  • Osteoarthritis is to the abnormality of the inflammatory response of joint tissue caused by various causes such as aging, and muscle loss. In this study, the activity in joint inflammation was verified using SKBE, a plant extract, and the expression levels of arthritis-inducing proteins including MMP-1, MMP-3, MMP-13, and collagen type II in vitro were compared and analyzed. Furthermore, we synthesized α-spinasterol, an active ingredient of SKBE, by the previously reported synthesis method and these findings could provide a new starting point for the development of treatments for osteoarthritis.

  • Objectives : This study was performed to develop a standard instrument of oriental medical evaluation for dementia. Methods: The advisor committee on this study was organized by 17 neuropsychiatry professors of oriental medical colleges. The items and structure of the instrument were based on review of published literature. We took consultation twice from the advisor committee and we also took additional advices bye-mail. Results: We discriminated between bian-zheng and su-zheng from the answers of the advisor committee. We got the mean weight of each symptom and sign from the answers of the advisor committee. We got the final weight from the combination of the ratio of bian-zheng to the number of all answers of the advisor committee and mean weight. Conclusions : The instrument of oriental medical evaluation for dementia was developed through experts' discussion. If the validity and reliability of this instrument is confirmed through additional clinical trial. the instrument of oriental medical evaluation for dementia is expected to be applied to the subsequent research.

  • Fibrous dysplasia is a bone disorder characterized by progressive replacement of normal bone by fibrous bone tissue. Common involving sites of fibrous dysplasia are the skeletal system including long bones, ribs, craniofacial bones and the pelvis. If maxilla were affected by fibrous dysplasia, antrum is almost always involved. And fibrous dyplasia in maxillary sinus were followed the shape of bone. In our case, the lesion involves antrum but, its shape was different from typical fibrous dysplasia pattern of maxillary sinus. Therefore we report a case of monostotic pediculated fibrous dysplasia in the maxillary sinus with a review of literature.

  • Diaphragmatic hernias have been reported in 0.8%-1.6% of patients who experience blunt chest trauma. The hernia is assumed to form as a result of direct diaphragmatic violation or significant intraabdominal or intrathoracic pressure caused by the trauma. Some reports have described cases of delayed diaphragmatic hernia and subsequent stomach perforation that occurred a few days to several years after an accident. We report an extremely rare case of diaphragmatic herniation in which the process from initial blunt trauma to visceral organ perforation took only 2 days, without any evidence of herniation on the initial X-ray or computed tomography. Delayed diaphragmatic herniation and subsequent visceral organ perforation should not be missed during the period immediately after blunt chest trauma.

  • Aims: This overview summarizes the existing evidence on acupuncture for cancer-related pain. Methods: Literature searches were conducted in 7 databases. All clinical studies of acupuncture, electroacupuncture, ear acupuncture and transcutaneous electrical nerve stimulation (TENS) in cancer patients with the main outcome measure of pain were included. Results: Of the 8 studies included, one high quality randomized clinical trial (RCT) of ear acupuncture showed statistically significant pain relief in comparison with placebo ear acupuncture. All the other studies were either non-blinded (n = 2) or uncontrolled clinical trials (n = 5). Most investigations suffered from methodological flaws such as inadequate study design, poor reporting of results, small sample size and overestimation of the results. Conclusions: The current evidence does not support the use of acupuncture as an adjunctive analgesic method for cancer patients. Only one high quality RCT showed promising results thus warranting further investigation into this under-researched area.

  • Whistler mode chorus waves, which are observed outside the plasmasphere of the Earth's magnetosphere, play a major role in accelerating and scattering energetic electrons in the radiation belts. In this study we developed a predicting scheme of the global distribution of chorus by using the Time History of Events and Macroscale Interactions during Substorms (THEMIS) satellite data. First, we determined global spatial distributions of chorus activity, and identified fit functions that best represent chorus intensities in specific L-MLT zones. Second, we determined the specific dependence of average chorus intensity on preceding solar wind conditions (e.g., solar wind speed, IMF Bz, energy coupling degree) as well as preceding geomagnetic states (as represented by AE, for example). Finally, we combined these two results to develop the predicting functions for the global distribution and intensity of chorus. Implementing these results in the radiation belt models should improve the local acceleration effect by chorus waves.

  • Central venous catheterization via an internal jugular vein or subclavian vein has become a common procedure in monitoring CVP and managing severely ill patients. However, there have beennumerous reports of complications associated with central venous catheterization. These include vessel injury, pneumothorax, hemothorax, nerve injury, arrhythmias, arteriovenous thrombosis, pulmonary embolism, and infection at the insertion site. We report a case of hemothorax after subclavian vein catheterization failure, along with successful treatment.
