인체 Histiocytic Lymphoma Cell Line U937의 종식 및 분화에 대한 새로운 $1.25(OH)_2D_3$ 유도체의 효과에 관한 연구 (Effects of a New 1,25(OH)$_2$-Vitamin $D_3$ Anglog on Proliferation and Differentiation of the Human Histiocytic Lymphoma Cell Line U937)

  • This study describes the effects of novel1, 25-dihydroxyvitamin D$_3$ analong[1,25(OH)$_2$-16ene-23yne-26, 27-F6-D$_3$] on proliferation of the human histiocytic lymphoma cell line U937 in vitro. We also examined the expression of c-myc oncogene in U937 cells was apparently inhibited to 62% and 87% of the control level after 4 days in the presence of 10-8M and 10-7 M of this analog, respectively. This compound morpholgically and functionally differentiated U937 cells to nonocyte-macrophage phenotype showing the increase of adherence ability to surface and a decrease of N/C ratio in Giemsa staining . Especially, nonspecific esterase activity which is a marker of cell differentiation to monocyte-macrophage was positive, and production of the positive stained cells increased in a dose dependent fashion . The expression of c-myc oncogene by 1, 25(OH)$_2$D$_3$ analog(10-7 M) was reduced by 60% at the mRNA level as determined by Northern blotting. The effects of this novel analog on cell proliferation and cell differentiation may open op new therapeutic strategies for human disorders such as psoriassis and may provide a tool to understand the mechanism of action of vitamin D$_3$ seco-steroids in malignancy.

Synthesis of Diacetoxy Acetal Derivatives of Santonin and their Enhancing Effects on HL-60 Leukemia Cell Differentiation

  • Several diacetoxy acetal analogues have been synthesized from santonin and assessed for their ability of inducing or enhancing the differentiation of human HL-60 leukemia cells. The compounds themselves had little effect on HL-60 cell differentiation. However, three analogues, 2a, 3a, and 5b, synergistically enhanced 1,25-dihydroxyvitamin $D_3[1,25-(OH)_2D_3]-induced$ HL-60 cell differentiation when combined with 5 nM of dihydroxyvitamin $D_3[1,25(OH)_2O_3]$, a well-known differentiation inducer. Especially, the compound 5b profoundly enhanced the $1,25-(OH)2O_3]-induced$ HL-60 cell differentiation.

  • Contents of vitamin $D_3$ and 25-OH-vitamin $D_3$ in marine animal products(20 species) were determined by HPLC. The isomers of vitamin D, $D_2$ and $D_3$, were not clearly separated on a reversed phase, ${\mu}$ Bonda Pak, with 20% methanol-acetonitrile, and on a normal phase, Zorbax SIL. with 0.4% isopropanol-hexane, but 25-OH-vitamin $D_2$ and-$D_3$ were separated on either ${\mu}$ Bonda Pak with 10% methanol-acetonitrile, or on Zorbax SIL with 2.2% isopropanol-hexane, respectively. Although levels of vitamin $D_3$ and 25-OH-vitamin $D_3$ varied remarkably according to species, their average value(fish : $l,l87{sim}36,007$ I.U/sample 100g, mussel : $58{\sim}1,706$ I.U/sample 100g, pickle: $1,208{\sim}79,358$ I.U/sample 100g) was greatly higher than that of meat($80{\sim}100$ I.U/sample 100g) and dairy products($400{\sim}800$ I.U/sample 100g). Fatty tissues of fish and pickled fish intestines contained high level of vitamin $D_3$ and 25-OH-vitamin $D_3$, while the clam and mussel known to have various kinds of sterol including ${\Delta}^7$-sterol showed very low levels of vitamin $D_3$ and its derivative.

Eugenol과 1α,25-dihydroxyvitamin D3의 병합처리에 의한 HL-60 세포의 분화 유도 (Cooperative Induction of HL-60 Cell Differentiation by Combined Treatment with Eugenol and 1α,25-Dihydroxyvitamin D3)

