갑상선 재발암의 진단과 치료에 대한 고찰 (Diagnosis and Treatment of the Recurrent Thyroid Cancer)

  • Objectives: To demonstrate effective diagnostic method and proper management of recurrent thyroid cancer through to compare treatment and surveillance of $I^{131}$ scanning detected recurrence and clinically detected recurrence. Material and Methods: We retrospectively analyzed clinical information about 46 patients who has recurrent thyroid cancer of 298 patients who have been primarily operated due to thyroid cancer in PMC at the over 10 years between 1986 and 1995. We examine incidence of recurrence due to pathologic types, site of recurrence, disease free interval, detection method of recurrence, and also treatment and progression of recurrence. A patients in which the clinical examination was entirely negative and the $I^{131}$ scan demonstrated either a new area of $I^{131}$ uptake or an increased area of concentration, compared to the previous scan, was designated as a recurrence detected by $I^{131}$ scan only. Recurrences that were obviously by physical examination or chest x-ray, etc were considered clinically detected recurrence, regardless of the the results of the thyroid scan. Results: Mean of disease tree interval(DFI) is 36months. When mean DFI of $I^{131}$ scan detected recurrence is 28months, whereas mean DFI of clinically detected recurrence is 47months. In statiscal analysis, p-value is 0.043 as significantly. In progression of recurrent patient, NED is 28case, AWD is Sease, DOD is 13case. Among the 13case, scan detected recurrence is lease of 20 patients(5%), whereas clinically detected recurrence is l2case of 26 patient(46%). In statiscal analysis, p-value is 0.003 as significantly. Conclusion: Early detection of the recurrent thyroid cancer by $I^{131}$ scanning leads to good progress compare with detection by clinical examination. Sperm chromatin structure assay results in Nigerian men with unexplained infertility

  • Objective: Several publications have established a relationship between sperm DNA damage and male factor infertility, based on data from America, Europe, and Asia. This study aimed to compare the extent of sperm DNA damage in sperm samples from Nigerian men with unexplained infertility and in sperm samples from a fertile group composed of sperm donors who had successfully impregnated a female partner naturally or through assisted conception. Methods: A total of 404 men underwent male fertility evaluation at Androcare Laboratories and Cryobank participated in this study. Semen analysis and a sperm chromatin structure assay (SCSA) were performed on all subjects. Results: The men in the unexplained infertility group were slightly older than the men in the fertile sperm group ($36{\pm}10$ years vs. $32{\pm}6$ years, p=0.051). No significant difference was observed between the two groups in semen analysis parameters ($p{\geq}0.05$). Men in the unexplained infertility group with normal semen parameters had a significantly higher DNA fragmentation index (DFI) than men in the fertile sperm group ($27.5%{\pm}7.0%$ vs. $14.1%{\pm}5.3%$, p<0.05). In the unexplained infertility group, 63% of the men had a DFI greater than 20%, compared to 4% in the fertile sperm group. In the unexplained infertility group, 15.2% of the subjects had a DFI greater than 30%, compared to 1% in the fertile sperm group. Conclusion: Our study showed that the SCSA may be a more reliable predictor of fertility potential than traditional semen analysis in cases of unexplained infertility.

The effect of temperature and storage time on DNA integrity after freeze-drying sperm from individuals with normozoospermia

  • Objective: This study evaluated the effects of temperature and storage time on the quality and DNA integrity of freeze-dried sperm from individuals with normozoospermia. Methods: Normal sperm samples from 15 men aged 24 to 40 years were studied. Each sample was divided into six groups: fresh, freezing (frozen in liquid nitrogen), freeze-dried then preserved at room temperature for 1 month (FD-1m-RT), freeze-dried then preserved at room temperature for 2 months (FD-2m-RT), freeze-dried then preserved at 4 ℃ for 1 month (FD-1m-4 ℃), and freeze-dried then preserved at 4 ℃ for 2 months (FD-2m-4 ℃). The morphology, progressive motility, vitality, and DNA integrity of the sperm were evaluated in all groups. Results: In all freeze-dried groups, sperm cells were immotile after rehydration. The freeze-dried groups also showed significantly less sperm vitality than the fresh and frozen groups. Significantly more morphological sperm abnormalities were found in the freeze-dried groups, but freeze-drying did not lead to a significantly higher DNA fragmentation index (DFI). The DFI was significantly higher in the FD-2m-RT group than in the other freeze-dried groups. Conclusion: The freeze-drying method preserved the integrity of sperm DNA. The temperature and duration of storage were also identified as factors that influenced the DFI. Accordingly, more research is needed on ways to improve sperm quality in the freeze-drying process.

