses are highly dependent on the stimulation voltage level, and not so much on stimulation frequency. This paper suggests that NIRS technology is highly suitable for the study of small animal brains.