  • Eugenol은 여러가지 향신료에 있는 필수 오일의 주요 성분으로서 악성 종양 세포의 성장을 저해하고 사멸을 유도한다고 보고되었다. 본 연구에서는 eugenol이 세포 분화 유도에 관여하는지를 조사하기 위하여 HL-60 전골 수성 백혈병 세포의 분화에 미치는 eugenol의 효과를 연구하였다. HL-60 세포에 eugenol (150 ${\mu}M$)을 가했을 때 세포성장이 저해되었으며 $1,25(OH)_{2}$ vit $D_{3}$ (3 nM)와 병합처리시에는 더 큰 저해효과를 보였다. 이 때, eugenol은 $1,25(OH)_{2}$ vit $D_{3}$에 의해 유도되는 세포주기의 $G_{0}/G_{1}$단계 정지를 더욱 증가시킴을 알 수 있었다. 또한, eugenol과 $1,25(OH)_{2}$ vit $D_{3}$를 병합처리 하였을 때에는 $G_{0}/G_{1}$단계의 정지와 관련된 세포주기 조절인자인 p27 level를 상호 협동적으로 증가시켰을 뿐만 아니라 cyclin A, cdk2, cdk4 level를 감소시켰다. 또한 유세포분석실험을 통하여, CD14 (단핵세포 표지 인자)의 발현이 eugenol과 $1,25(OH)_{2}$ vit $D_{3}$를 병합처리한 세포에서 단독처리시보다 더 증가함을 알 수 있었다. 이러한 결과들은 eugenol이 $1,25(OH)_{2}$ vit $D_{3}$와 상호협동적으로 작용하여 저농도의 $1,25(OH)_{2}$ vit $D_{3}$에 의해 자극된 세포 분화 신호를 더욱 더 증대 시킴을 나타낸다. 이러한 eugenol에 의한 세포 분화 유도 작용은 암의 화학적예방 효과에 유용하게 응용될 수 있을 것으로 기대된다.

정상 성인의 혈청 Vitamin D 수준과 이에 영향을 주는 변인에 관한 연구 (A Study on Vitamin D Status and Factors Affecting It in Young Adults)

  • In this study, the serum level of 25-hydroxyvitamin D(25-(OH)D) was measured by high pressure liquid chromatography(HPLC), and factors affecting it were investigated in 72 young adults age ranging from 21 years to 39 years with normal bone density. The mean level of serum 25-(OH)D was 20.0$\pm$6.8ng/ml in males and 26.1$\pm$12.3ng/ml in females, which was significantly higher in females (p<0.01). The serum level of parathyroid hormone(PTH) showed a negative correlation with that of 25-(OH)D(p<0.05). Time spent outdoors in a day correlated positively with the serum level of 25-(OH)D(p<0.01). During the day, a specific time between 12:00 a.m. and 2:00 p.m. showed the most significant correlation with the level of 25-(OH)D(p<0.005). Among the nutrients studied, fat and vitamin D intake were positively correlated with the serum 25-(OH)D level. Stepwise multiple regression analysis showed that the serum level of 25-(OH)D could be fit by vitamin D intake(34.7% explained), serum PTH level (27.3% explained) and the time spent outdoors during the specific time(28.4% explained).1996)

Efficacy and safety of vitamin D3 B.O.N intramuscular injection in Korean adults with vitamin D deficiency

  • Objective: There has been no prospective study that examined intramuscular injection of high-dose vitamin D in Korean adults. The aim of this study was to assess the efficacy and safety of high-dose vitamin $D_3$ after intramuscular injection in Korean adults with vitamin D deficiency. Method: This study was a 24-week, prospective, multicenter, randomized, double-blind, placebo-controlled trial. A total of 84 subjects ${\geq}19$ and <65 years of age were randomly allocated to either the vitamin $D_3$ or placebo group in a 2:1 ratio. After randomization, a single injection of plain vitamin $D_3$ 200,000 IU or placebo was intramuscularly administered. If serum 25-hydroxyvitamin D (25[OH]D) concentrations were <30 ng/mLon week 12 or thereafter, a repeat injection was administered. Results: After a single intramuscular injection of vitamin $D_3$ to adults with vitamin D deficiency, the proportion of subjects with serum 25(OH)D concentrations ${\geq}30ng/mL$ within 12 weeks was 46.4% in the vitamin $D_3$ group and 3.6% in the placebo group (p<0.0001). The proportion of subjects with serum 25(OH)D concentrations ${\geq}30ng/mL$ within 24 weeks was 73.2% in the vitamin $D_3$ group and 3.6% in the placebo group (p<0.0001). Mean change in serum 25(OH)D concentrations at weeks 12 and 24 after vitamin $D_3$ injection was $12.8{\pm}8.1$ and $21.5{\pm}8.1ng/mL$, respectively, in the vitamin $D_3$ group, with no significant changes in the placebo group. Serum parathyroid hormone concentrations showed a significant decrease in the vitamin $D_3$ group but no change in the placebo group. Conclusion: Intramuscular injection of vitamin $D_3$ 200,000 IU was superior to placebo in terms of its impact on serum 25(OH)D concentrations, and is considered to be safe and effective in Korean adults with vitamin D deficiency.