황복, Takifugu obscurus의 초기 생활사와 공식 행위에 있어 밀도와 먹이 공급 횟수의 영향 (Influence of Density and Feeding Frequency on Early Life History and Cannibalism of River Puffer, Takifugu obscurus)

  • 본 연구는 황복, T. obscurus 치어(TL $25.0{\pm}0.5$ mm, BW $0.53{\pm}0.03$ g)의 초기 생활사 동안 성장, 생존 및 공식행위에 있어 밀도와 먹이 공급횟수(공급량)의 관련성을 구명하기 위해 밀도와 먹이제한 조건에서 일렬의 실험을 실시하였다. 실험기간 중 실험 밀도는 1.43 inds./${\ell}$, 2.14 inds./${\ell}$, 2.86 inds./${\ell}$, 3.57 inds./${\ell}$, 4.29 inds./${\ell}$ & 7.14 inds./${\ell}$와 같이3반복으로 6개의 실험구를 설정하여 주었고, 먹이 공급은 1~15일까지 3회/일, 16~30일까지는 1회/일로 횟수를 제한하였다. 실험은 1톤 수조에서 30일간 실시되었다. 실험기간 동안, DFI, FE, 성장, 공식빈도, 공식 폐사율(생존율)을 조사하였다.DFI와 FE는 밀도에 따른 어떠한 경향도 보이지 않았다. 공식 빈도와 폐사율은 밀도가 증가하고, 먹이 공급횟수가 감소할수록 유의하게 증가하였다. 그에 따른 생존율은 밀도가 증가할수록 유의하게 감소하는 경향이 있었다. 결론적으로 군집밀도는 공식 행위와 정상관계를 보였지만, 먹이량은 공식과 역상의 관계를 지니고 있음을 확인할 수 있었다.

Prognostic factors for survivals from first relapse in breast cancer patients: analysis of deceased patients

  • Purpose: This study was performed to evaluate prognostic factors for survival from first relapse (SFFR) in stage I-III breast cancer patients. Materials and Methods: From June 1994 to June 2008, 3,835 patients were treated with surgery plus postoperative radiotherapy and adjuvant chemotherapy for stage I-III breast cancer at Samsung Medical Center. Among them, a total of 224 patients died by June 2009, and 175 deaths were of breast cancer. Retrospective review was performed on medical records of 165 patients who met the inclusion criteria of this study. Univariate and multivariate analysis were done on survivals according to variables, such as age, stage, hormone status of tumor, disease-free interval (DFI), sites of first failure, number of organs involved by recurrent disease (NOR), application of salvage treatments, and existence of brain or liver metastasis (visceral metastasis). Results: Patients' median overall survival time was 38 months (range, 8 to 123 months). Median SFFR was 17 months (range, 5 to 87 months). Ninety percent of deaths occurred within 40 months after first recurrence. The patients with SFFR ${\leq}1$ year had tendency of triple-negativity, shorter DFI (${\leq}2$ years), larger NOR (>3), visceral metastasis for first relapse than the patients with SFFR >1 year. In multivariate analysis, longer DFI (>2 vs. ${\leq}2$ years), absence of visceral metastasis, and application of salvage treatments were statistically significant prognosticators for longer SFFR. Conclusion: The DFI, application of salvage treatments, and visceral metastasis were significant prognostic factors for SFFR in breast cancer patients.

