A ranula is a bluish, transparent, and thin-walled swelling in the floor of the mouth. They originate from the extravasation and subsequent accumulation of saliva from the sublingual gland. Ranulas are usually limited to the sublingual space but they sometimes extend to the submandibular space and parapharyngeal space, which is defined as a plunging ranula. A 21-year-old woman presented with a complaint of a large swelling in the left submandibular region. On contrast-enhanced CT images, it dissected across the midline, and extended to the parapharyngeal space posteriorly and to the submandibular space inferiorly. Several septa and a fluid-fluid level within the lesion were also demonstrated. We diagnosed this lesion as a ranula rather than cystic hygroma due to the location of its center and its sublingual tail sign. As plunging ranula and cystic hygroma are managed with different surgical approaches, it is important to differentiate them radiologically.
Intraventricular arachnoid cyst has been rarely reported. Here we present two cases of symptomatic intraventricular arachnoid cysts in the fourth ventricle and right lateral ventricle. The first patient was a 38-year-old female who complained of headache and left facial hypesthesia. Computed tomography and MR scan revealed large cystic lesion in the fourth ventricle. After cyst wall removal, facial hypesthesia disappeared immediately and headache improved slowly. The second patient was a 9-year-old girl who complained of headache, vomiting and paresthesia in her right low extremity. Cystic lesion in the right lateral ventricle was detected in the CT and MR scan. The symptoms improved after cyst wall removal. Surgical findings of these two cases showed that the cyst walls were attached firmly to the choroid plexus. Symptomatic intraventricular arachnoid cyst must be treated appropriately and we recommend complete cyst wall removal.

Background: Odontogenic cysts associated with lower third molar are common. The prognosis for surgical treatment is relatively good. However, postoperative infection discourages the clinicians. Hence, we would like to investigate the factors associated with infection after surgical treatment of cysts associated with the mandibular third molar. Methods: We retrospectively reviewed the medical and radiographic records of 81 patients who were diagnosed with dentigerous cyst or odontogenic keratocyst and underwent cyst enucleation. The factors affecting postoperative infection were divided into host factor, treatment factor, and cystic lesion factor. To identify the factors associated with postoperative infection, we attempted to find out the variables with significant differences between the groups with and without infection. Results: A total of 81 patients (64 male and 17 female) were enrolled in this study. There was no statistical relationship about the postoperative infection between all variables (gender, smoking, diabetes mellitus, age, bone grafting, related tooth extraction, previous marsupialization or decompression, type of antibiotics, cortical perforation associated with cystic lesion, preoperative infection, preoperative cyst size). Conclusions: The results of this study suggest that it is not necessary to avoid bone grafts that are concerned about postoperative infection.

This study aimed to describe a rare case of gnathostomiasis in the vocal cord. A 54-yearold Chinese woman living in Korea visited with a chief complaint of voice change at the outpatient department of otorhinolaryngology in Hallym Sacred Heart Hospital, Hallym University on August 2, 2021. She had eaten raw conger a few weeks before the voice change developed, but her medical history and physical examinations demonstrated neither gastrointestinal symptoms nor other health problems. A round and red cystic lesion, recognized in the anterior part of the right vocal cord, was removed using forceps and scissors through laryngeal microsurgery. The histopathological specimen of the cyst revealed 3 cross-sections of a nematode larva in the lumen of the cyst wall composed of inflammatory cells and fibrotic tissues. They differ in diameter, from 190 ㎛ to 235 ㎛. They showed characteristic cuticular layers with tegumental spines, somatic muscle layers, and gastrointestinal tracts such as the esophagus and intestine. Notably, intestinal sections consisted of 27-28 lining cells containing 0-4 nuclei per cell. We tentatively identified the nematode larva recovered from the vocal cord cystic lesion as the thirdstage larva of Gnathostoma, probably G. nipponicum or G. hispidum, based on the sectional morphologies.
폐에 발생하는 간엽 낭성 과오종(mesenchymal cystic hamartoma: MCH)은 매우 드문 종양으로 1986년에 처음으로 발표되었고 아직까지 유병율은 밝혀지지 않았다. 방사선 사진과 조직 검사에서 특징적으로 양측 폐에 다발성의 결절과 크기가 다양한 낭포들이 보인다. 결절들은 미성숙 간엽 세포들의 증식으로 이루어지고 결절의 크기가 점차 커지면서 낭포를 형성하게 되는데 낭포의 내경은 정상적 또는 화성 호흡 상피 세포로 이루어지고, 그 벽의 중간층은 방추형의 간엽세포충으로 되어 있다. 주증상은 객혈과 재발되는 기흉, 그리고 혈흉이다. 비교적 양성종으로 알려져 있지만 악성 변화의 가능성이 있다. 폐기포 절제술 시 폐 전체 표면에 다양한 크기의 낭포성 병소와 결절들이 육안적으로 관찰되었고, 조직 검사상 낭포의 내경은 호흡 상피로 둘러져 있었고, 그 벽의 중간층은 원시 간엽 세포층로 이뤄져있었다. 육안적 소견과 광학현미경하 소견이 간엽 낭성 과오종에 적합한 소견이었다.. 저자들은 자주 재발되는 기흉과 객혈이 있었던 27세의 여자에서 간엽 낭성 과오종을 경험하였기에 보고하는 바이다.