뼈조직과 세포에서의 칼슘이동기전에 대한 1,25 dihydroxycholecaliciferol과 Ca Ionophore A23187의 영향 (Effects of 1,25 Dihydroxycholecalciferol and Ca Ionophore A23187 on Ca Transports in Bone and Bone Cells)

  • Various types of evidence suggest that some changes in cellular in cellular calcium may well signal the initiation of a chain of events leading to the physiological effects of the bone resorbing agents. The effects of 1,25-dihydorxycholecalciferol, $1.25\textrm{(OH)}_2\textrm{D}_3$, Ca ionophore A23187 and calcium antagonist, diltiazem on bone resprption and the cellular transport of Ca were investigated. Bone $^{45}\textrm{Ca}$ desaturation experiment was realized in isolated heterogenous rat bone cells after equilibrating the cells with $^{45}\textrm{Ca}$. Results of $^{45}\textrm{Ca}$ desaturation experiments were analysed by fitting the $^{45}\textrm{Ca}$ desaturation curve to a model of 2 exponential terms which indicated the presence of 2 exchangeable cellular calcium pools. $1.25\textrm{(OH)}_2\textrm{D}_3$ (0.5ng/$m\ell$) induced significantly bone resorption which was decreased by the physiological dose of diltiazeme(above 5nmol/$m\ell$) although it was ineffective alone. Ionophore A23187 (0.2$\mu\textrm{g}$/$m\ell$) decreased Ca release from bone but no additivity of effect with diltiazem(20nmol/$m\ell$) was observed. $1.25\textrm{(OH)}_2\textrm{D}_3$ (0.5ng/$10^{6}$ cells) had a moderate effect on the two kinetic phases of $^{45}\textrm{Ca}$ desaturation curve and these values were normalized when diltiazeme (20nmol/$10^{6}$ cells) was added along with $1.25\textrm{(OH)}_2\textrm{D}_3$. Ionophore($0.05\mu\textrm{g}$/$10^{6}$ cells) alone increased specifically the value of the slow turnover rate which was not affected by addition of diltiazem. The hypothesis concerning the involvement of calcium in bone resorption seems in fact to be verified in case of $1.25\textrm{(OH)}_2\textrm{D}_3$ but more unsettled for Ca inophore A23187.

영아에서 경련을 동반한 저칼슘혈증에 관한 연구 (Hypocalcemic Convulsion in Formula Feeding Young Infants)

  • 목적 : 최근 6개월 이하의 영아에서 저칼슘혈증의 임상 경험의 예가 증가하여 이의 원인, 증세, 치료 및 임상 경과를 알아보기 위하여 본 연구를 시행하였다. 방법 : 1994년 2월 부터 1991년 4월까지 경북대학교 병원 소아과에 경련을 주소로 내원하였던 환아 중 그 원인이 저칼슘혈증으로 확인되었던 11예를 대상으로 하였다. 이들은 모두 6개월 이하였으며 평균 $2.2{\pm}1.1$개월이었다. 이들 환아의 병력지를 통하여 후향적으로 각종 검사를 분석하였다. 결과 : 이 환아들의 저칼슘혈증의 원인으로는 비타민 D 결핍이 8예였고, 무기인의 과다한 섭취가 3예였다. 경련의 양상은 전신성 간-근대성이 9예였고 부분 근대성이 2례였다. 경련의 지속시간은 30초에서 20분까지 다양하였으며, 경련의 횟수는 평균 6.5회였다. $Bonky^{(R)}$, 칼슘 단독 또는 병합 요법 후 혈청 칼슘은 치료 전 $6.3{\pm}0.9$ mg/dL서 치료 후 $9.9{\pm}1.7$ mg/dL로 통계학적으로 의미있게 증가하였고(P=0.0008), 혈청 ALP는 치료 전 $1,418{\pm}864$ U/L에서 치료후 $772{\pm}503$ UL로 통계학적으로 의미있게 감소하였다(P=0.0112). 입원 당시 iPTH은 $176.7{\pm}129$ pg/mL로 증가되어 있었고 3명의 환아에서 치료 이후 추후 검사상 정상 범위로 회복 되었다. 모든 환아에서 치료 전$25(OH)D_3$는 감소되어 있었으며 평균 $3.9{\pm}2.7$ ng/mL였고, 치료 후 1예에서 정상으로 회복되었다. $1,25(OH)_{2}D_{3}$는 6예 중 2예에서 감소되어 있었다. 결론 : 최근 비타민 D가 보강된 것으로 알려진 조제 분유 영양아에서 발생된 경련을 동반한 저칼슘혈증의 원인은 상당수에서 비타민 D 결핍이었으며 $1,25(OH)_{2}D_{3}$의 단기간 투여로 성공적으로 치료되었으며, 조제 분유 영양아에서도 저칼슘혈증의 원인으로 비타민 D 결핍을 반드시 생각해야 한다고 사료 된다.