새로운 PGE1인 Lipo-AS013이 피판의 혈류와 생존에 미치는 영향 (An Effects of a New PGE1: Lipo-AS013 on Blood Flow and Survival of Skin Flap)

  • Prostaglandin $E_1$($PGE_1$) is known to have various physiological action such as vasodilatation, decrease of blood pressure, angiogenesis, inhibition of platelet aggregation and so forth. $PGE_1$ has been developed in many different formulations in order to overcome its chemical instability and deactivation in the lungs when administered parenterally. Lipo-AS013 is a potent drug with higher chemical stability and greater vascular wall targeting than others. The study was done on $3{\times}10cm$ model flap of dorsal skin of Sprague-Dawley rats and the flap perfusion survival were observed and documented. The flap treated with Lipo-AS013 beforehand was given intravenously Sodium fluorescein 10 minutes later, and then Percent Dye Fluorescence Index(% DFI) was calculated. The results were compared to a control group and the group administered locally epinephrine.. In the control group, the % DFI and flap survival rate increased from $54.1{\pm}6.7$ to $65.0{\pm}2.6$(p<0.01) while in Lipo-AS013 group from $55.3{\pm}2.2$ to $67.4{\pm}1.9$(p<0.01), respectively. In the epinephrine group, the % DFI(p<0.05) and flap survival rate(p<0.001) decreased. In the both epinephrine and Lipo-AS013 group Percent DFI and flap survival rate are comparable with the control group.The result indicates that the potent Lipo-AS013 enhances the blood flow and flap survival. This highly potent Lipo-AS013 may have targeting ability and accumulate $PGE_1$ onto the vascular walls. A quantitative analysis of fluorescence on the skin surface is a reliable tool to measure the blood perfusion into an ischemic flap and its viability. Further comparative study with conventional $PGE_1$ and Lipo-$PGE_1$ is needed in order to clarify the action and efficiency of Lipo-AS013.

미세술기를 이용한 정계정맥류절제술이 정자 핵 내 DNA Integrity에 미치는 효과 (Effect of Microsurgical Varicocelectomy on Human Sperm Nucleus DNA Integrity)

  • 목 적: 인간 정자 핵 내의 DNA integrity는 배아의 발달 및 임신 유지에 중요한 역할을 하여 DNA integrity가 손상된 경우 불임과 유산의 원인이 된다고 하며, 정계정맥류는 DNA 손상을 일으키는 대표적인 원인 중 하나이다. 본 연구에서는 미세술기를 이용한 정계정맥류절제술로 정계정맥류를 교정을 하였을 때 정자 핵 내 DNA integrity가 어떠한 영향을 받는지에 대하여 알아보았다. 연구방법: 2006년 4월부터 2007년 4월까지 불임을 주소로 미세술기를 이용한 정계정맥류절제술을 받았던 18명의 환자에서 수술 전 후에 정액검사의 다른 지표들과 함께 정자 핵 내 DNA integrity가 어떻게 변화하였는지 조사하였다. 정자 핵 내 DNA integrity를 측정하는 방법으로 comet assay를 시행하였고, comet assay를 통한 DNA 손상 정도는 DNA fragmentation index (DFI)로 나타내었다. 결 과: 수술 후 4개월에 모든 환자에서 재발의 소견은 보이지 않았으며, DNA 손상 정도를 나타내는 평균 DFI는 수술 전에 19.3%, 수술 후에 13.7%로 유의한 변화를 보였다. 수술 전 DFI가 10 이상으로 비정상인 14명의 환자들 중 12명 (85%)에서 개선 소견을 보였으나, 수술 전 DFI가 10 미만인 정상 환자 4명에서는 1명 (25%)만이 개선 소견을 보였다. 수술 후 정자의 밀도, 운동성, 생존성에서 호전 양상을 보였으나 유의한 차이는 없었다. 결 론: 미세술기를 이용한 정계정맥류절제술을 통한 수술적 교정은 정액검사상의 다른 지표의 개선 뿐 아니라, 정자 핵 내 DNA 손상을 감소시킬 수 있다. 이상에서 정계정맥류의 수술적 교정으로 정자 핵 내 DNA integrity의 개선을 기대할 수 있으며, 이는 보다 양호한 정자를 많이 얻을 수 있어 자연임신이나 보조 생식술의 성공 가능성을 높일 수 있다는 점을 제시한다.