A 61-year-old male who complained of right upper quadrant pain was referred to the authors for evaluation after his computed tomography suggested biliary adenocarcinoma. The lesion consisted of multiple cysts with papillary mass and peri-ampullay mass. The patient underwent an operation due to a clinical suspicion of biliary cystadenocarcinoma, but the pathology confirmed biliary papillomatosis (BP) after diagnosing intrahepatic papillary neoplasm with high-grade dysplasia and invasive adenocarcinoma with papillary neoplasm from the distal common bile duct to the duodenum. BP is a disease characterized by multiple papillary masses. Its cause has yet to be discovered. It commonly manifests as bile duct dilation but rarely as a ductal cystic change. Under computed tomography or magnetic resonance imaging, both the BP and the cystic neoplasm can show bile duct dilation and a papillary mass, which makes their differential diagnosis difficult. A confirmative diagnosis can be made through a pathologic examination. BP is classified as a benign disease that can become malignant and may recur, though rarely. Its treatment of choice is surgical resection. Laser ablation or photodynamic therapy can be used for unresectable lesions. In the case featured in this paper, biliary papillomatosis was difficult to differentiate from cystic adenocarcinoma due to diffusely scattered multiple large cystic lesions in the liver, and it was histologically confirmed to have become malignant with cystic duct dilation after the operation. This case is reported herein with a literature review.
Adenoid cystic carcinoma(ACC) is the second most common malignant neoplasm in the salivary gland. In spite of ACC shows slowly growing nature, it is sometimes highly detrimental that it readily invades adjacent tissues and metastasize to distant organs at the early stage of disease. Hence, treatment outcome may be misfortunate due to wide regional infiltration, pathognomonic perineural spread and the tendency of hematogenous metastasis. We present a unusal case of ACC of the scalp in which the patient initially presented with bumpy mass of the scalp who had been diagnosed as the primarily developed ACC of the right hard palate that extended to infratemporal fossa, nasal cavity, and paranasal sinuses and had been treated by total excision and post-operative chemotherapy and radiation therapy for 10 years ago. Although this lesion occured at the scalp, which is a frequent site of primary dermatologic ACC, its histomorphology was the same with that of previous tumor. Complete clinical examination showed no recurrence sign at the primary site and suggested the scalp as a sole treatment failure site. Accordingly, it would be reasonable to consider the present case as either a scalp metastasis or a second primary lesion of salivary gland ACC.
섬유성 골이형성증은 골의 발육 이상으로 해면골이 섬유성 조직으로 대치되어 나타나는 양성 병변으로 대퇴골, 경골, 두개골, 늑골, 상완골 등에 호발하며 동통, 진행하는 병변, 병적 골절 등을 일으키기 쉬워 여러 가지 치료법이 시도되어 왔으나 그 중 골 소파 및 골 이식술이 현재까지 널리 시행되고 있다. 본 교실에서는 17세 여자 환자로 우측 상완골 전장에 걸친 다발성 골 낭종을 동반한 섬유성 골이형성증 환자에 steroid 주입법을 약 3개월 간격으로 총 2차례 시행하였고 처음 주입 후 약 11개월 후에 시행한 추시 단순 방사선 촬영상 병변의 크기가 감소하였고, 골음영 및 피질골의 두께가 증가하였으며, 자기공명영상촬영 검사상 병변의 크기가 상당히 감소하였고, T2 강조 영상에서 정상 골수의 신호강도와 비슷하게 변화 하였으며, 처음 주입 후 약 2년 8개월 후의 외래 추시상 통증 호소하지 않고 무리하지 않는 한도내에서 큰 불편 없이 지내고 있어 섬유성 골이형성증의 호전 소견을 보였기에 문헌고찰과 함께 보고하는 바이다.
초음파 진단기의 설정은 영상의 질에 영향을 준다. sonographer는 최적의 영상을 얻기 위해 초음파 영상의 질에 영향을 주는 설정 변수에 대한 효과를 이해해야 한다. 본 연구에서는 4가지 대표적인 영상 조절 변수 즉 TGC (Time Gain Control), 이득 (Gain), 주파수 (Frequency), DR (Dynamic Range)를 고려하였다. 초음파 영상의 질은 LCS (Law Contrast Sensitivity) 관점에서 정량적으로 비친 평가하였다. 실험은 임상용 초음파 진단기 (SA-9000 PRIME, Medison, Korea)를 사용하여 초음파 평가 팬텀 (539, ATS, USA)의 LCS 타겟을 영상화하였다. 영상 조절 변수의 설정을 변화하면서, 각 설정에 대한, 6개의 LCS 영상 (+15 dB, +C dB, +3 dB, -3 dB, -6 dB, -15 dB)을 취득하고, 영상에 대한 LCS픽셀 값을 계산하였다. 실험결과 TGC가 최대, Gain은 중간에서 최대사이, 주파수가 Pen모드, DR이 40-66 dB일 때 타겟 영상 (LCS)의 평균 픽셀 값이 높았다. 모든 타겟 영상에서 DR이 40 dB일 때에 LCS가 좋은 영상을 얻었다. 본 결과는 임상에서 잘 발견되는 solid lesion (+15, +6, +3 dB 타겟과 유사) 또는 cystic lesion (-15, -6, -3 dB 타겟과 비슷)이 있는 mass 평가 시 적절한 영상조절 변수 설정에 유용한 정보를 제공할 것으로 기대된다.
The glandular odontogenic cyst is a rare lesion described in 1987. It generally occurs at anterior region of mandible in adults over the age of 40 and has a slight tendency to recur. Histopathologically, a cystic cavity lined by a nonkeratinized, stratified squamous, or cuboidal epithelium varying in thickness is found including a superficial layer with glandular or pseudoglandular structures. A 21-year-old male visited Dankook University Dental Hospital with a chief complaint of swelling of the left posterior mandible. Radiographically, a huge multilocular radiolucent lesion involving impacted 3rd molar at the posterior mandible was observed. Buccolingual cortical expansion with partial perforation of buccal cortical bone was also shown. Histopathologically, this lesion was lined by stratified squamous epithelium with glandular structures in areas of plaque-like thickening. The final diagnosis was made as a glandular odontogenic cyst.