Relationship between 25-hydroxyvitamin D and metabolic syndrome among Jordanian adults

  • Evidence of the association between 25-hydroxyvitamin D (25(OH)D) and metabolic syndrome (MeS) remains uncertain and incongruent. This study aimed to determine the association between 25(OH)D and MeS among Jordanian adults. A complex multistage sampling technique was used to select a national population-based household sample. The present report deals exclusively with adults aged > 18 years who had complete information on all components of MeS (n = 3,234). A structured questionnaire was used to collect all relevant information. Anthropometric, clinical, and laboratory measurements were obtained. MeS was defined according to the International Diabetes Federation (IDF) definition. Of the total, 42.0% had MeS and 31.7% had 25(OH)D < 30 ng/ml. In a stratified analysis, the prevalence of MeS did not differ significantly between subjects with low and normal 25(OH)D levels for men and women in all age groups. In the multivariate analysis, the odds of MeS were not significantly different between subjects with low and normal 25(OH)D levels (OR = 0.85, 95% CI: 0.70, 1.05, P-value = 0.133). The association between 25(OH)D and MeS remained non-significant when 25(OH)D was analyzed as a continuous variable (OR = 1.004, 95% CI; 1.000, 1.008, P = 0.057) and when analyzed based on quartiles. None of the individual components of MeS were significantly associated with 25(OH)D level. This study does not provide evidence to support the association between 25(OH)D level and MeS or its individual components. Prospective studies are necessary to better determine the roles of 25(OH)D levels in the etiology of MeS.

Cholecalciferol 대사연구(代謝硏究)에 관(關)한 일정리(一整理) (Cholecalciferol Metabolism Metabolic Conversion and Mode of Action)

  • Cholecalciferol은 피하에서 자외선과 체온의 작용을 받아 7-dehydrocholesterol로부터 Previtamin D를 거쳐 합성이 되며 이어서 간에 빨리 축적, 25 hydroxylation을 거치게 되고 신장에서 가장 활성을 띤 형태인 $1.25-(OH)_{2}CC$로 변하게 된다. 한편 신장에서는 체내의 Ca, P이 정상으로 존재하게 되면 1-hydroxylation이 억제되는 대신 24-hydroxylation이 일어나 또 다른 active form인 $24,\;25-(OH)_{2}CC$가 되는데 24-hydroxylation의 역활이 무엇인가에 대해서는 아직 구체적으로 밝혀지지 않았다. $24,\;25-(OH)_{2}CC$$1.25-(OH)_{2}CC$는 모두 공통적으로 또 다른 active form인 $1.24,\;25-(OH)_{3}CC$를 형성할 수도 있다. 그중 $1.25-(OH)_{2}CC$는 Steroid H.으로 일컬어지기도 하는 Vit. D metabolite중에서 가장 활성을 가진 형태이며 표적기관으로 크게 소장, 뼈, 신장을 들 수 있다. 소장에서의 역활은 무엇보다도 Ca흡수에 관련되는 것으로 소장에서의 Ca흡수와 Ca BP합성에 관여한다. 골격 형성과 뼈의 mineralization에 관여하는 Vit. D metabolite를의 효과에 관해서는 아직까지 일관성있는 보고가 없다. 한편 $1.25-(OH)_{2}CC$는 side chain oxidation을 거쳐 $CO_{2}$와 미지의 물질을 생성하는데 이 mechanism이 어떤 의미를 갖는지는 분명치 않다. 그밖에 또 다른 표적기관으로서 최근 타액선의 존재가 알려졌으며 Vit. D가 배설되는 경로에 대해서는 새로운 바가 없다. Vit. D가 담즙을 통해서 우선적으로 배설되고 소량이 뇨를 통해 배설되는데 metabolite들의 배설경로는 더욱 규명되어야 할 과제이다.