넙치(Paralichthys olivaceus) 치어의 성장 및 양면착색 현상에 있어 잠입기질의 효과 (Effect of the Burrowing Substratum on the Growth and Ambicoloration of Juvenile Flounder Paralichthys olivaceus Cultured at High Density)

  • To evaluate the influence of burrowing substrate on the rearing performance and ambicoloration of cultured flounder, Paralichthys olivaceus, we compared the daily food intake (DFI), feed efficiency (FE), survival, growth, proportion of pigmented skin on the blind side, and proportion of ambicolored fish. We reared juvenile flounders [total length (TL) $4.46{\pm}0.06cm$, body weight (BW) $0.77{\pm}0.03g$] in dark-green fiberglass-reinforced plastic (FRP) aquariums without (control) or with gravel substrate at a density of 200 fishes/ton for 120 days. While there was no difference in survival rate or growth, the DFI was lower and FE higher in the group raised with substrate than in the control. The proportions of pigmented area on the blind side and ambicolored fish were significantly higher in the control tank. Therefore, the supplement of substrate on the aquarium bottom positively affects the feeding efficiency, and inhibits abnormal pigmentation on the blind side in flounder farming at high density.

State-of-the-art update for diagnosing diabetic foot osteomyelitis: a narrative review

  • Recently, the International Working Group on the Diabetic Foot and the Infectious Diseases Society of America divided diabetic foot disease into diabetic foot infection (DFI) and diabetic foot osteomyelitis (DFO). DFI is usually diagnosed clinically, while numerous methods exist to diagnose DFO. In this narrative review, the authors aim to summarize the updated data on the diagnosis of DFO. An extensive literature search using "diabetic foot [MeSH]" and "osteomyelitis [MeSH]" or "diagnosis" was performed using PubMed and Google Scholar in July 2023. The possibility of DFO is based on inflammatory clinical signs, including the probe-to-bone (PTB) test. Elevated inflammatory biochemical markers, especially erythrocyte sedimentation rate, are beneficial. Distinguishing abnormal findings of plain radiographs is also a first-line approach. Moreover, sophisticated modalities, including magnetic resonance imaging and nuclear medicine imaging, are helpful if doubt remains after a first-line diagnosis. Transcutaneous bone biopsy, which does not pass through the wound, is necessary to avoid contaminating the sample. This review focuses on the current diagnostic techniques for DFOs with an emphasis on the updates. To obtain the correct therapeutic results, selecting a proper option is necessary. Based on these numerous diagnosis modalities and indications, the proper choice of diagnostic tool can have favorable treatment outcomes.

Photochemical Response Analysis on Drought Stress for Red Pepper (Capsiumannuum L.)

  • The aim of this study is to determine the drought stress index through photochemical analysis in red pepper (Capsiumannuum L.). The photochemical interpretation was performed in the basis of the relation between Kautsky effect and Photosystem II (PSII) following the measurement of chlorophyll, pheophytin contents, and $CO_2$ assimilation in drought stressed 5-week-old red pepper plants. The $CO_2$ assimilation rate was severely lowered with almost 77% reduction of chlorophyll and pheophytin contents at four days after non-irrigation. It was clearly observed that the chlorophyll fluorescence intensity rose from a minimum level (the O level), in less than one second, to a maximum level (the P-level) via two intermediate steps labeled J and I (OJIP process). Drought factor index (DFI) was also calculated using measured OJIP parameters. The DFI was -0.22, meaning not only the initial inhibition of PSII but also sequential inhibition of PSI. In real, most of all photochemical parameters such as quantum yield of the electron transport flux from Quinone A ($Q_A$) to Quinone B ($Q_B$), quantum yield of the electron transport flux until the PSI electron acceptors, quantum yield of the electron transport flux until the PSI electron acceptors, average absorbed photon flux per PSII reaction center, and electron transport flux until PSI acceptors per cross section were profoundly reduced except number of QA reducing reaction centers (RCs) per PSII antenna chlorophyll (RC/ABS). It was illuminated that at least 6 parameters related with quantum yield/efficiency and specific energy fluxes (per active PSII RC) could be applied to be used as the drought stress index. Furthermore, in the combination of parameters, driving forces (DF) for photochemical activity could be deduced from the performance index (PI) for energy conservation from photons absorbed by PSII antenna until the reduction of PSI acceptors. In conclusion, photochemical responses and their related parameters can be used as physiological DFI.
